Sex in the Time of COVID-19: Results of an Online Survey of Gay, Bisexual and Other Men Who Have Sex with Men’s Experience of Sex and HIV Prevention During the US COVID-19 Epidemic
This paper presents data from a recent cross-sectional survey of gay, bisexual and other men who have sex with men (GBMSM) in the US, to understand changes in sexual behavior and access to HIV prevention options (i.e. condoms and pre-exposure prophylaxis (PrEP)) during the COVID-19 lockdown period. The Love and Sex in the Time of COVID-19 survey was conducted online from April to May, 2020. GBMSM were recruited through advertisements featured on social networking platforms, recruiting a sample size of 518 GBMSM. Analysis considers changes three in self-reported measures of sexual behavior: number of sex partners, number of anal sex partners and number of anal sex partners not protected by pre-exposure prophylaxis (PrEP) or condoms. Approximately two-thirds of the sample reported that they believed it was possible to contract COVID-19 through sex, with anal sex reported as the least risky sex act. Men did not generally feel it was important to reduce their number of sex partners during COVID-19, but reported a moderate willingness to have sex during COVID-19. For the period between February and April–May 20,202, participants reported a mean increase of 2.3 sex partners during COVID-19, a mean increase of 2.1 anal sex partners (range − 40 to 70), but a very small increase in the number of unprotected anal sex partners. Increases in sexual behavior during COVID-19 were associated with increases in substance use during the same period. High levels of sexual activity continue to be reported during the COVID-19 lockdown period and these high levels of sexual activity are often paralleled by increases in substance use and binge drinking. There is a clear need to continue to provide comprehensive HIV prevention and care services during COVID-19, and telehealth and other eHealth platforms provide a safe, flexible mechanism for providing services.
Mpox Epidemiology and Vaccine Effectiveness, England, 2023
Reported mpox cases in England continued at a low but steady frequency during 2023. Of 137 cases reported in 2023, approximately half were acquired overseas and half were in vaccinated persons. Estimated effectiveness of 2-dose vaccine was 80%, and no vaccinated mpox patient was hospitalized.
Mpox knowledge, behaviours and barriers to public health measures among gay, bisexual and other men who have sex with men in the UK: a qualitative study to inform public health guidance and messaging
Background The 2022-23 mpox epidemic is the first-time sustained community transmission had been reported in countries without epidemiological links to endemic areas. During that period, the outbreak almost exclusively affected sexual networks of gay, bisexual, or other men who have sex with men (GBMSM) and people living with HIV. In efforts to control transmission, multiple public health measures were implemented, including vaccination, contact tracing and isolation. This study examines knowledge, attitudes, and perceptions of mpox among a sample of GBMSM during the 2022-23 outbreak in the UK, including facilitators for and barriers to the uptake of public health measures. Methods Interviews were conducted with 44 GBMSM between May and December 2022. Data were analysed using reflexive thematic analysis. Positive and negative comments pertaining to public health measures were collated in a modified version of a ‘table of changes’ to inform optimisations to public health messages and guidance. Results Most interviewees were well informed about mpox transmission mechanisms and were either willing to or currently adhering to public health measures, despite low perceptions of mpox severity. Measures that aligned with existing sexual health practices and norms were considered most acceptable. Connections to GBMSM networks and social media channels were found to increase exposure to sexual health information and norms influencing protective behaviours. Those excluded or marginalized from these networks found some measures challenging to perform or adhere to. Although social media was a key mode of information sharing, there were preferences for timely information from official sources to dispel exaggerated or misleading information. Conclusions There are differential needs, preferences, and experiences of GBMSM that limit the acceptability of some mitigation and prevention measures. Future public health interventions and campaigns should be co-designed in consultation with key groups and communities to ensure greater acceptability and credibility in different contexts and communities.
Harm Reduction Practices and Needs in a Belgian Chemsex Context: Findings from a Qualitative Study
Chemsex is a growing public health concern, with little evidence-based care and support available. The aim of this study is to understand current risk reduction practices, and the information and care needs of gay, bisexual, and other men who have sex with men (GBMSM) who engage in chemsex. Between January and March 2020, semi structured in-depth interviews with drug-using GBMSM (n = 20) were conducted. Data were analyzed thematically. The reported preparatory measures were: deliberately scheduling chemsex sessions, and discussing preferences regarding setting and attendees. During the event, a logbook is kept to monitor drugs taken by each participant. People try to take care of each other, but this is often counteracted. Respondents highlighted needs: reliable and easily-accessible information, anonymous medical and psychological healthcare, chemsex-specific care, and a value-neutral safe space to talk about chemsex experiences. Results imply two types of users: planned and impulsive users. Adherence to intended harm reduction practices are complicated by drug effects, peer pressure, and feelings of distrust among users.
Mpox on Reddit: a Thematic Analysis of Online Posts on Mpox on a Social Media Platform among Key Populations
The 2022–2023 mpox outbreak has disproportionately impacted gay, bisexual, and other men who have sex with men (GBMSM). The US CDC recommended individuals to explore safer sexual practices that minimize the potential risk of exposure and also strongly encouraged for eligible individuals to prioritize vaccination. This study aimed to analyze social media data related to mpox on Reddit since the mpox outbreak and identify themes associated with the impact on social behaviors and social processes among targeted population. Publicly available data were collected from the social media Reddit. We extracted the summarized mpox-related posts since the beginning of May 2022 from popular subreddits that were popular among GBMSM. We thematically analyzed the content to identify the overall themes related to the GBMSM’s responses to the outbreak. There is an overall increase in the number of daily mpox-related posts, with three upticks in late May, late July, and early August 2022, which may correspond to the dates that the first mpox case was identified in the USA, the WHO declared a global public health emergency, and the US Department of Health and Human Services declared a public health emergency. Four themes were identified: (1) changes in sexual behaviors and social activities; (2) mpox vaccine attitude, uptake, and hesitancy; (3) perceived and experienced stigma and homophobia, and mental distress; and (4) online information-seeking and mutual aid and support. GBMSM changed their sexual behaviors and social activities to mitigate their exposure to the virus during this outbreak and actively sought and shared information about mpox vaccination in their respective settings, while some were hesitant due to concerns about side effects and potential effectiveness. Perceived and experienced stigma and discrimination on gay- and same sex-identify have impacted GBMSM’s mental health. Interventions to promote the mpox vaccine must address the historical medical mistrust and vaccine hesitancy among GBMSM.
Drivers, barriers, and implications of mpox vaccine uptake in Australia among gay, bisexual, and other men who have sex with men: results from a prospective observational mixed-methods study (2022–2024)
Unlike other high-income countries that experienced mpox outbreaks in 2022, Australia had low case numbers before a significant resurgence in 2024, with 99 % of cases occurring in males. Vaccine coverage among gay, bisexual, and other men who have sex with men (GBMSM) was estimated at 50 %. We identified drivers and barriers to vaccine uptake and discuss implications for mpox control in Australia. The TraX study, established in 2022, was a mixed-method prospective observational study of GBMSM and their sexual partners tracking mpox vaccine uptake through weekly surveys. Two multivariable logistic regression models identified predictors of receiving one and two doses. We analyzed qualitative responses from unvaccinated participants using the Framework Method. A total of 3596 participants were recruited through a waitlist, vaccine hubs, and community networks. In 2024, 1224 responded and were included in this analysis. Among community-recruited participants, 82.8 % had received at least one dose, associated with living in areas where ≥5 % of residents were gay men, living with HIV, recent sexually transmitted infections (STIs), greater COVID-19 vaccine history, non-relationship sex, group sex, international travel, and mpox concern. Among all participants, 83.7 % had completed the second dose, associated with older age and non-relationship sex. In qualitative analysis of 57 unvaccinated participants, most cited low perceived risk based on geography or sexual behavior. Reported barriers included difficulty accessing services, limited vaccine knowledge, and concerns about side effects. This study found high uptake of the first mpox vaccine dose and a high rate of second dose completion. Most participants made decisions aligned with public health guidance, though some underestimated their risk. With low vaccine coverage and natural immunity, Australia's 2024 mpox outbreak likely reflected insufficient population-level protection. Australia likely entered 2024 with little infection induced immunity because few cases had occurred in 2022–2023, and with incomplete vaccine coverage among people at ongoing risk. Our findings point to practical steps to prevent future outbreaks by increasing vaccination rates, including addressing side-effect concerns, streamlining vaccination, and improving awareness of personal risk factors. •Weekly surveys among 3596 participants tracked mpox vaccine uptake in Australia.•Highest uptake occurred through government-led waitlists and vaccination hubs.•Vaccine uptake was associated with health, behavioral, and geographic factors.•Some unvaccinated participants underestimated their personal risk.•Australia's 2024 mpox outbreak likely reflected low population-level protection.
A practice already in use: a snapshot survey on the use of doxycycline as a preventive strategy (Doxy-PEP and Doxy-PrEP) in the GBMSM population in Spain
Purpose . While doxycycline shows promise as a preventative measure against certain STIs (post-exposure prophylaxis or PEP, and pre-exposure prophylaxis or PrEP), very few medical and scientific associations favor its community use. Nevertheless, sexual health organizations for gay, bisexual and other men who have sex with men (GBMSM) community have noticed an increase in requests of its use. Methods . The sexual health organization “Stop Sida” in Spain launched an anonymous snapshot survey to evaluate the current use of doxycycline as prevention strategy (both PEP and PrEP) in Spain through its social networks and its support groups in telegram. Results . 150 valid answers were obtained from different parts of Spain. 82 respondents (54,6% of total) had ever taken doxycycline, out of which 29 (35,4%) indicated the use as doxy-PEP and 7 (8,5%) as doxy-PrEP. The self-reported rate of STI diagnoses (mainly of syphilis and chlamydia) in the past 12 months was lower among those participants who reported having used doxy-PEP compared with those who did not report using doxy-PEP. The most common ways to obtain the doxycycline were telling a specialist that they have had a risky contact or by using leftover pills from a previous treatment. Conclusion The current study is the first study to describe the use of doxycycline as a preventive strategy among the GBMSM community in Spain, and the first designed, launched and analyzed entirely by an NGO dealing with sexual health for the GBMSM community. The results obtained are aligned with other studies in other parts of Europe, such as Germany.
A systematic review of mHealth interventions for HIV prevention and treatment among gay, bisexual, and other men who have sex with men
Mobile health (mHealth) interventions are an acceptable and potentially effective means to address the HIV prevention and treatment needs of gay, bisexual, and other men who have sex with men. Abstract The use of mobile health (mHealth) technologies addressing HIV disparities among gay, bisexual, and other men who have sex with men (GBMSM) has increased. A systematic review of mHealth interventions for HIV prevention and treatment among GBMSM was conducted to summarize the current evidence and provide recommendations for future research. PRISMA guidelines were followed (PROSPERO ID: 148452). Studies identified via PubMed, PsychInfo, or Embase were included that (i) were in English, (ii) were published in a peer-reviewed journal prior to July 1, 2019, (iii) presented primary results, (iv) included only GBMSM, and (v) reported the results of an mHealth intervention (e.g., text message, phone/mobile application [app]) to improve HIV prevention or treatment outcomes. Of 1,636 identified abstracts, 16 published studies met inclusion criteria. Eleven studies were conducted in the United States. One study was a fully powered randomized controlled trial (RCT), seven were single-arm pilots with pre–post assessments, four were pilot RCTs, and four tested public health campaigns with post-assessments. Seven developed study-specific apps, five used text messaging, and four used existing social networking apps. Most (81%) targeted prevention outcomes. Nine cited a specific behavioral theory. All studies found that a mHealth approach was feasible and acceptable. All interventions provided evidence of preliminary efficacy or promising trends on primary outcomes. Although mHealth interventions for HIV prevention and treatment appear feasible and acceptable, most published studies are small pilot trials. Additional research assessing the efficacy and mechanisms of mHealth interventions is needed.
Co-creation using crowdsourcing to promote PrEP adherence in China: study protocol for a stepped-wedge randomized controlled trial
Background Adherent pre-exposure prophylaxis (PrEP) uptake can prevent HIV infections. Despite the high HIV incidence, Chinese key populations have low PrEP uptake and adherence. New interventions are needed to increase PrEP adherence among key populations in China. Co-creation methods are helpful to solicit ideas from the community to solve public health problems. The study protocol aims to describe the design of a stepped-wedge trial and to evaluate the efficacy of co-created interventions to facilitate PrEP adherence among key populations in China. Methods The study will develop intervention packages to facilitate PrEP adherence among Chinese key populations using co-creation methods. The study will then evaluate the efficacy of the co-created intervention packages using a stepped-wedge randomized controlled trial. This four-phased closed cohort stepped-wedge design will have four clusters. Each cluster will start intervention at three-month intervals. Seven hundred participants who initiated PrEP will be recruited. Participants will be randomized to the clusters using block randomization. The intervention condition includes receiving co-created interventions in addition to standard of care. The control condition is the standard of care that includes routine clinical assessment every 3 months. All participants will also receive an online follow-up survey every 3 months to record medication adherence and will be encouraged to use a WeChat mini-app for sexual and mental health education throughout the study. The primary outcomes are PrEP adherence and retention in PrEP care throughout the study period. We will examine a hypothesis that a co-created intervention can facilitate PrEP adherence. Generalized linear mixed models will be used for the primary outcome analysis. Discussion Developing PrEP adherence interventions in China faces barriers including suboptimal PrEP uptake among key populations, the lack of effective PrEP service delivery models, and insufficient community engagement in PrEP initiatives. Our study design addresses these obstacles by using co-creation to generate social media-based intervention materials and embedding the study design in the local healthcare system. The study outcomes may have implications for policy and intervention practices among CBOs and the medical system to facilitate PrEP adherence among key populations. Trial registration The study is registered in Clinical Trial databases in China (ChiCTR2100048981, July 19, 2021) and the US (NCT04754139, February 11, 2021).
Modeling the 2022 mpox outbreak with a mechanistic network model
Background The 2022 outbreak of mpox affected more than 80,000 individuals worldwide, most of whom were gay, bisexual, and other men who have sex with men (GBMSM) who likely contracted the disease through close contact during sex. Given the unprecedented number of mpox infections and the new route of infection, there was substantial uncertainty about how best to manage the outbreak. Methods We implemented a dynamic agent-based network model to simulate the spread of mpox in a United States-based GBMSM population. This model allowed us to implement data-informed dynamic network evolution to simulate realistic disease spreading and behavioral adaptations. Results We found that behavior change, the reduction in one-time partnerships, and widespread vaccination are effective in preventing the transmission of mpox, and that earlier vaccination and behavior adaptation has a greater effect, even when only a high-activity portion of the population participates. With no vaccination or behavior adaptation, 16% of the population was infected (25th percentile, 75th percentiles of simulations: 15.3%, 16.6%). With vaccination and behavior change in only the 25% of GBMSM most likely to have a one-time partner, cumulative infections were reduced by 30%, or a total reduction in nearly 500 infections (mean: 11.3%, and : 9.4%, 13.4%). Earlier vaccination and behavior adaptation further reduce cumulative infections; beginning vaccination a year before the outbreak results in only 2.6% of GBMSM being infected, averting 1300 infections or nearly 13% of the total population in our model. We also show that sustained partnerships drive the early outbreak, while one-time partnerships drive transmission after the first initial weeks. The median effective reproductive number, , at days is 1.40 for casual partnerships, 1.00 for main, and 0.35 for one-time. By , the median for one-time partnerships more than tripled to 1.47, while it decreases for casual and main partnerships: 0.37 and 0.19, respectively. Conclusion With the ability to model individuals’ behavior, mechanistic networks are particularly well suited to studying sexually transmitted infections, the spread and control of which are often governed by individual-level action. Our results contribute valuable insights into the role of different outbreak mitigation strategies and relationship types in mpox transmission dynamics.
