Artificial Intelligence in Cosmetic Dermatology with Regard to Laser Treatments: A Comparative Analysis of AI and Dermatologists’ Decision-Making

Introduction: Artificial intelligence (AI) has developed into an increasingly important tool in dermatology. While new technologies integrated within laser devices are emerging, there is a lack of data on the applicability of publicly available AI models. Methods: The prospective study used an online questionnaire where participants evaluated diagnosis and treatment for 25 dermatological cases shown as pictures. The same questions were given to AI models: ChatGPT-4o, Claude Sonnet 4, Gemini 2.5 Flash, and Grok-3. Results: Dermatologists outperformed AI in diagnostic accuracy (suspected primary diagnosis-SD 75.6%) in pooled dermatologists vs. pooled AI (SD 57.0%), with laser specialists achieving the highest accuracy (SD 82.0%) and residents the lowest (SD 66.0%). There was a high heterogeneity across AI models. Gemini approached dermatologist performance (SD 72.0%), while Claude showed a low accuracy (SD 40.0%). While AI models reached near 100% accuracy in some classic/common diagnoses (e.g., acne, rosacea, spider angioma, infantile hemangioma), their accuracy dropped to near 0% on rare or context-dependent cases (e.g., blue rubber bleb nevus syndrome, angiosarcoma, hirsutism, cutaneous siderosis). Inter-rater agreement was high among laser experts in terms of diagnostic accuracy and treatment choice. Agreement between residents and AI models was highest for diagnostic accuracy and treatment choice, while it was lowest between experts and AI models. Conclusions: Before AI-based tools can be integrated into daily practice, particularly regarding diagnosis and appropriate laser treatment recommendations, specific supervised medical training of the AI model is necessary, as open-source platforms currently lack the ability to contextualize presented data.

CO[sub.2] laser processing has emerged as an efficient dry-finishing technique capable of inducing controlled chemical and morphological transformations in cotton and denim textiles. The strong mid-infrared absorption of cellulose enables localised photothermal heating, leading to selective dye decomposition, surface oxidation, and micro-scale ablation while largely preserving the bulk fabric structure. These laser-driven mechanisms modify colour, surface chemistry, and topography in a predictable, parameter-dependent manner. Low-fluence conditions predominantly produce uniform fading through fragmentation and oxidation of indigo dye; in comparison, moderate thermal loads promote the formation of carbonyl and carboxyl groups that increase surface energy and enhance wettability. Higher fluence regimes generate micro-textured regions with increased roughness and anchoring capacity, enabling improved adhesion of dyes, coatings, and nanoparticles. Compared with conventional wet processes, CO[sub.2] laser treatment eliminates chemical effluents, strongly reduces water consumption and supports digitally controlled, Industry 4.0-compatible manufacturing workflows. Despite its advantages, challenges remain in standardising processing parameters, quantifying oxidation depth, modelling thermal behaviour, and assessing the long-term stability of functionalised surfaces under real usage conditions. In this review, we consolidate current knowledge on the mechanistic pathways, processing windows, and functional potential of CO[sub.2] laser-modified cotton substrates. By integrating findings from recent studies and identifying critical research gaps, the review supports the development of predictable, scalable, and sustainable laser-based cotton textile processing technologies.

We explore the wettability modulation induced on alumina (Al[sub.2]O[sub.3]) targets by femtosecond laser texturing to demonstrate the stable and durable hydrophilic character of the surface. Specifically, we identify a suitable operational regime to tailor micro-nanostructures onto Al[sub.2]O[sub.3] plates and accurately assess the ablation threshold in our experimental conditions. A periodic geometry with triangular patterns of various groove depths, ranging from 3.2 ± 0.1 to 17.1 ± 0.1 µm, was optimized for establishing a long-term wetting response. The latter was monitored on daily basis over a time interval exceeding 40 days by collecting the contact angle measurements of samples with and without a post-process thermal annealing, adopted to stabilize the surface wettability soon after the laser treatment. The results show that deeper grooves significantly enhance and maintain the hydrophilic character, particularly in samples without post-process thermal annealing, where superhydrophilicity (θ < 5°) is demonstrated to persist the entire time throughout the test. These findings disclose the potential for an effective fine-tuning of the alumina wettability, thus opening up the possibility of specific applications requiring long-term control of surface–liquid interactions, such as biomedical implants, and orthopedic and dental prostheses.

Perifoveal anomalous exudative vascular complex (PEVAC) was first described in 2011. Since then, individual clinical cases and a couple of case series have been published, and their characteristics have been studied in multi-modal images. To date, there is no consensus on its treatment. Initially, it was thought that PEVACs could be left to spontaneous evolution because they presented a slow progression. But it has been shown that the growth of the lesion covers a spectrum from non-exudative lesions to PEVAC with vision impairment over time. We present an updated bibliographic review of this pathology. We explain the changes in the diagnostic criteria that have been undergone. And we focus the discussion on selective treatment with focal laser, which has shown an excellent anatomical response and visual improvement or stabilization in the cases described and in our series of three patients systematically treated with focal laser. Background
Keloids are pathologic scars that pose a significant functional and cosmetic burden. They are challenging to treat, despite the multitude of treatment modalities currently available.
Objective
The aim of this study was to conduct an evidence-based review of all prospective data regarding keloid treatments published between 2010 and 2020.
Methods
A systematic literature search of PubMed (National Library of Medicine), Embase (Elsevier), and Cochrane Library (Wiley) was performed in November of 2020. Search strategies with the keywords “keloid” and “treatment” were performed by a medical librarian. The search was limited to prospective studies that were peer-reviewed, reported on clinical outcomes of keloid therapies, and were published in the English language between January 1, 2010, and November 24, 2020.
Results
A total of 3462 unique citations were identified, of which 108 studies met inclusion criteria. Current literature supports silicone gel or sheeting with corticosteroid injections as first-line therapy for keloids. Adjuvant intralesional 5-fluorouracil (5-FU), bleomycin, or verapamil can be considered, although mixed results have been reported with each. Laser therapy can be used in combination with intralesional corticosteroids or topical steroids with occlusion to improve drug penetration. Excision of keloids with immediate post-excision radiation therapy is an effective option for recalcitrant lesions. Finally, silicone sheeting and pressure therapy have evidence for reducing keloid recurrence.
Conclusions
This review was limited by heterogeneity of subject characteristics and study outcome measures, small sample sizes, and inconsistent study designs. Larger and more robust controlled studies are necessary to further understand the variety of existing and emerging keloid treatments, including corticosteroids, cryotherapy, intralesional injections, lasers, photodynamic therapy, excision and radiation, pressure dressings, and others.

PurposeBreast cancer-related lymphedema (BCRL) represents a lifelong risk for breast cancer survivors and once acquired becomes a lifelong burden. This review summarizes current BCRL prevention and treatment strategies.FindingsRisk factors for BCRL have been extensively studied and their identification has affected breast cancer treatment practice, with sentinel lymph node removal now standard of care for patients with early stage breast cancer without sentinel lymph node metastases. Early surveillance and timely management aim to reduce BCRL incidence and progression, and are further facilitated by patient education, which many breast cancer survivors report not having adequately received. Surgical approaches to BCRL prevention include axillary reverse mapping, lymphatic microsurgical preventative healing (LYMPHA) and Simplified LYMPHA (SLYMPHA). Complete decongestive therapy (CDT) remains the standard of care for patients with BCRL. Among CDT components, facilitating manual lymphatic drainage (MLD) using indocyanine green fluorescence lymphography has been proposed. Intermittent pneumatic compression, nonpneumatic active compression devices, and low-level laser therapy appear promising in lymphedema management. Reconstructive microsurgical techniques such as lymphovenous anastomosis and vascular lymph node transfer are growing surgical considerations for patients as well as liposuction-based procedures for addressing fatty fibrosis formation from chronic lymphedema. Long-term self-management adherence remains problematic, and lack of diagnosis and measurement consensus precludes a comparison of outcomes. Currently, no pharmacological approaches have proven successful.ConclusionProgress in prevention and treatment of BCRL continues, requiring advances in early diagnosis, patient education, expert consensus and novel treatments designed for lymphatic rehabilitation following insults.

Purpose: To determine rates of enhancement and visual prognosis following photorefractive keratectomy (PRK) enhancement of small-incision lenticule extraction (SMILE). Patients and Methods: This retrospective, single-site study reviewed all cases of primary SMILE at Hoopes Vision in Draper, Utah between March 14, 2017 and April 8, 2022 to identify any cases that required follow-up enhancement. Primary SMILE was performed using Visumax 500 kHz femtosecond laser (Carl Zeiss Meditec, Jena, Germany). All enhancements were performed with alcoholassisted PRK, using a WaveLight EX500 excimer laser (Alcon Laboratories, Inc., Fort Worth, TX). Results: Four hundred and five eyes underwent primary SMILE, of which 15 later underwent PRK enhancement (enhancement rate of 3.7%). No significant difference in pre-SMILE data was identified between the enhancement and non-enhancement groups. The average age of those who underwent PRK enhancement was 33.8[+ or -]6.3 years old and ranged from 25 to 45. Following primary SMILE, 13 eyes (87%) had an uncorrected distance visual acuity (UDVA) of 20/40 or better, and none had a UDVA of 20/20 or better. After one year of post-enhancement follow-up, all eyes had a UDVA of 20/40 or better, and 13 eyes (87%) had a UDVA of 20/20 or better (Figure 1). All were within one diopter of target spherical equivalent (SEQ), 13 (87%) were within 0.50 D, and 10 (67%) were within 0.25 D. Of those with 12-month follow-up data, none had UDVA worse than corrected distance visual acuity (CDVA), and none had lost lines of CDVA. Efficacy and safety indices were 1.03 and 0.99, respectively. Conclusion: Following SMILE, ophthalmologists may anticipate an enhancement rate of one to seven percent. In these cases, PRK is a safe and effective procedure for enhancement of SMILE.
