Cat-transmitted sporotrichosis caused by Sporothrix brasiliensis is an emerging zoonosis in Latin America. Because treatment of feline sporotrichosis is often not effective, we sought to determine whether treatment failure results from S. brasiliensis strains that have existing elevated MICs for itraconazole, the primary treatment for this disease. During 2021-2023 at the triple border region of Brazil, Paraguay, and Argentina, 108 S. brasiliensis strains were isolated from felines before antifungal treatment. The main clinical manifestation was cutaneous disseminated sporotrichosis (61%), which was the only form resulting in sporotrichosis-induced deaths (61%). We conducted antifungal susceptibility testing for 9 antifungal compounds, evaluating for both mycelial and yeast phases. MIC levels were low for most antifungal agents but were higher in the mycelial phase than in the yeast phase, especially for voriconazole and isavuconazole. We conclude that the varying clinical manifestations of sporotrichosis and large differences in mortality rates were not caused by elevated itraconazole MICs.

We describe a large urban outbreak of feline sporotrichosis caused by Sporothrix brasiliensis fungi in Argentina. Over a 7-month period in Puerto Iguazú, which borders Brazil, we identified culture-proven sporotrichosis in 9 cases across 7 households. Public health officials should coordinate cross-border One Health actions and institute context-specific interventions.

Yeast bloodstream infections lead to high mortality and morbidity and are mostly observed in immunocompromised patients. In Africa, only a few studies have characterized clinical yeasts. To increase insight into yeast resistance and transmission in Africa, we identified various yeasts from Alexandria, Egypt and performed antifungal susceptibility testing (AFST) and genotyping. A total of 1307 single isolates from unique patients, recovered from different anatomical sites including the bloodstream, retrieved from a reference laboratory in Alexandria, Egypt were studied. All isolates were identified with MALDI-TOF MS, while some were initially identified with a Vitek 2 Compact system. Short tandem repeat (STR) genotyping was performed for the most common species, and AFST was performed with microbroth dilution. Among bloodstream isolates (n = 71), C. albicans was the most common etiological agent, followed by C. tropicalis and C. parapsilosis. Comparison of yeast identification methods demonstrated that 22% of isolates were incorrectly identified with the Vitek 2 Compact system compared to MALDI-TOF MS. Multiple rare yeasts showed reduced antifungal susceptibility. STR genotyping demonstrated potential events of nosocomial transmission with N. glabratus and C. parapsilosis. Moreover, an azole-resistant C. tropicalis clade identified earlier in Alexandria was still present. To conclude, clinical yeasts in Alexandria, Egypt, are overall susceptible common species.

First Cases of Feline Sporotrichosis Caused by Sporothrix brasiliensis in Paraguay

Sporothrix brasiliensis is an emerging fungal pathogen causing cat-transmitted sporotrichosis, an epi-zoonosis affecting humans, cats and dogs in Brazil and now spreading to neighboring South American countries. Here, we report the first two autochthonous cases of cat-transmitted sporotrichosis in Paraguay. The first case was a four-year-old male cat showing several ulcerative lesions, nasal deformity and respiratory symptoms. The second case was a one-year-old male cat showing a single ulcerated lesion, respiratory symptoms and nasal deformity. Both cases were admitted to a veterinary clinic in Ciudad del Este, Paraguay. Isolates were recovered from swabs of the two cases. Using molecular methods, the isolates were identified as S. brasiliensis.

Environmental isolation of Sporothrix spp. in veterinary settings

Background This study investigated environmental contamination by Sporothrix spp. in a veterinary facility specialised in treating cats with sporotrichosis. Methods Twelve samples were collected from frequently touched surfaces and cages of three cats at different treatment stages. Sampling sites included a procedure table, door handle, faucet, ethanol sprayer, an identification card with visible secretions, and cage components. Samples were cultured on Mycosel agar. Fungal growth was assessed through micromorphological analysis. Results Sporothrix was isolated from four samples (33%), one from the stainless‐steel procedure table and the others associated with the cages of two symptomatic cats. Cat #1, with active ulcerative lesions, had positive cultures from both the cage grid and latch. Cat #2, in early treatment and showing respiratory signs, had fungal growth on the cage grid. No contamination was detected from Cat #3's environment, and for the remaining sports swapped. Conclusion These findings suggest that cage surfaces in veterinary settings can be a source of environmental contamination and potential occupational exposure.
