MGB-OAGB: Effect of Biliopancreatic Limb Length on Nutritional Deficiency, Weight Loss, and Comorbidity Resolution
IntroductionMini gastric bypass-one anastomosis gastric bypass (MGB-OAGB) has been approved as a mainstream metabolic/bariatric procedure by IFSO. Still there are lots of concerns regarding nutritional deficiency after MGB-OAGB. The purpose of this retrospective analysis is to evaluate the effect of biliopancreatic limb (BPL) length on weight loss, comorbidity resolution, and nutritional deficiencies in patients 1 year after MGB-OAGB and to find suitable BPL length.Material and MethodsOne hundred and one patients who underwent MGB-OAGB were divided into three groups of 150 cm, 180 cm, and 250 cm depending on the length of BPL bypassed. The nutritional parameters (vitamin D3, vitamin B12, serum iron, serum ferritin, total protein, serum albumin, serum globulin), anthropometric measurements (weight, BMI), and comorbidity resolution (T2DM, hypertension) were compared between the three groups at 1-year follow-up.ResultsThere was statistically significant difference in number of patients having deficiencies in all the nutritional parameters except globulin between 150 cm and 250 cm groups (P < 0.05). While on comparing 180- and 250-cm group, a statistically significant difference was present in vitamin D3, vitamin B12, and total protein (P < 0.05) only. The difference was statistically insignificant between the three groups on T2DM, hypertension resolution, and %EWL but TWL between 150 cm vs 180 cm and 150 cm vs 250 cm showed significant difference.ConclusionA 150-cm BPL length is adequate with very minimal nutritional complications and good results. A 180-cm BPL can be used in super obese while a 250-cm BPL should be used with utmost care as it results in significant nutritional deficiencies.
Long Biliopancreatic Limb Roux-En-Y Gastric Bypass Versus One-Anastomosis Gastric Bypass: a Randomized Controlled Study
Background Roux-en-Y gastric bypass (RYGB) is the gold standard in bariatric surgery. The one-anastomosis gastric bypass (OAGB) procedure, first introduced by Dr. Rutledge, has demonstrated a 25% greater weight loss efficiency than the traditional Roux-en-Y gastric bypass (RYGB) procedure due to the substantially longer biliopancreatic limb (BPL). Aim of the study The current work aimed to compare the outcomes of OAGB and long BPL RYGB regarding weight loss and comorbidity resolution. Patients and methods This randomized controlled trial was done at our institution between September 2019 and January 2021. Patients who were candidates for bariatric surgery were randomly and equally allocated to two groups. Group A underwent OAGB, while group B underwent long BPL RYGB. Patients were followed up for 6 months postoperatively. Results This study included 62 patients equally allocated to OAGB or long BPL RYGB, with no dropouts during follow-up. At 6 months, there was no statistically significant difference between the two groups regarding postoperative BMI ( P = 0.313) and the EBWL ( P = 0.238). There was comparable remission of diabetes mellitus ( P = 0.708), hypertension ( P = 0.999), OSA ( P = 0.999), joint pain ( P = 0.999), and low back pain ( P = 0.999). Seven patients in the OAGB group experienced reflux symptoms ( P = 0.011), which were managed by proton pump inhibitors. Conclusion Extending the BPL in RYGB provides weight loss and comorbidity remission comparable to that of OAGB. Some OAGB-related reflux cases remain a concern. However, they were sufficiently controlled with PPIs. Due to OAGB superior technical simplicity, long BPL RYGB should be preserved for cases whom are more risky for bile reflux. Graphical Abstract
Upgrading the Power Grid Functionalities with Broadband Power Line Communications: Basis, Applications, Current Trends and Challenges
This article reviews the basis and the main aspects of the recent evolution of Broadband Power Line Communications (BB-PLC or, more commonly, BPL) technologies. The article starts describing the organizations and alliances involved in the development and evolution of BPL systems, as well as the standardization institutions working on PLC technologies. Then, a short description of the technical foundation of the recent proposed technologies and a comparison of the main specifications are presented; the regulatory activities related to the limits of emissions and immunity are also addressed. Finally, some representative applications of BPL and some selected use cases enabled by these technologies are summarized, together with the main challenges to be faced.
Integrating Wi-Fi, Li-Fi, and BPL Technologies for a Secure Indoor Communication System
In today’s digital age, there is an increasing demand for integrated wireless and wired technologies; however, there is a difficulty in achieving secure and reliable communications within buildings and facilities. This paper presents a proposal for maintaining the infrastructure while expanding it to implement communication technologies with high transmission and reception speeds and high levels of data confidentiality to enhance the operational efficiency of organizations. Three main technologies have emerged as promising solutions for this purpose: Wi-Fi, Li-Fi, and BPL. Despite the advantages that each technology offers, some drawbacks appear in these technologies that affect data transmission. Wi-Fi, Li-Fi, and BPL can be combined to achieve maximum security and reduce noise and interference via the ESP8266 module. This combination could be an important step toward achieving an integrated and secure indoor communication system. The paper provides a comprehensive overview of the three techniques and how they are applied in practice. In addition, OSE, ABPF, and ASBF filters are used to detect and eliminate interference and attack secure internal networks.
A Longer Biliopancreatic Limb and Shorter Common Channel Enhance Weight Loss But May Have Harmful Effects in Mouse Models of Roux-en-Y Gastric Bypass
Background RYGB consists of the Roux limb (RL), the biliopancreatic limb (BPL), and the common channel (CC). There is no consensus on the optimal limb lengths. Methods Using a mouse model of RYGB, 30 diet-induced obese mice were divided into two groups with varying BPL and CC lengths: a standard BPL with a long CC (RYGB S) and a long BPL with a short CC (RYGB L). Additionally, 9 age-matched, lean control mice (LC) were also included in this study. Results RYGB S had limb lengths of RL = 17%, BPL = 24%, and CC = 59%. RYGB L had limb lengths of RL = 17%, BPL = 32%, and CC = 51%. RYGB S and RYGB L had 67% and 40% survival, respectively. Mortality in RYGB L included more instances where the cause of death was not apparent. RYGB L demonstrated greater weight loss, lower energy expenditure, and lower heart mass as compared to RYGB S. Both RYGB groups had lower epidydimal fat mass, spleen mass, and bone mineral density compared to LC. RYGB L had a lower heart mass than RYGB S and LC. While the relative abundance of Eubacterium was lower in RYGB L than in RYGB S, no other gut microbiota differences were observed. Conclusions A longer BPL with a shorter CC induces greater weight loss but may lead to adverse effects, including lower heart mass, reduced bone density, and deaths with unclear causes. Detection of sub-centimeter lesions using digital TOF-PET/CT system combined with Bayesian penalized likelihood reconstruction algorithm
ObjectiveMany advances in PET/CT technology can potentially improve image quality and the ability to detect small lesions. A new digital TOF-PET/CT scanner based on silicon photomultipliers (SiPM) integrated with a Bayesian penalized likelihood (BPL) PET reconstruction algorithm (Q.Clear; GE Healthcare) has been introduced into clinical practice. The present study aimed to quantify the ability of a digital TOF-PET/CT scanner combined with BPL reconstruction to detect small lesions, and to determine the optimal penalization factor (β) in BPL to accurately detect such lesions.MethodsAll PET data were acquired from a NEMA body phantom using a Discovery MI (DMI) PET/CT system (GE Healthcare). The phantom included six spheres with diameters of 4, 5, 6, 8, 10, and 13 mm, and contained a background activity level of 5.3 kBq/mL, with target-to-background ratios (TBR) of 4:1 and 8:1. Images were reconstructed using a baseline OSEM algorithm, with OSEM + PSF, OSEM + TOF, OSEM + PSF + TOF, and BPL + PSF + TOF (β: 50–400). The matrix size was 192 × 192 and 384 × 384. Data acquired in 100-min list mode were re-binned into acquisition times ranging from 2 to 100 min. The quantitative accuracy and detectability of small hot spheres were evaluated by physical assessment of a recovery coefficient (RC) and a detectability index (DI), as well as visual assessment of PET images at each acquisition time.ResultsThe RC and DI of sub-centimeter spheres were improved, because the digital TOF-PET/CT scanner has a larger TOF performance gain due to better timing resolution. The RC and DI were higher with BPL in sub-centimeter spheres, than with other OSEM-based types of reconstruction. The BPL for an 8-mm sphere overestimated uptake due to edge artifact overshoot induced by PSF modeling. The variability of RC and DI for acquisition times and TBR differed considerably according to β values. The RC for ~ 8-mm spheres were > 1 at β values between 50 and 100, but were close to 1 at β value of 200. The visual scores for β = 200 in BPL were maximal, whereas those for spheres that were ≥ 6 mm exceeded the criterion of 3.ConclusionThe BPL in the digital TOF-PET/CT scanner improved the quantitation and detectability of sub-centimeter spheres compared with OSEM-based reconstruction. Optimization of the β value in BPL might allow the detection of lesions ≤ 6 mm, although detectability depended on the TBR of lesions. A β value of 200 seemed optimal for detecting sub-centimeter lesions.
One Anastomosis Gastric Bypass–Mini Gastric Bypass with Tailored Biliopancreatic Limb Length Formula Relative to Small Bowel Length: Preliminary Results
BackgroundOne Anastomosis Gastric Bypass–Mini Gastric Bypass (OAGB-MGB) is rapidly gaining popularity and is currently being performed by an increasing number of bariatric surgeons worldwide. However, excessive postoperative weight loss and malnutrition still remain a major concern regarding this procedure. The aim of this observational retrospective study was to investigate whether a tailored biliopancreatic limb (BPL) length relative to small bowel length (SBL) is superior to a fixed BPL length of 200 cm in terms of weight loss results and nutritional deficiencies in morbidly obese patients 1 year following OAGB-MGB.Materials and MethodsSixty-four patients who underwent OAGB-MGB were divided into two consecutive groups depending on the BPL length used: fixed 200-cm BPL and tailored BPL groups. Anthropometric measurements (%EWL, TWL, %TWL) and nutritional parameters (vitamin A, vitamin D3, vitamin B12, serum iron, serum albumin, total protein) were compared between the two groups at 1-year follow-up.ResultsNo statistically significant differences were observed between the patients in two groups in terms of %EWL, TWL, %TWL. The number of patients with deficiencies of vitamin A (p = 0.030), vitamin D3 (p = 0.020), and albumin (p = 0.030) was significantly higher in fixed 200-cm BPL group as compared with tailored BPL group, 1 year following OAGB-MGB. No statistically significant differences were seen between the patients in two groups in terms of vitamin B12, iron, and total protein deficiencies.ConclusionTailoring BPL length by bypassing about 40% of the SBL seems to be safe and effective. According to preliminary results of this study, a tailored BPL length relative to SBL is even likely to be superior to the fixed 200-cm BPL as it is associated with less nutritional deficiencies while providing similar weight loss results. Further randomized studies with larger sample sizes and longer follow-up periods are necessary to confirm the primary results of this study.
Factors affecting biochemical pregnancy loss (BPL) in preimplantation genetic testing for aneuploidy (PGT-A) cycles: machine learning-assisted identification
Purpose To determine the factors influencing the likelihood of biochemical pregnancy loss (BPL) after transfer of a euploid embryo from preimplantation genetic testing for aneuploidy (PGT-A) cycles. Methods The study employed an observational, retrospective cohort design, encompassing 6020 embryos from 2879 PGT-A cycles conducted between February 2013 and September 2021. Trophectoderm biopsies in day 5 (D5) or day 6 (D6) blastocysts were analyzed by next generation sequencing (NGS). Only single embryo transfers (SET) were considered, totaling 1161 transfers. Of these, 49.9% resulted in positive pregnancy tests, with 18.3% experiencing BPL. To establish a predictive model for BPL, both classical statistical methods and five different supervised classification machine learning algorithms were used. A total of forty-seven factors were incorporated as predictor variables in the machine learning models. Results Throughout the optimization process for each model, various performance metrics were computed. Random Forest model emerged as the best model, boasting the highest area under the ROC curve (AUC) value of 0.913, alongside an accuracy of 0.830, positive predictive value of 0.857, and negative predictive value of 0.807. For the selected model, SHAP (SHapley Additive exPlanations) values were determined for each of the variables to establish which had the best predictive ability. Notably, variables pertaining to embryo biopsy demonstrated the greatest predictive capacity, followed by factors associated with ovarian stimulation (COS), maternal age, and paternal age. Conclusions The Random Forest model had a higher predictive power for identifying BPL occurrences in PGT-A cycles. Specifically, variables associated with the embryo biopsy procedure (biopsy day, number of biopsied embryos, and number of biopsied cells) and ovarian stimulation (number of oocytes retrieved and duration of stimulation), exhibited the strongest predictive power.
Evaluation of Stability, Inactivation, and Disinfection Effectiveness of Mpox Virus
Background: Mpox virus (MPXV) infections have increased in many countries since May 2022, increasing demand for diagnostic tests and research on the virus. To ensure personnel safety, appropriate and reliable measures are needed to disinfect and inactivate infectious samples; Methods: We evaluated the stability of infectious MPXV cultures stored at different temperatures and through freeze–thaw cycles. Heat physical treatment (56 °C, 70 °C, 95 °C), chemical treatment (beta-propiolactone (BPL)) and two commercialized disinfectants (Micro-Chem Plus (MCP) and ethanol) were tested against infectious MPXV cultures; Results: The results indicated that MPXV stability increases with lower temperatures. The MPXV titer was stable within three freeze–thaw cycles and only decreased by 1.04 log10 (lg) 50% cell culture infective dose (CCID50) per milliliter (12.44%) after twelve cycles. MPXV could be effectively inactivated at 56 °C for 40 min, 70 °C for 10 min, and 95 °C for 5 min. For BPL inactivation, a 1:1000 volume ratio (BPL:virus) could also effectively inactivate MPXV. A total of 2% or 5% MCP and 75% ethanol treated with MPXV for at least 1 min could reduce >4.25 lg; Conclusions: MPXV shows high stability to temperature and freeze–thaw. Heat and BPL treatments are effective for the inactivation of MPXV, while MCP and ethanol are effective for disinfection, which could help laboratory staff operate the MPXV under safer conditions and improve operational protocols.
Green Synthesis of BPL-NiONPs Using Leaf Extract of Berberis pachyacantha: Characterization and Multiple In Vitro Biological Applications
An eco-friendly biogenic method for the synthesis of nickel oxide nanoparticles (NiONPs) using phytochemically rich Berberis pachyacantha leaf extract (BPL) was established. To achieve this purpose, 80 mL of BPL extract was used as a suitable reducing and capping agent for the synthesis of NiONPs. The synthesis of BPL-based nickel oxide nanoparticles (BPL@NiONPs) was confirmed using different microscopic and spectroscopic techniques: UV Visible spectroscopy (UV-Vis), Fourier-transform infrared spectroscopy (FTIR), X-ray diffraction (XRD), energy dispersive X-ray (EDX), dynamic light scattering (DLS) and scanning electron microscopy (SEM) analysis. Spectroscopically, BPL-NiONPs was found with a pure elemental composition (oxygen and nickel), average size (22.53 nm) and rhombohedral structure with multiple functional groups (-OH group and Ni-O formation) on their surface. In the next step, the BPL extract and BPL@NiONPs were further investigated for various biological activities. As compared to BPL extract, BPL@NiONPs exhibited strong biological activities. BPL@NiONPs showed remarkable antioxidant activities in terms of 2,2-diphenyl-1-picrylhydrazyl (76.08%) and total antioxidant capacity (68.74%). Antibacterial action was found against Pseudomonas aeruginosa (27 mm), Staphylococcus aureus (20 mm) and Escherichia coli (19.67 mm) at 500 µg/mL. While antifungal potentials were shown against Alternaria alternata (81.25%), Fusarium oxysporum (42.86%) and Aspergillus niger (42%) at 1000 µg/mL. Similarly, dose-dependent cytotoxicity response was confirmed against brine shrimp with IC50 value (45.08 µg/mL). Additionally, BPL@NiONPs exhibited stimulatory efficacy by enhancing seed germination rate at low concentrations (31.25 and 62.5 µg/mL). In conclusion, this study depicted that BPL extract has important phytochemicals with remarkable antioxidant activities, which successfully reduced and stabilized the BPL@NiONPs. The overall result of this study suggested that BPL@NiONPs could be used as nanomedicines and nanofertilizers in biomedical and agrarian fields.
