The efficacy of functional-analytic psychotherapy and acceptance and commitment therapy (FACT) for public employees

The literature is replete with evidence regarding the impact of psychological distress in the workplace. Traditionally, worksite interventions to enhance mental health have been carried out in groups. This study aimed to implement a brief individual program in the workplace through the combination of Functional Analytic Psychotherapy (FAP) and Acceptance and Commitment Therapy (ACT) for Public Administration employees.
One hundred and six public employees from a Spanish city council completed pretest measures and forty-three met the inclusion criteria. The participants´ scores on distress, burnout, psychological flexibility, depression, anxiety and stress were examined in a pretest-posttest design with a waiting list control group and random assignment. Thirty-eight employees completed the intervention (FACT group= 19; Waiting list control group= 19). The intervention lasted three individual sessions using a protocol with the processes of FAP and ACT, resulting in a protocol named FACT.
The FACT group showed statistically significant improvements in distress, burnout, psychological flexibility and anxiety compared with the waiting list control group.
These results provide a breakthrough and initial support for the inclusion of FAP in the workplace along with the integration with brief ACT in individual sessions to improve employees' mental health.

Quality of life in women with lipoedema: a contextual behavioral approach

Purpose Lipoedema is a chronic, progressive adipose disorder of unknown etiology, often underdiagnosed or misdiagnosed as obesity. It manifests itself with accumulation of the fat in lower parts of the body and associated edema and, due to numerous physical and psychological consequences, affects the quality of life (QOL) of those who suffer. The aim of this study was to investigate the psychological factors that might have an impact on the QOL of women with lipoedema from the contextual behavioral viewpoint. Methods In an Internet-based cross-sectional study, women suffering from lipoedema (N = 120) were asked to fill in questionnaires assessing: symptom severity, QOL (WHOQOL-BREF), satisfaction with life (SWLS), psychological flexibility (Acceptance and Action Questionnaire-II), social connectedness (Social Connectedness ScaleRevised) and other psychological factors. The majority of participants were from the USA, the UK, and Australia. Results Multiple hierarchical regression analyses showed that a higher level of QOL was predicted by higher levels of psychological flexibility and social connectedness, while controlling for symptom severity. Higher level of SWL was predicted only by higher level of social connectedness. Conclusions Acceptance and Commitment Therapy with psychological flexibility as the target of change and Functional Analytic Psychotherapy with social connectedness as the target of change might be useful in treating women with lipoedema; however, further research in this area is needed. The authors conclude that psychological and biomedicai interventions for women with lipoedema and their QOL merit more attention from researchers and the medical community than is currently received.

Intimacy is an interpersonal repertoire related to physical and behavioral health indicators. Functional analytic psychotherapy (FAP) is a behavioral intervention that has demonstrated utility in improving intimacy repertoires, particularly in experimental single-case designs. The FAP Intimacy Scale (FAPIS) was developed with the aim of using a theoretically sound measure to evaluate FAP effects on a large scale. This study sought to culturally adapt and validate the FAPIS for Spanish-speaking populations. A total sample of 509 Spanish-speaking participants completed the survey. Exploratory and confirmatory factor analyses were conducted to examine FAPIS psychometric properties. A network analysis was also performed to assess the relation among scale factors. As expected, the FAPIS Spanish version demonstrated a good fit, meeting the same three-factor structure as the original version. Network loading along three factors was also found, and item 3 was identified as the most central item in the scale. Low and negative correlations among FAPIS, experiential avoidance, emotional sensitivity, and social impact were found. Findings showed that FAPIS is a valid and reliable measure for assessing intimacy in Spanish-speaking populations.

Job burnout is a condition that interferes significantly with quality of life and is related to stressful working environments, organizational climate, and client outcomes. The introduction of burnout prevention and “Helping the Helper” programs may produce significant benefits with respect to this issue. In this study, we assessed a weekly online “Helping the Helper” intervention based on the behavioral principles of Functional Analytic Psychotherapy (FAP) to improve social relationships, reduce burnout, and improve organizational climate and quality of life in mental health professionals. Six therapists of the Borderline Personality Disorder Clinic of Mexico’s National Institute of Psychiatry participated in this study. Subjects answered three questionnaires (Maslach Burnout Inventory, Multidimensional Scale of Organizational Climate, and World Health Organization Quality of Life-BREF) at seven time points throughout the duration of the study. The intervention was implemented between time points three and five. Robust Improvement Rate Difference (R-IRD) scores were computed for each subject to assess for the intervention’s efficacy. Results suggest preliminary support for the possibility of a “Helping the Helper” intervention based on FAP for reducing burnout and improving organizational climate among colleagues in a high-stress, public mental health setting.

Spanish Validation and Psychometric Properties of the FIAT-Q-Brief Scale for Functional
Idiographic Assessment. Contextual therapies address psychological problems and human suffering
through verbal changes and natural contingencies given in therapy. Among them, Functional Analytic
Psychotherapy (FAP) has developed a clinical process focused on the functional analysis of clinically
relevant behaviors, which have to appear and be modified directly by the therapist within the
consultation. The Functional Idiographic Assessment Template-Questionnaire is used to assess these
functional classes. Here we present the Spanish adaptation and scoring of an abbreviated format in
the FIAT-Q-breve. The sample consisted of 400 participants (52.3% female and 47.8% male), with a
mean age of 36.21 (SD= 14.24), and a small sample of 27 participants was also used for test-retest
reliability. The FIAT-Q-breve questionnaire itself, together with other life problems and life values
questionnaires (SA-45, VLQ and LSI) have been applied on-line. Results show a very high overall
factor reliability (α= 0.95, ω= 0.96); factor reliability α between .63 and .90; and test-retest reliability
r between .65 and .80. The FIAT-Q-breve correlates with the SA-45, both about psychopathological
problems, and does not correlate with the values questionnaires (VLQ and LSI). All five factors of
the original questionnaire appear in the CFA, although the model does not fully fit. We provide the
data for the assessment of other consultants and discuss the usefulness and use of the FIAT-Q-breve
for assessment in contextual therapies

Application of functional analytic psychotherapy to manage schizophrenia

Introduction: Cognitive behavioral therapy (CBT) and third-wave CBT approaches have been shown to ameliorate symptoms of schizophrenia. However, this study uses the functional analytic psychotherapy which focuses on the environmental causes of behavior which includes the client's present environment as well as his history of past interactions with it, to bring about a change in the client and reduce the symptoms. This therapy has never been used earlier with patients having schizophrenia in India; therefore, it was taken up for this study. Methodology: A pre-post design with control group was used in the present study. Ten patients were taken. Positive and Negative Syndrome Scale for measuring symptoms of schizophrenia, Schizophrenia Quality of Life Scale for assessing quality of life, Coping Response Inventory-Adult form for assessing coping skills, Apathy Evaluation Scale-Clinician Version for assessing apathy, Family Adaptability and Cohesion Evaluation Scale for assessing perceived interpersonal relationship were used for assessment pre- and postintervention. Fifteen sessions were done with each patient within a period of 10 months at the rate of one session per week. Chi-square test, Wilcoxon signed-rank test, and Mann-Whitney U-test were used for the analysis of data. Results: Significant improvements were found in positive symptoms, general psychopathology, approach coping mechanisms, psychosocial and motivation domains in quality of life, and perceived cohesion in family among the patients postintervention. Conclusion: Functional analytic psychotherapy is an effective method for treating patients having schizophrenia with the application of its specifically modified rules for use with patients having psychosis.

Functional Analytic Psychotherapy Enhanced Behavioral Activation for Depression: A Concurrent and Non-Concurrent Between-Participants Study

Functional analytic psychotherapy enhanced behavioral activation (FEBA) is a therapeutic approach that combines strategies from behavioral activation and functional analytic psychotherapy to improve therapeutic outcomes in clients who lack stable sources of reinforcement in their natural environments. In FEBA, therapists modify clients’ behaviors within the therapeutic session and simultaneously change clients’ environmental conditions in their natural contexts. A concurrent and nonconcurrent multiple-baseline design between participants was conducted. Four participants—2 men and 2 women—who reported depressive symptoms were independently treated in a psychological services center by 2 male therapists. FEBA increased the frequency of healthy behaviors (e.g., engaging in meaningful conversations with friends) in session and out of session. However, depressive behaviors in therapeutic and natural settings did not show stable changes. The implications for implementing some of the results of this study with clients with depression are discussed. Methodological recommendations for using FEBA with specific populations and in other contexts are also presented.

Functional Analytic Psychotherapy for Nursing Home Residents: A Single-Subject Investigation of Session-by-Session Changes

Nursing home settings contain unique environmental factors that may promote the acquisition and maintenance of problematic interpersonal behaviors. Functional Analytic Psychotherapy (FAP) is a contextually-based behavior therapy that can be used to modify problematic interpersonal behaviors via carefully provided and contingent in-session responses of the therapist to client behaviors. Thus, FAP would appear to be a particularly well-suited intervention approach for nursing home residents. To date, the effects of FAP have not been reported for nursing home residents. The current single case study assessed the effects of FAP on three nursing home residents using a single-subject AB design. The AB design consisted of a 2-weeks baseline phase followed by a 4-week FAP treatment phase. The nursing home residents completed pre-treatment and post-treatment measures of interpersonal relating. Client interpersonal behaviors were also recorded during each session and coded into two categories: Maladaptive in-session behaviors and adaptive in-session behaviors. Using the reliable change index and Swanson’s dsw to evaluate questionnaire outcomes, results indicated that all three participants reported a significant change in interpersonal functioning. Analysis of in-session behavior indicated that there were significant decreases in maladaptive behaviors and an increase in adaptive behaviors for two clients. Finally, observed in-session behavior changes were congruent with self-reported changes in interpersonal behavior. These results indicate that FAP was an effective intervention for these nursing home residents.

Children’s mental problems remain one of the central topics in governments’ agendas because of their negative impact at social and economic levels. The present paper is a single case study, with A-B design with concurrent control through behaviors and participants, aimed to identify the effect of the Functional Analytic Psychotherapy (FAP) on four mothers who informed about their children’s disruptive behaviors in the school setting. A case-control who received psychoeducationbased intervention was d in order to observe outcome differences among participants regarding the implemented treatment. Using the FIAT-Q and al analysis, participants’ problematic behaviors were identified. Mothers treated with FAP were expected to display behavioral changes within the sessions and generalized them to the natural context in the interaction with their children in order to promote children’s prosocial behaviors. Results indicated that FAP reduced the frequency of mothers’ problematic behaviors and, in turn, children reduced disruptive behaviors. Implications of the study results are discussed both at theoretical and applied levels.

This study aimed to evaluate the application of Functional Analytic Psychotherapy to an adolescent who was serving probation providing community service. The Functional Analytic Psychotherapy was applied. The criteria for evaluating behavior modification were the increase of desirable behavior (expression of positive feelings, expression of genuine repentance, self-revelation); the decrease of undesirable behavior (hostility, lying, blaming others for their own acts); school attendance, professional training activities, and psychotherapy, as planned in the Statute of the Child and Adolescent. The adolescent participated in ten 50-min sessions, delivered weekly and individually. The psychotherapy was interrupted because the adolescent was killed. Results indicated that therapeutic alliance helped reducing avoidance behaviors and increased the frequency of self-disclosure behaviors and positive feelings expression. The lack of family encouragement, difficulty to access the place where psychotherapy was held, and poor adherence to treatment compromised the development of this intervention. Those factors must be taken into account by clinicians.
