A Review of Patient and Provider Satisfaction with Telemedicine

Purpose of ReviewThe purpose of this review is to describe the determinants of satisfaction with telemedicine (TM) and how they compare with in-person visits from both the perspective of patients and of providers.Recent FindingsThe use of TM will expand only if patients and providers are at least as satisfied with it as they are with in-person visits. Since deviations from expected care can result in reduced satisfaction regardless of the quality of the visit or objective medical outcomes, it is important to understand and to help form those expectations when possible. Patients consistently report 95–100% satisfaction rate with TM when compared with in-person appointments. They tend to cite the convenience of decreased travel times and costs as the main drivers for satisfaction with TM. Providers tend to be satisfied with TM if they have input into its development, there is administrative support, the technology is reliable and easy to use, and if there is adequate reimbursement for its use.SummarySatisfaction with TM is necessary for adoption of this new technology. To improve satisfaction it is important to consider factors that drive it both for patients and for providers.

But has telemedicine truly changed the way healthcare is delivered or is it merely a temporary fix for a temporary pandemic?Recent FindingsThis global public health emergency has exposed vulnerabilities in our healthcare system and telemedicine has proven to be an effective tool to help increase access to care and improve affordability for patients across all racial, economic, geographic, and technological demographics.SummaryLooking back on the last 20 months or so since the pandemic started, this review attempts to summarize what has gone well and what has not in the telehealth space and concludes that while far from perfect, telemedicine is here to stay.

It also sought to provide readers a foundation of human factors thinking and methods that could be employed within their own practice.Recent FindingsThere are only a handful of articles that discuss the importance of user-centered design and human factors principles in relation to telemedicine systems.SummaryMost articles come to the conclusion that design flaws could have been avoided by involving stakeholders in the design and implementation of telemedicine. However, many of them lack the guidance for those who find themselves having to choose, implement, or use unwieldy systems. With this in mind, this paper provides a series of human factors principles, real-world questions, methods, and resources for those who may find themselves considering, implementing, sustaining, or using telemedicine in their own healthcare settings.

The Mayo Clinic has reworked its entire telemedicine interest to a model with centralized operations, one virtual technology platform, standardized training, and connectedness for all of its locations. The National Quality Forum spent 2016 and 2017 formulating 70 some pages of recommendations for expanded measures to valuate telemedicine over the foreseeable future.
