Do highly physically active workers die early? A systematic review with meta-analysis of data from 193 696 participants
ObjectiveRecent evidence suggests the existence of a physical activity paradox, with beneficial health outcomes associated with leisure time physical activity, but detrimental health outcomes for those engaging in high level occupational physical activity. This is the first quantitative systematic review of evidence regarding the association between occupational physical activity and all-cause mortality.DesignSystematic review with meta-analysis.Data sourceA literature search was performed in electronic databases PubMed, Embase, CINAHL, PsycINFO and Cochrane.Eligibility criteria for selecting studiesWe screened for peer reviewed articles from prospective studies assessing the association of occupational physical activity with all-cause mortality. A meta-analysis assessed the association of high (compared with low) level occupational physical activity with all-cause mortality, estimating pooled hazard ratios (HR) (with 95% CI).Results2490 unique articles were screened and 33 (from 26 studies) were included. Data from 17 studies (with 193 696 participants) were used in a meta-analysis, showing that men with high level occupational physical activity had an 18% increased risk of early mortality compared with those engaging in low level occupational physical activity (HR 1.18, 95% CI 1.05 to 1.34). No such association was observed among women, for whom instead a tendency for an inverse association was found (HR 0.90, 95% CI 0.80 to 1.01).ConclusionsThe results of this review indicate detrimental health consequences associated with high level occupational physical activity in men, even when adjusting for relevant factors (such as leisure time physical activity). These findings suggest that research and physical activity guidelines may differentiate between occupational and leisure time physical activity.
Nonrelocatable Occupations at Increased Risk During Pandemics: United States, 2018
Objectives. To characterize which occupations in the United States could likely work from home during a pandemic such as COVID-19. Methods. I merged 2018 US Bureau of Labor Statistics (BLS) national employment and wage data with measures ranking the importance of computer use at work and the importance of working with or performing for the public from the BLS O*NET survey. Results. Approximately 25% (35.6 million) of US workers are employed in occupations (such as technology, administrative, financial, and engineering) that could be done from home; the remaining 75% work in occupations (including health care, manufacturing, retail, and food services) that are challenging to do from home. Conclusions. Most US workers are employed in occupations that cannot be done at home, putting 108.4 million workers at increased risk for adverse health outcomes related to working during a pandemic. These workers tend to be lower paid. The stress experienced by lower-income groups, coupled with job insecurity, could result in a large burden of mental health disorders in the United States in addition to increased cases of COVID-19 from workplace transmission.
Suicide by occupation: Systematic review and meta-analysis
Previous research has shown that those employed in certain occupations, such as doctors and farmers, have an elevated risk of suicide, yet little research has sought to synthesise these findings across working-age populations. To summarise published research in this area through systematic review and meta-analysis. Random effects meta-analyses were used to calculate a pooled risk of suicide across occupational skill-level groups. Thirty-four studies were included in the meta-analysis. Elementary professions (e.g. labourers and cleaners) were at elevated risk compared with the working-age population (rate ratio (RR) = 1.84, 95% CI 1.46-2.33), followed by machine operators and deck crew (RR = 1.78, 95% CI 1.22-2.60) and agricultural workers (RR = 1.64, 95% CI 1.19-2.28). Results suggested a stepwise gradient in risk, with the lowest skilled occupations being at greater risk of suicide than the highest skill-level group. This is the first comprehensive meta-analytical review of suicide and occupation. There is a need for future studies to investigate explanations for the observed skill-level differences, particularly in people employed in lower skill-level groups.
Danish registers on personal labour market affiliation
Introduction: Statistics Denmark has a number of registers about labour market affiliation, which includes information on employees, establishments and the relation between employees and establishments. Content: The registers describe a person’s attachment to the labour market, information on the establishments and information on the relations between persons and establishments. The registers presented either include information at one specific point in time each year (end of November) or during the year. Validity and coverage: Both the validity and coverage of the registers are considered to be high. Conclusion: These registers provide valuable information on personal labour market affiliation and can be used to study determinants and consequences of labour market affiliation.
COVID-19 and Essential Workers: A Narrative Review of Health Outcomes and Moral Injury
The COVID-19 pandemic has introduced a number of added obstacles to safe employment for already-challenged essential workers. Essential workers not employed in the health sector generally include racially diverse, low-wage workers whose jobs require close interaction with the public and/or close proximity to their coworkers, placing them at increased risk of infection. A narrative review facilitated the analyses of health outcome data in these workers and contributing factors to illness related to limited workplace protections and a lack of organizational support. Findings suggest that this already marginalized population may also be at increased risk of “moral injury” due to specific work-related factors, such as limited personal protective equipment (PPE) and the failure of the employer, as the safety and health “duty holder,” to protect workers. Evidence suggests that ethical and, in some cases, legally required safety protections benefit not only the individual worker, but an employer’s enterprise and the larger community which can retain access to resilient, essential services.
OPERAS decision support system versus manual job coding: a quantitative analysis on coding time and inter-coder reliability
ObjectivesThe manual coding of job descriptions is time-consuming, expensive and requires expert knowledge. Decision support systems (DSS) provide a valuable alternative by offering automated suggestions that support decision-making, improving efficiency while allowing manual corrections to ensure reliability. However, this claim has not been proven with expert coders. This study aims to fill this omission by comparing manual with decision-supported coding, using the new DSS OPERAS.MethodsFive expert coders proficient in using the French classification systems for occupations PCS2003 and activity sectors NAF2008 each successively coded two subsets of job descriptions from the CONSTANCES cohort manually and using OPERAS. Subsequently, we assessed coding time and inter-coder reliability of assigning occupation and activity sector codes while accounting for individual differences and the perceived usability of OPERAS, measured using the System Usability Scale (SUS; range 0–100).ResultsOPERAS usage substantially outperformed manual coding for all coders on both coding time and inter-coder reliability. The median job description coding time was 38 s using OPERAS versus 60.8 s while manually coding. Inter-coder reliability (in Cohen’s kappa) ranged 0.61–0.70 and 0.56–0.61 for the PCS, while ranging 0.38–0.61 and 0.34–0.61 for the NAF for OPERAS and manual coding, respectively. The average SUS score was 75.5, indicating good usability.ConclusionsCompared with manual coding, using OPERAS as DSS for occupational coding improved coding time and inter-coder reliability. Subsequent comparison studies could use OPERAS’ ISCO-88 and ISCO-68 classification models. Consequently, OPERAS facilitates large, harmonised job coding in large-scale occupational health research.
Malaria risk factors in northern Namibia: The importance of occupation, age and mobility in characterizing high-risk populations
In areas of low and unstable transmission, malaria cases occur in populations with lower access to malaria services and interventions, and in groups with specific malaria risk exposures often away from the household. In support of the Namibian National Vector Borne Disease Program’s drive to better target interventions based upon risk, we implemented a health facility-based case control study aimed to identify risk factors for symptomatic malaria in Zambezi Region, northern Namibia. A total of 770 febrile individuals reporting to 6 health facilities and testing positive by rapid diagnostic test (RDT) between February 2015 and April 2016 were recruited as cases; 641 febrile individuals testing negative by RDT at the same health facilities through June 2016 were recruited as controls. Data on socio-demographics, housing construction, overnight travel, use of malaria prevention and outdoor behaviors at night were collected through interview and recorded on a tablet-based questionnaire. Remotely-sensed environmental data were extracted for geo-located village residence locations. Multivariable logistic regression was conducted to identify risk factors and latent class analyses (LCA) used to identify and characterize high-risk subgroups. The majority of participants (87% of cases and 69% of controls) were recruited during the 2016 transmission season, an outbreak year in Southern Africa. After adjustment, cases were more likely to be cattle herders (Adjusted Odds Ratio (aOR): 4.46 95%CI 1.05–18.96), members of the police or other security personnel (aOR: 4.60 95%CI: 1.16–18.16), and pensioners/unemployed persons (aOR: 2.25 95%CI 1.24–4.08), compared to agricultural workers (most common category). Children (aOR 2.28 95%CI 1.13–4.59) and self-identified students were at higher risk of malaria (aOR: 4.32 95%CI 2.31–8.10). Other actionable risk factors for malaria included housing and behavioral characteristics, including traditional home construction and sleeping in an open structure (versus modern structure: aOR: 2.01 95%CI 1.45–2.79 and aOR: 4.76 95%CI: 2.14–10.57); cross border travel in the prior 30 days (aOR: 10.55 95%CI 2.94–37.84); and outdoor agricultural work at night (aOR: 2.09 95%CI 1.12–3.87). Malaria preventive activities were all protective and included personal use of an insecticide treated net (ITN) (aOR: 0.61 95%CI 0.42–0.87), adequate household ITN coverage (aOR: 0.63 95%CI 0.42–0.94), and household indoor residual spraying (IRS) in the past year (versus never sprayed: (aOR: 0.63 95%CI 0.44–0.90). A number of environmental factors were associated with increased risk of malaria, including lower temperatures, higher rainfall and increased vegetation for the 30 days prior to diagnosis and residing more than 5 minutes from a health facility. LCA identified six classes of cases, with class membership strongly correlated with occupation, age and select behavioral risk factors. Use of ITNs and IRS coverage was similarly low across classes. For malaria elimination these high-risk groups will need targeted and tailored intervention strategies, for example, by implementing alternative delivery methods of interventions through schools and worksites, as well as the use of specific interventions that address outdoor transmission.
The COVID-19 pandemic
The COVID-19 pandemic is discussed. This report points to the importance of occupation as a risk factor but also to the availability and use of appropriate personal protection to mitigate the risk of becoming infected. In addition, well-established socioeconomic factors of health inequalities intermingled with occupations at risk, demonstrated by the fact that most taxi drivers belonged to the same ethnic group and that taxi drivers had higher mortality rates when residing in London. These findings are mirrored in a recent preprint publication from the US state of California, reporting that relative excess mortality was particularly high among food/agriculture, transportation/logistics, facilities, and manufacturing workers.
The Future of Agricultural Jobs in View of Robotization
Robotics and computerization have drastically changed the agricultural production sector and thus moved it into a new automation era. Robots have historically been used for carrying out routine tasks that require physical strength, accuracy, and repeatability, whereas humans are used to engage with more value-added tasks that need reasoning and decision-making skills. On the other hand, robots are also increasingly exploited in several non-routine tasks that require cognitive skills. This technological evolution will create a fundamental and an unavoidable transformation of the agricultural occupations landscape with a high social and economic impact in terms of jobs creation and jobs destruction. To that effect, the aim of the present work is two-fold: (a) to map agricultural occupations in terms of their cognitive/manual and routine/non-routine characteristics and (b) to assess the susceptibility of each agricultural occupation to robotization. Seventeen (17) agricultural occupations were reviewed in relation to the characteristics of each individual task they entail and mapped onto a two-dimensional space representing the manual versus cognitive nature and the routine versus non-routine nature of an occupation. Subsequently, the potential for robotization was investigated, again concerning each task individually, and resulted in a weighted average potential adoption rate for each one of the agricultural occupations. It can be concluded that most of the occupations entail manual tasks that need to be performed in a standardised manner. Considering also that almost 81% of the agricultural work force is involved with these activities, it turns out that there is strong evidence for possible robotization of 70% of the agricultural domain, which, in turn, could affect 56% of the total annual budget dedicated to agricultural occupations. The presented work silhouettes the expected transformation of occupational landscape in agricultural production as an effort for a subsequent identification of social threats in terms of unemployment and job and wages polarization, among others, but also of opportunities in terms of emerged skills and training requirements for a social sustainable development of agricultural domain.
