Challenges and opportunities of English as the medium of instruction in diploma midwifery programs in Bangladesh: a mixed-methods study
Background English is generally recognized as the international language of science and most research on evidence-based medicine is produced in English. While Bangla is the dominant language in Bangladesh, public midwifery degree programs use English as the medium of instruction (EMI). This enables faculty and student access to the latest evidence-based midwifery content, which is essential for provision of quality care later. Yet, it also poses a barrier, as limited English mastery among students and faculty limits both teaching and learning. Methods This mixed-methods study investigates the challenges and opportunities associated with the implementation of EMI in the context of diploma midwifery education in Bangladesh. Surveys were sent to principals at 38 public midwifery education institutions, and 14 English instructors at those schools. Additionally, ten key informant interviews were held with select knowledgeable stakeholders with key themes identified. Results Surveys found that English instructors are primarily guest lecturers, trained in general or business English, without a standardized curriculum or functional English language laboratories. Three themes were identified in the key informant interviews. First, in addition to students’ challenges with English, faculty mastery of English presented challenges as well. Second, language labs were poorly maintained, often non-functional, and lacked faculty. Third, an alternative education model, such as the English for Specific Purposes (ESP) curriculum,  has potential to strengthen English competencies within midwifery schools. Conclusions ESP, which teaches English for application in a specific discipline, is one option available in Bangladesh for midwifery education. Native language instruction and the middle ground of multilingualism are also useful options. Although a major undertaking, investing in an ESP model and translation of technical midwifery content into relevant mother tongues may provide faster and more complete learning. In addition, a tiered system of requirements for English competencies tied to higher levels of midwifery education could build bridges to students to help them access global evidence-based care resources. Higher levels might emphasize English more heavily, while the diploma level would follow a multilingualism approach, teach using an ESP curriculum, and have complementary emphasis on the mother tongue.
Transformation of the education of health professionals in China: progress and challenges
In this Review we examine the progress and challenges of China's ambitious 1998 reform of the world's largest health professional educational system. The reforms merged training institutions into universities and greatly expanded enrolment of health professionals. Positive achievements include an increase in the number of graduates to address human resources shortages, acceleration of production of diploma nurses to correct skill-mix imbalance, and priority for general practitioner training, especially of rural primary care workers. These developments have been accompanied by concerns: rapid expansion of the number of students without commensurate faculty strengthening, worries about dilution effect on quality, outdated curricular content, and ethical professionalism challenged by narrow technical training and growing admissions of students who did not express medicine as their first career choice. In this Review we underscore the importance of rebalance of the roles of health sciences institutions and government in educational policies and implementation. The imperative for reform is shown by a looming crisis of violence against health workers hypothesised as a result of many factors including deficient educational preparation and harmful profit-driven clinical practices.
Subspecialisation in radiology in Europe, a survey of the accreditation council of imaging
BackgroundTo provide an overview of existing Subspecialty Exams and Diplomas in Radiology and their endorsement as well as to providing an insight into the status of subspecialisation in radiology in Europe. The European Training Curriculum for Subspecialisation in Radiology mentions thirteen fields of subspecialisation within radiology. The websites of the corresponding subspecialty societies were checked for Subspecialty Exams and Diplomas. In addition, we performed a survey among European radiologists regarding subspecialisation in radiology.ResultsTen out of 13 European subspecialty societies offer a European subspecialty diploma. At least 7 out of the 10 European subspecialties societies in radiology offering a European subspecialty diploma obtained European Society of Radiology (ESR) endorsement. Two out of 10 obtained European Union of Medical Specialists—Council of European Specialist Medical Assessment endorsement. Survey among European radiologists who were ESR full members in March 2021 demonstrated that almost 20% of respondents indicated that they have no subspecialisation. Another 15% indicated that their area of subspecialisation is not recognised in their country of work. Eighty-four percent of respondents would like their area of subspecialisation in radiology to be officially recognised. According to the respondents, the major benefit of having their subspecialisation in radiology officially recognised is personal interest (45%).ConclusionsThere is a desire for more subspecialty recognition in radiology among European radiologists. Therefore, European subspecialty diplomas in radiology fulfil a need. Furthermore, there is room for further harmonisation and implementation on a European level regarding subspecialty training and recognition in radiology.Critical relevance statementAs there is a desire for more subspecialty recognition in radiology among European radiologists, European subspecialty diplomas in radiology fulfil a need and there is still room for further harmonisation and implementation on a European level regarding subspecialty training in radiology.Key points• Radiology has 13 subspecialties as per the European Training Curriculum for Subspecialisation.• Currently, 15 subspecialty diplomas are offered by European subspecialty societies in radiology• Members of the European Society of Radiology seek greater recognition of radiology subspecialties.
Oncology teaching in undergraduate nursing at public institutions courses in Brazil
ABSTRACT Objectives: to identify the occurrence of oncology teaching in undergraduate nursing curricula. Methods: descriptive study, which data was obtained from curriculum and from Pedagogical Course Projects available on the websites from Brazilian public institutions, between June and July 2020. Results: 143 undergraduate nursing courses were identified. From them, 132 nursing courses (corresponding to 89 education institutions) had available the curriculum and/or Pedagogical Course Projects. Only 7 (5.3%) of them had oncology as a mandatory subject, 4 of them in the Midwest Region. Only 35 (26.5%) had elective subject in Oncology, most of them in the Northeast (45.7%). Conclusions: on third of nursing courses at public institutions has Oncology subject in the curriculum, which is few considering the high incidence and mortality from cancer in the country. The findings contribute for discussions, in curricular accommodation, on the relevance of oncology teaching in nursing curricula. RESUMEN Objetivos: identificar ocurrencia de enseñanza de oncología en currículos de grado en Enfermería. Métodos: estudio descriptivo, datos obtenidos de la matriz curricular y del Proyecto Pedagógico de Cursos disponibles en el sitio de instituciones públicas brasileñas, entre junio y julio de 2020. Resultados: identificados 143 cursos de grado en Enfermería. De estos, 132 cursos de Enfermería (correspondientes a 89 instituciones de enseñanza) proveían la matriz curricular y/o Proyecto Pedagógico de Cursos. Solo 7 (5,3%) de ellos poseían disciplina obligatoria de oncología, siendo 4 en el Centro-Oeste. Solamente 35 (26,5%) proveían disciplina optativa en oncología, la mayoría en el Nordeste (45,7%). Conclusiones: un tercio de los cursos de Enfermería de instituciones públicas brasileñas tienen enseñanza de oncología en el currículo, lo que es poco considerando la alta incidencia y mortalidad por cáncer en el país. Esto contribuye para discusiones, en reformas curriculares, sobre la relevancia de enseñanza de oncología en currículos de Enfermería. RESUMO Objetivos: identificar a ocorrência de ensino de oncologia nos currículos de graduação em Enfermagem. Métodos: estudo descritivo, cujos dados foram obtidos da matriz curricular e do Projeto Pedagógico de Cursos disponíveis nos sites de instituições públicas brasileiras, entre junho e julho de 2020. Resultados: foram identificados 143 cursos de graduação em Enfermagem. Destes, 132 cursos de Enfermagem (correspondentes a 89 instituições de ensino) disponibilizavam a matriz curricular e/ou Projeto Pedagógico de Cursos. Apenas 7 (5,3%) deles possuíam disciplina obrigatória de oncologia, sendo 4 na região Centro-Oeste. Somente 35 (26,5%) disponibilizavam disciplina optativa em oncologia, a maioria no Nordeste (45,7%). Conclusões: um terço dos cursos de Enfermagem de instituições públicas brasileiras tem o ensino de oncologia no currículo, o que é pouco considerando a alta incidência e mortalidade por câncer no país. On the occasion of her death a hundred years ago, this commemoration presents the bibliography of the first Hungarian woman. Hugonnai Vilma obtained the medical degree in Zurich, which was accredited only after eighteen years. She is an example of giving help and care for those in need.
Blockchain Smart Contract to Prevent Forgery of Degree Certificates: Artificial Intelligence Consensus Algorithm
Certificates are often falsified, such as fake diplomas and forged transcripts. As such, many schools and educational institutions have begun to issue diplomas online. Although diplomas can be issued conveniently anytime, anywhere, there are many cases wherein diplomas are forged through hacking and forgery. This paper deals with the required Blockchain diploma. In addition, we use an automatic translation system, which incorporates natural language processing, to perform verification work that does not require an existing public certificate. The hash algorithm is used to authenticate security. This paper also proposes the use of these security protocols to provide more secure data protection. In addition, each transaction history, whether a diploma is true or not, may be different in length if it is presented in text, but converting it into a hash function means that it is always more than a certain length of SHA-512 or higher. It is then verified using the time stamp values. These chaining codes are designed. This paper also provides the necessary experimental environment. At least 10 nodes are constructed. Blockchain platform development applies and references Blockchain standardization, and a platform test, measurement test, and performance measurement test are conducted to assess the smart contract development and performance measurement. A total of 500 nodes were obtained by averaging 200 times, and a Blockchain-based diploma file was agreed upon at the same time. It shows performance information of about 4100 TPS. In addition, the analysis of artificial intelligence distribution diagram was conducted using a four-point method, and the distribution chart was evenly distributed, confirming the diploma with the highest similarity. The verified values were then analyzed. This paper proposes these natural language processing-based Blockchain algorithms.
The signaling value of a high school diploma
This paper distinguishes between the human capital and signaling theories by estimating the earnings return to a high school diploma. Unlike most indicators of education (e.g., a year of school), a diploma is essentially a piece of paper and, hence, by itself cannot affect productivity. Any earnings return to holding a diploma must therefore reflect the diploma’s signaling value. Using regression discontinuity methods to compare the earnings of workers who barely passed and barely failed high school exit exams—standardized tests that students must pass to earn a high school diploma—we find little evidence of diploma signaling effects.
Perioperative nursing education in Brazil: reviewing the past to survive the future
ABSTRACT Objectives: to summarize historical aspects related to perioperative nursing education in Brazil and to outline future perspectives. Methods: a descriptive-reflexive essay about the historical process of Brazilian perioperative teaching at undergraduate and graduate levels. Results: the reflection addresses the historical axes of perioperative nursing, teaching perioperative nursing, curriculum changes, graduate studies and relationship with the perioperative period and trends in perioperative nursing education. Curriculum changes reduced time available for teaching and learning, with a focus on the perioperative period, and diluted contents of surgical nursing in other subjects, which could cause irreversible losses for generalist nurses’ training. Final Considerations: when reviewing historical aspects of national nursing curricula, it was found that the nomenclature perioperative nursing was never used in the curriculum, however, the surgical area has always been contemplated in undergraduate and graduate studies. Future trends indicate the need to integrate themes and prepare professionals with perspectives focused on global health and technology. RESUMEN Objetivos: resumir aspectos históricos relacionados con la educación en enfermería perioperatoria en Brasil y esbozar perspectivas futuras. Métodos: se trata de un ensayo descriptivo-reflexivo sobre el proceso histórico de la enseñanza perioperatoria brasileña a nivel de pregrado y posgrado. Los cambios curriculares redujeron el tiempo disponible para la enseñanza y el aprendizaje, con foco en el período perioperatorio, y diluyeron los contenidos de enfermería quirúrgica en otras disciplinas, lo que podría ocasionar pérdidas irreversibles para la formación de enfermeros generalistas. Resultados: la reflexión aborda los ejes históricos de la enfermería perioperatoria, la enseñanza de enfermería perioperatoria, los cambios curriculares, los estudios de posgrado y la relación con el período perioperatorio y las tendencias en la formación de enfermería perioperatoria. Consideraciones Finales: al revisar aspectos históricos de los currículos nacionales de enfermería, se encontró que la nomenclatura enfermería perioperatoria nunca fue utilizada en los currículos, sin embargo, el área quirúrgica siempre ha sido contemplada en los estudios de grado y posgrado. Las tendencias futuras indican la necesidad de integrar temas y preparar profesionales con perspectivas centradas en la salud global y la tecnología. RESUMO Objetivos: sumarizar aspectos históricos relacionados ao ensino de enfermagem perioperatória no Brasil e traçar perspectivas futuras. Métodos: trata-se de um ensaio descritivo-reflexivo acerca do processo histórico do ensino perioperatório brasileiro no âmbito da graduação e pós-graduação. Resultados: a reflexão aborda os eixos histórico da enfermagem perioperatória, ensino da enfermagem perioperatória, mudanças curriculares, pós-graduação e relação com o perioperatório e tendências do ensino de enfermagem perioperatória. Alterações curriculares reduziram o tempo disponível de ensino-aprendizagem, com enfoque no perioperatório, e diluíram os conteúdos de enfermagem cirúrgica em outras disciplinas, podendo causar perdas irreversíveis para a formação de enfermeiros generalistas. Considerações Finais: ao rever aspectos históricos dos currículos nacionais da enfermagem, constatou-se que a nomenclatura enfermagem perioperatória nunca foi utilizada nos currículos, contudo a área cirúrgica sempre foi contemplada na graduação e pós-graduação. Tendências futuras assinalam a necessidade de integrar temáticas e preparar profissionais com perspectivas direcionadas à saúde mundial e tecnologia.
