Use of outpatient medical care by headache patients in Germany: a population-based cross-sectional study

BackgroundHeadache sufferers in need of professional health care often do not utilize the care available, and factors influencing headache-specific physician consultation are not yet understood. Objectives of this study are (1) to assess self-reported headache-specific physician consultations and (2) to identify headache-related and sociodemographic predictors.MethodsData of a random sample of the general population in Germany aged ≥14 years were analyzed (N = 2461). A multivariate binary logistic regression was conducted to identify a parsimonious model to predict physician consultation.Results50.7% of the participants with headache reported at least one headache-specific physician consultation during lifetime. Of these, 53.6% had seen one, 26.1% two, and 20.3% more than two physicians because of their headaches. The odds of physician consultation increased with the number of headache days per month (HDM) [(reference HDM < 1) HDM 1–3 (OR = 2.29), HDM 4–14 (OR = 2.41), and HDM ≥15 (OR = 4.83)] and increasing Headache Impact Test score (HIT-6) [(reference “no or little impact”) moderate impact (OR = 1.74), substantial impact (OR = 3.01), and severe impact (OR = 5.08)]. Middle-aged participants were more likely to have consulted than younger and older ones [(reference 14–34 years) 35–54 years (OR = 1.90), 55–74 years (OR = 1.96), ≥75 years (OR = 1.02)]. The odds of physician consultation among self-employed subjects were lower than among employed manual workers (OR = 0.48). The living environment (rural versus urban) did not have an influence on the consultation frequency.ConclusionThe results indicate that apart from burden-related factors (headache frequency; headache impact), health care utilization patterns are also influenced by patients’ occupational status and age. Further research is needed to analyze whether the lower consultation rate means that the self-employed have a higher risk of chronification or that they have more effective self-management strategies regarding headache.

Influence of Occupational Status on the Quality of Life of Chinese Adult Patients with Epilepsy

Background: Epilepsy is one of the most common serious neurological disorders. The present study aimed to investigate the influence of occupational status on the quality of life of Chinese adult patients with epilepsy.
Methods: This study surveyed 819 subjects clinically diagnosed with epilepsy for more than 1 year in 11 hospitals in Beijing; 586 were employed (71.55%). All subjects completed the case report form with inquiries on demographic data, social factors, and illness. The patients' quality of life was assessed using the quality of life in patients with epilepsy-31 items (QOLIE-31) questionnaire.
Results: The QOLIE-31 score in the employed group was significantly higher than that in the unemployed group. Furthermore, the scores in all the sections (overall quality of life, energy/fatigue, emotional well-being, seizure worry, cognition, social function, and medication effects) of the employed group were higher than those of the unemployed group. Both the employed and unemployed groups achieved the highest difference in social function. The QOLIE-31 score of students was higher than those of farmers and workers. Both the students and workers scored higher in the quality of life compared with the adult peasants living with epilepsy. The students and farmers showed significant differences in QOLIE-31 score, cognition, emotional well-being, overall quality of life, energy/fatigue, and social function. In contrast, no significant difference was noted in seizure worry and medication effects across the three different kinds of occupation.
Conclusion: Occupational status might affect the quality of life of Chinese adult patients with epilepsy, and social function is the most important contributing factor.

Despite its inclusion in models of social and ecological determinants of health, work has not been explored in most health inequity research in the United States. Leaving work out of public health inequities research creates a blind spot in our understanding of how inequities are created and impedes our progress toward health equity. We first describe why work is vital to our understanding of observed societal-level health inequities. Next, we outline challenges to incorporating work in the study of health inequities, including (1) the complexity of work as a concept; (2) work’s overlap with socioeconomic position, race, ethnicity, and gender; (3) the development of a parallel line of inquiry into occupational health inequities; and (4) the dearth of precise data with which to explore the relationships between work and health status. Finally, we summarize opportunities for advancing health equity and monitoring progress that could be achieved if researchers and practitioners more robustly include work in their efforts to understand and address health inequities.

What is your mother doing? The influence of mother’s occupational status on her children’s educational outcomes and how it is related to father’s influenceIn this article, we investigated the influence of mother’s occupational status on her child’s educational outcomes and to what extent this depends on father’s occupational status. Based on the Breen and Goldthorpe (1997) model, we expected that mother’s occupational status positively affects the educational outcomes of her child, although this influence may be weaker than father’s influence due to remains of traditional gender roles in the Netherlands. Second, we expected that daughters benefit more from mother’s occupational status than sons. Finally, we derived from the dominance model (Erikson, 1984) that the positive influence of mother’s occupational status becomes weaker when father’s occupational status is higher. The three hypotheses were tested with datasets from the LISS panel in 2012 on 1.176 Dutch participants who grew up in dual earner households. The results of multilevel regression analyses show that mother’s occupational status indeed has a moderately positive effect on her child’s education level. This effect does not differ between sons and daughters and is independent of fathers’ occupational status. Unexpectedly, father’s occupational status does not affect his child’s educational level when mother’s occupational status is taken into account. This suggests that in dual earner households mother’s occupational status is more relevant than father’s occupational status.

The Work Ability Index and single-item Question: Associations with Sick Leave, Symptoms, and health-a Prospective Study of Women on long-term Sick Leave

Objectives This study investigated the association between the work ability index (WAI) and the single-item question on work ability among women working in human service organizations (HSO) currently on long-term sick leave. It also examined the association between the WAI and the single-item question in relation to sick leave, symptoms, and health. Predictive values of the WAI, the changed WAI, the single-item question, and the changed single-item question were investigated for degree of sick leave, symptoms, and health. Methods This cohort study comprised 324 HSO female workers on long-term (>60 days) sick leave, with follow-ups at 6 and 12 months. Participants responded to questionnaires. Data on work ability, sick leave, health, and symptoms were analyzed with regard to associations and predictability. Spearman correlation and mixedmodel analysis were performed for repeated measurements over time. Results The study showed a very strong association between the WAI and the single-item question among all participants. Both the WAI and the single-item question showed similar patterns of associations with sick leave, health, and symptoms. The predictive value for the degree of sick leave and health-related quality of life (HRQoL) was strong for both the WAI and the single-item question, and slightly less strong for vitality, neck pain, both self-rated general and mental health, and behavioral and current stress. Conclusion This study suggests that the single-item question on work ability could be used as a simple indicator for assessing the status and progress of work ability among women on long-term sick leave.

Knowledge management behavior and individual creativity

Creativity is an increasingly important domain of performance largely based on knowledge held and exchanged among employees. Despite the necessity of knowledge exchange, individual employees tend to experience mixed motivation caused by the inherent social dilemma of knowledge sharing. To pragmatically explain how individuals deal with this motivational dilemma, we propose an expanded framework of knowledge management behavior (KMB) that includes knowledge sharing, hiding, and manipulation. Individual choices among these KMBs may be driven by dispositional goal orientations. We also propose that the effects of KMB on creativity of employees vary depending on their social status in a work group. Our analyses based on 214 employees from 37 teams reveal that (i) learning goal orientation increases knowledge sharing and decreases knowledge manipulation; (ii) avoiding goal orientation increases knowledge sharing and manipulation; and (iii) proving goal orientation increases knowledge hiding and manipulation. Knowledge hiding is negatively related to employee creativity, particularly for employees with high social status. Knowledge manipulation is positively related to creativity, particularly for those with high social status. This study develops and validates a theoretical framework explaining the formative process and distinct outcomes of the multifaceted and strategic approaches to KMB at the individual level.

Dirty Work profiles a number of occupations that society deems tainted. The volume's vivid, ethnographic reports focus on the communication that helps workers manage the moral, social, and physical \"stains\" that derive from engaging in such occupations. The creative ways that those who perform such dirty work learn to communicate with each other--and with outsiders--regulate the negative aspects of the work itself and emphasize the positives so that workers can maintain a sense of self-value even while performing devalued occupations.
The Structure of Deference

Current theories of occupational status conceptualize it as either a function of cultural esteem or the symbolic aspect of the class structure. Based on Weber’s definition of status as rooted in either cultural or class conditions, we argue that a consistent operationalization of occupational status must account for both of these dimensions. Using quantitative measures of cultural sentiments for occupational identities, we use affect control theory to model the network deference relations across occupations. We calculate a measure of the extent to which one occupational actor deferring to another is incongruent with cultural expectations for all possible combinations of 304 occupational titles. Because high-status actors are less likely to defer to low-status actors, the degree to which these events violate cultural expectations provides an indicator of the relative status position of different occupations. We assess the construct validity of our new deference score measure using Harris Poll data. Deference scores are more predictive of status rankings from poll data than are occupational prestige scores. We establish criterion validity using five theoretically relevant workplace outcomes: subjective work attachment, job satisfaction, general happiness, the importance of meaningful work, and perceived respect at work.

Employing a comparative experimental design drawing on over 18,000 interviews across eleven countries on four continents, this article revisits the discussion about the economic and cultural drivers of attitudes towards immigrants in advanced democracies. Experiments manipulate the occupational status, skin tone and national origin of immigrants in short vignettes. The results are most consistent with a Sociotropic Economic Threat thesis: In all countries, higher-skilled immigrants are preferred to their lower-skilled counterparts at all levels of native socio-economic status (SES). There is little support for the Labor Market Competition hypothesis, since respondents are not more opposed to immigrants in their own SES stratum. While skin tone itself has little effect in any country, immigrants from Muslim-majority countries do elicit significantly lower levels of support, and racial animus remains a powerful force.

This article provides field-experimental evidence on status goods. We work with an Indonesian bank that markets platinum credit cards to high-income customers. In a first experiment, we show that demand for the platinum card exceeds demand for a nondescript control product with identical benefits, suggesting demand for the pure status aspect of the card. Transaction data reveal that platinum cards are more likely to be used in social contexts, implying social image motivations. In a second experiment, we provide evidence of positional externalities from the consumption of these status goods. A final experiment provides suggestive evidence that increasing self-esteem causally reduces demand for status goods, indicating that social image might be a substitute for self-image.
