C61 TUBERCULOSIS CASE REPORTS: Koch's With Kikuchi Disease Together In A Young Male
When new cervical nodes develop in an already established case of Tuberculous mass in the mediastinum, it can so easily be passed off as an extension of Koch's rather than suspecting a superimposed condition like Kikuchi disease. A repeat CT scan of chest done on completion of 6 months of ATT had shown complete resolution of the mediastinal lesion.
Status of Histopathology Services in Ethiopia
Abstract Objectives The main purpose of this study is to evaluate the accessibility of pathology services in Ethiopia. Methods Checklist was prepared for the assessment of the availability of histopathology services in Ethiopia, types of equipment, turnaround time (TAT), and intended number of population to use it. Results There are 13 histopathology laboratories in the Ethiopia and a survey was conducted at all facilities in the nation. Out of 13 institutions, 7 (53.8%) of them are located in the capital, Addis Ababa. Six (46.2%) of them are out of the capital located in four different regions. No service is given in regions of afar, Somali, Benishangul-gumuz, Gambella, Harari, and Diredawa city administration where 40,239,100 population resides. Conclusion There are very limited histopathology laboratories in the country; as a result, diagnosis and prevention of cancer diseases are inadequate throughout the country.
Confocal Endomicroscopy of Solid Pseudopapillary Tumor of the Pancreas
A 69-year-old woman was found to have a 1.9 cm partially cystic hypodense lesion of the pancreatic tail with a 6-mm calcification on magnetic resonance imaging upon presenting with an initial episode of acute pancreatitis. EUS identified an oval 20 mm solid-cystic mass in the pancreatic tail with hyperechoic foci (Video 1). Patient was enrolled in an ongoing single center prospective study (NCT02516488) evaluating needle-based confocal laser endomicroscopy (nCLE) for the evaluation of pancreatic cysts. EUS-nCLE of the lesion was performed with an AQ-Flex-19 miniprobe (Cellvizio; Mauna Kea Technologies Inc, Paris, France). This demonstrated dark trabeculae and clusters of cells separated by interstitial space (Figure 1A). Fine needle aspiration (FNA) immunostaining and presence of CTNNB1 mutation revealed a diagnosis of solid pseudopapillary tumor (SPT) of the pancreas. The patient underwent a robotic distal pancreatectomy with splenectomy. Histopathology demonstrated pseudopapillary arrangements of neoplastic cells separated by vascular stalks and areas of hyalinization (Figure 2). Nuclear staining was present for beta-catenin, SOX11, and TFE3, consistent with a pancreatic SPT. Negative chromogranin and weak synaptophysin positivity differentiated this lesion from a neuroendocrine tumor (NET). Pancreatic SPTs are rare masses primarily affecting women in the second and third decade of life.1 EUS-nCLE features of this lesion are similar to those previously described in cystic-NETs of the pancreas (Figure 1B). Both lesions demonstrate trabeculae of cells separated by interstitial spaces and vasculature.2 Similar to EUS-nCLE imaging, SPTs are difficult to differentiate from pancreatic NETs on histopathology without immunostaining.1 SPTs demonstrate solid and pseudopapillary structures of poorly cohesive cells forming around blood vessels with background cystic spaces and hyalinization.1 This is the first report of EUS-nCLE features delineating a SPT and suggesting similarities with cystic-NETs.
417 Endometrial stromal sarcoma-how to treat recurrance?
ObjectivesEndometrial stromal sarcoma(ESS) is a rare gynaecological malignancy accounting only.2percent of uterine neoplasm.According to WHO, one third of the ESS recurs.But there is no protocol to treat recurrent ESS. This study is to find out a standard protocol of treatment incase of recurrent endometrial stromal sarcoma.MethodsIts a retrospective study of nineteen patients of ESS attending NICRH.All had hysterectomy outside our hospital and came with histopathology report of ESS.Time period was three years (2016–2018).ResultsThe recurrence rate of nineteen cases were42.1percent.The median age of diagnosis was47.55 years and median time of first relapse was9 months.The number of recurrance ranges from 1 to 5.Incase of first relapse,second surgery was done in eight patients but three had third relapse.out of eleven patients who received adjuvant chemotherapy,six had relapse and out of seven patients who had adjuvant radiation,five had recurrance.As all primary histopathology was done outside NICRH,grade was undetermined in eleven patients.Out of four high grade patients,three had relapse.Incase of four low grade, two received aeromatase inhibitor and they remain relapse free.ConclusionsIn our study,it is clear that neither secondary cytoreduction nor adjuvant chemoradiation can prevent recurrence.There is limited data of immuno or target therapy.So long term multicentric study should be performed to set up a standard guideline of treatment in case of recurrent endometrial stromal sarcoma.
