5PSQ-097 Triple whammy interaction: improving patients’ safety

BackgroundConcomitant treatment with renin-angiotensin system inhibitors (ACEI/ARB), diuretics and non-steroidal anti-inflammatory drugs (NSAID) has been named as triple whammy (TW). This interaction can produce acute kidney injury (AKI).PurposeTo implement a strategy in order to avoid the development of AKI due to TW interaction.Material and methodsA so-called ‘Avoiding TW strategy’ was implemented including the following activities: a multidisciplinary group (nephrologists, general practitioners (GP) and clinical pharmacists (CP)) was established to design the strategy; evidence on TW interaction and AKI was assessed; criteria for selection of candidates for intervention was agreed (concomitant use of ACEI/ARB, diuretics and NSAID); CP presented the programme to GPs; patients who were candidates for intervention were retrieved through an in-house developed software (OBSERVA) integrated in electronic clinical records in our region; a deprescription proposal was included in all retrieved clinical records with information about the risk of developing AKI due to the combination, suggesting the doctor to withdraw the NSAID and, if this was not possible, monitoring renal function and serum potassium levels was recommended; and valuation of NSAID withdrawal was planned.ResultsThe TW optimisation strategy was created and 1699 proposals were sent in August 2018. NSAID deprescription proposals were distributed among the different groups: M01AE (propionic acid derivatives): 54.3%; M01AH (coxibs): 27.8%; M01AB (acetic acid derivatives): 15.0%; M01AC (oxicams): 2.7%; M01AG (fenamates): 0.1%; and M01AX (other NSAID): 0.1%.Preliminary results, 2 months after the implementation, showed that 15% of proposals were evaluated by GPs, with an acceptance rate of 82%.ConclusionPharmacological interactions must be considered even more when they cause important morbidity such as AKI.CP intervention through electronic clinical records optimises pharmacotherapy and may reduce adverse events and improve patients’ safety.References and/or acknowledgements1. The academic grant process, if not broken, has become highly inefficient. In the 1970s, 40 percent to 50 percent of applications were green-lighted, and it was relatively easy for agencies to pick and fund the best proposals. Today, less money and more applicants mean only 10 percent to 20 percent receive funding. Instead of a competition between best ideas, it's now a contest to see who can write the best proposals, says Carl Bergstrom, a University of Washington biologist. One easy fix would be to give agencies more money for grants, but that's a heavy lift politically.

The European Huntington’s Disease Network (EHDN), established in 2003, is a non-profit research network with the mission of advancing research, facilitating conduct of clinical trials, and improving clinical care in HD. EHDN forms a platform for clinicians, scientists, academics, patients, and family members to work together to achieve these goals.The EHDN offers membership to those with an interest in/directly affected by HD; >200 European HD clinical and basic science centres and >3400 individuals are members.EHDN hosts a bi-annual plenary meeting, one of the world’s largest conferences dedicated to Huntington’s disease. A fellowship exchange programme has been established, to facilitate training of young HD professionals from countries where HD care and facilities are developing.The EHDN is governed by an Executive Committee, responsible for overseeing activities and establishing scientific strategy, with a Scientific Bioethical Advisory Committee responsible for reviewing research proposals. EHDN Central Coordination manages network operations, with regional Language Area Coordinators as the bridge between the EHDN and the clinical centres, liaising with the HD patient and research community and monitoring Enroll-HD study and platform data.EHDN offers review of clinical trial and study protocols, with endorsement given for protocols of high scientific and ethical quality. This statement of endorsement is valued within the HD community.EHDN Working Groups and Task Forces address key HD research topics, supported by the Think Tank, experts with in-depth knowledge of EHDN scientific activities. EHDN supports researchers with identifying grant and funding opportunities and by awarding seed funds. Background Chatbots are computer programs that use artificial intelligence (AI) and natural language processing (NLP) to simulate conversations with humans. One such chatbot is ChatGPT, which uses the third-generation generative pre-trained transformer (GPT-3) developed by OpenAI. ChatGPT has been praised for its ability to generate text, but concerns have been raised about its accuracy and precision in generating data, as well as legal issues related to references. This study aims to investigate the frequency of AI hallucination in research proposals entirely drafted by ChatGPT. Methodology An analytical design was employed to investigate AI hallucination by ChatGPT. A total of 178 references listed by ChatGPT were verified for inclusion in the study. Statistical analysis was performed by five researchers who entered their data into a Google Form, and the final results were represented using pie charts and tables. Results Out of the 178 references analyzed, 69 references did not have a Digital Object Identifier (DOI), and 28 references neither turned up on Google search nor had an existing DOI. Three references were listed from books and not research articles. These observations suggest that ChatGPT's ability to generate reliable references for research topics may be limited by the availability of DOI and the accessibility of online articles. Conclusions The study highlights the potential limitations of ChatGPT's ability to generate reliable references for research proposals. AI hallucination is a problem that may negatively impact decision-making and may give rise to ethical and legal problems. Improving the training inputs by including diverse, accurate, and contextually relevant data sets along with frequent updates to the training models could potentially help address these issues. However, until these issues are addressed, researchers using ChatGPT should exercise caution in relying solely on the references generated by the AI chatbot.
