\"This book aims to highlight the gaps and the transparency issues in the clinical research and trials processes and how there is a lack of information flowing back to researchers and patients involved in those trials. Lack of data transparency is an underlying theme within the clinical research world and causes issues of corruption, fraud, errors and a problem of reproducibility. The benefits and drawbacks of various technologies, as well as the scope of their application, are thoroughly discussed. The use of anonymity technology and differential privacy in data collection can aid in the prevention of attacks based on background knowledge gleaned from data integration and fusion. The majority of medical big data are stored on a cloud computing platform during the storage stage. To ensure the confidentiality and integrity of the information stored, encryption and auditing procedures are frequently used. Access control mechanisms are mostly used during the data sharing stage to regulate the objects that have access to the data. The privacy protection of medical and health big data is carried out under the supervision of machine learning during the data analysis stage. Finally, acceptable ideas are put forward from the management level as a result of the general privacy protection concerns that exist throughout the life cycle of medical big data throughout the industry.

Coronavirus disease 2019 and sudden sensorineural hearing loss

Severe acute respiratory syndrome coronavirus 2 emerged in December 2019 in Wuhan, China, and was declared a global health emergency of international concern by the World Health Organization on 30th January 2020. It has resulted in almost 600 000 deaths to date worldwide. Sudden sensorineural hearing loss is a known complication of a number of viral infections, but there is little in the literature to date on its association with coronavirus disease 2019.
This paper presents the case of a 30-year-old female staff nurse who contracted coronavirus disease 2019 and presented to our department with a significant unilateral sensorineural hearing loss confirmed on audiogram. She was treated with a course of oral steroids, but unfortunately there was no improvement in her hearing.
This case report is important as it highlights the importance of having a low index of suspicion when patients present with a variety of symptoms not previously associated with coronavirus disease 2019. The paper also discusses the controversy surrounding the use of steroids in the management of this disease.

Olfactory epithelium histopathological findings in long-term coronavirus disease 2019 related anosmia

Olfactory dysfunction represents one of the most frequent symptoms of coronavirus disease 2019, affecting about 70 per cent of patients. However, the pathogenesis of the olfactory dysfunction in coronavirus disease 2019 has not yet been elucidated.
This report presents the radiological and histopathological findings of a patient who presented with anosmia persisting for more than three months after infection with severe acute respiratory syndrome coronavirus-2.
The biopsy demonstrated significant disruption of the olfactory epithelium. This shifts the focus away from invasion of the olfactory bulb and encourages further studies of treatments targeted at the surface epithelium.

Treatment of auricular pseudocysts using enhanced negative drainage: a prospective study of 21 cases
Auricular pseudocysts are rare, painless, benign intracartilaginous cysts of the auricle that are not lined by epithelium and have no known aetiology.
This was a prospective study conducted in an ENT department from January 2020 to June 2022. In 21 patients, complete aspiration of the pseudocyst with enhanced negative drainage was performed. They were followed for a minimum of six months.
All patients completely responded to the negative drainage treatment. No cases of recurrence or obvious deformities were observed.
Aspiration with intensified negative drainage was associated with a positive response in patients with auricular pseudocysts. Complete resolution of the swelling can be achieved without any serious complications. Thus, it appears to be a simple and effective method for managing the condition.

Delayed extensive surgical emphysema after Valsalva manoeuvre post Eustachian tuboplasty

ObjectiveThis paper reports a case of balloon Eustachian tuboplasty with delayed presentation of extensive surgical emphysema.MethodsThis is a clinical case report with a review of literature.ResultsEustachian tube dysfunction is a functional disorder that results in inadequate middle-ear ventilation, causing aural fullness and tinnitus. A 36-year-old male presented with the sudden onset of an isolated, painful, left-sided neck swelling. The patient underwent balloon Eustachian tuboplasty, which was uneventful, but subsequently developed a sudden onset of isolated left-sided neck swelling on the 5th post-operative day during Valsalva manoeuvre. Neck examination revealed extensive crepitus on the left side of the neck. Examination findings were confirmed by imaging. The patient was conservatively managed and subsequently discharged home.ConclusionBalloon Eustachian tuboplasty is a safe procedure; however, extra care must be taken to avoid possible complications. Patients should be counselled against Valsalva manoeuvre and heavy weightlifting. They also should be instructed to sneeze with an open mouth and consider the use of stool softeners.

Predicting a patient’s future health state through the analysis of their clinical records is an emerging area in the field of intelligent medicine. It has the potential to assist healthcare professionals in prescribing treatments safely, making more accurate diagnoses, and improving patient care. However, clinical notes have been underutilized due to their complexity, high dimensionality, and sparsity. Nevertheless, these clinical records hold significant promise for enhancing clinical decision. To tackle these problems, a novel feedback attention–based bidirectional long short-term memory (FABiLSTM) model has been proposed for more effective diagnosis using clinical records. This model incorporates PubMedBERT for filtering irrelevant information, enhances global vector word embeddings for numerical representations and K-means clustering, and performs to explore term frequency and inverse document frequency intricacies. The proposed approach excels in capturing information, aiding accurate disease prediction. The predictive capability is further enhanced with the help of a billiards-inspired optimization algorithm. The effectiveness of the FABiLSTM method has been assessed with the MIMIC-III dataset, yielding impressive results in accuracy, precision, F1 score, and recall score of 98.52%, 98%, 98.2%, and 98.2% individually. These results reveal ways in which the proposed technique excels in comparison with current practices.
