Polycystic ovarian syndrome (PCOS) is one of the most common heterogeneous endocrine and metabolic disorders in premenopausal women. It is a complex multifactorial disorder with strong epigenetic and environmental influences, including factors related to eating habits and lifestyle. There is a close relationship between obesity and PCOS. Weight gain and obesity are often clinical symptoms manifested by biochemical markers. Moreover, abdominal obesity in women with PCOS is involved in the development of inflammatory changes. A significant share of balanced therapies correcting the lifestyle of patients is suggested, e.g., with the implementation of appropriate diets to minimize exposure to inflammatory factors and prevent abnormal immune system stimulation. In the case of obese patients with PCOS, planning a diet program and supporting the motivation to change eating habits play an important role to lose weight and lower BMI. Probiotics/synbiotic supplementation may enhance weight loss during the diet program and additionally positively affect metabolic and inflammatory factors by improving the intestinal microbiome.

Effects of spray dried yeast (Saccharomyces cerevisiae) on growth performance and carcass characteristics, gut health, cecal microbiota profile and apparent ileal digestibility of protein, amino acids and energy in broilers

The aim of present study was to determine the effects of supplementation of either synbiotic or probiotic on growth performance and carcass characteristics, gut health, cecal microbiota prolife and apparent ileal digestibility of protein, amino acids, and energy in broilers. Two hundred and forty-day-old straight-run broilers (Ross 308) were allotted randomly to 1 of 5 dietary treatments including basal diet (control), supplemented with either synbiotic (Nutromax P) or probiotic (Actera), each at 0.5 and 1 g/kg of the diet for 5 weeks. The overall findings of the study indicated better (p < 0.05) growth performance of broilers by synbiotic supplementation (1 g/kg) compared with those fed probiotic (1 g/kg) supplemented and control diets. The broilers consuming diet supplemented with 1 g/kg synbiotic has an increased carcass yield in comparison with those fed control diet. The findings of gut health indicated significantly increased villus height and goblet cells, by synbiotic supplementation (1 g/kg), compared with control diet in broilers. The broilers fed 1 g/kg synbiotic supplemented diets had 18% increased protein, 9 to 31% higher amino acid, and 34% better energy digestibility, whereas 8.4% decreased protein digestibility in broilers fed probiotic (1 g/kg) supplemented compared with control diet in broilers. The broilers fed synbiotic (1 g/kg) supplemented diets had increased cecal Lactobacillus and decreased Salmonella, E. coli, and Clostridium count compared with those fed control diet. In conclusion, synbiotic supplementation (1 g/kg) resulted in improved production performance, balanced cecal microbial composition, and better digestibility of nutrients in broilers compared with those fed control and diets supplemented with probiotics.

Background
We and others have previously shown that alterations in the mammalian gut microbiome are associated with diet, notably early life exposure to a maternal high fat diet (HFD). Here, we aimed to further these studies by examining alterations in the gut microbiome of juvenile Japanese macaques (
Macaca fuscata
) that were exposed to a maternal HFD, weaned onto a control diet, and later supplemented with a synbiotic comprised of psyllium seed and
Enterococcus
and
Lactobacillus
species.
Results
Eighteen month old offspring (
n
= 7) of 36% HFD fed dams were fed a control (14% fat) diet post weaning, then were synbiotic supplemented for 75 days and longitudinal stool and serum samples were obtained. All stool samples were subjected to 16S rRNA metagenomic sequencing, and microbiome profiles and serum lipids and triglycerides were compared to untreated, healthy age matched and diet matched controls (
n
= 7). Overall, 16S-based metagenomic analysis revealed that supplementation exerted minimal alterations to the gut microbiome including transient increased abundance of
Lactobacillus
species and decreased abundance of few bacterial genera, including
Faecalibacterium
and
Anaerovibrio
. However, serum lipid analysis revealed significant decreases in triglycerides, cholesterol, and LDL (
p
< 0.05). Nevertheless, supplemented juveniles challenged 4 months later were not protected from HFD-induced gut dysbiosis.
Conclusions
Synbiotic supplementation is temporally associated with alterations in the gut microbiome and host lipid profiles of juvenile Japanese macaques that were previously exposed to a maternal HFD. Despite these presumptive temporal benefits, a protective effect against later HFD-challenge gut dysbiosis was not observed.

Synbiotic (FamiLact) administration in idiopathic male infertility enhances sperm quality, DNA integrity, and chromatin status: A triple-blinded randomized clinical trial

Background: Idiopathic male infertility is often treated empirically. A recent body of evidence has indicated the association between pro ± prebiotics administration and improvement in semen parameters.
Objective: To assess the effect of FamiLact (probiotic + prebiotic) administration on male subjects with idiopathic infertility.
Materials and Methods: Fifty-six men with idiopathic male infertility were randomly/equally divided into two groups. Men in the case and control groups received 500 mg of FamiLact and an identical placebo for 80 days, respectively. A semen sample was obtained from each of the participants before initiation and after the termination of the treatment course. Samples underwent regular semen analysis and were further analyzed to assess the level of DNA damage (sperm chromatin structure assay), oxidative stress (BODIPY C11 staining), and protamine deficiency (chromomycin-A3 staining) in spermatozoa.
Results: No significant difference was observed between the baseline values of both groups. After intervention, mean sperm concentration, motility, and normal morphology were significantly higher in the FamiLact group compared to the placebo group (p < 0.05). In the FamiLact receivers, we detected improvement regarding the following parameters: concentration, motility, abnormal morphology, sperm lipid peroxidation, and DNA fragmentation (p ≤ 0.02). Likewise, in the placebo group, we noticed a decrease in the post-medication mean value of DNA fragmentation (p = 0.03) while observing no significant difference regarding other parameters.
Conclusion: FamiLact administration improves sperm concentration, motility, and abnormal morphology and decrease sperm DNA damage, possibly through alleviating oxidative stress in the seminal fluid.

This review investigates dietary strategies aimed at enhancing insulin sensitivity and reducing the risk of type 2 diabetes (T2D), with a special focus on recent meta-analysis evidence supporting the use of probiotics and synbiotics. Dietary interventions, such as the Mediterranean, low-carbohydrate, high-fiber, and plant-based diets, have shown significant promise in modulating glucose metabolism through mechanisms involving insulin receptor activation, glucagon-like peptide 1 (GLP-1) receptor signaling, and peroxisome proliferator–activated receptors (PPARs) regulation. Additionally, probiotics and synbiotics have emerged as complementary approaches, enhancing gut microbiota composition and improving insulin sensitivity through reduced inflammation and oxidative stress. The integration of molecular insights, including the role of siRNA and miRNA in gene regulation, highlights the potential for personalized nutritional therapies. This review provides evidence-based recommendations for daily dietary practices and suggests future research directions targeting combined dietary and molecular interventions for T2D management.

Alongside advances in understanding the pathophysiology of sepsis, there have been tremendous strides in understanding the pervasive role of the gut microbiota in systemic host resistance. In pre-clinical models, a diverse and balanced gut microbiota enhances host immunity to both enteric and systemic pathogens. Disturbance of this balance increases susceptibility to sepsis and sepsis-related organ dysfunction, while restoration of the gut microbiome is protective. Patients with sepsis have a profoundly distorted composition of the intestinal microbiota, but the impact and therapeutic potential of the microbiome is not well-established in human sepsis. Modulation of the microbiota consists of either resupplying the pool of beneficial microbes by administration of probiotics, improving the intestinal microenvironment to enhance the growth of beneficial species by dietary interventions and prebiotics, or by totally recolonizing the gut with a fecal microbiota transplantation (FMT). We propose that there are three potential opportunities to utilize these treatment modalities over the course of sepsis: to decrease sepsis incidence, to improve sepsis outcome, and to decrease late mortality after sepsis. Exploring these three avenues will provide insight into how disturbances of the microbiota can predispose to, or even perpetuate the dysregulated immune response associated with this syndrome, which in turn could be associated with improved sepsis management.

Objective: This experiment investigated the effects of synbiotic supplementation produced from probiotics and prebiotics from trimmed asparagus by-products (TABP) on broiler chicken diets in order to yield designer meat production. Materials and Methods: A total of 320 one-day-old Ross 308® chicks were randomly allocated to dietary treatments with four replications each (n = 20). The dietary treatments were composed of a control group without supplementation and treatment groups fed with 10, 30, and 50 gm/kg of TABP supplementation in diets combined with 2 gm/kg probiotics. Results: The results showed that broilers fed dietary supplementation of TABP with 2 gm/kg had a lower level of total cholesterol and low-density lipoprotein cholesterol in the serum, which reduced the atherogenic indices of the serum, such as cardiac risk ratio and atherogenic coef¬ficient (p < 0.05). In addition, the cholesterol content and the fatty acid profile of breast meat, including palmitic acid, oleic acid, saturated fatty acid, and omega 9 levels, also declined with the increasing levels of TABP inclusion (p < 0.05). Moreover, the supplementation of TABP in diets caused a decline in the atherogenic and thrombogenicity indices and a rise in Δ-9 desaturase (16) index and hypocholesterolemic to the hypercholesterolemic ratio of meat (p < 0.05). Conclusion: Synbiotic supplementation of up to 30 gm/kg TABP combined with 2 gm/kg probiotics in the chicken diet can potentially be utilized for the production of designer meat.

This present study investigates the possibility of enriching adult Artemia franciscana with singular or combined administration of Pediococcus acidilactici and fructooligosaccharide (FOS). The experiment was conducted in a completely randomized design with four treatments, including synbiotic, P. acidilactici +FOS, probiotic, P. acidilactici, prebiotic, FOS and control. To evaluate the enrichment of adult Artemia with each treatment, sampling was performed at 2, 4 and 6 hrs post enrichment. The bacterial counts was measured using the microbial culture and expressed as log CFU per g Artemia. A pre-experiment has been designed and probiotic was used in three levels and prebiotic was used in three levels of 1, 2 and 5 g per litter of suspension. Based on pre experiment results, 10^sup 8^ CFU per litter of probiotic and 5 g per litter of prebiotic was selected. The results of this experiment showed that over time, consumed bacteria increased by adult Artemia and there was a significant difference between sampling in terms of ingested bacteria.

In vitro digestive system simulation and anticancer activity of soymilk fermented by probiotics and synbiotics immobilised on agro-industrial residues

In this study, a variety of probiotic strains, including
Lactiplantibacillus plantarum, Lacticaseibacillus casei, Lactobacillus acidophilus, Streptococcus thermophilus, Bifidobacterium longum, Limosilactobacillus reuteri, Lactobacillus delbrueckii
subsp.
bulgaricus, Lacticaseibacillus rhamnosus,
and
Bifidobacterium bifidum
, were utilized for soymilk fermentation both as free cells and as synbiotics on agro-industrial residuals such as okara, whey protein, banana peels, apple pomace, sugarcane bagasse, orange peels, and lemon peels. Among these,
Lacticaseibacillus rhamnosus
emerged as the most significant strain for soymilk fermentation, exhibiting a viability of 10.47 log cfu/mL, a pH of 4.41, total acidity of 1.12%, and organic acid contents (lactic and acetic acid) of 11.20 and 7.50 g/L, respectively. As a synbiotic
Lacticaseibacillus rhamnosus
immobilised on okara, showed even more impressive results, with a viability of 12.98 log cfu/mL, a pH of 4.31, total acidity of 1.27%, and organic acid contents of 13.90 and 9.30 g/L, respectively. Over a 12-h fermentation period, cell viability values increased by 10.47-fold in free cells and 11.19-fold in synbiotics. Synbiotic supplementation of fermented soymilk proved more beneficial than free cells in terms of viability, acidity, and organic acid content. Furthermore, when synbiotic fermented soymilk was freeze-dried to simulate the digestive system in vitro, synbiotics and freeze-dried cells demonstrated superior gastrointestinal tract survival compared to free cells. Both the probiotic bacteria and the synbiotics exhibited cytotoxicity against colon and liver cancer cell lines, with half-maximal inhibitory concentrations ranging from 41.96 to 61.52 μL/well.

Does the use of probiotics/synbiotics prevent postoperative infections in patients undergoing abdominal surgery? A meta-analysis of randomized controlled trials

Background Advances in surgery have considerably lowered postoperative morbidity. However, infection remains a considerable morbidity factor. The aim of this review is to identify the potential benefit(s) of the perioperative administration of probiotics/synbiotics to patients undergoing abdominal surgery. Methods We searched PubMed, Scopus, Web of Science, and Cochrane library to identify randomized controlled trials (RCTs) that studied the perioperative administration of probiotics/synbiotics to patients undergoing abdominal surgery. Results Nine RCTs studying 733 patients were included in our review. The incidence of postoperative pneumonia, cholangitis, and any infections as well as the duration of postoperative hospital stay and length of antibiotic therapy were lower among patients receiving probiotics than in the control group [six RCTs, 355 patients, odds ratio (OR) 0.24, 95% confidence interval (CI) 0.09-0.68; three RCTs, 209 patients, OR 0.18, 95% CI 0.05-0.57; seven RCTs, 514 patients, OR 0.26, 95% CI 0.12-0.55; five RCTs, 313 patients, OR -2.70, 95% CI -5.15 to -0.25; four RCTs, 250 patients, OR -4.01, 95% CI -5.11 to -2.92, respectively], while the incidence of postoperative wound infection, urinary tract infection, intra-abdominal abscess, and mortality was not different between patients of the compared groups (six RCTs, 355 patients, OR 0.52, 95% CI 0.23-1.18; five RCTs, 313 patients, OR 0.44, 95% CI 0.04-5.54; four RCTs, 226 patients, OR 0.44, 95% CI 0.12-1.59; nine RCTs, 685 patients, OR 0.98, 95% CI 0.29-3.29, respectively). Conclusion The use of probiotics/synbiotics may reduce postoperative infections after abdominal surgery. This is a promising infection-preventive measure that may decrease morbidity, length of antibiotic therapy, duration of hospital stay, and pressure for emergence of antimicrobial resistance. However, the results of this meta-analysis should be interpreted with caution due to the significant heterogeneity of the studies included.
