The Influence of Doctor-Patient Communication on Health Outcomes: A Systematic Review

Hintergrund: Für den Einfluss der Arzt-Patient-Beziehung sowie -kommunikation auf den Gesundheitszustand ist eine weitere empirische Fundierung notwendig. Anhand der Unterscheidung zentraler Bereiche (bspw. Beziehungsqualität, Informationsvermittlung, Patientenedukation) können spezifische Ergebnisse beschrieben werden. Um die empirische Basis dieser Annahmen zu überprüfen haben wir einen systematischen Review durchgeführt. Methoden: In einem systematischen Review der Publikationen in Medline, Embase, Cochrane, PsychLIt und Psyindex zwischen 2000 und 2015 wurden RCTs, kontrollierte und unkontrollierte peer-reviewte Studien mit erwachsenen klinischen Stichproben identifiziert und analysiert. Studien mit psychiatrischen Stichproben, psychologischen oder psychotherapeutischen Interventionen wurden ausgeschlossen. Ergebnisse: Siebzehn RCTs, siebzehn kontrollierte Studien sowie acht qualitative Studien erfüllten alle Einschlusskriterien. Die generelle Studienqualität der RCTs und kontrollierten Studien war zufriedenstellend. 60% der Studien konnten einen positiven Effekt der Arzt-Patient-Beziehung auf den Gesundheitszustand zeigen. Die Rolle von Informationsermittlung sowie -vermittlung (Patientenedukation) wurde am häufigsten untersucht, wobei ein eindeutiger positiver Effekt gefunden wurde. Auch die Kommunikationsfähigkeit führte zu verbesserten Ergebnissen sowie zur verbesserten Unterstützung behandlungsbezogener Verhaltensweisen und Emotionen. Zwei Studien, in denen eine ökonomische Evaluation mit eingeschlossen wurde, zeigten verringerte Kosten für das Gesundheitssystem. Zusammenfassung: Im vorliegenden systematischen Review wurden überzeugende Effekte für unterschiedliche Bereiche der Arzt-Patient-Beziehung sowie Aspekte der Kommunikation auf verschiedene objektive und subjektive Gesundheitsparameter gefunden. Objective: In order to evaluate the influence of the doctor-patient relationship and communication on healthcare outcomes, further reliable results are needed. More specific results can be achieved depending on the differentiation of several domains (e.g., relationship, information gathering, or patient education). We conducted a systematic review including publications from 2000 to 2015 to gather further empirical support. Methods: A systematic review was carried out (with information drawn mainly from Medline, Embase, Cochrane, PsychLit, and Psyndex) to identify RCTs as well as controlled and uncontrolled studies in clinical adult samples. Studies with psychiatric samples and psychologicalpsychotherapeutic interventions were excluded. Results: A total of 17 RCTs, 17 controlled studies, and 8 qualitative studies met the inclusion criteria. The quality of the RCTs and controlled studies was satisfying. In the doctor-patient relationship (building), 60% of the studies showed a positive effect on objective health parameters. Information gathering and provision (patient education) were the most investigated domains with unequivocally positive effects. Communication (skills) showed improved results and enabled treatment-related emotions and behavior. Two studies included an economical evaluation with reduced healthcare costs. Conclusions: A systematic review revealed different domains of the doctor-patient relationship and communication with convincing effects regarding different objective and subjective health outcomes.

Website Based Application of Doctor Selection Classification Derive From Patient Complaints Using the C4.5 Method and K-Nearest Neighbor

This paper is about web based application of doctor selection classification, derive from patient complaints by utilizing the C4.5 method and K-Nearest Neighbor. Classification is the process of grouping things based on classes with the characteristics of similarities and differences. This objects or entities are labeled as user complaints. User complaints are classified and produce a category of doctor that matches the user's complaints. This application will be using sample data from ascertain the opinions and experience of Doctor Yohannes Cahyadi who practices at the Griya Kasih Indah Clinic and from the website alodokter, hellosehat, docdoc.com, cicendoeyehospital, and the eye clinic Nusantara. The purpose of making a doctor selection classification application based on patient complaints is so that the user can find out the doctor's category recommendations in accordance with user complaints input on this application. The test results obtained said that the best classification accuracy is to use the K-Nearest Neighbor method with an accuracy value of 100%, this shows that the use of the C4.5 method and K-Nearest Neighbor can provide recommendations for the right doctor with user complaints.

Horizontal necklines correction with absorbable braided polydioxanone threads: Case series

Introduction The prevalence of horizontal neck lines as a cosmetic concern is widely acknowledged, yet the available treatment options are limited, and no studies have investigated the use of polydioxanone‐barbed threads. These threads, characterized by a finely braided structure, function as a scaffold to attract regenerative factors and facilitate the migration and proliferation of cells. This study aims to evaluate the outcomes of concurrent application of braided polydioxanone‐barbed threads for addressing horizontal neck wrinkles. Methodology A retrospective case series involving four female participants (aged 41, 43, 45, and 46) treated with polydioxanone‐barbed threads for horizontal neck wrinkles between January 2023 and July 2023 was conducted. Adult patients were assessed at an 8‐week follow‐up, revealing a significant reduction in wrinkle intensity based on the Horizontal Neck Wrinkle Severity Scale. Results The analysis of horizontal neck lines demonstrated a notable decrease in wrinkle intensity according to the Horizontal Neck Wrinkle Severity Scale at the 8‐week mark, and this improvement maintained statistical significance. Both patient Global Aesthetic Improvement Scale (GAIS) scores (90%–100%) and physician GAIS scores (100%) were rated as excellent. Conclusion The subdermal application of polydioxanone‐barbed threads for horizontal neck lines proves to be a secure and efficacious approach for treating horizontal neck wrinkles, with no observed Tyndall effect. This technique shows promise for rejuvenating the skin in the horizontal neckline region.

Associations between psychosocial work factors and provider mental well-being in emergency departments

Emergency departments (ED) are complex and dynamic work environments with various psychosocial work stressors that increase risks for providers' well-being. Yet, no systematic review is available which synthesizes the current research base as well as quantitatively aggregates data on associations between ED work factors and provider well-being outcomes.
We aimed at synthesizing the current research base on quantitative associations between psychosocial work factors (classified into patient-/ task-related, organizational, and social factors) and mental well-being of ED providers (classified into positive well-being outcomes, affective symptoms and negative psychological functioning, cognitive-behavioural outcomes, and psychosomatic health complaints).
A systematic literature search in eight databases was conducted in December 2017. Original studies were extracted following a stepwise procedure and predefined inclusion criteria. A standardized assessment of methodological quality and risk of bias was conducted for each study with the Quality Assessment Tool for Quantitative Studies from the Effective Public Health Practice Project. In addition to a systematic compilation of included studies, frequency and strength of quantitative associations were synthesized by means of harvest plots. Subgroup analyses for ED physicians and nurses were conducted.
N = 1956 records were retrieved. After removal of duplicates, 1473 records were screened for titles and abstracts. 199 studies were eligible for full-text review. Finally, 39 original studies were included whereof 37 reported cross-sectional surveys. Concerning the methodological quality of included studies, the majority was evaluated as weak to moderate with considerable risk of bias. Most frequently surveyed provider outcomes were affective symptoms (e.g., burnout) and positive well-being outcomes (e.g., job satisfaction). 367 univariate associations and 370 multivariate associations were extracted with the majority being weak to moderate. Strong associations were mostly reported for social and organizational work factors.
To the best of our knowledge, this review is the first to provide a quantitative summary of the research base on associations of psychosocial ED work factors and provider well-being. Conclusive results reveal that peer support, well-designed organizational structures, and employee reward systems balance the negative impact of adverse work factors on ED providers' well-being. This review identifies avenues for future research in this field including methodological advances by using quasi-experimental and prospective designs, representative samples, and adequate confounder control.

Early diagnosis of necrotizing fasciitis: Imaging techniques and their combined application

Necrotizing fasciitis is a clinical, surgical emergency characterized by an insidious onset, rapid progression, and a high mortality rate. The disease's mortality rate has remained high for many years, mainly because of its atypical clinical presentation, which prevents many cases from being diagnosed early and accurately, resulting in patients who may die from uncontrollable septic shock and multi‐organ failure. But unfortunately, no diagnostic indicator can provide a certain early diagnosis of NF, and clinical judgement of NF is still based on the results of various ancillary tests combined with the patient's medical history, clinical manifestations, and the physician's experience. This review provides a brief overview of the epidemiological features of NF and then discusses the most important laboratory indicators and scoring systems currently employed to diagnose NF. Finally, the latest progress of several imaging techniques in the early diagnosis of NF and their combined application with other diagnostic indices are highlighted. We point out promising research directions based on an objective evaluation of the advantages and shortcomings of different methods, which provide a basis for further improving the early diagnosis of NF.

Current practices and perceived effectiveness of polynucleotides for treatment of facial erythema by cosmetic physicians

Background Polynucleotides (PN) are increasingly used for the treatment of facial erythema in the Republic of Korea. However, there are limited pre‐clinical and clinical data on the efficacy of polynucleotides for facial erythema. In this study, we investigated the current practice and perceived effectiveness of polynucleotide treatment for facial erythema among cosmetic physicians. Methods By conducting a survey among clinicians who use PN in clinical practice, we explored the current practices and assessed the perceived effectiveness of polynucleotides in treating facial erythema. Results A total of 557 physicians who used polynucleotides for facial erythema participated in the survey. Polynucleotides were used by 84.4%, 66.4%, and 47.4% of physicians for facial erythema caused by inflammatory facial dermatosis, repeated laser/microneedle radiofrequency, and steroid overuse, respectively. Among those users, 88.1%, 90%, and 83.7% respectively in those same categories answered that polynucleotides were “highly effective” or “effective.” Furthermore, they agreed that polynucleotides have the following properties: wound healing/regeneration (95.8%), protection of skin barrier (92.2%), hydration (90.5%), vascular stabilization (81.0%), and anti‐inflammation (79.5%). Conclusion Our findings showed that cosmetic physicians in the Republic of Korea have used PN as a part of combination treatment for facial erythema resulting from inflammatory facial dermatosis and repeated laser/ microneedle radiofrequency, rather than from steroid overuse. Also, most clinicians agreed that PN was effective for treatment of facial erythema. Given the lack of pre‐clinical and clinical trial evidence, the empirical responses of practicing physicians provide useful information to guide clinical practice and further research.

Evaluation of eyebrow position and upper eyelid laxity after endolift laser treatment

The periorbital area is one of the most sensitive areas in the face, and many techniques are used to change the eyelid laxity and position of the eyebrows. Recently the use of noninvasive or minimally invasive techniques is developed so, the amount of surgical procedures has decreased. In the present study. We evaluate the effect of Endolift laser as a non‐invasive method on upper eyelid laxity and eyebrow position. Nine patients underwent Endolift laser and evaluated for eyelid laxity and change in eyebrow position. Two blinded physicians assessed the cosmetic outcome of improvement in upper eyelid tightening via photography 6 months post‐treatment. The physicians evaluated the patient's improvement degree using the photographs by two clinical factors: skin laxity and total cosmetic result. Also, eyebrow height from the center of the pupil (CPEBH), Central eyebrow height (CEBH), Lateral eyebrow height (LEBH), Medial eyebrow height (MEBH) were measured by ImagJ before and 6 months post Endolift laser treatment. Our results showed eyelid laxity and eyebrow height were significantly changed after treatment.

A systematic review of health care workers' knowledge and related factors towards burn first aid

The present systematic review was conducted to investigate the knowledge of health care workers (HCWs) regarding first aid in burns. A comprehensive, systematic search was performed in different international electronic databases, such as Scopus, PubMed, Web of Science, and Persian electronic databases such as Iranmedex, and Scientific Information Database using keywords extracted from Medical Subject Headings such as ‘Knowledge’, ‘First aid’, ‘Health personnel’ and ‘Burns’ from the earliest to 1 February 2023. The appraisal tool for cross‐sectional studies (AXIS tool) evaluates the quality of the included studies. A total of 3213 HCWs participated in seven cross‐sectional studies. Of the HCWs, 44.50% were physicians. The studies included in this systematic review were conducted in Saudi Arabia, Australia, Turkey, the UK, Ukraine, and Vietnam. The knowledge rate of HCWs related to first aid for burns was 64.78 out of 100, which indicates their relatively desirable knowledge. The factors of first aid training experience, age, and experienced burn traumas had a significant positive effect on the knowledge of HCWs related to first aid for burns. Also, factors such as gender, nationality, marital status, and job position had a significant relationship with the knowledge of HCWs about first aid for burns. Therefore, it is suggested that health care managers and policymakers implement training programs and practical workshops related to first aid, especially first aid for burns.
