Contextual factors related to aging determine force-based manipulation dosage: a prospective cross-sectional study

Background
Contextual factors influence clinicians’ delivery of force-based manipulation (FBM), like spinal manipulative therapy (SMT). It is particularly important to discern how contextual factors interact with therapeutic forces delivered to an older adult population, to minimize risk and identify ideal dosage. This study aimed to determine whether contextual factors pertaining to aging result in the modulation of kinetic and kinematic parameters used by experienced clinicians when delivering SMT.
Methods
Participants were randomly presented with a series of 12 AI-generated patient vignettes, featuring both visual and auditory content and representing varying age-related contextual factors. Factors included chronological (35-, 65- and 85-year-old), pathological (“healthy” vs degenerative spine), and felt (perceived as “young” vs. “old”) age. Participants delivered SMT to a human analogue manikin based on each vignette, presented six times in randomized order. Kinetic and kinematic parameters were collected and analyzed for differences between “young” and “old” contextual factors of age, using a 3-way repeated measures ANOVA model.
Results
Sixteen licensed chiropractors (8 female, 8 male) participated, with an average age of 45.4 (SD = 9.7, range 34–64) years and 18.3 (SD = 10.8, range 5–39) years of experience. A main effect in peak force was found for both chronological (F(

Conclusion
Contextual factors of aging modulated certain kinetic and kinematic characteristics when delivering SMT. This provides evidence that practitioners differentially discern aspects of aging to inform how they deliver FBM dosage. Future research is needed to identify ideal kinetic and kinematic characteristics based on considerations of aging.

Which lumbar spinal stenosis patients will improve with nonsurgical treatment? A secondary analysis of a randomized controlled trial

Background
Lumbar spinal stenosis (LSS) can be disabling and is a leading reason for spinal surgery in older adults. While nonsurgical treatments are recommended as first-line treatment, it remains unclear which patients will benefit most.
Purpose
To identify patient characteristics associated with larger improvements or larger treatment effects among adults receiving nonsurgical LSS interventions.
Design
Secondary analysis of a randomized controlled trial.
Setting
Outpatient research clinics.
Subjects
216 older adults with symptomatic LSS.
Methods
Participants, recruited from November 2013 to June 2016, were randomized to receive: (1) manual therapy with an individualized exercise program (MTE); (2) a group exercise (GE) program; or (3) medical care (MC). We evaluated the association of baseline characteristics with 2-month change in primary outcomes: symptoms and function on the Swiss Spinal Stenosis questionnaire (SSSQ); and walking capacity in meters (m) on the self-paced walking test (SPWT). Baseline characteristics included sociodemographic and clinical variables. To explore heterogeneity of treatment effects, we evaluated unadjusted stratified estimates when comparing MTE to GE/MC. Additionally, we included an interaction term in models to test for statistical interaction.
Results
At baseline, participants (mean age = 72, 54% female, 23% non-white) had moderate LSS-related symptoms/impairment (mean SSSQ score = 31.3) and limited walking capacity on SPWT (mean = 451 m). The overall improvement on SSSQ was 2.5 points with larger improvements observed among younger, non-white, non-smoking participants, and those with worse baseline LSS or back-related symptoms/impairment. Overall improvement on the SPWT was 205 m with larger improvements observed among younger participants, those with higher baseline physical activity levels and participants without knee osteoarthritis. For SSSQ, the treatment effect was larger among adults aged < 70 versus older adults (MTE vs. GE/MC; mean difference [MD] = − 4.06, 95% CI = − 6.29 to − 1.83 vs. MD = − 0.47. 95% CI = − 2.63 to 1.69, respectively;

Conclusions
In a sample receiving nonsurgical treatments for LSS, we identified patient-level characteristics associated with larger improvements and/or treatment effects. If confirmed in larger randomized controlled trials, these findings may guide clinical decision-making to enhance clinical outcomes.

A systematic review on the associations between low back pain and frailty in community-dwelling older adults

Background
Frailty and low back pain (LBP) may negatively affect each other in older adults, yet no systematic review has summarized their cross-sectional, temporal, or causal associations. Exploring these associations could provide valuable insights for reducing frailty risk in older adults with LBP. This systematic review aimed to consolidate evidence on the association between frailty and LBP in older adults.
Methods
Four databases (EMBASE, CINHAL, MEDLINE, and SPORTDiscus) were systematically searched from their inception until July 31, 2024. Studies investigating the association between LBP, regardless of chronicity, or LBP-related disability and frailty in older adults were included. LBP was defined as pain occurring between the 12th ribs and inferior gluteal folds. Due to the lack of consensus on the definitions of older adults or frailty, studies were included based on the authors’ definitions. Three pairs of independent reviewers screened abstracts and full texts, extracted data, assessed risk of bias, and determined the certainty of evidence.
Results
Out of 1,690 articles identified, six cross-sectional studies and one prospective study were included. Low-certainty evidence from four cross-sectional studies suggested that both acute and chronic LBP, with odds ratios from 1.34 to 7.50, separately showed significant correlation with frailty. Pre-frail or frail older adults reported higher chronic LBP intensity, scoring 0.5 to 0.8 points more on the numeric rating scale, and greater LBP-related disability, with scores 1.7 to 7.2 points higher on the Roland Morris Disability Questionnaire, compared to non-frail counterparts. However, there was low-certainty evidence that acute LBP intensity was unrelated to frailty. Very low-certainty evidence from the prospective study indicated that higher acute LBP intensity and disability were associated with transitioning from non-frail to prefrail or frail status.
Conclusions
Our systematic review revealed that older adults with higher LBP intensity or associated disability were more likely to have prefrail or frail status, albeit with low-certainty evidence. However, the findings are limited by the small number of studies, especially prospective research. Future high-quality research should clarify the causation between LBP intensity or disability and frailty in community-dwelling older adults. Research should also explore potential mediators or moderators influencing the LBP-frailty association. These findings could help develop effective prevention and rehabilitation strategies to mitigate the impacts of LBP on frailty, or vice versa.

“They knew how to take care of people”: a qualitative study on older adults with chronic low back pain perspectives of an exercise plus education program

Background
Exercise therapy and education are first-line care for those with chronic low back pain (CLBP), but information on its applicability in older adults is limited, and adherence to non-pharmacological treatments in this population is challenging. This study explored perspectives of older adults with CLBP on a group-based exercise plus education program supported by text messages—PAT-Back.
Methods
This descriptive qualitative study was embedded in a feasibility randomized controlled trial (RCT) with older adults aged ≥ 60 with CLBP who participated PAT-Back. PAT-Back consisted of pain education, group and home-based exercises, with the latter being supported by text messages. After the feasibility RCT finished, we conducted 14 (13 women, 1 man) semi-structured interviews with participants. Interview questions were designed to elicit participants’ reflections on the program, including its impact on symptoms and factors that facilitated and hindered participation. Transcripts were thematically analyzed.
Results
We identified four themes related to participants’ perspectives on PAT-Back: 1)
It takes more than intrinsic motivation
: Participation in the PAT-Back program was linked to family and peer social support, clinician’s supervision, caring clinicians and inaccessibility to treatments; 2)
PAT-Back exceeded expectations
: Before the intervention older adults did not expect or believed exercise would reduce pain, but this changed as they experienced improvements while engaging in the program; 3)
Coexisting comorbidities, challenges with mobility and transport got in the way
: at times, having pain in other body sites and not having access to transport and/or safe environments hindered exercise participation; 4)
Technology can be both helpful and challenging:
motivational text messages were perceived as beneficial by some, but others highlighted challenges related to access and technology literacy.
Conclusions
Older adults’ experiences in exercise and education programs for CLBP may be enhanced by social support (from clinicians, peers, family members), access to healthcare and embodied experiences of improvement. Such experiences can also be hindered by comorbidities and broader factors, such as safety to exercise outdoors. Exercise and education interventions for this population should be tailored at the conception phase to meet their needs, especially regarding social support and environmental infrastructure to promote participation and improve symptoms.

Differences in demographics and clinical outcomes in older, middle-aged, and younger adults with low back pain receiving chiropractic care

Background
The evidence on age-related differences in outcomes in patients seeking chiropractic care for low back pain is limited. The aims of this study were to (i) to explore differences in patient characteristics and symptoms between older, middle-aged and younger patients with LBP seeking chiropractic care, (ii) to investigate whether age was associated with changes in physical function at 2, 13 and 52 weeks follow-up and (iii) to evaluate if other specific demographic variables were associated with changes in physical function over time.
Methods
This observational cohort study (November 2016 to December 2018) used data from the Danish Chiropractic Low Back Pain Cohort (ChiCo). Participants ≥ 18 years seeking chiropractic care for new onset low back pain were categorised into three age groups: young adults (< 40 years), middle-aged adults (40–59 years), and older adults (≥ 60 years). Disability was assessed at baseline and at 2, 13 and 52 weeks follow-up using the Roland Morris Disability Questionnaire. Associations between age groups and disability outcomes were analysed using linear regression, while associations with demographics, and social and psychological factors were examined using backward stepwise linear regression.
Results
2777 participants were included. At baseline, there were no significant differences in disability scores between age groups. Older patients reported more non-musculoskeletal comorbidities and prescription pain medication use, compared to younger and middle-aged patients. Younger patients showed higher depression and anxiety levels compared to middle-aged and older patients. Younger and middle-aged patients more frequently reported multiple musculoskeletal comorbidities than older patients. At all follow-ups, older patients had slightly higher disability scores and showed less improvement over time compared to younger and middle-aged patients, indicating a modest association between age and poorer outcomes. Higher baseline disability, more musculoskeletal comorbidities, worse self-reported health, and higher depression scores were more consistently associated with less improvement in disability over time.
Conclusion
Older chiropractic patients with low back pain had slightly higher disability scores compared to younger and middle-aged patients, but age was not the strongest factor associated with disability outcomes. Baseline disability, depression, self-perceived general health, and MSK comorbidities were more consistently linked to higher disability scores across all follow-up time points.

Prognostic ability of the sTarT back screening tool for disability and pain intensity outcomes in older adults with low back pain seeking chiropractic care: a multi-national external validation study

Background
Low back pain (LBP) is common among older adults, and it is a frequent reason for seeking chiropractic care. The STarT Back Screening Tool (SBT) was developed to stratify patients with LBP into low, medium, and high-risk treatment pathways, so that the treatment can be matched to each participant’s risk profile. But its prognostic performance varies across settings and populations. No studies have focused on the SBT’s utility as a stratified-care tool in older adults with LBP in a chiropractic setting. Therefore, our aim was to evaluate the ability of the SBT to predict three-, six-, and 12-month disability and pain outcomes in older adults (≥55 years) with a new episode of LBP consulting chiropractors in the Netherlands, Sweden, and Australia.
Methods
This was a secondary analysis of the Back Complaints in Older Adults – Chiropractic (BACE-C) cohort. Participants visiting chiropractors with LBP completed baseline questionnaires for demographic and clinical characteristics, including the SBT. Follow-up questionnaires assessed disability (Roland Morris Disability Questionnaire (RMDQ)) and pain intensity (11-point Numerical Rating Scale (NRS)). “No improvement” on disability and pain intensity was defined as less than 30% reduction in baseline scores. We used logistic regression models to estimate discrimination metrics including the area under the receiver operating characteristic curve (AUC). Subgroup analyses were conducted by country, sex, and LBP duration; sensitivity analyses employed alternative “no improvement” definitions and linear regression on continuous outcome scores.
Results
A total of 738 participants were included. The mean age of the study sample was 66.2 ± 7.5 years and 50.9% of the participants were female. The SBT showed poor discrimination for predicting no improvement in disability and pain intensity. All AUC values were below 0.60 regardless of whether SBT risk subgroups (i.e. low/medium/high) or the SBT sum score were used. Subgroup and sensitivity analyses did not meaningfully improve discrimination.
Conclusion
The SBT presented limited prognostic ability to predict outcomes of disability and pain intensity in older adults with LBP in a chiropractic setting. These findings suggest insufficient evidence for the prognostic ability of the SBT risk stratification tool. Future research should explore reasons behind the limited prognostic accuracy and consider potential modifications or alternative tools.

The association between individual radiographic findings and improvement after chiropractic spinal manipulation and home exercise among older adults with back-related disability: a secondary analysis

Some chiropractors use spinal x-rays to inform care, but the relationship between radiographic findings and outcomes is unclear. This study examined the association between radiographic findings and 30% improvement in back-related disability in older adults after receiving 12 weeks of chiropractic spinal manipulation and home exercise instruction.
Methods
This IRB-approved secondary analysis used randomized trial data of community-dwelling adults age
≥
65 with chronic spinal pain and disability. Data were collected during the parent trial between January 2010-December 2014. The primary outcome of the parent study was ≥ 30% improvement in Oswestry Disability Index (ODI) at 12 weeks, a clinically important response to care. In this secondary analysis, two chiropractic radiologists independently assessed digital lumbar radiographs for pre-specified anatomic, degenerative, and alignment factors; differences were adjudicated. The unadjusted association between baseline radiographic factors and 30% ODI improvement was determined using chi-square tests.
Results
From the parent trial, 120 adults with baseline lumbar radiographs were included in this study. Mean age was 70.4 years (range 65–81); 59.2% were female. Mean baseline disability (ODI = 25.6) and back pain (5.2, 0–10 scale) were moderate. Disc degeneration (53.3% moderate, 13.3% severe), anterolisthesis (53.3%), retrolisthesis (36.6%) and scoliosis (35.0%) were common among the participant sample. After 12-weeks of treatment, 51 (42.5%) participants achieved 30% improvement in back disability. No alignment, degenerative, or anatomic factors were associated with ODI improvement at 12 weeks (all
p
> 0.05), regardless of severity of radiographic findings.
Conclusion
We found no association between a predetermined subset of radiographic findings and improvement in back-related disability among this sample of older adults. As such, this study provides preliminary data suggesting that imaging may be unhelpful for predicting response to chiropractic spinal manipulation and home exercise.

No drugs, more sex? And Rock’n Roll: effective non-operative treatments and practical management strategies for older adults with lumbar spinal stenosis

Lumbar spinal stenosis is a growing problem among older adults, associated with significant disability and socio-economic burden. Neurogenic claudication is the most common clinical syndrome caused by LSS with pain being the predominant symptom and limited walking the main impairment. Lumbar spinal stenosis can also impact sexual function in older adults, necessitating greater awareness of this association. Pain and impaired function can lead to psychosocial distress, including hopelessness, anxiety, and isolation, further compounding disability. Recent clinical practice guidelines recommend non-operative treatment as the first-line approach, including manual therapy, exercise, and education. However, these guidelines lack details on the practical application of these interventions in clinical practice. This narrative review explores the epidemiology of lumbar spinal stenosis, the evidence supporting non-operative care, and practical management strategies. It also highlights the relationship between sexual dysfunction and lumbar spinal stenosis.

Diagnostic imaging in the management of older adults with low back pain: analysis from the BAck Complaints in Elders: Chiropractic – Australia cohort study

Background
Diagnostic imaging is commonly used in the management of low back pain (LBP), with approximately one-quarter of those who present to primary care referred for imaging. Current estimates of imaging frequency commonly exclude older adults; however, pathology detected with imaging (e.g., osteoporosis, cancer) may occur more frequently in older populations. The aims of this study were to: (i) determine the frequency and forms of diagnostic imaging use in older adults presenting for chiropractic care for LBP in Australia; (ii) describe participant characteristics associated with imaging use; and (iii) describe the types of radiographic findings.
Methods
Data were collected from the BAck Complaints in Elders: Chiropractic-Australia (BACE: C-A) study, a 12-month, prospective cohort study of adults aged ≥ 55 years with a new episode of LBP. Self-reported frequency of imaging use (baseline, 2 and 6 weeks, 3, 6, 9, and 12 months) was reported descriptively by imaging modality. Imaging reports were obtained, and imaging findings were independently extracted and categorised. Baseline characteristics were assessed for differences in those who received imaging compared to those who did not. Proportions of imaging use and imaging findings were presented descriptively with 95% confidence intervals.
Results
The BACE: C-A cohort comprised 217 participants of whom 60.8% reported receiving diagnostic imaging for their current episode of LBP. X-ray was performed most (44.7%), followed by computed tomography (CT) (30.8%). Participants receiving imaging reported higher low back disability, more healthcare use for LBP, more frequent leg pain, more suspected serious pathology, and stronger beliefs that imaging was important. Degenerative changes were the most common imaging finding (96.6%). Pathology of possible clinical significance, including compression fracture or suspected osteoporosis, was present in 15.5% of participants.
Conclusion
Three out of five older adults with LBP who sought chiropractic care received imaging over one-year. Participants receiving imaging tended to have more complex presentations (e.g., more disability, suspected underlying pathology) or stronger beliefs that imaging was necessary for the management of LBP. Degenerative changes were the most common imaging finding. Pathology of potential clinical relevance was present on approximately 15% of imaging reports received. No conditions requiring immediate medical attention were reported.

Age differences in demographic and clinical characteristics among veterans with chronic low back pain: a cross-sectional study of baseline findings from the Veteran Response to Dosage in Chiropractic Therapy (VERDICT) trial

Background
Veteran Response to Dosage in Chiropractic Therapy (VERDICT) was a pragmatic randomized trial testing chiropractic dosage effects in 766 veterans with chronic low back pain (CLBP) of ≥ 3 months. This cross-sectional analysis compares baseline characteristics of younger (18-to-64 years) and older veterans (≥ 65 years).
Methods
Data were collected from February 22, 2021 to May 21, 2025 via electronic health records and REDCap questionnaires. Descriptive statistics and tests of group differences were performed using SAS.
Results
VERDICT enrolled 188 older veterans (25%; mean 72 years) and 578 younger veterans (75%; mean 44 years). More female (24.7% vs. 10.6%,

< .001), Black (18.9% vs. 12.2%), and Hispanic (11.8% vs. 3.7%,
p
= .001) veterans comprised the younger cohort. Employment differed (
p
< .001) with older veterans retired (78.2% vs. 14.2%) and younger veterans employed (59% vs. 16.5%). About 14% lived rurally and period of military service was similar. Pain profiles were similar between younger and older veterans for > 5 years duration (78.4% vs. 73.4%), high-impact chronic pain (64.5% vs. 62.2%), mean pain interference [63.8(4.8) vs. 63.2(5.0)], and mean back-related disability (primary outcome) [11.9(5.2) vs. 13.3(4.9)]. < .001). NSAIDs use was highest among both younger (62.8%) and older (56.9%) veterans. While two-thirds had tried exercise in the past 3 months, only 16% reported exercising for their pain condition, with older veterans more likely to report providers encouraging physical activity.
Conclusions
Similar pain profiles were reported among older and younger veterans seeking chiropractic care for CLBP within a clinical trial. However, potentially important age differences were noted in demographics, mental health and substance use, and CLBP treatments.
