Background
The purpose of this study is to evaluate the effect of a six-week combined manual therapy (MT) and stabilizing exercises (SEs), with a one-month follow-up on neck pain and improving function and posture in patients with forward head and rounded shoulder postures (FHRSP).
Methods
Sixty women with neck pain and FHRSP were randomized into three groups: Group 1 performed SE and received MT (
n
= 20), Group 2 performed SE (n = 20) and Group 3 performed home exercises (n = 20) for six weeks. The follow-up time was one month after the post test. The pain, function, and head and shoulder angles were measured before and after the six-week interventions, and during a one-month follow-up.
Results
There were significant within-group improvements in pain, function, and head and shoulder posture in groups 1 and 2. There were significant between-group differences in groups 1 and 2 in head posture, pain, and function favoring group 1 with effect size 0.432(
p
= 0.041), 0.533 (
P
= 0.038), and 0.565(
P
= 0.018) respectively. There were significant between-group differences in both intervention groups versus the control group favoring the intervention groups.
Conclusion
These findings suggest that both interventions were significantly effective in reducing neck pain and improving function and posture in patients. However, the improvement in function and pain were more effective in Group 1 as compared to Group 2, suggesting that MT can be used as a supplementary method to the stabilizing intervention in the treatment of neck pain. More researches are needed to confirm the result of this study.

Evidence for performance decrements following prolonged static stretching (SS) has led to a paradigm shift in stretching routines within a warm-up. Rather than SS, dynamic stretching (DS) and dynamic activity (DA) have replaced SS within warm-up routines. The objective of the present study was to compare the effect of differing lower limb SS durations (30 [SS30s], 60 [SS60s] or 120 s [SS120s] of SS per muscle group or no-stretch control) within a comprehensive warm-up protocol consisting of aerobic activity, DS and DA. Sixteen male participants completed the four stretching conditions in a randomized order, after a 5-min low-intensity (cycle) warm-up and before a DS/DA component on separate days. Tests included passive hip and knee ranges of motion (ROM), maximum voluntary knee extensor/flexor force, force produced at 100 ms (F100), vertical jump height and evoked knee extensor contractile properties. For hip flexion (hamstrings) ROM, SS120s provided the largest increase (5.6–11.7%) followed by SS60s (4.3–11.4%), control (4.4–10.6%) and SS30s (3.6–11.1%). For knee flexion (quadriceps) ROM, SS30s provided the largest increase (9.3–18.2%) followed by SS120s (6.5–16.3%), SS60s (7.2–15.2%) and control (6.3–15.2%). There were decreases in quadriceps F100 following SS in SS120s (29.6%) only. There were increases in vertical jump performance in the control (6.2%), SS60s (4.6%) and SS30s (3.3%). While 120 s SS per muscle increased ROM, even within a comprehensive warm-up routine, it also elicited notable performance decrements. However, moderate durations of SS were observed to improve ROM whilst either having negligible or beneficial (but not detrimental) effects on specific aspects of athletic performance.

Dynamic stretching (DS) is often performed during warm-up to help avoid hamstring muscle injuries, increase joint flexibility, and optimize performance. We examined the effects of DS of the hamstring muscles on passive knee extension range of motion (ROM), passive torque (PT) at the onset of pain (as a measure of stretch tolerance), and passive stiffness of the muscle-tendon unit over an extended period after stretching. Twenty-four healthy subjects participated, with 12 each in the experimental and control groups. Stretching was performed, and measurements were recorded using an isokinetic dynamometer pre-intervention, and at 0, 15, 30, 45, 60, 75, and 90 min post-intervention. DS consisted of ten 30-s sets of 15 repetitions of extension and relaxation of the hamstrings. ROM increased significantly (range, 7%-10%) immediately after DS, and the increase was sustained over 90 min. PT at the onset of pain also increased immediately by 10% but returned to baseline by 30 min. Passive stiffness decreased significantly (range, 7.9%-16.7%) immediately after DS, and the decrease was sustained over 90 min. Post-DS values were normalized to pre-DS values for the respective outcomes in both groups. ROM was significantly higher (range, 7.4%-10%) and passive stiffness was significantly lower (range, 5.4%-14.9%) in the experimental group relative to the control group at all time points. Normalized PT values at the onset of pain were significantly higher in the experimental group at 0-15 min than in the controls, but the differences were smaller at 30-45 min and not significant thereafter. We conclude that DS increases ROM and decreases passive stiffness in a sustained manner, and increases PT at the onset of pain for a shorter period. Overall, our results indicate that when performed prior to exercise, DS is beneficial for the hamstring muscles in terms of increasing flexibility and reducing stiffness.

Dynamic stretching alone can impair slower velocity isokinetic performance of young male handball players for at least 24 hours

There are many adult studies reporting static stretch (SS)-induced deficits and dynamic stretch (DS) performance improvements shortly after the intervention. However, there is only a single study examining stretch-induced performance changes with youth at 24 hours' post-stretch. The objective of this study was to examine physiological responses of young trained athletes at 24-hours after experiencing SS or DS protocols. Eight young male, elite handball players (age: 16.1±5.1 years) were tested prior to-, 3-minutes and 24-hours following the three conditions (DS, SS, Control) in a randomized and counterbalanced order. Similar volumes of SS (2 repetitions of 75s for each leg) and DS (5 repetitions of 30s for each leg) involved one stretch each for the quadriceps and hamstrings. Tests included (i) two 4s maximal voluntary isometric contractions (MVC) at 60° of knee flexion with 2-min rest, (ii) two maximal isokinetic contractions each at 60°/sec and 300°/sec with 1-min rest, and (iii) two drop jumps with 30-sec rest. To simulate a full warm-up, dynamic activity including 5 minutes of aerobic cycling (70 rpm; 1 kilopond), 4 submaximal isometric contractions and 4 drop jumps were instituted before the pre-tests and following the interventions. Two-way repeated measures ANOVAs revealed that 1) both the SS and control conditions exhibited knee extensor 60°.s-1 (SS:-10.3%; p = 0.04, Control: -8.7%; p = 0.07) and 300°.s-1 (SS: -12.9%; p = 0.005, Control: -16.3%; p = 0.02) isokinetic deficits at post-test, 2) DS impaired knee flexor 60°.s-1 isokinetic work and power-related measures at post-test (Work: -10.1%; p = 0.0006; Power: -19.1%; p = 0.08) and at 24-hours' post-test (Work: 9.9%; p = 0.023; Power: -9.6%; p = 0.01), 3) DS (12.07% and 10.47%) and SS (13.7% and 14.6%) enhanced knee flexor 300°.s-1 isokinetic force and power-related measures compared to control. In conclusion, testing-induced knee extensor isokinetic impairments were counterbalanced by DS, however the hip flexion DS could have produced minor muscle damage for at least 24-hours decreasing knee flexor forces and power at 60°.s-1.

The use of stretching devices for treatment of trismus in head and neck cancer patients: a randomized controlled trial

Purpose
To compare the effects of two stretching devices, the TheraBite® Jaw Motion Rehabilitation System™ and the Dynasplint Trismus System®, on maximal mouth opening in head and neck cancer patients.
Methods
Patients were randomly assigned to one of two exercise groups: the TheraBite® Jaw Motion Rehabilitation System™ group or the Dynasplint Trismus System® group. Patients performed stretching exercises for 3 months. During the three study visits, maximal mouth opening was measured and the patients completed questionnaires on mandibular function and quality of life.
Results
In our study population (
n
= 27), five patients did not start the exercise protocol, eight patients discontinued exercises, and two patients were lost to follow-up. No significant differences regarding the change in mouth opening between the two devices were found. Patients had an increase in MMO of 3.0 mm (IQR − 2.0; 4.0) using the TheraBite® Jaw Motion Rehabilitation System™ and 1.5 mm (IQR 1.0; 3.0) using the Dynasplint Trismus System®. Exercising with either stretching device was challenging for the patients due to the intensive exercise protocol, pain during the exercises, fitting problems with the stretching device, and overall deterioration of their medical condition.
Conclusions
The effects of the two stretching devices did not differ significantly in our study population. The factors described, influencing the progression of stretching exercises, need to be taken into account when prescribing a similar stretching regimen for trismus in head and neck cancer patients.

Objectives To examine the correction of posture, increase in strength and decrease in shoulder pain and dysfunction in varsity swimmers. Design and Setting Randomised clinical trial. Participants Twenty-eight National Collegiate Athletic Association division I varsity swimmers. Measurements Two testing sessions were conducted before and after an 8-week time period. Posture, strength and shoulder pain and function were assessed. Forward head angle was measured using a digital inclinometer, forward head translation was measured using a ruler and total scapular distance was measured with unmarked string. Average and peak values (N) of strength were measured with the hand-held dynamometer. The intervention subjects then participated in an 8-week exercise training programme to correct posture. The procedures were then repeated in the post-test. Results Significant group by time interactions (p<0.05) were found in forward head angle and forward shoulder translation indicating a decrease in forward head angle and forward shoulder translation. Significant main effects for time (p<0.05) were found in strength measures for all muscle groups indicating increased strength for shoulder girdle muscles tested. Conclusions The exercise intervention was successful at decreasing forward head and rounded shoulder postures in elite swimmers. This study supports the theoretical basis for clinical rehabilitation of posture and the shoulder.

Introduction and hypothesisPerineal preparation techniques for childbirth have been used with the aim of reducing perineal tears during the expulsive phase of labor. However, no studies were found to investigate the effects of instrument-assisted stretching versus perineal massage on pelvic floor muscle (PFM) variables. Therefore, the aim of this study was to evaluate the effect of instrument-assisted stretching versus perineal massage on the extensibility and strength of the PFMs.MethodsPrimiparous women were randomized to the instrument-assisted stretching (IStr) group (n = 13) and perineal massage (PnM) group (n = 14). The groups participated in eight sessions, twice weekly, beginning at the 34th gestational week. The IStr group underwent the intervention for 15 min using EPI-NO®. The PnM group underwent a perineal massage protocol for 10 min. Each woman was evaluated by a blinded physiotherapist before, after four and after eight sessions for primary (PFM extensibility using the EPI-NO® circumference) and secondary (PFM strength using a manometer) outcomes. Covariate analysis (ANCOVA) was used to compare the groups using the baseline values as a covariate.ResultsBoth groups showed an increase in PFM extensibility compared with the evaluations before and after four and eight sessions (PnM group from 17.6 ± 1.8 to 20.2 ± 1.9 cm; IStr group from 19.9 ± 1.6 to 22.9 ± 1.6 cm;p < 0.001). There was no difference between groups. Regarding muscle strength, no statistical differences were observed between evaluations or between groups.ConclusionsInstrument-assisted stretching and perineal massage increase extensibility and do not alter the strength of PFMs in pregnant women.

Acute Effects of Dynamic Stretching on Mechanical Properties Result From both Muscle-Tendon Stretching and Muscle Warm-Up

We investigated the acute effects of dynamic stretching on mechanical properties of plantar flexor muscles and tested the hypothesis that it would result from an interaction between muscle-tendon stretching and muscle warm-up. To test the stretching effect, dynamic stretching (DS) was compared to static stretching (SS). To test the warm-up effect, DS was compared to submaximal isometric muscle activity (SIMA) with similar contraction intensity. A control condition served as reference. These four conditioning activities were time matched (2×20s) and tested on separate days on 13 volunteers. Electrical neurostimulation was applied to investigate muscle mechanical properties (peak doublet torque (PDT) and rate of torque development (RTD)), before assessing maximal voluntary isometric torque (MVIT). Passive torque and muscle fascicles length, were measured respectively with a dynamometer and ultrasonography during a maximal passive dorsiflexion. These parameters were recorded before and after each conditioning activities. MVIT, PDT, RTD and fascicles length remained unaffected, while passive torque was significantly reduced (-11.6 ± 14.8%) after DS. SS significantly decreased MVIT (-7.7 ± 3.8%) and enhanced fascicles length (45.3 ± 49.2%), whereas SIMA increased PDT (5.8 ± 5.2%) and RTD (7.4 ± 8.3%) without affecting passive torque or fascicles length. These results may suggest that, during dynamic stretching, muscle-tendon stretching effects would partly counteract muscle warm-up effects.

Lower-extremity resistance training on unstable surfaces improves proxies of muscle strength, power and balance in healthy older adults: a randomised control trial
Background
It is well documented that both balance and resistance training have the potential to mitigate intrinsic fall risk factors in older adults. However, knowledge about the effects of simultaneously executed balance and resistance training (i.e., resistance training conducted on unstable surfaces [URT]) on lower-extremity muscle strength, power and balance in older adults is insufficient. The objective of the present study was to compare the effects of machine-based stable resistance training (M-SRT) and two types of URT, i.e., machine-based (M-URT) and free-weight URT (F-URT), on measures of lower-extremity muscle strength, power and balance in older adults.
Methods
Seventy-five healthy community-dwelling older adults aged 65–80 years, were assigned to three intervention groups: M-SRT, M-URT and F-URT. Over a period of ten weeks, all participants exercised two times per week with each session lasting ~60 min. Tests included assessment of leg muscle strength (e.g., maximal isometric leg extension strength), power (e.g., chair rise test) and balance (e.g., functional reach test), carried out before and after the training period. Furthermore, maximal training load of the squat-movement was assessed during the last training week.
Results
Maximal training load of the squat-movement was significantly lower in F-URT in comparison to M-SRT and M-URT. However, lower-extremity resistance training conducted on even and uneven surfaces meaningfully improved proxies of strength, power and balance in all groups. M-URT produced the greatest improvements in leg extension strength and F-URT in the chair rise test and functional reach test.
Conclusion
Aside from two interaction effects, overall improvements in measures of lower-extremity muscle strength, power and balance were similar across training groups. Importantly, F-URT produced similar results with considerably lower training load as compared to M-SRT and M-URT. Concluding, F-URT seems an effective and safe alternative training program to mitigate intrinsic fall risk factors in older adults.
