Some properties of new general fractal measures
In this research, we adopt a comprehensive approach to address the multifractal and fractal analysis problem. We introduce a novel definition for the general Hausdorff and packing measures by considering sums involving certain functions and variables. Specifically, we explore the sums of the form ∑ i h - 1 ( q h ( μ ( B ( x i , r i ) ) ) + t g ( r i ) ) , where μ represents a Borel probability measure on R d , and q and t are real numbers. The functions h and g are predetermined and play a significant role in our proposed intrinsic definition. Our observation reveals that estimating Hausdorff and packing pre-measures is significantly easier than estimating the exact Hausdorff and packing measures. Therefore, it is natural and essential to explore the relationships between the Hausdorff and packing pre-measures and their corresponding measures. This investigation constitutes the primary objective of this paper. Additionally, the secondary aim is to establish that, in the case of finite pre-measures, they possess a form of outer regularity in a metric space X that is not limited to a specific context or framework.

Reduced gastrointestinal iron uptake in pulmonary arterial hypertension: a prospective cross-sectional study
Background Iron deficiency (ID) is common in patients with pulmonary arterial hypertension (PAH) and is associated with worse clinical outcomes. The etiology of ID in PAH is poorly understood. The aim of this study was to systematically determine whether differences in oral iron absorption exist between PAH patients with and without chronic ID compared with healthy controls. Methods This single-center prospective, cross-sectional cohort study enrolled 45 subjects: 15 PAH patients with chronic ID, 15 PAH patients without ID and 15 healthy age and sex matched controls. Chronic ID was defined by either recorded ID anemia or clinical indication for i.v. iron supplementation in the past 3 years. Plasma iron levels and transferrin saturation (TSAT) were measured before and after a standardized oral iron absorption test with 200 mg ferrous iron. Additional iron and inflammation laboratory parameters were determined. Hepcidin and erythroferrone levels were measured using enzyme-linked immunosorbent assay, and tumor necrosis factor alpha and ferroportin expression were determined by quantitative polymerase chain reaction. Results Both PAH groups showed a similar increase of plasma iron and TSAT after 3 h. The increase in plasma iron and TSAT was significantly lower in both PAH groups (with chronic ID: 71.5 (IQR 44.1–188.8) µg/dl; 22 (IQR 14–49) %; without ID: 86.0 (IQR 27.9–105.6) µg/dl; 26 (IQR 11–42) %) compared to healthy controls (154.1 (IQR 129.0–181.5) µg/dl; 45 (IQR 34–54) %, p  = 0.015 and p  = 0.031, respectively). Conclusions This study is the first to demonstrate a significantly reduced gastrointestinal iron uptake in PAH patients compared to healthy age and sex matched controls. Interestingly, PAH patients with chronic ID showed similar iron uptake levels as those without, suggesting that factors other than iron stores, such as chronic inflammation, may impair iron absorption in this patient population.

Spectral Properties of Ruelle Transfer Operators for Regular Gibbs Measures and Decay of Correlations for Contact Anosov Flows
In this work we study strong spectral properties of Ruelle transfer operators related to a large family of Gibbs measures for contact Anosov flows. The ultimate aim is to establish exponential decay of correlations for Hölder observables with respect to a very general class of Gibbs measures. The approach invented in 1997 by Dolgopyat in “On decay of correlations in Anosov flows” and further developed in Stoyanov (2011) is substantially refined here, allowing to deal with much more general situations than before, although we still restrict ourselves to the uniformly hyperbolic case. A rather general procedure is established which produces the desired estimates whenever the Gibbs measure admits a Pesin set with exponentially small tails, that is a Pesin set whose preimages along the flow have measures decaying exponentially fast. We call such Gibbs measures regular. Recent results in Gouëzel and Stoyanov (2019) prove existence of such Pesin sets for hyperbolic diffeomorphisms and flows for a large variety of Gibbs measures determined by Hölder continuous potentials. The strong spectral estimates for Ruelle operators and well-established techniques lead to exponential decay of correlations for Hölder continuous observables, as well as to some other consequences such as: (a) existence of a non-zero analytic continuation of the Ruelle zeta function with a pole at the entropy in a vertical strip containing the entropy in its interior; (b) a Prime Orbit Theorem with an exponentially small error.

Supportive Therapie der pulmonal arteriellen Hypertonie nach den neuen Leitlinien
In den neuen Leitlinien (LL) für pulmonale arterieller Hypertonie (PAH) sind die allgemeinen Maßnahmen ein integraler Bestandteil der Behandlung der Patienten. Auch die systemischen Auswirkungen der PH und Rechtsherzinsuffizienz sollten angemessen berücksichtigt und behandelt werden. Im folgenden Artikel werden die in den LL genannten Maßnahmen mit Berücksichtigung der bestehenden Empfehlungsgrade und Evidenzen beschrieben. Leider sind die meisten Allgemeinmaßnahmen, wie die Gabe von Diuretika, Sauerstoff, psychosozialer Support und Impfungen nicht oder unzureichend in randomisierten kontrollierten Studien untersucht worden. So haben sie zwar einen hohen Empfehlungsgrad, aber einen niedrigen Evidenzgrad. Nur bei dem spezialisierten körperlichen Training liegen bislang insgesamt 7 randomisierte, kontrollierte Studien und 5 Metaanalysen vor, die eine Verbesserung der maximalen Sauerstoffaufnahme, körperlichen Belastbarkeit, Beschwerden (WHO-Funktionsklasse), Lebensqualität und Hämodynamik nachgewiesen haben (daher neue IA Empfehlung). Auch weitere Maßnahmen wie die Antikoagulation und Eisensubstitution werden im Folgenden besprochen.
