Problem-Focused Psychodynamic Psychotherapy

For many years, the general rule of thumb in psychoanalysis and psychodynamic psychotherapy has been to refrain from focusing too much on specific symptoms or problems so as to not interfere with free association or the effectiveness of the psychoanalytic approach.
When Fredric Busch, M.D., and colleagues developed panic-focused psychodynamic therapy in the 1990's and subsequently psychodynamic approaches to depression, they emphasized the value of more active interventions, a focus on symptoms and associated dynamics, and occasional psychoeducation.
In this new volume, he expands the scope of that work, articulating how a focused psychodynamic psychotherapeutic approach can be adapted for patients in general. Rather than one specific aspect of patients' difficulties, Problem Focused Psychodynamic Psychotherapy (PrFPP) focuses on the set of problems (e.g., symptoms, relationship issues, behavioral difficulties) a particular patient brings into the consulting room.
Through numerous tables and a wealth of case vignettes, this guide provides novice and experienced clinicians alike with a general template for working with patients to identify and address the overlapping and unique dynamics of various problems.
It describes how to use psychodynamic exploratory techniques to make problem lists and examine the context and emotions surrounding each issue. It also discusses how to develop a psychodynamic formulation to provide a framework for identifying and addressing the dynamic contributors to the various problems.
Therapist and patient can then undertake the \"working through\" process to identify how specific dynamics emerge in different contexts and overlap in contributing to problems.
All of these approaches help spur patients' self-reflective capacities and the identification of their own dynamics—making it possible to more rapidly address core difficulties. The work also enables the continued use of these modes of managing problems after the treatment is completed.
And because PrFPP is suitable for short- and longer-term interventions, it is valuable for patients who either cannot commit to long-term treatment or only have access to brief interventions.
Control of measles in a juvenile custodial setting in the wake of the recent US outbreak

The recent US measles outbreak is the largest since 1992. It is just a matter of time before measles is introduced into a juvenile custodial setting. Are we prepared? Should we be prepared? This short article addresses steps institutional settings should take to prevent the spread of measles in a contained setting.

Isolation and characterisation of Klebsiella pneumoniae bacteriophages vB_(K)pn_(L)illa1 and vB_(K)pn_(L)illa2

Klebsiella pneumoniae is an opportunistic pathogen that is commonly associated with pulmonary infections. It is of particular concern due to its increasing resistance to antibiotics, particularly carbapenem. Alternative treatments for these bacterial infections are required, and phage application is an attractive option. In this study, we isolated two novel Klebsiella pneumonia bacteriophages from wastewater, vB_(K)pn_(L)illa1 and vB_(K)pn_(L)illa2, both of which have lytic activity against Klebsiella oxytoca and Klebsiella michiganensis. Genome sequencing and phylogenetic analysis revealed that both phages belong to the family Straboviridae, with vB_(K)pn_(L)illa1 belonging to the genus Jiaodavirus and vB_(K)pn_(L)illa2 belonging to the genus Slopekvirus. Growth kinetics showed an effective reduction of K. pneumoniae populations when infected with either phage.

Depletion of the Nb CORE receptor drastically improves agroinfiltration productivity in older Nicotiana benthamiana plants

Nicotiana benthamiana is increasingly used for transient gene expression to produce antibodies, vaccines, and other pharmaceutical proteins but transient gene expression is low in fully developed, 6–8‐week old plants. This low gene expression is thought to be caused by the perception of the cold shock protein (CSP) of Agrobacterium tumefaciens . The CSP receptor is contested because both Nb CSPR and Nb CORE have been claimed to perceive CSP. Here, we demonstrate that CSP perception is abolished in 6‐week‐old plants silenced for Nb CORE but not Nb CSPR. Importantly, older Nb CORE‐silenced plants support a highly increased level of GFP fluorescence and protein upon agroinfiltration. The drastic increase in transient protein production in Nb CORE‐depleted plants offers new opportunities for molecular farming, where older plants with larger biomass can now be used for efficient protein expression.

JNK signaling dominance in hyperthermia

Hyperthermia is a promising anticancer treatment that induces heat stress, stimulating various signal transduction pathways to maintain cellular homeostasis. Mitogen-activated protein kinases (MAPKs) link various extracellular stimuli with cytoplasmic and nuclear mediators through a three-tiered cascade of kinases, including MAPKs, MAP2Ks, and MAP3Ks. In mammals, three major groups of MAPKs have been characterized: extracellular signalregulated protein kinases (ERK), p38 MAPKs, and c-Jun NH₂-terminal kinases (JNK). Each group of MAPKs is heatactivated and exhibits distinct biological functions. However, the differences and advantages of the regulation of each MAPK with temperature changes remain unknown. Our results demonstrated that JNK was activated in a temperature-dependent manner, with degradation of the JNK phosphatases despite transient phosphorylation of ERK with induction of the ERK phosphatases. This brief insight deepens our current understanding of the deregulation of the ERK and JNK cascades in hyperthermia.
