PurposeTo explore the prospective association of objectively measured and self-reported occupational physical activity (OPA) with multisite musculoskeletal pain (MSP) among Danish eldercare workers.MethodsThe study population consisted of eldercare workers in 20 Danish nursing homes (N = 553, response rate 59%, 525 female). Baseline data were collected in 2013–2014 and the 1-year follow-up was completed in 2016. At baseline, we measured objective OPA by a thigh-worn ActiGraph GT3X + accelerometer during work and self-reported OPA by a questionnaire survey. Information on musculoskeletal pain during the past four weeks in seven different body sites was reported by a structured questionnaire at baseline (n = 389) and by SMS and telephone interview during follow-up (n = 284). MSP was defined as having pain in two or more body sites. Using log-binomial models we calculated risk ratios (RRs) with their 95% confidence intervals (CIs) to estimate the association between objectively measured and self-reported OPA and MSP.ResultsWe found statistically significant positive associations between self-reported OPA (RR for high OPA 1.24, 95% CI 1.05–1.46) and MSP while there was no significant association found between objective OPA and MSP.ConclusionOur study indicates that self-reported, but not objectively measured OPA is positively associated with MSP. This finding highlights the need for better understanding, use, and interpretation of self-reported and objectively measured OPA in the study of MSP.

Physical activity at work may not be health enhancing. A systematic review with meta-analysis on the association between occupational physical activity and cardiovascular disease mortality covering 23 studies with 655 892 participants

Emerging evidence suggests contrasting health effects for leisure-time and occupational physical activity. In this systematic review, we synthesized and described the epidemiological evidence regarding the association between occupational physical activity and cardiovascular disease (CVD) mortality.
A literature search was performed in PubMed, Embase, CINAHL, PsycINFO and Evidence-Based Medicine Reviews, from database inception to 17 April 2020. Articles were included if they described original observational prospective research, assessing the association between occupational physical activity and CVD mortality among adult workers. Reviews were included if they controlled for age and gender and at least one other relevant variable. We performed meta-analyses on the associations between occupational physical activity and CVD mortality.
We screened 3345 unique articles, and 31 articles (from 23 studies) were described in this review. In the meta-analysis, occupational physical activity showed no significant association with overall CVD mortality for both males [hazard ratio (HR) 1.00, 95% confidence interval (CI) 0.87-1.15] and females (HR 0.95, 95% CI 0.82-1.09). Additional analysis showed that higher levels of occupational physical activity were non-significantly associated with a 15% increase in studies reporting on the outcome ischemic heart disease mortality (HR 1.15, 95% CI 0.88-1.49).
While the beneficial association between leisure-time physical activity and CVD mortality has been widely documented, occupational physical activity was not found to have a beneficial association with CVD mortality. This observation may have implications for our appreciation of the association between physical activity and health for workers in physically demanding jobs, as occupational physical activity may not be health enhancing.

Background and objective
Physical activity has benefits for the cardiovascular system, however, what levels and types of activity provide optimal cardiovascular health is unclear. We aimed to determine the level of physical activity that has the most benefits against cardiovascular diseases (CVD).
Methods
PubMed, Scopus, and Web of Science were searched for prospective cohort studies on leisure-time (LTPA) or occupational physical activity (OPA) as the exposure and major types of CVD (total CVD, coronary heart disease [CHD], stroke, and atrial fibrillation [AF]) incidence as the outcome. Risk of bias of studies was evaluated using the ROBINS-I tool. Summary hazard ratios (HR) were calculated using random-effects pairwise model.
Results
A total of 103 studies were included in the analysis. The highest versus the lowest LTPA was associated with a lower risk of overall CVD (HR = 0.81; 95% CI: 0.77–0.86), CHD (HR = 0.83; 0.79–0.88), and stroke (HR = 0.83; 0.79–0.88), but not AF (HR = 0.98; 0.92–1.05). Linear dose-response analyses showed a 10%, 12%, 9%, and 8% risk reduction in CVD, CHD, stroke, and AF incidence, respectively, for every 20 MET-hours/week increase in LTPA. In nonlinear dose-response analyses, there were inverse associations up to 20 MET-hours/week with 19% and 20% reduction in CVD and CHD risk, and up to 25 MET-hours/week with 22% reduction in stroke, with no further risk reduction at higher LTPA levels. For AF, there was a U-shaped nonlinear association with the maximum 8% risk reduction at 10 MET-hours/week of LTPA. Higher levels of OPA were not associated with risk of CVD, CHD, stroke, or AF.
Conclusions
Overall, results showed an inverse dose-response relationship between LTPA and risk of CVD, CHD, stroke, and AF. Running was the most beneficial LTPA but the risk was similar among various LTPA intensities. OPA showed no benefits in total or any type of CVD.

Work, travel, or leisure: comparing domain-specific physical activity patterns based on rural–urban location in Canada

Background
Physical activity occurs across various domains including leisure/recreation, for transportation, or for work or household reasons. Rural and urban active living environments are characterized by different opportunities for physical activity within each domain which may translate into different patterns of behavior. The aim of this study was to compare rural–urban differences in physical activity across different domains, and explore interactions between sociodemographic factors, physical activity domains, and rurality.
Methods
We used self-reported data collected across three physical activity domains (active transportation, recreation, occupational/household) and relevant sociodemographic variables from the Canadian Community Health Survey. Adjusting for sociodemographic factors, we did two separate cross-sectional analyses: 1) binary logistic regression to determine the odds of reporting any activity in each domain, and 2) ordinary least squares regression using the sub-samples reporting > 0 min per week of activity to compare how much activity was reported in each domain.

Our final survey weighted sample of Canadian adults (mean age 47.4 years) was

= 47,266). Rural residents were less likely to report any active transportation (OR = 0.59, 95% CI [0.51, 0.67],

< .0001). For recreational physical activity, rural males had lower odds (OR = 0.75, 95% CI [0.67, 0.83],

< .0001) and rural females had higher odds (OR = 1.19, 95% CI [1.08, 1.30],

= .0002) of reporting any participation compared to urban residents. Rural males (OR = 1.90, 95% CI [1.74, 2.07],

< .0001) and females (OR = 1.33, 95% CI [1.21, 1.46],
p
< .0001) had higher odds of reporting any occupational or household physical activity.
Conclusions
Urban residents tend to participate in more active transportation, while rural residents participate in more occupational or household physical activity. Location-based differences in physical activity are best understood by examining multiple domains and must include appropriate sociodemographic interactions, such as income and sex/gender.

Objectives Recent meta-analyses suggest a physical activity health paradox: high levels of occupational physical activity (OPA) increase cardiovascular disease (CVD) risk, while leisure-time physical activity (LTPA) decreases risk. However, studies of women and cerebrovascular disease are limited. This report examines physical activity effects on stroke and transient ischemic attack (TIA) among working women in the United States. Methods OPA history, health status, and lifestyle were assessed by baseline interviews of 31 270 employed Sister Study participants aged 35-74 years. OPA was assessed at six intensity levels (lowest: \"mostly sitting\"); the highest three were combined as \"high intensity work.\" Independent OPA and LTPA effects on 6-year cerebrovascular disease incidence were estimated in adjusted Cox proportional hazard models. Results Stroke (N=441) and TIA (N=274) risk increased with more standing and higher intensity work at current and longest held job. Compared with mostly sitting, high intensity work at the current job increased TIA risk by 57% [hazard ratio (HR) 1.57, 95% confidence interval (CI) 1.04-2.38]. High intensity OPA at the longest held job increased risk for stroke by 44% (HR 1.44; 95% CI 1.08-1.93). Among women with CVD, sitting and standing equally, especially at the current job, increased risks up to two-fold (TIA HR 1.98, 95% CI 1.10-3.55) compared with mostly sitting at work. LTPA showed inverse associations. Conclusions Higher intensity levels of OPA increased stroke and TIA risks, while LTPA decreased risks; results corroborate the physical activity health paradox for women and cerebrovascular disease. More standing at work increased cerebrovascular disease risks, especially for women with CVD.

The “sweet- and sour-spot” of occupational physical activity for back pain: a prospective accelerometer study among eldercare workers

OBJECTIVES: Both high and low levels of occupational physical activity are associated with back pain. Thus, there might be a “sweet- and sour-spot” of occupational physical activity for back pain. Our aim was to investigate if there exists an occupational physical activity “sweet- (lowest risk) and sour-spot” (highest risk) for back pain.
METHODS: A total of 396 eldercare workers from 20 Danish nursing homes participated. Occupational physical activity was measured between 1–4 working days using thigh-worn accelerometry. Back pain intensity was reported monthly on a scale from 0–10 over 1-year. A zero-inflated mixed-effects model was developed regressing occupational physical activity against back pain, adjusted for confounders. The “sweet- and sour-spot” were defined as the occupational physical activity compositions (sitting, standing, light, and moderate-to-vigorous) associated with the 5% lowest and highest risk for back pain, respectively.
RESULTS: The composition associated with the lowest risk of back pain – the “sweet-spot”– consisted of 71% worktime spent sitting, 18% spent standing, 5% spent on light physical activity and 6% spent on moderate-to-vigorous physical activity. The composition associated with highest risk for back pain –the “sour-spot”– consisted of 8% worktime spent sitting, 66% spent standing, 4% spent on light physical activity, and 21% spent on moderate-to-vigorous physical activity.
CONCLUSIONS: The “sweet-spot” of occupational physical activity for back pain among eldercare workers involves more sitting and light physical activity time, while the “sour-spot” involves more standing and moderate-to-vigorous physical activity time. Research on the occupational physical activity “sweet- and sour-spot” is needed.

Exploring the ‘fit for work’ principle: The association between occupational physical activity, cardio-respiratory fitness, and mortality – a meta-analysis of male worker data

OBJECTIVES: This individual participant data meta-analysis investigates the association between occupational physical activity (OPA) and both cardiovascular mortality and all-cause mortality across different cardio-respiratory fitness (CRF) groups among male workers.
METHODS: Data were pooled from five European cohort studies. OPA was categorized into three levels and CRF into low, moderate, and high tertiles. OPA was assessed using self-reports and CRF through objective measurements. Two-stage meta-analyses were conducted. First, we analyzed each cohort using Cox-regression models then we pooled results with random effects model to evaluate the associations between OPA and both cardiovascular and all-cause mortality, stratified by CRF. Models were adjusted for age, body mass index, smoking status, leisure-time physical activity, and educational level.
RESULTS: Among 9922 men (mean age 46.8, standard deviation 6.7, years), 55.7% died during an average 25.6-year follow-up, of which 29.3% died from cardiovascular causes. Individuals with low CRF and high levels of OPA showed increased risks of cardiovascular [hazard ratio (HR) 1.27, 95% confidence interval (CI) 1.04–1.55] and all-cause mortality (HR 1.22, 95% CI 1.07–1.40) compared to those with low CRF and low levels of OPA. High CRF mitigated cardiovascular mortality risk (HR 1.08, 95% CI 0.79–1.48) but not all-cause mortality (HR 1.27, 95% CI 0.98–1.83) for those with high OPA.
CONCLUSIONS: Our findings for cardiovascular mortality suggest that high CRF levels may protect workers with physically demanding jobs from adverse cardiovascular outcomes, supporting the ‘fit for work’ principle. However, this protective effect was not observed for all-cause mortality.

PurposeTo examine the association of exposure to Occupational Physical Activity (OPA) with all-cause mortality and incidence of cardiovascular diseases (CVD).MethodsThe study population was composed of three Italian cohorts: a national cohort of employees participating in the National Health Survey 2005, followed-up until 2014 (ILS 2005), and two urban cohorts of employees resident in Turin at 2001 and 2011 censuses (TLS 2001 and TLS 2011, respectively), both followed-up until 2018. Follow-up was conducted through individual record-linkage with death registries and hospital admissions archives. Exposure to OPA was assigned through an Italian job-exposure matrix (JEM). Relative Risks of both CVD incidence and overall mortality associated with OPA quartiles (IRR) were estimated using Poisson regression models adjusted for socio-demographics and health, and in the national cohort, also for leisure time physical activity, BMI, smoking, diabetes, and hypertension.ResultsCompared to the lowest quartile, the highest OPA quartile was associated in both genders with significantly increased mortality in TLS 2001 (IRR = 1.11 among men, IRR = 1.20 among women) and in TLS 2011 (IRR = 1.27 among men and IRR = 1.73 among women), whereas in the ILS 2005 cohort no association was found. Among women, high OPA was also associated with CVD risk in TLS 2001 and 2011 (IRR = 1.39 and IRR = 1.16 for the highest quartile, respectively), while in the ILS cohort in both genders only the third quartile showed a significantly higher risk.ConclusionOur results indicate that OPA does not have a beneficial effect on CVD and mortality, but rather suggest that it may produce deleterious health effects.

OBJECTIVES: Occupational physical activity (OPA) during pregnancy has been linked to adverse pregnancy outcomes, but crude exposure assessment remains an issue in causal inference. We aimed to develop a quantitative trimester-specific job exposure matrix (JEM) for standing, walking, and forward bending among pregnant workers.
METHODS: Accelerometer measurements from 403 female workers across 109 DISCO-08 job codes were obtained in Denmark between January 2023 and June 2024. Full workdays were measured during two weeks among pregnant workers and one week among non-pregnant workers. We used linear mixed-effects models to estimate exposure levels of occupational standing, walking, and forward bending for all 1171 DISCO-08 codes, including age, trimester, and expert ratings as fixed effects, and job codes and workers as random effects.
RESULTS: The between-job variances relative to total variances were 56% for standing, 51% for walking, and 45% for forward bending. The fixed effect trimester reduced standing time by 0.38 hours during the 3rd trimester compared to non-pregnant participants, whereas no differences were observed for walking or forward bending. Based on the trimester-specific JEM for occupational standing time, bakers had the highest exposure (range from non-pregnant to 3rd trimester, 5.41–5.03 hours/workday). For walking and forward bending, the highest exposed jobs from the pregnancy-specific JEM were waiters (1.76 hours/workday) and livestock/dairy producers (1.24 hours/workday), respectively.
CONCLUSIONS: The JEM enhances independent objective exposure assessment in epidemiological studies of OPA and pregnancy outcomes and may advance guidelines and potentially prevent adverse pregnancy outcomes.

Daily physical activity patterns among aging workers: the Finnish Retirement and Aging Study (FIREA)

ObjectivesPhysical activity is associated with the aging workers' ability to work and predicts working beyond retirement age. To better understand physical activity behaviour in this growing population group, we aimed at characterising 24-hour physical activity patterns among aging workers, and to describe the association between occupational category and total, occupational and leisure-time physical activities.MethodsWe included 878 workers (mean age 62.4 years, SD 1.1, 85% women) from the Finnish Retirement and Aging Study, who wore an accelerometer on their non-dominant wrist for 1 week. We plotted mean hourly activity counts per minute (CPM) for working days and days off. We also compared mean daily CPM between genders and occupations between working days and days off, and work and leisure time by using repeated measures analysis of variance.ResultsActivity patterns were different between genders, occupations and types of the day. Women (2580, 95% CI 2540 to 2620) had higher daily mean CPM than men (2110, 95% CI 2020 to 2000). Women in manual occupations were more active than women in non-manual occupations during working days. The differences among men were in the same direction but less pronounced than among women. We found no differences in activity levels between occupations during days off and leisure time on working days.ConclusionsIn aging workers, physical activity differs by gender and occupation during working time, but not during leisure time. As low physical activity is associated with increased risk of early exit from employment, physical activity should be promoted at workplaces, especially among men and people in non-manual occupations.
