Children in Foster Care: A Vulnerable Population at Risk
TOPIC:  Nationally, 542,000 children are in foster care. Many of these children have prior histories of maltreatment such as abuse and neglect, with neglect being the most common form of maltreatment and the reason for many children requiring foster care services. Painful experiences associated with maltreatment and the trauma of being removed from one's parents (foster care) may affect the developmental and mental health of children. PURPOSE:  This paper synthesizes the experiences associated with foster care and reveals foster care outcomes obtained through a literature search of published research. Specifically, the notions of oppression and domination defined by Young (1990) experienced by children in foster are explored. SOURCES:  Review of the literature and clinical practice. CONCULSIONS:  Most children in foster care, if not all, experience feelings of confusion, fear, apprehension of the unknown, loss, sadness, anxiety, and stress. Such feelings and experiences must be addressed and treated early to prevent or decrease poor developmental and mental health outcomes that ultimately affect a child's educational experience and the quality of adulthood. Systemic orientation for all children entering foster care is proposed as a preventative intervention that addresses associated experiences of children in foster care.
Placement in Foster Care Enhances Quality of Attachment Among Young Institutionalized Children
This study examined classifications of attachment in 42-month-old Romanian children (N = 169). Institutionalized since birth, children were assessed comprehensively, randomly assigned to care as usual (CAU) or to foster care, and compared to family-reared children. Attachment classifications for children in foster care were markedly different from those in the CAU. Importantly, children placed in foster care before 24 months were more likely to have secure attachments and if placed earlier were less likely to have disorganized or insecureother attachments. Cognitive status predicted greater likelihood of organized attachment in the CAU and greater likelihood of secure attachment in the foster care and never-institutionalized groups. Foster care is an important intervention to reduce the adverse effects following early deprivation.
Understanding Foster Placement Instability for Looked After Children: A Systematic Review and Narrative Synthesis of Quantitative and Qualitative Evidence
Ensuring the stability of foster placements for looked after children is a priority for social services. Many previous studies have highlighted the negative psychological, social and academic consequences of placement breakdown for foster children, but less is known about how services can effectively promote placement stability. A systematic review and narrative synthesis of research examining correlates of placement moves and breakdown were undertaken in order to inform practice in this area. Qualitative studies were included alongside quantitative research, providing additional insights into the processes that facilitate and impede placement stability. Correlates of increased placement instability with the strongest evidence included older age of children, externalising behaviour, longer total time in care, residential care as first placement setting, separation from siblings, foster-care versus kinship care and experience of multiple social workers. Key protective factors included placements with siblings, placements with older foster-carers, more experienced foster-carers with strong parenting skills, and placements where foster-carers provide opportunities for children to develop intellectually. Following from these findings, a conceptual framework is proposed that distinguishes vulnerability and protective factors as well as background and immediate factors. Implications for front line social work practice, including the development of manualised tools, are discussed.
A Sense of Belonging: Meanings of Family and Home in Long-Term Foster Care
Permanency has been a key goal of the child welfare system for nearly forty years, with most discussion of this issue focusing how best to achieve legal and physical permanence. Although there has been some attention to the subjective dimensions of permanence, there has been no exploration of how fostered children develop a sense of belonging to their substitute families. This article draws on interviews with fostered children and their foster carers, conducted in the course of a larger study of outcomes in permanent placements, to present a qualitative analysis of belonging in long-term foster care. Interviews with fostered children revealed four types of perceived belonging: 'as if', 'just like', qualified and provisional. These were shaped by the interplay of a variety of factors, including day-to-day family practices in foster families, the actions and commitment of foster and birth parents, children's mental representations of their past and current experiences in these families and the meaning that children ascribed to blood and non-blood relationships.
Child Guidance Centres in Japan
This book represents a Copernican change to our understanding of alternative care in Japan and answers the question of Japan’s continued use of institutional care over foster care. – Tsuzaki Tetsuo In contemporary Japan, 85 per cent of children in alternative care remain housed in large welfare institutions, as opposed to family-based foster care. This publication examines how Japan has been isolated from the global discourse on alternative care, urging a shift in social work and alternative care policies. As the first ethnographic account from inside child guidance centres, it makes a key contribution towards understanding the closed world of Japan’s social services, including the decision-making processes by which a child is removed from the family and placed into care. In addition, regional variation in policy implementation for alternative care is outlined, with reference to detailed case studies and a discussion around organisational cultures of the child guidance centres. Where foster care is constructed as anything other than professional, it is often seen as a threat to the child’s family-bond with their natal parent and therefore not used. Child Guidance Centres in Japan destabilises this construction of the family-bond as singular and discrete, highlighting new practices in alternative care. Child Guidance Centres in Japan: Alternative Care, Social Work, and the Family will be a vital resource for students, scholars of social work and Japanese studies, as well as practitioners and lobbyists involved in alternative care.
Observational measure of implementation progress in community based settings: The Stages of implementation completion (SIC)
Background An increasingly large body of research is focused on designing and testing strategies to improve knowledge about how to embed evidence-based programs (EBP) into community settings. Development of strategies for overcoming barriers and increasing the effectiveness and pace of implementation is a high priority. Yet, there are few research tools that measure the implementation process itself. The Stages of Implementation Completion (SIC) is an observation-based measure that is used to track the time to achievement of key implementation milestones in an EBP being implemented in 51 counties in 53 sites (two counties have two sites) in two states in the United States. Methods The SIC was developed in the context of a randomized trial comparing the effectiveness of two implementation strategies: community development teams (experimental condition) and individualized implementation (control condition). Fifty-one counties were randomized to experimental or control conditions for implementation of multidimensional treatment foster care (MTFC), an alternative to group/residential care placement for children and adolescents. Progress through eight implementation stages was tracked by noting dates of completion of specific activities in each stage. Activities were tailored to the strategies for implementing the specific EBP. Results Preliminary data showed that several counties ceased progress during pre-implementation and that there was a high degree of variability among sites in the duration scores per stage and on the proportion of activities that were completed in each stage. Progress through activities and stages for three example counties is shown. Conclusions By assessing the attainment time of each stage and the proportion of activities completed, the SIC measure can be used to track and compare the effectiveness of various implementation strategies. Data from the SIC will provide sites with relevant information on the time and resources needed to implement MTFC during various phases of implementation. With some modifications, the SIC could be appropriate for use in evaluating implementation strategies in head-to-head randomized implementation trials and as a monitoring tool for rolling out other EBPs.
Fostering on the Farm
From 1870 until after World War I, reformers led an effort to place children from orphanages, asylums, and children's homes with farming families. The farmers received free labor in return for providing room and board. Reformers, meanwhile, believed children learned lessons in family life, citizenry, and work habits that institutions simply could not provide. Drawing on institution records, correspondence from children and placement families, and state reports, Megan Birk scrutinizes how the farm system developed--and how the children involved may have become some of America's last indentured laborers. Between 1850 and 1900, up to one-third of farm homes contained children from outside the family. Birk reveals how the nostalgia attached to misplaced perceptions about healthy, family-based labor masked the realities of abuse, overwork, and loveless upbringings endemic in the system. She also considers how rural people cared for their own children while being bombarded with dependents from elsewhere. Finally, Birk traces how the ills associated with rural placement eventually forced reformers to transition to a system of paid foster care, adoptions, and family preservation.
Trauma exposure and PTSD among older adolescents in foster care
Purpose Youth in foster care represent a highly traumatized population. However, trauma research on this population has focused primarily on maltreatment rather than the full spectrum of trauma experiences identified within the DSM-IV. The current study aims to fill this gap by reporting the prevalence of exposure to specific types of traumatic events for a large sample of youth with foster care experience. The study also reports the likelihood of lifetime PTSD diagnoses associated with each specific type of trauma. Method Data are from a longitudinal panel study of 732 adolescents aged 17 and 18 who were in foster care. Lifetime trauma exposure and PTSD diagnosis were assessed using the Composite International Diagnostic Interview . Statistical comparisons were made using logistic regressions. Results The majority of respondents had experienced at least one trauma in their lifetime. While overall trauma prevalence did not differ by gender, males were more likely to experience interpersonal violence and environmental trauma, while females were more likely to experience sexual trauma. Caucasian participants reported higher rates of trauma exposure than African-American participants did. The types of trauma associated with the highest probability of a lifetime PTSD diagnosis were rape, being tortured or a victim of terrorists, and molestation. Conclusions Youth in foster care are a highly traumatized population and meet diagnostic criteria for PTSD at higher rates than general youth populations. The ongoing impact of trauma may be particularly problematic for these young people given their abrupt transition to independence.
Parenting stress, dyadic coping and endocrine markers of stress and resilience in foster and biological mothers
Foster parents have been shown to report higher levels of parenting stress but also more dyadic coping (DC) behaviors in their partnership than biological parents, which might be an important protective factor that helps them cope with daily stressors. Here, we examined how parenting stress and DC are related in foster and biological parents and whether these are reflected in long-term alterations of hypothalamic-pituitary-adrenocortical (HPA) axis activity. A total of 79 foster mothers and 131 biological mothers participated in a longitudinal study. At the initial assessment, children were aged 2–7 years and lived for an average of 18 months in their current foster family. Mothers’ cortisol and dehydroepiandrosterone (DHEA) concentrations and their cortisol/DHEA ratios were assessed in scalp hair twice with approximately 11 months in between, while their perceived parenting stress and DC were measured by self-report questionnaires. Results showed no significant differences between foster mothers and biological mothers in cortisol, DHEA and cortisol/DHEA concentrations. While more DC was longitudinally related to lower levels of parenting stress across both study groups, no significant associations were found to endocrine markers. Thus, these findings indicate that increased parenting stress levels were not, or not strongly, reflected in HPA axis alterations as assessed in hair. Our findings thus add evidence for non-significant associations between self-reported perceived stress and chronic HPA axis markers. Future studies may explore whether early interventions, including those aimed at promoting and maintaining positive DC, are beneficial in preventing the development of stress-related illnesses in foster parents.
Permanency and Safety Among Children in Foster Family and Kinship Care
Over the past 25 years, kinship care placements have risen dramatically, such that when a child enters into care, child welfare agencies must first attempt to identify safe living arrangements with relatives or individuals known to the child before searching for alternatives. Despite the growing emphasis on kinship care, little is known about its impact on child outcomes in comparison to other placement types (e.g., foster family). Therefore, the aim of this scoping review was to evaluate quantitative research on children in out-of-home care from 2007 to 2014 with regard to the following outcomes: (1) permanency (i.e., reunification, reentry, placement stability, and adoption/guardianship) and (2) safety (e.g., additional reports to child welfare). Based on these objectives, the review identified 54 studies that examined permanency and safety among children in two major placement types, namely foster family and kinship care. Across studies, children in kinship care experienced greater permanency in terms of a lower rate of reentry, greater placement stability, and more guardianship placements in comparison to children living with foster families. Children in kinship care, however, had lower rates of adoption and reunification. The findings also indicated that differences in these variables diminish over time. Findings for safety outcomes were mixed. Study methodological limitations and recommendations for future research are considered.
