Consumption of ultra-processed food products and diet quality among children, adolescents and adults in Belgium

PurposeTo assess the dietary share of ultra-processed foods (UPF) among Belgian children, adolescents and adults and associations with diet quality.MethodsData from the national Food Consumption Surveys 2004 (N = 3083; ≥ 15 years) and 2014–2015 (N = 3146; 3–64 years) were used. Two 24-h recalls (dietary records for children) were used for data collection. Foods consumed were classified by the level of processing using the NOVA classification. The usual proportion of daily energy intake from UPF was determined using SPADE (Statistical Program to assess dietary exposure).ResultsIn 2014/2015, 36.4% of foods consumed were ultra-processed, while 42.4% were unprocessed/minimally processed. The usual proportion of daily energy intake from UPF was 33.3% (95% CI 32.1–35.0%) for children, 29.2% (95% CI 27.7–30.3%) for adolescents and 29.6% (95% CI 28.5–30.7%) for adults. There were no differences in UPF consumption between 2004 and 2014/2015. The products contributing most to UPF consumption were processed meat (14.3%), cakes, pies, pastries (8.9%), sweet biscuits (7.7%) and soft drinks (6.7%). The UPF dietary share was significantly lower during consumption days when participants met the WHO salt intake recommendation (≤ 5 g/day) and when saturated fat was ≤ 10% of their total energy intake. The dietary share of unprocessed/minimally processed foods was significantly higher during consumption days when participants met the WHO salt and fruit/vegetable intake (≥ 400 g/day) recommendations and when saturated fat was ≤ 10% of their total energy intake.ConclusionsThe UPF dietary share is substantial and associated with lower diet quality. Internationally recommended policies to limit UPF accessibility and marketing need to be implemented to reduce UPF consumption.

Ultra-processed food consumption in adults across Europe

PurposeThe purpose of this study is to describe ultra-processed food and drinks (UPFDs) consumption, and associations with intake of total sugar and dietary fibre, and high BMI in adults across Europe.MethodsUsing food consumption data collected by food records or 24-h dietary recalls available from the European Food Safety Authority (EFSA) Comprehensive European Food Consumption Database, the foods consumed were classified by the level of processing using the NOVA classification. Diet quality was assessed by data linkage to the Dutch food composition tables (NEVO) and years lived with disability for high BMI from the Global Burden of Disease Study 2019. Bivariate groupings were carried out to explore associations of UPFDs consumption with population intake of sugar and dietary fibre, and BMI burden, visualised by scatterplots.ResultsThe energy share from UPFDs varied markedly across the 22 European countries included, ranging from 14 to 44%, being the lowest in Italy and Romania, while the highest in the UK and Sweden. An overall modest decrease (2–15%) in UPFDs consumption is observed over time, except for Finland, Spain and the UK reporting increases (3–9%). Fine bakery wares and soft drinks were most frequently ranked as the main contributor. Countries with a higher sugar intake reported also a higher energy share from UPFDs, as most clearly observed for UPF (r = 0.57, p value = 0.032 for men; and r = 0.53, p value = 0.061 for women). No associations with fibre intake or high BMI were observed.ConclusionPopulation-level UPFDs consumption substantially varied across Europe, although main contributors are similar. UPFDs consumption was not observed to be associated with country-level burden of high BMI, despite being related to a higher total sugar intake.

Food consumption trends and drivers

A picture of food consumption (availability) trends and projections to 2050, both globally and for different regions of the world, along with the drivers largely responsible for these observed consumption trends are the subject of this review. Throughout the world, major shifts in dietary patterns are occurring, even in the consumption of basic staples towards more diversified diets. Accompanying these changes in food consumption at a global and regional level have been considerable health consequences. Populations in those countries undergoing rapid transition are experiencing nutritional transition. The diverse nature of this transition may be the result of differences in socio-demographic factors and other consumer characteristics. Among other factors including urbanization and food industry marketing, the policies of trade liberalization over the past two decades have implications for health by virtue of being a factor in facilitating the ‘nutrition transition’ that is associated with rising rates of obesity and chronic diseases such as cardiovascular disease and cancer. Future food policies must consider both agricultural and health sectors, thereby enabling the development of coherent and sustainable policies that will ultimately benefit agriculture, human health and the environment.

Life cycle human health impacts of 875 pesticides

PURPOSE: Residues in field crops grown and harvested for human consumption are the main contributor to overall human exposure toward agricultural pesticides for the general population. However, exposure from crop residues is currently not considered in life cycle assessment practice. We therefore present a consistent framework for characterizing human toxicological impacts associated with pesticides applied to agricultural crops in the frame of life cycle impact assessment based on state-of-the-art data and methods. METHODS: We combine a dynamic multicrop plant uptake model designed for evaluating human exposure to residues for a wide range of pesticide-crop combinations with latest findings of pesticide dissipation kinetics in crops and post-harvest food processing. Outcome is a set of intake fractions and characterization factors for 875 organic pesticides and six major food crops along with specific confidence intervals for each factor. RESULTS AND DISCUSSION: Intake fractions aggregating exposure via crop residues and exposure via fractions lost to air and soil for pesticides applied to agricultural crops vary between 10â»â¸ and 10â»Â¹Â kg intake per kilogram applied as a function of pesticide and crop. Intake fractions are typically highest for lettuce and tomato and lowest for potato due to differences in application times before crop harvest and soil as additional barrier for uptake into potato tubers. Uncertainty in intake fractions is mainly associated with dissipation dynamics in crops, where results demonstrate that using pesticide- and crop-specific data is crucial. Combined with the uncertainty in effect modeling, characterization factors per pesticide and crop show squared geometric mean standard deviations ranging from 38 to 15,560 over a variability range across pesticide-crop combinations of 10 orders of magnitude. CONCLUSIONS: Our framework is operational for use in current life cycle impact assessment models, is made available for USEtox, and closes an important gap in the assessment of human exposure to pesticides. For ready use in life cycle assessment studies, we present pesticide-crop combination-specific characterization factors normalized to pesticide mass applied and provide default data for application times and loss due to post-harvest food processing. When using our data, we emphasize the need to consult current pesticide regulation, since each pesticide is registered for use on certain crops only, which varies between countries.
