Eculizumab is a C5 complement inhibitor approved by the FDA for the targeted treatment of four rare diseases, paroxysmal nocturnal hemoglobinuria (PNH), atypical hemolytic uremic syndrome (aHUS), generalized myasthenia gravis (gMG), and aquaporin-4 immunoglobulin G-positive optic neuromyelitis optica spectrum disorders (AQP4-IgG+NMOSD). The current study was conducted to assess real-world adverse events (AEs) associated with eculizumab through data mining of the FDA Adverse Event Reporting System (FAERS).
Disproportionality analyses, including Reporting Ratio Ratio (ROR), Proportional Reporting Ratio (PRR), Bayesian Confidence Propagation Neural Network (BCPNN), and Multi-Item Gamma Poisson Shrinker (MGPS) algorithms were used to quantify the signals of eculizumab-associated AEs.
A total of 46,316 eculizumab-related ADEs reports were identified by analyzing 19,418,776 reports in the U.S. Food and Drug Administration Adverse Event Reporting System (FAERS) database. A total of 461 PTs were identified as satisfying by all four algorithms. These PTs reported adverse reactions consistent with the specifications, such as fatigue, nasopharyngitis, meningococcal infection, fever, and anemia. Some PTs, such as aplastic anemia, gene mutation, mastication disorder, kidney fibrosis, BK virus infection, abnormal neutrophil count, C3 glomerulopathy, neuroblastoma, and glomerulonephritis membranoproliferative, were also detected outside the instructions. The median time to onset of eculizumab adverse events was 159 days (interquartile range [IQR] 11∼738 days). In addition, at the PT level, 51 PTs were determined to have an imbalance in the occurrence of ADEs between the sexes.
These findings provide valuable insights into the occurrence of ADEs following the use of eculizumab and could support clinical monitoring and risk identification efforts.

A Matrix of the Functions and Organizations that Ensure Continued Healthcare Services in a Disaster

Purpose: Japan is one of the most natural disaster-prone countries in the world. In the case of a natural disaster or mass casualty incident, a large number of injured people are likely to go to hospitals, which need to provide more services at such times. Thus, it is necessary for various bodies – such as hospitals, municipalities, medical associations, pharmaceutical associations, and trade associations – to collaborate. For example, coordination among multiple hospitals is important when transporting patients to another hospital if they cannot be examined at the first one. There is a pressing need to establish an Area Disaster Resilience Management System for Healthcare (ADRMS-H) to increase medical resilience. In order to develop an ADRMS-H and ensure that continuous healthcare is provided during calamities, it is necessary to clarify the functions (which we have already explained) and coordination that organizations must perform. Since medical care needs change by the hour during disasters, so do the functions that guarantee ongoing healthcare. This study aims to create a matrix of functions ensuring continued healthcare services in a disaster and relevant organizations to understand each one’s role and how they relate to each other.Methodology/Approach: We used the case of Kawaguchi city in Saitama Prefecture, Japan as an example to illustrate related organizations needed to fulfill the functions for ensuring continued healthcare services. Next, we created a matrix of the functions and organizations.Findings: This study identified related organizations such as municipalities, hospitals, medical associations, and trade associations in Kawaguchi city. Since functions for ensuring ongoing healthcare change by the hour, so do the organizations, whose transformations we analyzed by the hour.Originality/Value of paper: The results of this paper can help people understand how related organizations work together during disasters; in light of this, it will be possible to develop a model for an ADRMS-H in terms of coordination among organizations.

Integrated Justice in Pakistan: From Legal Pluralism to Normative Convergence

Legal pluralism, throughout most of developing countries, has been extant since the onset of colonial era. Manifested in a variety of forms, legal pluralism is inherently characterized by both promises as well as limitations. In Pakistan, legal pluralism is epitomized by the prevalence and functioning of parallel systems of justice such as formal courts and Alternate Dispute Resolution Mechanism (ADRM), such as “Jirga. Poor coordination and tenuous enforcement mechanisms, however, render the formal justice system in Pakistan one of poorest performers in terms of judicial efficacy world-wide. This article seeks to explore the possibility of a convergence between traditional and modern models of dispute resolution, i.e. Jirga and court system and the resultant efficacy thereof through devising a conceptual framework. The framework reveals that both formal courts and Jirga demonstrate marked discrepancies concerning their efficacy with respect to the provision of justice and dispute resolution. Findings from the field, however, evince that Jirga stand out to be a relatively more effective mechanism of dispute resolution than formal courts. The conceptual framework, however, implies that by converging both systems it is possible to cope with the limitations of each of the two systems such that while courts could provide legal legitimacy to the Jirga by improving its decency and accountability through regulations, Jirga could enhance the legitimacy of courts by improving its accessibility and transparency through feedback mechanisms. The article concludes by way of arguing that instead of parting ways with each other, both courts and jirga shall seek to go hand in hand in order avoid delays in the provision of justice.

It was on the basis of the obtained results that identify the key prerequisites for the integration of the management of logistics processes, management of the value creation process, and risk management that the methodological basis for the construction of the axiological dimension of the risk management (ADRM) model of logistics processes was determined. By taking into account the contribution of individual concepts to the new research area, its essence was defined as an integrated, structured instrumentation aimed at the identification and implementation of logistics processes supporting creation of the value added as well as the identification and elimination of risk factors disturbing the process of the value creation for internal and external customers. The base for the ADRM concept of logistics processes is the use of the potential being inherent in synergistic effects which are obtained by using prerequisites for the integration of the management of logistics processes, of value creation and risk management as the key determinants of the value creation.
