A SCOPING REVIEW ON BARRIERS AND FACILITATORS OF COMMUNITY ENGAGEMENT IN LEARNING HEALTH SYSTEMS

Abstract
This scoping review identifies the challenges and enablers of community engagement in Learning Health Systems (LHS), an area where elder and health care have growing interests on. LHS aims to improve healthcare delivery, translate knowledge into clinical practice, and emphasize patient-centred outcomes. We followed the Joanna Briggs Institute’s (JBI) scoping review methodology. First, keywords and index terms were identified from MEDLINE and CINAHL databases. Second, using the keywords and index terms identified in the first step, other databases like Web of Science, PsycINFO, and Google were searched. Third, the reference lists of the chosen literature were searched. The results include 25 papers. The Assessing Community Engagement (ACE) Conceptual Model by Aguilar-Gaxiola et al. was utilized to inform the analysis. Active community engagement enhances the relevancy and applicability of health research and fosters trust and rapport between healthcare providers and their communities. It is essential to have appropriate support, mutual respect, culturally sensitive and locally relevant processes with common-ground language. We identified several barriers to engagement: 1) meaningful motivations, 2) logistical challenges in processes, 3) disparities in power dynamics, 4) cultural misunderstanding, and 5) lack of knowledge and skills. The review underscores the need to move beyond tokenistic engagement towards genuine community collaborations. Future research should investigate the facilitators that enhance such meaningful partnerships.

P317 Implementation of continuous quality initiative for improving key indicators in HIV treatment cascade in western nigeria

BackgroundContinuous Quality Improvement (CQI) is a quality management process that encourages all health care team members to continuously ask critical questions especially using CQI initiative that employs a Plan-Do-Study-Act (PDSA) cycle to test a proposed change or initiative. Granular Site Management (GSM) was established to enhance identification of innovations or best practices and scale across facilities and ensuring resources application efficiency. The aim of the study was thus to improve key indicators in HIV program treatment cascade in Western Nigeria.MethodsThe CQI implementation was in four selected (secondary & tertiary) facilities where a cumulative 6000 patients living with HIV (PLHIV) in care. The major drivers of poor performance on key program indicators were identified with underlying causes, in-depth analysis & review of performance done then using CQI approach to implement change strategies for improvement, monitor and periodically evaluate change ideas for improved outcomes.ResultsA total of four facilities were included in this study. Escort service was implemented for all newly identified HIV positive patients which made linkage to care improve from 50% to 95% within a space of six months. Task shifting & sharing, improved health education for clients, introduction of biometrics capturing for all clients and creation of additional hub for sample logging to viral load reference laboratory all helped to improve viral load uptake & suppression from < 30% & < 80% to 79% & > 80% respectively. Other CQI initiatives also greatly improved the positivity rate, total number of positives placed on treatment and retention in care.ConclusionThis CQI initiative using GSM approach has been used to achieve peer learning and cross fertilization of change ideas among facilities thus encouraging them to innovate and have a problem solving approach to achieve programmatic best practices thus ensuring program & resources application efficiency.DisclosureNo significant relationships.

“One Belt – One Road” – the Chinese initiative to create the Silk Road Economic Belt (SREB) and the Maritime Silk Road (MSR) was first announced by the Chairman of the People Republic of China Xi Jinping during his official visit to Kazakhstan and Indonesia in 2013. The main goal of “One Belt – One Road” initiative lies in the exploration, formation, and promotion of a new model for international cooperation and development through strengthening of current regional bilateral and multilateral mechanism and structures of collaboration with the participation of China based on maintenance and development of the spirit Ancient Silk Road. Central Asia for many centuries was a dynamic center linking regional and international communities via the historic Silk Road. Nowadays Central Asia is tremendously important for China for several reasons. Firstly, Central Asia is the gate for China to the West. All land routes which going from China to Europe or South Asia passing through Central Asian countries. Secondly, three of Central Asian countries share a border with China. There are traditional -cultural links between peoples on both sides of the border. Lastly, Central Asian countries are rich in natural resources especially in hydrocarbon reserves which is more important for the lifeline of the Chinese economy. Silk Road to Belt Road : reinventing the past and shaping the future
This volume approaches China's Belt and Road Initiative as a process of culturalization, one that started with the Silk Road and continued over the millennium. In mainstream literature, the Belt and Road Initiative (BRI) has been portrayed as the geo-economic vision and geo-political ambition of China's current leaders, intended to shape the future of the world. However, this volume argues that although geo-politics and geo-economy may play their part, the BRI more importantly creates a venue for the meeting of cultures by promoting people-to-people interaction and exchange.
