Bariatric Surgery and Endoluminal Procedures: IFSO Worldwide Survey 2014
Background and aim Several bariatric surgery worldwide surveys have been previously published to illustrate the evolution of bariatric surgery in the last decades. The aim of this survey is to report an updated overview of all bariatric procedures performed in 2014.For the first time, a special section on endoluminal techniques was added. Methods The 2014 International Federation for the Surgery of Obesity and Metabolic Disorders (IFSO) survey form evaluating the number and the type of surgical and endoluminal bariatric procedures was emailed to all IFSO societies. Trend analyses from 2011 to 2014 were also performed. Results There were 56/60 (93.3%) responders. The total number of bariatric/metabolic procedures performed in 2014 consisted of 579,517 (97.6%) surgical operations and 14,725 (2.4%) endoluminal procedures. The most commonly performed procedure in the world was sleeve gastrectomy (SG) that reached 45.9%, followed by Roux-en-Y gastric bypass (RYGB) (39.6%), and adjustable gastric banding (AGB) (7.4%). The annual percentage changes from 2013 revealed the increase of SG and decrease of RYGB in all the IFSO regions (USA/Canada, Europe, and Asia/Pacific) with the exception of Latin/South America, where SG decreased and RYGB represented the most frequent procedure. Conclusions There was a further increase in the total number of bariatric/metabolic procedures in 2014 and SG is currently the most frequent surgical procedure in the world. This is the first survey that describes the endoluminal procedures, but the accuracy of provided data should be hopefully improved in the next future. We encourage the creation of further national registries and their continuous updates taking into account all new bariatric procedures including the endoscopic procedures that will obtain increasing importance in the near future.
Surgical endoscopy a historical perspective for the future
Background Flexible endoscopy today is often assumed to be the exclusive domain of medical gastroenterologists. Surgeons however, have been, and continue to be, pioneers and practitioners of endoscopy, significantly advancing this minimally invasive field through their contributions. Methods We present a historical review of the contributions surgeons have made in the creation and practice of flexible endoscopy — and offer our vision of the course of future collaborative development of the field. Results Surgical endoscopists have and continue to have a tremendous impact on both the technology and techniques of flexible endoscopy. While the involvement of surgeons in flexible endoscopy varies from country to country, it remains a fundamental component of surgical training and an important part of the practice of many surgeons. Conclusion Flexible endoscopy has always been and continues to be an important patient care tool that is shared between medical gastroenterology and digestive surgery. “Turf war” efforts to restrict access to this important tool are unjustified and only serve to deprive patients of the dramatic benefits this tool offers.
Exploring the clinical value of concept-based AI explanations in gastrointestinal disease detection
Complex artificial intelligence models, like deep neural networks, have shown exceptional capabilities to detect early-stage polyps and tumors in the gastrointestinal tract. These technologies are already beginning to assist gastroenterologists in the endoscopy suite. To understand how these complex models work and their limitations, model explanations can be useful. Moreover, medical doctors specialized in gastroenterology can provide valuable feedback on the model explanations. This study explores three different explainable artificial intelligence methods for explaining a deep neural network detecting gastrointestinal abnormalities. The model explanations are presented to gastroenterologists. Furthermore, the clinical applicability of the explanation methods from the healthcare personnel’s perspective is discussed. Our findings indicate that the explanation methods are not meeting the requirements for clinical use, but that they can provide valuable information to researchers and model developers. Higher quality datasets and careful considerations regarding how the explanations are presented might lead to solutions that are more welcome in the clinic.
Evolving Role and Future Directions of Natural Language Processing in Gastroenterology
In line with the current trajectory of healthcare reform, significant emphasis has been placed on improving the utilization of data collected during a clinical encounter. Although the structured fields of electronic health records have provided a convenient foundation on which to begin such efforts, it was well understood that a substantial portion of relevant information is confined in the free-text narratives documenting care. Unfortunately, extracting meaningful information from such narratives is a non-trivial task, traditionally requiring significant manual effort. Today, computational approaches from a field known as Natural Language Processing (NLP) are poised to make a transformational impact in the analysis and utilization of these documents across healthcare practice and research, particularly in procedure-heavy sub-disciplines such as gastroenterology (GI). As such, this manuscript provides a clinically focused review of NLP systems in GI practice. It begins with a detailed synopsis around the state of NLP techniques, presenting state-of-the-art methods and typical use cases in both clinical settings and across other domains. Next, it will present a robust literature review around current applications of NLP within four prominent areas of gastroenterology including endoscopy, inflammatory bowel disease, pancreaticobiliary, and liver diseases. Finally, it concludes with a discussion of open problems and future opportunities of this technology in the field of gastroenterology and health care as a whole.
Utility of Endoscopy in Hospitalized Patients with Gastrointestinal Hemorrhage and Pulmonary Hypertension
BackgroundGastrointestinal hemorrhage (GIH) has been reported as one of the most common GI complications in patients with pulmonary hypertension (PH). There is paucity of data on the national burden of GIH in patients with PH. We aimed to assess the prevalence, trends and outcomes of endoscopic interventions in patients with PH who were admitted with GIH.MethodWe queried National Inpatient Sample (NIS) database from 2005 to 2014 and identified the patients hospitalized with primary or secondary discharge diagnosis of PH (ICD 9 CM Code: 416.0, 416.8, and 416.9). Using Clinical Classification Software Coding system (153) patients with concurrent diagnosis of GIH were then identified. We studied the prevalence and trends of GIH in PH, factors associated with GIH, use of endoscopy, factors associated with utilization of endoscopic interventions, endoscopy outcomes including mortality, and overall healthcare burden.ResultsOut of 7,586,973 PH hospitalizations 3.2% (N = 246,358) had concurrent GIH, with a rising prevalence of GIH in PH patients during the last decade. Clinical predictors for GIH in PH included older age, congestive heart failure, anticoagulation therapy and concurrent alcohol abuse. Mean length of stay (LOS) in PH patients hospitalized with GIH was significantly higher than without GIH (8.6 vs. 6.4 days, p < 0.01) along with a significant increase in hospitalization cost ( $20,189 vs. $ 14,807, p < 0.01). Similarly, odds of in-hospital mortality increase by ~ 1.5 times in PH patients with GIH than those without it (adjusted odds ratio [aOR: 1.45, 95%CI: 1.43–1.47]). Endoscopic interventions were performed in 48.6% of patients with PH and GIH during their hospitalization. Older patients were more likely to undergo endoscopy, as well as the patients who received blood transfusion, and those with hypovolemic shock. Patients with acute respiratory failure and acute renal failure were less likely to get endoscopy. Mean LOS in patients undergoing endoscopic intervention was significantly higher than those who did not receive any intervention (8.7 vs. 8.4 days, p < 0.01), without a substantial increase in hospitalization cost ( $20,344 vs. $ 20,041, p < 0.01). Also, there was a significant decrease in in-hospital mortality in patients undergoing endoscopic interventions.ConclusionConcurrent GIH in patients with PH increases length of stay; healthcare costs and increases in-hospital mortality. Use of endoscopic interventions in these patients is associated with reduced length of stay, in-hospital mortality without significantly increasing the overall health care burden and should be considered in hospitalized patients with PH who are admitted with GIH. Future studies comparing GIH patients with and without PH should be done to assess if PH is a risk factor for worse outcomes.Clinical trial registration number: No IRB required due to use of national de-identified data.
Robotics in interventional endoscopy—evolution and the way forward
The integration of robotics into gastrointestinal (GI) endoscopy represents a transformative advancement and bears the potential to bridge the gap between traditional limitations by offering unprecedented precision and control in diagnostic and therapeutic procedures. This review explores the historical progression, current applications and future potential of robotic platforms in GI endoscopy. Originally designed for surgical applications, robotic systems have expanded their reach into endoscopy, potentially enhancing procedural accuracy and reducing ergonomic strain on practitioners. Natural Orifice Transluminal Endoscopic Surgery (NOTES) emerged as a promising technique, leveraging natural orifices to perform minimally invasive surgeries. Despite its initial potential, several factors, including limitations of the available instrumentations and lack of reliable closure techniques, hindered its widespread adoption and progress. Conventional endoscopic tools often fall short in terms of triangulation, traction and degrees of freedom, necessitating the adoption of robotic interventions. Over recent decades, robotic endoscopy has significantly evolved, focusing on both diagnostic and complex therapeutic procedures such as endoscopic sub-mucosal dissection (ESD) and endoscopic full-thickness resection (EFTR). Various robotic platforms demonstrate enhanced safety and efficiency in GI procedures. As the field progresses, the emphasis on clinical validation, advanced training and the exploration of new applications remains crucial. Continuous innovation in robotic technology and endoscopic techniques promises to overcome existing limitations, further revolutionizing the management of GI diseases and improving patient outcomes.
Underwater Endoscopy: Diving into the Future of Gastrointestinal Endoscopy
Purpose of Review With endoscopic advances water/saline have been widely adopted as alternatives for CO2 insufflation in endoscopy and its use has been advocated in wide array of endoscopic procedures, both diagnostic and therapeutic. This review aims to highlight the applications and advantages of underwater endoscopy in comparison to conventional techniques. Recent Findings Underwater endoscopy was initially described with water insertion and water exchange techniques to aid difficult insertions during colonoscopy. Its application has now expanded to U-EMR, U-ESD as well as underwater third space endoscopy, where in many scenarios it has been shown to be superior to conventional endoscopy. Summary Underwater endoscopy offers a new dimension in gastrointestinal endoscopic procedures with various advantages over conventional techniques, rapidly making it the preferred approach for various procedures which could otherwise be challenging.
Dieulafoy's lesion: current trends in diagnosis and management
Dieulafoy's lesion is a relatively rare, but potentially life-threatening, condition. It accounts for 1-2% of acute gastrointestinal (GI) bleeding, but arguably is under-recognised rather than rare. Its serious nature makes it necessary to include it in the differential diagnosis of obscure GI bleeding. The aim of this study was to review the current trends in the diagnosis and management of Dieulafoy's lesion. Using Medline, a literature search was performed for articles published in English, using the search words 'Dieulafoy'(s)' and 'gastrointestinal bleeding'. All retrieved papers were analysed and the findings are summarised in this review. There is no consensus on the treatment of Dieulafoy's lesions. Therapeutic endoscopy can control the bleeding in 90% of patients while angiography is being accepted as a valuable alternative to endoscopy for inaccessible lesions. Currently, surgical intervention is kept for failure of therapeutic endoscopic or angiographic interventions and it should be guided by pre-operative localisation. Advances in endoscopy have increased the detection of Dieulafoy's lesions and decreased the mortality from 80% to 8.6%. There are recent encouraging reports on the successful use of laparoscopic surgery in managing symptomatic Dieulafoy's lesions.
Colorectal endoscopic submucosal dissection : present status and future perspective, including its differentiation from endoscopic mucosal resection
Endoscopic submucosal dissection (ESD) allows en bloc resection of a lesion, irrespective of the size of the lesion. ESD has been established as a standard method for the endoscopic ablation of malignant tumors in the upper gastrointestinal (GI) tract in Japan. Although the use of ESD for colorectal lesions has been studied via clinical research, ESD is not yet established as a standard therapeutic method for colorectal lesions because colorectal carcinoma has unique pathological, organspecific characteristics that differ radically from those of the esophagus and stomach, and scope handling and control is more difficult in the colorectum than in the upper GI tract. Depending on the efficacy of endoscopic mucosal resection (EMR) and the clinicopathological characteristics of the colorectal tumor, the proposed indications for colorectal ESD are as follows: (1) lesions difficult to remove en bloc with a snare EMR, such as nongranular laterally spreading tumors (particularly the pseudodepressed type), lesions showing a type Vi pit pattern, and large lesions of the protruded type suspected to be carcinoma; (2) lesions with fibrosis due to biopsy or peristasis; (3) sporadic localized lesions in chronic inflammation such as ulcerative colitis; and (4) local residual carcinoma after EMR. Colorectal ESD is currently in the development stage, and a standard protocol will be available in the near future. We hope that colorectal tumors will be efficiently treated by a treatment method appropriately selected from among EMR, ESD, and surgical resection after precise preoperative diagnosis based on techniques such as magnifying colonoscopy.
