Posterior corneal features in patients with Down syndrome and their relation with keratoconus

AimsTo characterise posterior corneal surface features in patients with Down syndrome (DS) and to compare them with healthy and mild keratoconus corneas.MethodsThis restrospective, comparative, non-randomised, clinical study included 123 eyes, divided into three groups (37 eyes of patients with DS, 46 with mild keratoconus and 40 controls), and took place at Vissum Alicante. Only patients with no previous ocular surgery, no corneal scars and no active ocular disease other than keratoconus were included. The Sirius System topographer (CSO, Firenze, Italy) was used in order to analyse posterior corneal surface keratometry, shape and keratoconus screening indices, posterior corneal aberrations, corneal volume and pachymetry.ResultsPatients with DS, when compared with healthy controls, have a steeper (mean keratometry 7 mm (KM): −6.30±0.44 vs −6.15±0.22; p<0.05) and more irregular (root mean square per unit of area: 4.5 mm 0.22±0.22 vs 0.09±0.03, p<0.001; posterior vertex of the ectatic area: 33.22±44.29 vs 10.63±2.88, p<0.001) posterior corneal surface, with higher aberrations (high-order aberrations (HOAs): 1.07±1.43 vs 0.15±0.06, p<0.001; coma-like: 0.88±1.09 vs 0.13±0.07, p<0.001) and thinnest pachymetry (497.68±26.88 vs 538.95±31.67, p<0.001). At the same time, no statistically significant difference was found between patients with DS and patients with mild keratoconus (p>0.05) in KM (−6.38±0.34), HOA (0.56±0.36), coma-like (0.51±0.34) and pachymetry (500.56±36.83).ConclusionsPosterior corneal surface of patients with DS is steeper, more irregular and shows more higher order aberrations, as well as reduced volume and thinner pachymetry than patients with healthy corneas. Additionally, posterior corneal surface in patients with DS shows similar characteristics to those found in mild keratoconus.

Background
To compare central corneal thickness (CCT) values measured by three different devices: slit-scanning topography (SST), ultrasonic pachymetry (UP), and optical coherence tomography (OCT) in diabetic eyes and compare the CCT values in patients with and without diabetic retinopathy.
Methods
Ninety-six patients with diabetes mellitus were included in this prospective study and divided into two groups according to the presence of diabetic retinopathy, as Group I with retinopathy and Group II without. The CCT of 96 eyes was measured by three different devices; SST (Orbscan II), UP and OCT. The results of CCT measurements with three different devices were compared. Also, the intergroup differences in CCT measurements were evaluated.
Results
The CCT was statistically insignificantly different between the two groups. Although the three methods of CCT measurements correlated well with each other, SST showed significantly (
p
< 0,0001) higher CCT results compared to both UP and OCT.
Conclusions
According to our results, neither the duration of DM nor the presence of diabetic retinopathy did have a significant effect on the CCT. The CCT values obtained with three devices were all in correlation. However, the results of SST were significantly higher compared to the other two. Our findings emphasize the value anterior segment OCT in CCT measurements, since it is a non-contact method and correlate very well with UP.

Repeatability and agreement of central corneal thickness measurement with non-contact methods: a comparative study

PurposeTo compare the central corneal thickness (CCT) measurements and reliability of RTVue XR-100 anterior segment optical coherence tomography (AS-OCT), AL-scan optical biometer and Schwind Sirius anterior segment analysis system.MethodsThe CCT was measured in one hundred and twenty-seven eyes of 127 healthy subjects with AS-OCT, AL-scan and Sirius system. Mean CCT was compared among the instruments, and the level of agreement was assessed using Bland–Altman plots. One eye each of 30 subjects was randomly assigned for intrasession intraoperator and interoperator repeatability which was assessed using coefficient of variation and intraclass correlation coefficient.ResultsMean CCT with AS-OCT, AL-scan and Sirius system was 496.72 ± 32.75, 507.43 ± 33.54 and 512.08 ± 33.1 µm, respectively. There was no statistically significant difference between AL-scan and Sirius system (p = 0.26). Significant difference was found between AS-OCT/AL-scan (p = 0.01) and AS-OCT/Sirius system (p < 0.0001). Bland–Altman analysis showed a high level of agreement between AL-Scan/Sirius system (Mean difference −4.6 µm) and a low level of agreement between AS-OCT/AL-scan (Mean difference −10.7 µm) and OCT/Sirius system (Mean difference −15.4 µm).ConclusionsAS-OCT underestimated CCT measurements when compared to other two devices in healthy subjects. Hence, one must be cautious when analyzing the results from different machines and should be aware that the measurement values are not interchangeable.

Aim: To study the relative effects of high and low fluidic parameters on endothelial cell density (ECD), central corneal thickness (CCT), and central macular thickness (CMT) after phacoemulsification with torsional ultrasound. Settings and Design: Prospective, randomized clinical trial based on a tertiary eye hospital. Subjects and Methods: The study included 65 patients in each group. Patients were randomized to either the high or the low flow group using a computerized random number table. The study was patient and examiner masked. All patients underwent phacoemulsification with torsional ultrasound. Visual acuity, ECD, CCT, and CMT were measured for all patients preoperatively at 2 weeks and 6 weeks postoperatively. Statistical Analysis Used: The Shapiro-Wilks test was used to assess the normality of the data. Mann-Whitney U-test with the P value set at 0.05 was used to compare the two groups. Results: Cumulative dissipated energy was significantly higher in the low flow group (16.44 ± 9.07 vs. 11.74 ± 6.68; P = 0.002). No statistically significant difference was noted between the two groups in the ECD, CCT, CMT, or corrected distance visual acuity at the end of 6 weeks. Conclusions: No significant difference was noted in the postoperative outcome between high and low flow groups. Parameters can be modified to suit the surgeon's preference, as both high and low flow parameters were found to have comparable postoperative outcomes.

Evaluation of keratoconus progression

To define variables for the evaluation of keratoconus progression and to determine cut-off values.
In this retrospective cohort study (2010-2016), 265 eyes of 165 patients diagnosed with keratoconus underwent two Scheimpflug measurements (Pentacam) that took place 1 year apart ±3 months. Variables used for keratoconus detection were evaluated for progression and a correlation analysis was performed. By logistic regression analysis, a keratoconus progression index (KPI) was defined. Receiver-operating characteristic curve (ROC) analysis was performed and Youden Index calculated to determine cut-off values.
Variables used for keratoconus detection showed a weak correlation with each other (eg, correlation r=0.245 between RPImin and Kmax, p<0.001). Therefore, we used parameters that took several variables into consideration (eg, D-index, index of surface variance, index for height asymmetry, KPI). KPI was defined by logistic regression and consisted of a Pachymin coefficient of -0.78 (p=0.001), a maximum elevation of back surface coefficient of 0.27 and coefficient of corneal curvature at the zone 3 mm away from the thinnest point on the posterior corneal surface of -12.44 (both p<0.001). The two variables with the highest Youden Index in the ROC analysis were D-index and KPI: D-index had a cut-off of 0.4175 (70.6% sensitivity) and Youden Index of 0.606. Cut-off for KPI was -0.78196 (84.7% sensitivity) and a Youden Index of 0.747; both 90% specificity.
Keratoconus progression should be defined by evaluating parameters that consider several corneal changes; we suggest D-index and KPI to detect progression.

The purpose of this study was to compare central corneal thickness, thinnest corneal thickness, and the thinnest point of the cornea between Pentacam and anterior segment optical coherence tomography (ASOCT) in patients with dry eye disease (DED). This cross-sectional study included 195 participants between November 2015-June 2017. DED was diagnosed using the Asia Dry Eye Society criteria and further divided into mild and severe DED based on kerato-conjunctival vital staining. Central corneal thickness, thinnest corneal thickness, and the thinnest point of the cornea measured by Pentacam and ASOCT were compared, and Pearson's correlation coefficients were estimated. The differences in central corneal thickness and the thinnest corneal thickness between Pentacam and ASOCT were analysed using Bland-Altman and multivariate regression analyses adjusted for age and sex. This study included 70 non-DED subjects and 52 patients with mild and 73 with severe DED. The Pentacam and ASOCT measurements of central corneal thickness and thinnest corneal thickness were strongly correlated, but the respective values were higher when measured with Pentacam. The Bland-Altman analysis revealed differences in central corneal thickness (non DED, 11.8; mild DED, 13.2; severe DED, 19.6) and in thinnest corneal thickness (non DED, 13.1; mild DED, 13.4; severe DED, 20.7). After adjusting for age and sex, the differences in central corneal thickness (β = 7.029 μm, 95%CI 2.528-11.530) and thinnest corneal thickness (β = 6.958 μm, 95%CI 0.037-13.879) were significantly increased in the severe-DED group. The distribution of the thinnest point of the cornea in the cornea's inferior temporal quadrant between Pentacam and ASOCT deviated in severe DED (Pentacam: 90.4% vs. ASOCT: 83.6%). Clinicians should consider that there were significant differences in corneal-morphology assessment between the measurements with Pentacam and ASOCT in severe DED.

Assessment of corneal epithelial thickness mapping in epithelial basement membrane dystrophy

To investigate the corneal epithelial thickness topography with optical coherence tomography (OCT) and its relationship with vision quality in epithelial basement membrane dystrophy (EBMD).
45 eyes of EBMD patients, 26 eyes of dry eye (DED) patients and 22 eyes of normal subjects were enrolled. All participants were subjected to 9-mm corneal epithelial mapping with OCT and vision quality was assessed with the optical quality analysis system using the objective scatter index (OSI). Central, superior, inferior, minimum, maximum, and standard deviation of epithelium thickness (Irregularity), were analysed and correlations with the OSI were calculated.
The mean (±SD) central, inferior and maximum epithelial thicknesses of the EBMD patients (respectively, 56.4 (±8.1) μm, 58.9 (±6.4) μm, and 67.1 (±8.3) μm) were thicker compared to DED patients (P<0.05) and normal subjects (P<0.05). We found greater irregularity of epithelial thickness in EBMD (5.1±2.5 μm) compared to DED patients (2.6±1.0 μm) (P = 4.4.10-6) and normal subjects (2.1±0.7 μm) (P = 7.6.10-7). The mean OSI was worse in EBMD patients than in DED patients (P = 0.01) and compared to normal subjects (P = 0.02). The OSI correlated with the epithelial thickness irregularity (Spearman coefficient = 0.54; P = 2.65.10-5).
The OCT pachymetry map demonstrated that EBMD patients had thicker corneal epithelium in the central and inferior region. These changes were correlated with objective measurements of vision quality. This OCT characterisation of the EMBD provides a better understanding of the epithelial behaviour in this dystrophy and its role in vision quality.

Reproducibility, and repeatability of corneal topography measured by Revo NX, Galilei G6 and Casia 2 in normal eyes

To test the repeatability and reproducibility of the topography module in posterior segment spectral domain optical coherence tomography with Revo NX (new device) and to compare keratometry values obtained by a Scheimpflug tomography (Galilei G6) and a swept source OCT (Casia 2).
In this prospective study, healthy subjects with nonoperated eyes had their central corneal thickness (CCT), anterior and posterior K1/K2 corneal power measured with the new device. Two operators made 6 measurements on the new device to check intraobserver repeatability and reproducibility, and measurement on Casia 2 and Galilei G6. Bland-Altman plots were used to assess the agreement between the devices for each analyzed variable.
94 eyes (94 patients) were studied. All devices produced significantly different mean CCT, the highest for Galilei 569.13±37.58 μm followed by Casia 545.00 ±36.15 μm and Revo 537.39±35.92 μm. The mean anterior K1 was 43.21 ± 1.37 for Casia 2 43.21 ± 1.55 for Revo NX and 43.19 ± 1.39 for Galilei G6, and the differences were insignificant p = 0.617. The posterior K1 for Revo NX was -5.77 ± 0.25 whereas for Casia 2 it was -5.98±0.22 and for Galilei G6-6.09±0.28 D p< 0.0001. The Revo NX showed intraclass correlation coefficient ranging from 0.975 for the posterior K2 surface, and 0.994 for anterior K1 and 0.998 for CCT.
Revo NX is independent of the user and offers a high level of repeatability for the anterior and posterior cornea. The wide range of differences between the devices suggests they should not be used interchangeably.

Detection of subclinical keratoconus using a novel combined tomographic and biomechanical model based on an automated decision tree

Early detection of keratoconus is a crucial factor in monitoring its progression and making the decision to perform refractive surgery. The aim of this study was to use the decision tree technique in the classification and prediction of subclinical keratoconus (SKC). A total of 194 eyes (including 105 normal eyes and 89 with SKC) were included in the double-center retrospective study. Data were separately used for training and validation databases. The baseline variables were derived from tomography and biomechanical imaging. The decision tree models were generated using Chi-square automatic interaction detection (CHAID) and classification and regression tree (CART) algorithms based on the training database. The discriminating rules of the CART model selected metrics of the Belin/Ambrósio deviation (BAD-D), stiffness parameter at first applanation (SPA1), back eccentricity (Becc), and maximum pachymetric progression index in that order; On the other hand, the CHAID model selected BAD-D, deformation amplitude ratio, SPA1, and Becc. Further, the CART model allowed for discrimination between normal and SKC eyes with 92.2% accuracy, which was higher than that of the CHAID model (88.3%), BAD-D (82.0%), Corvis biomechanical index (CBI, 77.3%), and tomographic and biomechanical index (TBI, 78.1%). The discriminating performance of the CART model was validated with 92.4% accuracy, while the CHAID model was validated with 86.4% accuracy in the validation database. Thus, the CART model using tomography and biomechanical imaging was an excellent model for SKC screening and provided easy-to-understand discriminating rules.

Comparison of variables measured with a Scheimpflug device for evaluation of progression and detection of keratoconus

Keratoconus is a progressive ectatic corneal disorder, which can result in severe visual impairment. The new ABCD keratoconus classification system allows differentiated description of the disease. Aim of the study was to evaluate the components of this novel staging system (ARC, PRC, thinnest pachymetry) as well as topometric indices, deviation of normality indices, and other parameters in terms of repeatability and reliability. 317 eyes with keratoconus were examined twice with a Scheimpflug device (Pentacam, Oculus). Bland Altman analysis and intraclass correlations were carried out to evaluate the parameters repeatability and reliability. Apart from IHA (ICC = 0.730), all parameters showed excellent reliability (ICC > 0.900). ARC, PRC, thinnest pachymetry, Kmax, CKI, KI, Rmin, and Progression Avg were the best repeatable parameters with relative repeatability values < 2.5%. Other parameters performing well in terms of repeatability were IHD, ISV, IVA, and final D (RR < 13%). Regression analysis revealed consistently high repeatability along all stages of keratoconus for PRC, thinnest pachymetry, and CKI. All parameters of the ABCD staging system showed excellent reliability and repeatability, PRC and thinnest pachymetry even at all stages of keratoconus and can consequently be reliably used in the determination of keratoconus progression.
