Sex and Gender Differences in Occupational Hazard Exposures: a Scoping Review of the Recent Literature

Purpose of Review
Comparative research on sex and/or gender differences in occupational hazard exposures is necessary for effective work injury and illness prevention strategies. This scoping review summarizes the peer-reviewed literature from 2009 to 2019 on exposure differences to occupational hazards between men and women, across occupations, and within the same occupation.
Recent Findings
Fifty-eight studies retrieved from eight databases met our inclusion criteria. Of these, 30 studies were found on physical hazards, 38 studies on psychological/psychosocial hazards, 5 studies on biological hazards, and 17 studies on chemical hazards. The majority of studies reported that men were exposed to noise, vibration, medical radiation, physically demanding work, solar radiation, falls, biomechanical risks, chemical hazards, and blood contamination; while women were exposed to wet work, bullying and discrimination, work stress, and biological agents. Within the same occupations, men were more likely to be exposed to physical hazards, with the exception of women in health care occupations and exposure to prolonged standing. Women compared to men in the same occupations were more likely to experience harassment, while men compared to women in the same occupations reported higher work stress. Men reported more exposure to hazardous chemicals in the same occupations as women.
Summary
The review suggests that men and women have different exposures to occupational hazards and that these differences are not solely due to a gendered distribution of the labor force by occupation. Findings may inform prevention efforts seeking to reduce gender inequalities in occupational health. Future research is needed to explain the reasons for sex/gender inequality differences in exposures within the same occupation.

Healthy Aging Requires a Healthy Home Care Workforce: the Occupational Safety and Health of Home Care Aides

Purpose of Review
To identify important home care (HC) aide occupational safety and health (OSH) hazards and examine how addressing these can improve aide health and the delivery of HC services overall. Specifically, this review seeks to answer: Why is HC aide OSH important? What are the most significant OSH challenges? How can improving HC aide OSH also improve the safety and health of their clients? What implications do the findings have for future research?
Recent Findings
HC is one of the fastest growing US industries. Aides comprise its largest workforce and are increasingly needed to care for the rapidly aging population. There is an aide shortage due in part to instabilities in HC work organization and to serious job-specific hazards, resulting in aides losing work time.
Recent social, economic, and technological factors are rapidly changing the nature of HC work, creating OSH hazards similar to those found in nursing homes. At the same time, aides are experiencing social and economic inequities that increase their vulnerability to OSH hazards. These hazards are also a burden on employers who are challenged to recruit, retain, and train aides. OSH injuries and illness interrupt the continuity of care delivery to clients. Many OSH hazards also put HC clients and families at risk.
Summary
A new framework and methodologies are needed to assess aide and client safety together in order to guide future HC research, policies, and practices. Government, industry, and labor commitment is needed to fund and coordinate a comprehensive, multidisciplinary research program.

Recent Research on Occupational Animal Exposures and Health Risks: A Narrative Review

Purpose of Review
In the last year, an increasing number of studies have reported on methicillin-resistant
Staphylococcus aureus
(MRSA) transmission in Africa and Asia and in migrant workers. We reviewed original research on occupational health and safety of animal workers published from January 1, 2018, through June 30, 2019, with a targeted focus on infectious disease studies published in these populations.
Recent Findings
Studies focused on occupational exposures to infectious agents, dust and allergens, pesticides, and occupational injury. Research on zoonotic MRSA used whole genome–sequencing technologies to evaluate transmission in Africa and Asia. Swine worker exposure to porcine coronavirus and emerging influenza A viruses was documented in China. 16s RNA amplicon sequencing identified distinct microbiota compositions in households with active animal farmers. Multiple bioaerosol exposures were assessed for industrial dairy workers. Occupational injury studies highlighted the struggles of Latino animal workers in the USA.
Summary
These studies highlighted the global expansion of zoonotic antibiotic resistance and identified novel occupational zoonoses of concern. The integration of microbiome assessment and compound mixtures into the evaluation of dust and endotoxin exposures for animal workers marks a new direction for this work.

Heat Exposure and Occupational Injuries: Review of the Literature and Implications

Purpose of Review
The burden of heat-related adverse occupational health effects, as well as traumatic injuries, is already substantial. Projected increases in mean temperatures and extreme events may increase the risk of adverse heat health effects and enhance disparities among exposed workers. This article reviews the emerging literature on the relationship between heat exposure and occupational traumatic injuries and discusses implications of this work.
Recent Findings
A recent meta-analysis of three case-crossover and five time series studies in industrialized settings reported an association of increasing occupational injuries with increasing heat exposure, with increased effect estimates for male gender and age less than 25 years, although heterogeneity in exposure metrics and sources of bias were demonstrated to varying degrees across studies. A subsequent case-crossover study in outdoor construction workers reported a 0.5% increase in the odds of traumatic injuries per 1 °C increase in maximum daily humidex (odds ratio 1.005 [95% CI 1.003–1.007]). While some studies have demonstrated reversed U-shaped associations between heat exposure and occupational injuries, different risk profiles have been reported in different industries and settings.
Summary
Studies conducted primarily in industrialized settings suggest an increased risk of traumatic injury with increasing heat exposure, though the exact mechanisms of heat exposure’s effects on traumatic injuries are still under investigation. The effectiveness of heat-related injury prevention approaches has not yet been established. To enhance the effectiveness of prevention efforts, prioritization of approaches should take into account not only the hierarchy of controls, social-ecological models, community and stakeholder participation, and tailoring of approaches to specific local work settings, but also methods that reduce local and global disparities and better address the source of heat exposure, including conservation-informed land-use planning, built environment, and prevention through design approaches. Participation of occupational health experts in transdisciplinary development and integration of these approaches is needed.

Current Review of Pneumoconiosis Among US Coal Miners

Purpose of Review
This review summarizes recent research on pneumoconiosis in coal workers following the identification of the resurgence of this disease among US coal miners in the early 2000s. We describe the impact of this research and how this has led to increased public attention, benefitting affected miners.
Recent Findings
The latest research shows that the prevalence of pneumoconiosis, including progressive massive fibrosis, continues to increase, especially in central Appalachia. Contributing factors may include mining of thin coal seams or cutting rock to access coal, which may expose miners to coal mine dust with a higher content of silica and silicates than in the past.
Summary
The impact of recently implemented changes, such as the reduced occupational exposure limit for respirable coal mine dust and the introduction of continuous personal dust monitors, will likely take years to appropriately evaluate.

Purpose of Review
To review the literature published from 2014 to 2019 on hair and nail salon workers concerning exposure assessment, reproductive and respiratory endpoints, and endocrine disruption, in relation to workplace exposures.
Recent Findings
We identified 29 relevant peer-reviewed publications. Overall, there were insufficient studies to determine whether working in these settings is associated with reproductive health endpoints, although prior studies suggest that reproductive effects are of concern. There is consistent evidence that working in hair and nail salons may increase the risk of respiratory effects. Also, despite the fact that many hair and nail care products contain endocrine disrupting compounds, no recent studies have evaluated endocrine-related endpoints. Moreover, few studies have evaluated chemical exposures in these settings and biomonitoring studies are sparse.
Summary
Improved exposure assessment of chemical hazards in hair and nail salons is necessary to properly characterize occupational exposures and assess their potential health risks. Further studies on endpoints related to endocrine disruption and reproductive health outcomes among hair and nail salon workers are needed. Improved exposure and epidemiologic studies will help inform chemical exposure mitigation efforts in a vulnerable occupational population, as well as policies related to workplace and consumer product safety.

Electronic Waste Recycling: Occupational Exposures and Work-Related Health Effects

Purpose of Review
Electronic waste (e-waste) is a global public health challenge. E-waste recycling workers may be exposed to chemical, physical, ergonomic, and psychosocial hazards. This review provides an overview of recent research on occupational exposures in e-waste recycling and work-related health effects that can impact e-waste workers.
Recent Findings
E-waste workers are exposed to a variety of chemicals including metals, particulates, persistent organic compounds, and flame retardants. Exposure varies according to job task with higher exposures observed for dismantling and burning e-waste. Exposure to job stress and physical hazards (e.g., noise) also occurs.
Summary
Many studies have measured workers’ exposure in the e-waste recycling industry; fewer have investigated health effects. Biological measures were reported more often than external exposure measures. In order to protect workers, efforts are required to better understand exposures and their health effects. Removing hazardous materials from electronic equipment and reducing e-waste production would benefit workers, communities, and the environment.

Parental Occupational Exposure and Neurodevelopmental Disorders in Offspring: a Systematic Review and Meta-analysis

Purpose of Review
Parental occupational exposures might be associated with neurodevelopmental disorders (NDDs) in offspring. We aimed to conduct a systematic review and meta-analysis to summarize and synthesize the current literature and to estimate the pooled magnitude of the underlying association(s) between parental occupational exposures and subsequent risk of NDDs.
Recent Findings
In the meta-analysis of 20 included studies, significant associations were found between parental occupational exposure to pesticides or solvents and the risk of attention deficit hyperactivity disorder in offspring. Prenatal occupational exposure to pesticides was significantly associated with motor development or cognition disorders in children. Furthermore, some evidence showed that metals might have a role in the development of autism spectrum disorders.
Summary
Further studies need to identify the level of parental occupational exposures that can be significantly associated with NDDs. Moreover, utilizing standardized outcome and exposure scales is recommended to incorporate paternal, maternal, and parental as well as both prenatal and postnatal exposure in future studies.

Occupational Exposures to Engineered Nanomaterials: a Review of Workplace Exposure Assessment Methods

Purpose of Review
The purpose of this review is to consolidate exposure assessment methods for occupational research on engineered nanomaterials (ENMs) published within the past 5 years (2015–2020).
Recent Findings
The three ENMs that generated the highest volume of new research include titanium dioxide, graphene, and aluminum oxide. A multi-metric approach, using both online and offline instruments and analyses, has been found to be a useful method to characterize ENM workplace exposures and was commonly used in the recently published literature. Particle number concentration was the most common online exposure metric used, followed by the metrics of mass and surface area. There are currently no consensus methods for offline analyses of most ENMs. Researchers generally used gravimetric or elemental analyses for carbonaceous nanomaterials, titanium dioxide, and other nanometals, but there was little overlap between other ENM materials reviewed. Using biological markers of exposure, such as urinary oxidative stress biomarkers, as an indication of chronic exposure may also be useful for some ENMs and should be further researched.
Summary
Generally, similar online instrumentation and offline electron microscopy methods were used for all ENMs. However, this consistency was not observed for offline mass analysis methods within specific ENMs. Consolidation of the most recent methods and results of exposure assessments within this broad material category can guide researchers toward future areas of study. Establishing consensus methods of exposure assessment for each individual ENM is crucial to characterizing workplace exposures, pooling data to fully understand their associated risks, and developing useful occupational exposure limits.

Using Decision Rules to Assess Occupational Exposure in Population-Based Studies

Purpose of Review
Population-based studies increasingly link task-based occupational questionnaire responses collected from subjects to exposure estimates via transparent, programmable decision rules. We reviewed recent applications and methodological developments of rule-based approaches.
Recent Findings
Agent-specific decision rules require interviews incorporating work-task-based questions. Some studies have developed rules before the interviews took place, while others developed rules after the interviews were completed. Agreement between rule-based estimates and exposures assigned using job-by-job expert review were generally moderate to good (Kappa = 0.4–0.8). Rules providing quantitative intensity levels using measurement data or that integrate multiple independent exposure sources for the same job represent further advances to improve the characterization of occupational exposures in population studies.
Summary
Decision rules have provided transparent and reproducible assessments, reduce job-by-job review, and facilitate sensitivity analyses in epidemiologic studies. Future studies should consider the development of decision rules concurrent with the questionnaire design to facilitate occupational exposure assessment efforts.
