INTERDISCIPLINARY APPROACH TO TRANSITIONS OF CARE
Purpose: The purpose of this ongoing project is to allow for seamless transition of the oncology patient from inpatient to outpatient care. An additional purpose is to improve metrics including increasing discharge to home and assuring follow up appointments within 7 days. Significance: Successful transitions of care for cancer patients is essential. As the care of this population can be quite complex, it is important to assure seamless transitions. The success of transitions is dependent on the interdisciplinary team coordinating the care and each member taking ownership for their role in the process. Methods: An interdisciplinary group consisting of nurses, providers, pharmacist, therapists, case managers, social workers, financial advisors, transition navigator and scheduler meet Monday through Friday to review all oncology inpatients to support transitions of care. All members of the team participate and discuss their role in this process. The team discuss barriers and solutions to overcome these barriers. Approximately 40 patients are reviewed during the rounds. The rounds are half an hour and scripted based on discipline. In addition to medical milestones needed to be reached for discharge, issues such as AMPAC scores and need for therapy consultation, new medications needing patient education and potential pre authorization, rides to and from appointments and social needs. Findings and Interpretations: The team has been in place for many years, but has made enhancements to achieve metrics. A transition nurse navigator is a new addition. The role is consulted for newly diagnosed cancer patients and at risk individuals. A patient service associate comes to rounds to assure appointments are made real time prior to discharge. Each patient has an appointment prior to discharge with a range of 72 hours to 7 days post discharge based on need. Discussion: Oncology Transitions of Care Rounds Team who won the Award for Excellence in Interprofessional Collaborative Practice for 2024. This presentation will include a discussion of the process, individual roles and outcomes related to discharge to home and timely appointment followup
Exploring transdisciplinarity in art and sciences

Mind style and cognitive grammar : language and worldview in speculative fiction
Mind Style and Cognitive Grammar advances our understanding of mind style: the experience of other minds, or worldviews, through language in literature. This book is the first to set out a detailed, unified framework for the analysis of mind style using the account of language and cognition set out in cognitive grammar. Drawing on insights from cognitive linguistics, Louise Nuttall aims to explain how character and narrator minds are created linguistically, with a focus on the strange minds encountered in the genre of speculative fiction. Previous analyses of mind style are reconsidered using cognitive grammar, alongside original analyses of four novels by Margaret Atwood, Kazuo Ishiguro, Richard Matheson and J.G. Ballard. Responses to the texts in online forums and literary critical studies ground the analyses in the experiences of readers, and support an investigation of this effect as an embodied experience cued by the language of a text. Mind Style and Cognitive Grammar advances both stylistics and cognitive linguistics, whilst offering new insights for research in speculative fiction.
