Mass Casualty Incident Training in Immersive Virtual Reality: Quasi-Experimental Evaluation of Multimethod Performance Indicators
Immersive virtual reality (iVR) has emerged as a training method to prepare medical first responders (MFRs) for mass casualty incidents (MCIs) and disasters in a resource-efficient, flexible, and safe manner. However, systematic evaluations and validations of potential performance indicators for virtual MCI training are still lacking. This study aimed to investigate whether different performance indicators based on visual attention, triage performance, and information transmission can be effectively extended to MCI training in iVR by testing if they can discriminate between different levels of expertise. Furthermore, the study examined the extent to which such objective indicators correlate with subjective performance assessments. A total of 76 participants (mean age 25.54, SD 6.01 y; 45/76, 59% male) with different medical expertise (MFRs: paramedics and emergency physicians; non-MFRs: medical students, in-hospital nurses, and other physicians) participated in 5 virtual MCI scenarios of varying complexity in a randomized order. Tasks involved assessing the situation, triaging virtual patients, and transmitting relevant information to a control center. Performance indicators included eye-tracking-based visual attention, triage accuracy, triage speed, information transmission efficiency, and self-assessment of performance. Expertise was determined based on the occupational group (39/76, 51% MFRs vs 37/76, 49% non-MFRs) and a knowledge test with patient vignettes. Triage accuracy (d=0.48), triage speed (d=0.42), and information transmission efficiency (d=1.13) differentiated significantly between MFRs and non-MFRs. In addition, higher triage accuracy was significantly associated with higher triage knowledge test scores (Spearman ρ=0.40). Visual attention was not significantly associated with expertise. Furthermore, subjective performance was not correlated with any other performance indicator. iVR-based MCI scenarios proved to be a valuable tool for assessing the performance of MFRs. The results suggest that iVR could be integrated into current MCI training curricula to provide frequent, objective, and potentially (partly) automated performance assessments in a controlled environment. In particular, performance indicators, such as triage accuracy, triage speed, and information transmission efficiency, capture multiple aspects of performance and are recommended for integration. While the examined visual attention indicators did not function as valid performance indicators in this study, future research could further explore visual attention in MCI training and examine other indicators, such as holistic gaze patterns. Overall, the results underscore the importance of integrating objective indicators to enhance trainers' feedback and provide trainees with guidance on evaluating and reflecting on their own performance.
Evaluation of system based psychological first aid training on the mental health proficiency of emergency medical first responders to natural disasters in China: a cluster randomised controlled trial
ObjectiveTo evaluate the effectiveness of a system based psychological first aid (PFA) training programme for emergency medical first responders in China.DesignParallel-group, assessor-blinded, cluster randomised controlled trial.Setting42 clusters of health workers from various health facilities in China.Participants1399 health workers who provide emergency service for survivors of disasters.InterventionsOne-day system based PFA training programme (PFA) or training as usual (TAU).Primary and secondary outcome measuresThe primary outcome was the PFA skills, knowledge and attitude (SKA-PFA) score at 2 months postintervention. Secondary outcomes included post-traumatic growth, self-efficacy and professional quality of life.ResultsThe intervention group (n=690) had significantly higher SKA-PFA scores than the control group (n=709) at 2 months postintervention (adjusted mean difference=4.44; 95% CI 1.17 to 7.52; p=0.007; Cohen’s d=0.35). The intervention group also had higher scores on post-traumatic growth (p=0.113, d=0.24), self-efficacy (p=0.032, d=0.20) and professional quality of life (p=0.281, d=0.04).ConclusionsThe system based PFA training programme was more effective than the TAU in enhancing the PFA knowledge and skills of the emergency medical first responders and in increasing their competence to provide emergency service for survivors in China.Trial registration numberChiCTR2200060464.
Ambulance Commanders’ Reluctance to Enter Road Tunnels in Simulated Incidents and the Effects of a Tunnel-Specific e-Learning Course on Decision-Making: Web-Based Randomized Controlled Trial
The optimal response to a major incident in a road tunnel involves efficient decision-making among the responding emergency services (fire and rescue services, police, and ambulances). The infrequent occurrence of road tunnel incidents may entail unfamiliarity with the tunnel environment and lead to uncertain and inefficient decision-making among emergency services commanders. Ambulance commanders have requested tunnel-specific learning materials to improve their preparedness. We aimed to assess decision-making among ambulance commanders in simulated road tunnel incidents after they had participated in a tunnel-specific e-learning course designed to support timely and correct decisions in this context. We conducted a web-based intervention study involving 20 participants from emergency medical services in Sweden who were randomly allocated to a test or control group. The control group (n=10, 50%) received a lecture on general incident management, while the intervention group (n=10, 50%) completed an e-learning course consisting of 5 modules focused on tunnel structure, safety, and collaboration in response. The participants took part in 2 simulation-based assessments for ambulance commander decision-making in major road tunnel incidents 1 month and 6 months after their allocated study intervention. In each simulation, the participants decided on the best course of action at 15 independent decision points, designed as multiple-choice questions. The primary outcome was the correct response to the question regarding how to appropriately enter the road tunnel. The secondary outcome measurements were correct or incorrect responses and the time taken to decide for each of the 15 decisions. Limited in-depth follow-up interviews were conducted with participants (n=5, 25%), and collected data were analyzed using qualitative content analysis. All 20 participants completed the first simulation, and 16 (80%) completed the second. The main finding was that none (0/20, 0%) of the participants correctly answered the question on entering the tunnel system in the 1-month assessment. There were no significant differences between the groups (P=.59; 2-sample test of proportions) in the second assessment. The e-learning course was not associated with more correct answers at the first assessment, including accounting for participant factors (mean difference between groups: -0.58 points, 95% CI -1.88 to 0.73; P=.36). The e-learning course was also not associated with a shorter time to completion compared to the nonintervention group in either assessment. Interviews identified 3 categories linked to the main outcome: information (lack of), risk (limited knowledge and equipment), and mitigation (access to maps and aide-mémoire). Participation in a tunnel-specific e-learning course did not result in a measurable change in ambulance commanders' decision-making behavior during simulated road tunnel incidents. The observed hesitation to enter the road tunnel system may have several plausible causes, such as the lack of actionable intelligence and tunnel-specific plans. This novel approach to assessing commander decision-making may be transferable to other educational settings.
Disaster response team FAST skills training with a portable ultrasound simulator compared to traditional training: pilot study
Pre-hospital focused assessment with sonography in trauma (FAST) has been effectively used to improve patient care in multiple mass casualty events throughout the world. Although requisite FAST knowledge may now be learned remotely by disaster response team members, traditional live instructor and model hands-on FAST skills training remains logistically challenging. The objective of this pilot study was to compare the effectiveness of a novel portable ultrasound (US) simulator with traditional FAST skills training for a deployed mixed provider disaster response team. We randomized participants into one of three training groups stratified by provider role: Group A. Traditional Skills Training, Group B. US Simulator Skills Training, and Group C. Traditional Skills Training Plus US Simulator Skills Training. After skills training, we measured participants' FAST image acquisition and interpretation skills using a standardized direct observation tool (SDOT) with healthy models and review of FAST patient images. Pre- and post-course US and FAST knowledge were also assessed using a previously validated multiple-choice evaluation. We used the ANOVA procedure to determine the statistical significance of differences between the means of each group's skills scores. Paired sample t-tests were used to determine the statistical significance of pre- and post-course mean knowledge scores within groups. We enrolled 36 participants, 12 randomized to each training group. Randomization resulted in similar distribution of participants between training groups with respect to provider role, age, sex, and prior US training. For the FAST SDOT image acquisition and interpretation mean skills scores, there was no statistically significant difference between training groups. For US and FAST mean knowledge scores, there was a statistically significant improvement between pre- and post-course scores within each group, but again there was not a statistically significant difference between training groups. This pilot study of a deployed mixed-provider disaster response team suggests that a novel portable US simulator may provide equivalent skills training in comparison to traditional live instructor and model training. Further studies with a larger sample size and other measures of short- and long-term clinical performance are warranted.
Using High-Fidelity Virtual Reality for Mass-Casualty Incident Training by First Responders – A Systematic Review of the Literature
First responders' training and learning regarding how to handle a mass-casualty incident (MCI) is traditionally based on reading and/or training through computer-based scenarios, or sometimes through live simulations with actors. First responders should practice in realistic environments to narrow the theory-practice gap, and the possibility of repeating the training is important for learning. High-fidelity virtual reality (VR) is a promising tool to use for realistic and repeatable simulation training, but it needs to be further evaluated. The aim of this literature review was to provide a comprehensive description of the use of high-fidelity VR for MCI training by first responders. A systematic integrative literature review was used according to Whittemore and Knafl's descriptions. Databases investigated were PubMed, CINAHL Complete, Academic Search Ultimate, Web of Science, and ERIC to find papers addressing the targeted outcome. The electronic search strategy identified 797 potential studies. Seventeen studies were deemed eligible for final inclusion. Training with VR enables repetition in a way not possible with live simulation, and the realism is similar, yet not as stressful. Virtual reality offers a cost-effective and safe learning environment. The usability of VR depends on the level of immersion, the technology being error-free, and the ease of use. This integrative review shows that high-fidelity VR training should not rule out live simulation, but rather serve as a complement. First responders became more confident and prepared for real-life MCIs after training with high-fidelity VR, but efforts should be made to solve the technical issues found in this review to further improve the usability.
Use of Self-Efficacy Scale in Mass Casualty Incidents During Drill Exercises
Introduction Medical First Responders (MFRs) in the emergency department SUMMA 112 are tasked with handling the initial management of Mass Casualty Incidents (MCI) and building response capabilities. Training plays a crucial role in preparing these responders for effective disaster management. Yet, evaluating the impact of such training poses challenges since true competency can only be proven amid a major event. As a substitute gauge for training effectiveness, self-efficacy has been suggested. Objective The purpose of this study is to employ a pre- and post-test assessment of changes in perceived self-efficacy among MFRs following an intervention focused on the initial management of MCI. It also aimed to evaluate a self-efficacy instrument for its validity and reliability in this type of training. Method In this study, we used a pretest (time 1 = T1) – post-test (time 2 = T2) design to evaluate how self-efficacy changed after a training intervention with 201 MFRs in initial MCI management. ANOVA within-subjects and between subjects analyses were used. Results The findings reveal a noteworthy change in self-efficacy before and after training among the 201 participants. This suggests that the training intervention positively affected participants’ perceived capabilities to handle complex situations like MCI. Conclusion The results allow us to recommend a training program with theory components together with practical workshops and live, large-scale simulation exercises for the training of medical first responders in MCI, as it significantly increases their perception of the level of self-efficacy for developing competencies associated with disaster response.
Exploring medical first responders’ perceptions of mass casualty incident scenario training: a qualitative study on learning conditions and recommendations for improvement
ObjectiveDespite participating in scenario training, many medical first responders (MFRs) perceive themselves as inadequately prepared to respond to mass casualty incidents (MCIs). The objective of this study was to conduct a comprehensive examination of traditional MCI scenario training methods, focusing on their inherent strengths and limitations. An investigation into the perceptions of MFRs who had participated in MCI scenario training was carried out to identify potential areas for improvement and provide recommendations for refining MCI training protocols.DesignQualitative inductive approach using semistructured interviews that took place between October 2021 and February 2022. Data were analysed with qualitative content analysis.SettingMCI scenario training involving four organisations (three emergency medical services and one search-and-rescue organisation) tasked with responding to MCIs, collectively representing four European Union countries.Participants27 MFRs (17 emergency medical services personnel and 10 search-and-rescue volunteers) were recruited to participate in the study.ResultsTwo categories and seven associated subcategories (shown in parentheses) were identified as influencing the learning outcomes for MFRs: Training in a context mirroring real-world incidents (conducting incident scene risk assessment, realistic representation in casualties, incorporating scenario variety into the curriculum, interagency collaboration, role alignment when training incident site management) and use of a pedagogical framework (allowing for mistakes, the importance of post-training evaluation).ConclusionsThis study reaffirms the value of traditional MCI scenario training and identifies areas for enhancement, advocating for realistic scenarios, interagency collaboration, improved incident site management skills and thorough post-training evaluation. It suggests a shift in MCI training conceptualisation and delivery. The potential of virtual reality technologies as a valuable addition to training methods is explored, with a note on the need for further research to ascertain the long-term effectiveness of these technologies. However, the selection of a training method should consider programme goals, target population and resources.
Clinical simulation training for first responders in pediatric emergencies with family interaction: a scoping review
ABSTRACT Objective: To map studies on clinical simulation training directed at first responders during pediatric emergencies, focusing on interaction with families. Methods: A scoping review based on the guidelines of the JBI Manual for Evidence Syntheses and reported according to the PRISMA-ScR checklist, covering eight databases and gray literature, without time or language restrictions. Results: The ten selected studies indicated that most publications were from the United States. Simulations were predominantly conducted in hospital settings, with only one study associated with the pre-hospital context. The main designs used involved pediatric resuscitation scenarios and high-fidelity simulated environments. Conclusion: Simulation training is effective, increasing professionals’ confidence and improving communication with families. However, the concentration in developed countries and hospital settings highlights the need for training in other settings, such as pre-hospital care, to integrate technical and family-centered approaches. RESUMEN Objetivo: Mapear estudios sobre entrenamientos de simulación clínica dirigidos a los socorristas durante emergencias pediátricas, con un enfoque en la interacción con las familias. Métodos: Revisión de alcance basada en las directrices del Manual de Síntesis de Evidencia de JBI y reportada según la lista de verificación PRISMA-ScR, cubriendo ocho bases de datos y literatura gris, sin restricciones de tiempo o idioma. Resultados: Los diez estudios seleccionados indicaron que la mayoría de las publicaciones eran de los Estados Unidos. Las simulaciones se realizaron predominantemente en entornos hospitalarios, con solo un estudio asociado al contexto prehospitalario. Los principales diseños utilizados involucraron escenarios de reanimación pediátrica y entornos simulados de alta fidelidad. Conclusión: El entrenamiento en simulación es efectivo, aumenta la confianza de los profesionales y mejora la comunicación con las familias. Sin embargo, la concentración en países desarrollados y entornos hospitalarios resalta la necesidad de capacitación en otros contextos, como la atención prehospitalaria, para integrar enfoques técnicos y centrados en la familia. RESUMO Objetivo: Mapear estudos sobre treinamentos por simulação clínica direcionados aos socorristas durante emergências com crianças, focados na interação com as famílias. Métodos: Revisão de escopo fundamentada conforme diretrizes do referencial JBI Manual for Evidence Syntheses e relatada conforme o checklist PRISMA-ScR, em oito bases de dados e literatura cinzenta, sem recorte temporal ou idiomático. Resultados: Dez estudos selecionados indicaram que as publicações são majoritariamente dos Estados Unidos. Predominaram os treinamentos por simulação conduzidos em ambientes hospitalares, com apenas um estudo associado ao contexto pré-hospitalar. Os principais designs utilizados envolvem a temática de reanimação pediátrica e cenários em ambientes simulados de alta fidelidade. Conclusão: Os treinamentos de simulação são eficazes, aumentam a confiança dos profissionais e aprimoram a comunicação com as famílias. Contudo, a concentração em países desenvolvidos e hospitais ressalta a necessidade de capacitação em outras ambiências, como o atendimento pré-hospitalar, para integração do contexto técnico com a abordagem familiar.
Beyond one-size-fits-all: leveraging mixed-methods qualitative analysis of local stakeholders to tailor validated prehospital care solutions in the Cameroonian cultural context
IntroductionPrehospital care, though critical to injury survival, is largely unavailable in many low and middle-income countries, including Cameroon. Lay first responder (LFR) programmes train persons with high injury exposure in first-aid and emergency transport, but stakeholder buy-in from trainees and healthcare workers (HCWs) is essential. To design a context-appropriate prehospital care system, we evaluated barriers and facilitators of implementing a driver-based LFR programme in Cameroon.MethodsIn April 2023, we performed a mixed-methods evaluation targeting commercial mototaxi drivers and HCW in Limbe, Cameroon. Drivers were recruited for focus groups through union leaders. Trauma HCW at Limbe Regional Hospital completed Likert surveys and a subgroup completed semistructured interviews. Data collection focused on perceptions, barriers and facilitators of LFR programme implementation. Survey data were summarised using median and IQR. Interviews were recorded, transcribed, translated and analysed with open and axial coding using reflexive thematic analysis.ResultsOverall, 92 mototaxi drivers and 34 HCWs participated in the LFR programme assessment. Among the HCW surveyed, 93% felt mototaxi drivers were capable of training as LFR but only 44% felt that drivers would be able to provide care safely. Interviews identified negative HCW perceptions of drivers, including drivers being uneducated and financially motivated, as key barriers, whereas driver exposure to injury was identified as a facilitator to LFR programme implementation. Driver groups demonstrated a positive perception of LFR training but identified unpaid time spent transporting injured persons as a significant barrier. Both groups described a need for hospital involvement in trainings and bidirectional standardised communication with HCW.ConclusionsIn Cameroon, driver-based LFR may facilitate increased prehospital care but further exploration of possible systems of collaboration that promote long-term success of the programme is required. Specifically, sustainable implementation will need to include clear bidirectional communication guidelines and provide driver incentive commensurate to effort.
Lay First Responder Training in Eastern Uganda: Leveraging Transportation Infrastructure to Build an Effective Prehospital Emergency Care Training Program
Background Though road traffic injuries (RTIs) are a major cause of mortality in East Africa, few countries have emergency medical services. The aim was to create a sustainable and efficient prehospital lay first responder program, creating a system with lay first responders spread through the 53 motorcycle taxi stages of Iganga Municipality. Methods One hundred and fifty-four motorcycle taxi riders were taught a first aid curriculum in partnership with a local Red Cross first aid trainer and provided with a first aid kit following WHO guidelines for basic first aid. Pre- and post-survey tests measured first aid knowledge improvement over the course. Post-implementation incident report forms were collected from lay first responders after each patient encounter over 6 months. Follow-up interviews were conducted with 110 of 154 trainees, 9 months post-training. Results Improvement was measured across all five major first aid categories: bleeding control (56.9 vs. 79.7%), scene management (37.6 vs. 59.5%), airway and breathing (43.4 vs. 51.6%), recovery position (13.1 vs. 43.4%), and victim transport (88.2 vs. 94.3%). From the incident report findings, first responders treated 250 victims (82.8% RTI related) and encountered 24 deaths (9.6% of victims). Of the first aid skills, bleeding control and bandaging was used most often (55.2% of encounters). Lay first responders provided transport in 48.3% of encounters. Of 110 lay first responders surveyed, 70 of 76 who had used at least one skill felt “confident” in the care they provided. Conclusion A prehospital care system composed of lay first responders can be developed leveraging existing transport organizations, offering a scalable alternative for LMICs, demonstrating usefulness in practice and measurable educational improvements in trauma skills for non-clinical lay responders.
