Food Fortification: The Advantages, Disadvantages and Lessons from Sight and Life Programs
Deficiencies in one or more micronutrients such as iron, zinc, and vitamin A are widespread in low- and middle-income countries and compromise the physical and cognitive capacity of millions of people. Food fortification is a cost-effective strategy with demonstrated health, economic and social benefits. Despite ongoing debates globally and in some countries regarding the performance and safety of food fortification, the practice offers significant benefits across each of the main vehicles for food fortification (large-scale food fortification, biofortification and point-of-use or home fortification) ranging from reducing the prevalence of nutritional deficiencies and economic benefits to societies and economies. Using Sight and Life’s global and national experiences in implementing food fortification efforts, we demonstrate how different programs in LMICs have successfully addressed challenges with food fortification and in doing so, find that these efforts are most successful when partnerships are formed that include the public and private sector as well as other parties that can provide support in key areas such as advocacy, management, capacity building, implementation and regulatory monitoring.
A blueprint for fortification planning and programming: Lessons learned from an analytical review of existing fortification frameworks
With multiple food fortification frameworks, countries can find it challenging to determine optimal methods for planning and implementing food fortification programmes to combat vitamin and mineral deficiencies, especially without additional technical support. To address this challenge, this study aimed to review existing frameworks to determine consistencies, differences, strengths, and weaknesses across the frameworks, and based on the review findings, formulate an enhanced and streamlined fortification framework. Nineteen frameworks were ultimately examined following a comprehensive literature review and key informant interviews. Generally, the reviewed frameworks amply describe motives and methods for the determination of fortification need and feasibility, industry engagement/quality assurance and quality control, and impact evaluations/surveillance. However, there was limited inclusion or discussion throughout the reviewed frameworks around harmonization of fortification with existing micronutrient interventions; fortification policy and/or strategy; enforcement, incentives, and penalties to ensure producer compliance with industry standards; and periodic fortification programme review and reassessment. The findings were used to develop a comprehensive Fortification Blueprint that aims to provide structured guidance and a library of tools and resources to fortification programme managers and key stakeholders to ensure optimal and sustainable programme design. The proposed Fortification Blueprint provides a call to action (and a mechanism to operationalize the call) around the need to take a renewed look at fortification programmes to address historically persistent challenges. Key messages Multiple food fortification frameworks currently exist to guide the design, implementation, scale‐up, and monitoring of mandatory large‐scale fortification programmes. Four design elements cited the least frequently throughout 19 reviewed frameworks included: (1) Fortification Policy and Strategy; (2) Harmonization with Existing Interventions/Policies; (3) Enforcement/Incentives/Penalties; and (4) Reassessment. Elements cited the most frequently in the frameworks included (1) Impact Evaluations/Surveillance; (2) Determining Need and Feasibility; and (3) Industry Engagement/Quality Assurance and Quality Control. A revised novel approach is proposed based on the research findings to refresh programme thinking and priorities and guide scaling fortification initiatives.

Trench Warfare under Grant and Lee
Continuing the study of field fortifications he began inField Armies and Fortifications in the Civil War, Earl J. Hess turns to the 1864 Overland campaign to cover battles from the Wilderness to Cold Harbor. A grueling form of trench warfare became a key feature of tactical operations during this phase of the war in Virginia.Drawing on meticulous research in primary sources and careful examination of trench remnants at the Wilderness, Spotsylvania, North Anna, Cold Harbor, and Bermuda Hundred, Hess describes Union and Confederate earthworks and how Grant and Lee used them in this new era of field entrenchments. According to Hess, the heavy reliance on earthworks by both armies in the Overland campaign was driven by Grant's relentless attacks against Lee, not by the widespread use of rifle muskets, as historians have previously argued. Entrenchments kept the armies within striking distance and compelled soldiers to dig in for protection. Despite suffering massive casualties, Grant seized control of the strategic initiative and retained it for the rest of the war in the eastern theater.Illustrated by rare, historic photographs and new detailed maps of the trench remnants, this book constitutes the second installment of a three-volume study of field fortifications in the eastern campaigns.
The Unfinished Agenda for Food Fortification in Low- and Middle-Income Countries: Quantifying Progress, Gaps and Potential Opportunities
Large-scale food fortification (LSFF) is a cost-effective intervention that is widely implemented, but there is scope to further increase its potential. To identify gaps and opportunities, we first accessed the Global Fortification Data Exchange (GFDx) to identify countries that could benefit from new fortification programs. Second, we aggregated Fortification Assessment Coverage Toolkit (FACT) survey data from 16 countries to ascertain LSFF coverage and gaps therein. Third, we extended our narrative review to assess current innovations. We identified 84 countries as good candidates for new LSFF programs. FACT data revealed that the potential of oil/ghee and salt fortification is not being met due mainly to low coverage of adequately fortified foods (quality). Wheat, rice and maize flour fortification have similar quality issues combined with lower coverage of the fortifiable food at population-level (<50%). A four-pronged strategy is needed to meet the unfinished agenda: first, establish new LSFF programs where warranted; second, systems innovations informed by implementation research to address coverage and quality gaps; third, advocacy to form new partnerships and resources, particularly with the private sector; and finally, exploration of new fortificants and vehicles (e.g. bouillon cubes; salt fortified with multiple nutrients) and other innovations that can address existing challenges.
