The author writes a letter to a friend, the noted journalist James Foley that was killed, revisiting the moments they shared together in an MFA program, specifically a workshop where the author felt unsafe. This friend becomes an anchor and a person to cycle back to in memory. In writing the letter, the past is invoked to aid the author in making sense of the lingering grief.

In the growing field of sonic interaction design, increasing emphasis is being placed on walking-based interactions within an array of applications, including virtual reality, interactive media, and rehabilitation. Our study focuses on recreating the aural experience of wading through water, specifically the challenge of eliciting accurate and natural movement-sound associations for wading, both in interactive and listening-only contexts. We engineered a real-time wading simulation using a digital Foley technique that maps lower limb angular velocity contours to the intensity of steady-state flowing sounds. Our first iteration was evaluated in a perceptual experiment involving 16 participants, as well as an interactive test with 9 participants, which revealed the need for additional sensors along with time-domain preprocessing to ensure a consistently natural sound envelope across walking cadences (step tempi). We then refined the mapping function and incorporated more sound layers. In our subsequent listening test, 55 participants compared the realism of the initial and refined versions with real-life wading sounds at various step cadences. While the refined version demonstrated a notable improvement over the initial one and was deemed fairly realistic overall, it fell just short of the authenticity of the real-life recordings at faster cadences, suggesting room for further improvement of our simulation. Nevertheless, this research marks a stride in the evolution of walking-based sonic interactions, instigating wider acceptance and application of such systems in the future.

Intracervical Foley Catheter Plus Intravaginal Misoprostol vs Intravaginal Misoprostol Alone for Cervical Ripening: A Meta-Analysis

Currently, there is no meta-analysis comparing intravaginal misoprostol plus intracervical Foley catheter versus intravaginal misoprostol alone for term pregnancy without identifying risk factors. Therefore, the purpose of this study is to conduct a systematic review and meta-analysis of randomized control trials (RCTs) comparing concurrent intravaginal misoprostol and intracervical Foley catheter versus intravaginal misoprostol alone for cervical ripening. We systematically searched Embase, Pubmed, and Cochrane Collaboration databases for randomized controlled trials (RCTs) comparing intracervical Foley catheter plus intravaginal misoprostol and intravaginal misoprostol alone using the search terms “Foley”, “misoprostol”, “cervical ripening”, and “induction” up to 29 January 2019. Data were extracted and analyzed by two independent reviewers including study characteristics, induction time, cesarean section (C/S), clinical suspicion of chorioamnionitis, uterine tachysystole, meconium stain, and neonatal intensive care unit (NICU) admissions. Data was pooled using random effects modeling and calculated with risk ratio (RR) and 95% confidence interval (CI). Pooled analysis from eight studies, including 1110 women, showed that labor induction using a combination of intracervical Foley catheter and intravaginal misoprostol decreased induction time by 2.71 h (95% CI −4.33 to −1.08, p = 0.001), as well as the risk of uterine tachysystole and meconium staining (RR 0.54, 95% CI 0.30–0.99 and RR 0.48, 95% CI 0.32–0.73, respectively) significantly compared to those using intravaginal misoprostol alone. However, there was no difference in C/S rate (RR 0.93, 95% CI 0.78–1.11) or clinical suspicion of chorioamnionitis rate (RR 1.22, CI 0.58–2.57) between the two groups. Labor induction with a combination of intracervical Foley catheter and intravaginal misoprostol may be a better choice based on advantages in shortening induction time and reducing the risk of uterine tachysystole and meconium staining compared to intravaginal misoprostol alone.

A Foley catheter ‘the jack of all trades’: a literature review of its common and novel uses

The Foley catheter is one of the most commonly used devices in modern surgical practice. Developed for draining the urinary bladder, this humble catheter has been employed for many other purposes ranging from urine output monitoring to complex urological investigations. Over time, it has evolved into being applied in more complex and innovative ways in various other specialties apart from urology. In this review article, we describe some of the common and novel uses of this deceptively simple device, and discuss the scope of its application in modern medicine.

Introduction Foley catheterization is a routine urologic procedure, yet complications may arise, particularly in patients with long‐term indwelling catheters. Case Presentation We present a rare case of iatrogenic ureteral obstruction caused by inadvertent Foley catheter insertion into the right ureter in a 55‐year‐old male nursing home resident. The condition led to right‐sided hydronephrosis and recurrent urinary tract infections. Diagnosis was established using computed tomography, which confirmed the catheter's entry into the ureter, and cystoscopy revealed polypoid lesions around the right ureteral orifice. After management with an antegrade Double‐J stent, Foley catheter reinsertion, and antibiotic therapy, the patient's condition improved. Follow‐up cystoscopy during hospitalization demonstrated a marked reduction in the size of the polypoid lesions, with the right ureteral orifice located within a cellule between two gate‐like trabeculations. Conclusion This case highlights the importance of vigilance in long‐term catheter management and imaging in recurrent urinary tract infections. Keynote Message Foley catheter misplacement into the ureter is a rare but serious complication, especially in patients with long‐term indwelling catheters or bladder dysfunction. This case demonstrates that catheter‐induced trauma, combined with bladder trabeculation, can cause ureteral obstruction. Careful verification of catheter placement and vigilance for unexplained hydronephrosis or recurrent urinary tract infections are essential to prevent such complications.

Offering women a choice in induction of labour: a prospective cohort study

PurposeTo evaluate women’s choice in the method of labour induction between oral misoprostol, PGE2 pessary and the Foley catheter. To compare women’s satisfaction according to their choice and to identify factors associated with patient satisfaction.MethodsWe conducted a comparative, prospective cohort study of 520 women who chose their preferred method for labour induction, in a French tertiary hospital, from July 2019 to October 2020. Before and after the delivery, they were asked to argue their choice and to evaluate their satisfaction through the use of questionnaires. The primary outcome was global level of satisfaction.ResultsOf the 520 women included, 67.5% of women chose oral misoprostol compared to 21% PGE2 pessary and 11.5% Foley catheter. Regarding global satisfaction, we found no significant difference between the three groups: 78.4%, 68.8% and 71.2% (p = 0.107) for, respectively, oral misoprostol, PGE2 pessary and Foley catheter. Factors that seem to improve women’s satisfaction were nulliparity (aOR = 2.03, 95% CI [1.19–3.53]), delivery within 24 h after the start of induction (aOR = 3.46, 95% CI [2.02–6.14]) and adequate information (aOR = 4.21, 95% CI [1.869.64]). Factors associated with lower satisfaction rates were postpartum haemorrhage (aOR = 0.51, 95% CI [0.30–0.88]) and caesarean section (aOR = 0.31, 95% CI [0.17–0.54]).ConclusionWomen satisfaction rates were not different between the three methods, when chosen by the patients themselves. These finding should encourage caregivers to promote shared decision making when possible.Trial registrationThe protocol was approved by the French ethics committee for research in obstetrics and gynaecology (CEROG, reference number 2019-OBS-0602) on 1st June 2019.

Foley catheter with noble metal alloy coating for preventing catheter-associated urinary tract infections: a large, multi-center clinical trial

Background
Catheter-associated urinary tract infections (CAUTI) are among the most frequent healthcare-associated infections in the world. They are associated with increased mortality, prolonged hospital stay and increased healthcare costs. The objective of this study was to evaluate the efficacy of the noble metal alloy (NMA) coated BIP Foley Catheter in preventing the incidence of symptomatic CAUTI in a large cohort of patients in India.
Methods
This multi-center, prospective study included 1000 adult patients admitted to six hospitals across India for urology, surgery and ICU requiring urethral catheterization and admission for ≥ 48 h. Patients were allocated to the NMA-coated BIP Foley Catheter group or a non-coated control catheter group, with a randomization ratio of 3:1. CAUTI surveillance was conducted at study entry, upon catheter removal, and 2 days after catheter removal. For statistical analysis, categorical data (e.g. gender) were compared using the chi-square or Fischer test, and numerical data were compared using the two-sample
t
-test. Associations were evaluated using logistic regression.
Results and conclusions
The incidence of symptomatic CAUTI was reduced by 69% in the BIP Foley Catheter group compared to the control group (6.5 vs 20.8 CAUTI/1000 catheter days), with an incidence rate ratio of 0.31 (95% confidence interval: 0.21–0.46;
p
< 0.001). A reduction in the cumulative CAUTI incidence was evident in the BIP Foley Catheter group within 3 days after catheterization; this reduction was maintained up to ~ 30 days, and the largest reductions were seen between 3 and 11 days. There were no serious adverse events related to either catheter, and the percentage of patients with ≥ 1 adverse event was significantly lower in the NMA-coated BIP Foley Catheter group than in the control group (21.6% vs. 48.4%;
p
= 0.001). In conclusion, the NMA-coated BIP Foley Catheter was effective in reducing CAUTI and was well tolerated, with a lower incidence of adverse events compared to the uncoated catheter.

Audience perceptions of Foley footsteps and 3D realism designed to convey walker characteristics

Foley artistry is an essential part of the audio post-production process for film, television, games, and animation. By extension, it is as crucial in emergent media such as virtual, mixed, and augmented reality. Footsteps are a core activity that a Foley artist must undertake and convey information about the characters and environment presented on-screen. This study sought to identify if characteristics of age, gender, weight, health, and confidence could be conveyed, using sounds created by a professional Foley artist, in three different 3D humanoid models, following a single walk cycle. An experiment was conducted with human participants (n=100) and found that Foley manipulations could convey all the intended characteristics with varying degrees of contextual success. It was shown that the abstract 3D models were capable of communicating characteristics of age, gender, and weight. A discussion of the literature and inspection of related audio features with the Foley clips suggest signal parameters of frequency, envelope, and novelty may be a subset of markers of those perceived characteristics. The findings are relevant to researchers and practitioners in linear and interactive media and demonstrate mechanisms by which Foley can contribute useful information and concepts about on-screen characters.

Phage φAB6-Borne Depolymerase Combats Acinetobacter baumannii Biofilm Formation and Infection

Biofilm formation is one of the main causes of increased antibiotic resistance in Acinetobacter baumannii infections. Bacteriophages and their derivatives, such as tail proteins with depolymerase activity, have shown considerable potential as antibacterial or antivirulence agents against bacterial infections. Here, we gained insights into the activity of a capsular polysaccharide (CPS) depolymerase, derived from the tailspike protein (TSP) of φAB6 phage, to degrade A. baumannii biofilm in vitro. Recombinant TSP showed enzymatic activity and was able to significantly inhibit biofilm formation and degrade formed biofilms; as low as 0.78 ng, the inhibition zone can still be formed on the bacterial lawn. Additionally, TSP inhibited the colonization of A. baumannii on the surface of Foley catheter sections, indicating that it can be used to prevent the adhesion of A. baumannii to medical device surfaces. Transmission and scanning electron microscopy demonstrated membrane leakage of bacterial cells treated with TSP, resulting in cell death. The therapeutic effect of TSP in zebrafish was also evaluated and the results showed that the survival rate was significantly improved (80%) compared with that of the untreated control group (10%). Altogether, we show that TSP derived from φAB6 is expected to become a new antibiotic against multi-drug resistant A. baumannii and a biocontrol agent that prevents the formation of biofilms on medical devices.
