Improving the content validity of the mixed methods appraisal tool: a modified e-Delphi study
The mixed methods appraisal tool (MMAT) was developed for critically appraising different study designs. This study aimed to improve the content validity of three of the five categories of studies in the MMAT by identifying relevant methodological criteria for appraising the quality of qualitative, survey, and mixed methods studies. First, we performed a literature review to identify critical appraisal tools and extract methodological criteria. Second, we conducted a two-round modified e-Delphi technique. We asked three method-specific panels of experts to rate the relevance of each criterion on a five-point Likert scale. A total of 383 criteria were extracted from 18 critical appraisal tools and a literature review on the quality of mixed methods studies, and 60 were retained. In the first and second rounds of the e-Delphi, 73 and 56 experts participated, respectively. Consensus was reached for six qualitative criteria, eight survey criteria, and seven mixed methods criteria. These results led to modifications of eight of the 11 MMAT (version 2011) criteria. Specifically, we reformulated two criteria, replaced four, and removed two. Moreover, we added six new criteria. Results of this study led to improve the content validity of this tool, revise it, and propose a new version (MMAT version 2018).
Mental health problems among Chinese adolescents during the COVID-19: The importance of nutrition and physical activity
Mental health problems are common among adolescents and greatly influenced by stressful events. This study sought to assess the prevalence and correlates of insomnia, depressive and anxiety symptoms among Chinese adolescents during the COVID-19. Method: Cross-sectional study (N = 1,794 adolescents, mean age = 15.26) was conducted in May 2020. An online survey was used to collect socio-demographic data, COVID-related fear (COVID-fear), nutrition, physical activity (PA) level and the symptoms of insomnia, depression and anxiety. Results: The prevalence of insomnia, depressive and anxiety symptoms was 37.80%, 48.20% and 36.70%, respectively, among Chinese adolescents during the pandemic. Generalized linear models revealed that female, left behind children, and students with greater COVID-fear tended to report symptoms of insomnia, depression and anxiety concurrently. After adjusting for socio-demographic factors and COVID-fear, better nutritional status and moderately active PA were both associated with lower levels of depressive and anxiety symptoms, while highly active PA was associated with lower levels of insomnia, depressive and anxiety symptoms. Conclusions: These findings suggest that more attention should be paid to psychological health among adolescents while combating COVID-19. To promote adolescents’ mental health, educators should help adolescents develop a healthy lifestyle with balanced diet and regular exercise. Los problemas de salud mental son comunes en adolescentes y están influenciados por eventos estresantes. Se evaluó prevalencia y correlatos de síntomas de insomnio, depresión y ansiedad en adolescentes chinos durante el COVID-19. Método: En mayo de 2020 se realizó un estudio transversal (N = 1.794 adolescentes, edad media = 15,26) mediante una encuesta en línea para recopilar datos sociodemográficos, miedo relacionado con COVID-19, nutrición, actividad física (AF) y síntomas de insomnio, depresión y ansiedad. Resultados: La prevalencia de síntomas de insomnio, depresión y ansiedad fue del 37,80%, 48,20% y 36,70%, respectivamente. Modelos lineales generalizados revelaron que mujeres, niños abandonados y estudiantes con más miedo al COVID-19 tendían a informar síntomas de insomnio, depresión y ansiedad simultaneamente. Después de ajustar los factores sociodemográficos y el miedo al COVID-19, mejor estado nutricional y AF moderadamente activa se asociaron con niveles más bajos de síntomas de depresión y ansiedad, mientras que AF muy activa se asoció con niveles más bajos de insomnio, síntomas depresivos y de ansiedad. Conclusiones: Se debe prestar más atención a la salud psicológica de los adolescentes mientras se combate el COVID-19. Para promover su salud mental, los educadores deben ayudarles a desarrollar un estilo de vida saludable con una dieta equilibrada y ejercicio regular.
COVID-19 and stem cell transplantation; results from an EBMT and GETH multicenter prospective survey
This study reports on 382 COVID-19 patients having undergone allogeneic ( n  = 236) or autologous ( n  = 146) hematopoietic cell transplantation (HCT) reported to the European Society for Blood and Marrow Transplantation (EBMT) or to the Spanish Group of Hematopoietic Stem Cell Transplantation (GETH). The median age was 54.1 years (1.0–80.3) for allogeneic, and 60.6 years (7.7–81.6) for autologous HCT patients. The median time from HCT to COVID-19 was 15.8 months (0.2–292.7) in allogeneic and 24.6 months (−0.9 to 350.3) in autologous recipients. 83.5% developed lower respiratory tract disease and 22.5% were admitted to an ICU. Overall survival at 6 weeks from diagnosis was 77.9% and 72.1% in allogeneic and autologous recipients, respectively. Children had a survival of 93.4%. In multivariate analysis, older age ( p  = 0.02), need for ICU ( p  < 0.0001) and moderate/high immunodeficiency index ( p  = 0.04) increased the risk while better performance status ( p  = 0.001) decreased the risk for mortality. Other factors such as underlying diagnosis, time from HCT, GVHD, or ongoing immunosuppression did not significantly impact overall survival. We conclude that HCT patients are at high risk of developing LRTD, require admission to ICU, and have increased mortality in COVID-19.
Was that (cyber)bullying? Investigating the operational definitions of bullying and cyberbullying from adolescents’ perspective
Background/Objective: Despite the great interest that bullying and cyberbullying have received during the last decades, the problem of defining these phenomena is still debated. Recently, this discussion has also been articulated in terms of how young people who are directly involved in bullying and cyberbullying understand these notions. This study aimed at investigating the operational definitions of both bullying and cyberbullying provided by adolescent victims and perpetrators, by inquiring the weight of traditional criteria (i.e., frequency, deliberateness, imbalance of power, and harm) as well as dominance in the perception of these phenomena. Method: A total of 899 students aged between 11 and 16 years filled out the Student Aggression and Victimisation Questionnaire. Results: Common traits and differences between the operational definition of bullying and cyberbullying and between the perspectives of victims and perpetrators of aggression were found. The most relevant criterion for the perception of both these phenomena was clearly the presence of dominance. By contrast, the imbalance of power showed no significant relationship with the perception of being bullied or bullying others both offline and online. Conclusions: Findings emphasise that young people conceptualise bullying with a clear reference to relational and group processes, rather than to individual differences. Antecedentes/Objetivo: A pesar del gran interés que el bullying y el cyberbullying han despertado durante las últimas décadas, el problema de la definición de estos fenómenos es todavía motivo de debate. Recientemente, la literatura ha abarcado esta controversia a partir de la comprensión que los jóvenes tienen del bullying y cyberbullying. Este artículo investiga las definiciones operativas, proporcionadas por víctimas y agresores, tras analizar la envergadura que tienen estos factores: frecuencia, intencionalidad, desequilibrio de poder, daño y dominancia. Método: Un total de 899 alumnos entre 11 y 16 años rellenaron el Student Aggression and Victimisation Questionnaire. Resultados: Los resultados evidenciaron rasgos comunes y diferencias entre las definiciones operativas de bullying y cyberbullying, al igual que entre las perspectivas de víctimas y perpetradores. El criterio más relevante para la definición de ambos fenómenos fue la percepción de la dominancia. En cambio, el desequilibrio de poder no mostró relaciones significativas con la definición de bullying y cyberbullying. Conclusiones: Estos hallazgos hacen hincapié en cómo los jóvenes interpretan el bullying, con un enfoque en los procesos relacionales y grupales, más que en las diferencias individuales.

Patient-Reported Outcomes After Complex Adult Spinal Deformity Surgery: 5-Year Results of the Scoli-Risk-1 Study
Study Design: Prospective cohort. Objective: To prospectively evaluate PROs up to 5-years after complex ASD surgery. Methods: The Scoli-RISK-1 study enrolled 272 ASD patients undergoing surgery from 15 centers. Inclusion criteria was Cobb angle of >80°, corrective osteotomy for congenital or revision deformity, and/or 3-column osteotomy. The following PROs were measured prospectively at intervals up to 5-years postoperative: ODI, SF36-PCS/MCS, SRS-22, NRS back/leg. Among patients with 5-year follow-up, comparisons were made from both baseline and 2-years postoperative to 5-years postoperative. PROs were analyzed using mixed models for repeated measures. Results: Seventy-seven patients (28.3%) had 5-year follow-up data. Comparing baseline to 5-year data among these 77 patients, significant improvement was seen in all PROs: ODI (45.2 vs. 29.3, P < 0.001), SF36-PCS (31.5 vs. 38.8, P < 0.001), SF36-MCS (44.9 vs. 49.1, P = 0.009), SRS-22-total (2.78 vs. 3.61, P < 0.001), NRS-back pain (5.70 vs. 2.95, P < 0.001) and NRS leg pain (3.64 vs. 2.62, P = 0.017). In the 2 to 5-year follow-up period, no significant changes were seen in any PROs. The percentage of patients achieving MCID from baseline to 5-years were: ODI (62.0%) and the SRS-22r domains of function (70.4%), pain (63.0%), mental health (37.5%), self-image (60.3%), and total (60.3%). Surprisingly, mean values (P > 0.05) and proportion achieving MCID did not differ significantly in patients with major surgery-related complications compared to those without. Conclusions: After complex ASD surgery, significant improvement in PROs were seen at 5-years postoperative in ODI, SF36-PCS/MCS, SRS-22r, and NRS-back/leg pain. No significant changes in PROs occurred during the 2 to 5-year postoperative period. Those with major surgery-related complications had similar PROs and proportion of patients achieving MCID as those without these complications.
Optimism and pessimism in the general population: Psychometric properties of the Life Orientation Test (LOT-R)
Background/Objective: The Life Orientation Test-Revised (LOT-R) is often used to assess dispositional optimism. The aims of this study were to test psychometric properties of the LOT-R, to provide normative scores, and to test the association between optimism and several psychological, sociodemographic, and behavioral factors. Method: A randomly selected German general population community sample with an age range of 18-80 years (N=9,711) was surveyed. Results: The Confirmatory Factor Analysis (CFA) proved two (correlated) factors: Optimism and Pessimism. Invariance tests across gender and age groups confirmed metric invariance. There were only small gender differences in the LOT-R total score (M=16.4 for females and M=16.1 for males). The correlation between the subscales Optimism and Pessimism was strong for young and well educated people. Low optimism mean scores were observed for unemployed people, people with low income, smokers, and obese people. Normative scores of the LOT-R are provided. Conclusions: The study confirmed the bidimensional structure of the LOT-R and invariance across age and gender. We can recommend using this instrument for measuring dispositional optimism and pessimism in epidemiological research and clinical practice. Antecedentes/Objetivo: La versión revisada del Life Orientation Test (LOT-R) se emplea a menudo para evaluar el optimismo disposicional. Los objetivos de este estudio fueron establecer las propiedades psicométricas del LOT-R, y probar la asociación entre optimismo y varios factores psicológicos, socio-demográficos y conductuales. Método: Para ello se entrevistó una muestra comunitaria aleatoriamente seleccionada de la población general alemana, con un rango de edad de 19-80 años (N=9.711). Resultados: El Análisis Factorial Confirmatorio (CFA) sugiere dos factores (correlacionados): Optimismo y Pesimismo. Las pruebas de invarianza para género y edad confirmaron la invarianza métrica. Solamente se encontraron pequeñas diferencias de género en el puntaje total (M=16,4 para mujeres y M=16,1 para hombres). Se encontraron bajos puntajes medios en personas desempleadas, personas con bajos ingresos, fumadores y personas con obesidad. Se proveen valores normativos para el LOT-R. Conclusiones: El estudio confirma la estructura bidimensional del LOT-R y la invarianza en género y edad. El instrumento puede recomendarse para medir optimismo disposicional y pesimismo en investigación epidemiológica y en la práctica clínica.
Quality of life in breast cancer patients: Associations with optimism and social support
Background/Objective: The purpose of this study was to examine quality of life (QoL) in breast cancer patients from Colombia and to explore the relationship between QoL, habitual optimism, and social support. Method: A sample of 95 breast cancer patients treated in a hospital in Bogotá were administered the QoL instrument EORTC QLQ-C30 and the Life Orientation Test LOT-R. Additionally, they were asked to indicate from whom (physicians, friends, nurses, etc.) they wished and received social support. Reference data for the EORTC QLQ-C30 and the LOT-R were taken from a representative sample of the general Colombian population. Results: The breast cancer patients showed detriments to their QoL on most functioning scales and symptom scales of the EORTC QLQ-C30, while their general assessments of health and QoL were not worse than those of the controls. Optimism was positively correlated with QoL. Most patients wanted and received social support from their physicians and friends/family. Conclusions: The results suggest that optimism helps patients better cope with disease. A general assessment of global QoL cannot replace the more specific assessments of the functioning domains and symptoms. Antecedentes/Objetivo: El objetivo del estudio fue examinar la calidad de vida (QoL, por sus siglas en inglés) en pacientes con cáncer de mama colombianas, y explorar la asociación entre calidad de vida, optimismo disposicional y apoyo social. Método: Se entrevistó una muestra de 95 pacientes con cáncer de mama tratadas en un hospital de Bogotá y les fue aplicado el instrumento de medición de calidad de vida EORTC QLQ-C30 y el test de orientación ante la vida (LOT-R). Adicionalmente, se preguntó acerca de su apoyo social usando varias preguntas. Resultados: En la mayoría de las escalas de funcionamiento del EORTC QLQ-C30 y de las escalas de síntomas, las pacientes con cáncer mostraron detrimentos en su QoL, mientras en la evaluación general de calidad de vida y salud las medias de las pacientes no fueron más bajas que las de los controles. El optimismo estuvo positivamente correlacionado con la calidad de vida. La mayoría de las pacientes desearon y recibieron apoyo social de su médico y de sus amigos/familiares. Conclusiones: Los resultados sugieren que el optimismo ayuda a las pacientes a afrontar mejor la enfermedad. Una evaluación general de la QoL no parece poder sustituir la evaluación más específica de los síntomas y dominios de funcionamiento.
Nivel de conocimiento de Covid-19 entre estudiantes, docentes y personal administrativo que pertenecen a la Facultad de Odontología de la Universidad Central del Ecuador
Introducción: Covid-19, una enfermedad letal que ha provocado estragos a nivel mundial causó una pandemia, afectando a casi todos los aspectos de la vida humana, por lo cual, se debe adquirir conocimientos sobre prevención, formar actitudes y comportamientos positivos para evitar más contagios y muertes. El objetivo fue determinar el nivel de conocimiento de Covid-19 entre estudiantes, docentes y personal administrativo que pertenecen a la Facultad de Odontología de la Universidad Central del Ecuador. Métodos: estudio observacional, analítico transversal, con una muestra conformada por 275 participantes entre docentes odontólogos, personal administrativo, estudiantes de pregrado y posgrado. Previo al estudio se realizó la traducción del cuestionario: Covid-19 Awareness Among Healthcare Students and Professionals in Mumbai Metropolitan Region: A Questionnaire-Based Survey, de inglés a español, siguiendo los debidos procesos de preparación, conciliación, retrotraducción, e informe final. Una vez establecida la fiabilidad del cuestionario (consistencia interna CCI= 0,89 y Alpha de Cronbach α= 0,63) se aplicó a la muestra total previo diseño en Google Forms y enviado a través de correo electrónico. Los datos fueron analizados en el programa estadístico SPSS v. 26 mediante la prueba de Chi-Cuadrado. Resultados: el promedio establecido del valor de la mediana mostró que los estudiantes de pregrado y los profesionales con un nivel educativo de tercer nivel tuvieron un mayor nivel de conocimientos que los otros grupos (p=0,014 y 0,012) respectivamente. Conclusiones: el grupo etario de 18 a 30 años, los estudiantes de universitarios de pregrado y los profesionales con el título de tercer nivel tuvieron un mejor nivel de conocimientos sobre Covid-19.
