Food insecurity and maternal depression in rural, low-income families: a longitudinal investigation
The purpose of the present study was to examine the relationship between household food insecurity and maternal depression in a rural sample to determine whether food insecurity predicted mothers' depression over time or vice versa. The study employed a prospective design using three waves of data from 'Rural Families Speak', a multi-state study of low-income rural families in the USA. Food insecurity was measured using the Core Food Security Module and depression was measured using the Center for Epidemiologic Studies-Depression Scale. A structural equation model was fit to the data using the AMOS software package. Sixteen states in the USA (California, Indiana, Kentucky, Louisiana, Massachusetts, Maryland, Michigan, Minnesota, Nebraska, New Hampshire, New York, Ohio, Oregon, South Dakota, West Virginia, Wyoming) between 2000 and 2002. Subjects included 413 women with at least one child under the age of 13 years living in the home. Findings based on the 184 subjects with complete data indicated that the causal relationship between household food insecurity and depression is bidirectional (P = 0.034 for causation from depression to food insecurity, P = 0.003 for causation from food insecurity to depression, chi(2)/df = 1.835, root-mean-square error of approximation = 0.068, comparative fit index = 0.989). Findings based on all 413 subjects after imputation of missing values also indicated bidirectionality. The recursive relationship between food insecurity and depression has implications for US nutrition, mental health and poverty policies. The study highlights the need to integrate programmes addressing food insecurity and poor mental health for the population of rural, low-income women.

Rural Food Insecurity
Researchers have noted large spatial variations in rates of food insecurity. But little research exists on why this is so and the impacts it has on rural families. Drawing on a mixed-methods longitudinal study with 124 poor and working-class households in North Carolina, we analyze the processes that shape lower-income rural families’ access to food. We trace the narratives of three families whose stories are emblematic of themes from the larger data set to illumine how space and context influence families’ experiences across the life course. As the caregivers in our study navigated how to feed their families, living in a rural area shaped the resources and often precarious forms of support that they drew on from their social networks, local communities, and the state.

Reflection in Rural Family Medicine Education
Reflection in medical education is vital for students’ development as professionals. The lack of medical educators in rural family medicine can impinge on the effective reflection of residents’ learning. Hence, based on qualitative research, we proposed a framework regarding reflection in rural family medicine education, indicating when, where, and how reflection is performed and progresses. The contents of reflection include clinical issues regarding knowledge and skills, professionalism in clinical decisions, and work-life balance. The settings of reflection include conference rooms, clinical wards, residents’ desks, and hospital hallways. The timing of educational reflection includes during and after patient examination and discussion with various professionals, before finishing work, and during “doorknob” times (right before going back home). Rural medical teachers need competence as clinicians and medical educators to promote learning in medical residents and sustain rural medical care. Furthermore, medical teachers must communicate and collaborate with medical residents and nurses for educational reflection to take place in rural family medicine education, especially regarding professionalism. In rural family medicine education, reflection can be performed in various clinical situations through collaboration with learners and various medical professionals, aiding the enrichment of residents’ learning and sustainability of rural medical care.
Fostering on the Farm
From 1870 until after World War I, reformers led an effort to place children from orphanages, asylums, and children's homes with farming families. The farmers received free labor in return for providing room and board. Reformers, meanwhile, believed children learned lessons in family life, citizenry, and work habits that institutions simply could not provide. Drawing on institution records, correspondence from children and placement families, and state reports, Megan Birk scrutinizes how the farm system developed--and how the children involved may have become some of America's last indentured laborers. Between 1850 and 1900, up to one-third of farm homes contained children from outside the family. Birk reveals how the nostalgia attached to misplaced perceptions about healthy, family-based labor masked the realities of abuse, overwork, and loveless upbringings endemic in the system. She also considers how rural people cared for their own children while being bombarded with dependents from elsewhere. Finally, Birk traces how the ills associated with rural placement eventually forced reformers to transition to a system of paid foster care, adoptions, and family preservation.

The Influence of Aging Population in Rural Families on Farmers’ Willingness to Withdraw from Homesteads in Shenyang, Liaoning Province, China
Population aging presents a significant global challenge. In China, the aging of the rural population coincides with inefficient rural homestead utilization. While the Chinese government has enacted policies to address this, their impact remains limited. Utilizing survey data from 403 rural families in Shenyang, Liaoning Province, China, this study applies the binary Logit and mediating effect models to analyze the impact of rural family population aging on farmers’ willingness to withdraw from homesteads with compensation and their compensation preference. Key findings include: (1) Family population aging intensifies farmers’ willingness to withdraw from homesteads, with a stronger preference for non-monetary compensation as aging increases. (2) Regarding the willingness to withdraw with compensation, farmers’ cognition of homestead security value masks the effect by 4.71%, while asset value cognition has no mediating effect. (3) With regard to promoting non-monetary compensation choices, farmers’ homestead asset value cognition fully mediates at 16.01%, but security value cognition is without mediating effect. Based on these findings, it is recommended that the government crafts tailored homestead withdrawal policies considering farmers’ family age structure. Further, efforts should aim at refining farmers’ understanding of homestead values, promoting a blend of non-monetary and monetary compensations.
