PO-0140Whether Antimicrobial Therapy Affect Fecal Excretion Time In Paediatric Patients Of Nontyphoid Salmonellosis With Different Severity

ObjectiveTo investigate whether the administration of antibiotics affect faecal excretion time in paediatric salmonellosis with different severity.MethodsChildren with nontyphoid salmonellosis admitted to Kaohsiung Veterans General Hospital, Taiwan who consented to receive consecutive stool cultures every 4-7 days till two consecutive negative results between 2005 and 2013 were enrolled. Faecal excretion time was defined as the timeframe of the first positive stool culture and the first of two consecutive negative results. The Severity Score was used to stratify the patients as the severe, moderate and mild group according to their fever days before admission, band cells in peripheral blood and C-reactive protein. Patients were classified into no antibiotics (mild disease without antibiotic), concordant (severe patients receiving antibiotics active in vitro) and inappropriate therapy group (mild patients receiving antibiotics or severe patients receiving antibiotics resistant in vitro).Results53 patients were enrolled. Compared with no antibiotic group (n = 28), the concordant group (n = 11) had comparable faecal excretion time (12.36 vs 9.79 days), fever days after admission and hospital stay (2.45 vs 2.25; 6.45 vs 6.39 days). The inappropriate group (n = 14) had significantly longer faecal excretion time (24.50 vs 9.79 days), hospital stay (8.50 vs 6.39 days) and higher platelet count (333.43 vs 269.82 1,000/mm3) than no antibiotics group. With further multiple regression analysis, only inappropriate antibiotic use affected significantly (p = 0.008) on faecal excretion time.ConclusionsAntibiotics used in severe children with salmonellosis didn't prolong faecal excretion time, but inadequate use of antibiotics prolonged faecal excretion time in Salmonellosis.

super(18)F-FDG PET/CT: a review of diagnostic and prognostic features in multiple myeloma and related disorders

Conventional radiographic skeletal survey has been for many years the gold standard to detect the occurrence of osteolytic lesions in patients with multiple myeloma (MM). However, the introduction of more sensitive imaging procedures has resulted in an updated anatomic and functional Durie and Salmon \"plus\" staging system and has remarkably changed the diagnostic and prognostic approach to this tumor. It is now established that super(18)fluorine-fluorodeoxyglucose ( super(18)F-FDG) positron-emission tomography (PET) combined with low-dose computed tomography (CT), shortly designated PET/CT, exhibits a higher screening and diagnostic sensitivity and specificity over the skeleton X-ray. In patients with monoclonal gammopathy of undetermined significance and in those with smoldering MM, PET/CT is consistently unable to detect focal and/or diffuse marrow abnormalities. Conversely, based on a systematic review of 18 studies comprising almost 800 MM patients, PET/CT was able to detect MM osteolytic lesions with a sensitivity of approximately 80-90 % and a specificity of 80-100 %. Importantly, a poor degree of concordance has also been emphasized between PET/CT and whole-body magnetic resonance imaging (WB-MRI) in that when both techniques were applied to the same patients, double-positive results were recorded in approximately 30 % of the cases, but in the majority of them, a higher number of lesions were revealed with PET/CT than with MRI. Double-negative results, on the other hand, were found in about 22 % of the patients. Because PET/CT is able to identify tumor foci throughout the body, it can be usefully applied to the study of solitary bone plasmacytoma and extra-medullary plasmacytoma: In both conditions, the detection of additional, previously overlooked sites of skeletal involvement would falsify the diagnosis of single-district disease, upstage the tumor, and therefore require a different therapeutic approach. In addition, although PET/CT is poorly sensitive to diffuse bone marrow infiltration, it can anticipate a site of impending fracture throughout the body and can discriminate old from new pathologic fractures. MRI should, however, be preferred when vertebral bodies are suspected to be involved and the risk of vertebral fracture is to be assessed. PET/CT is a sensitive and reliable procedure to evaluate the response to chemotherapy and/or radiotherapy, which is shown by a remarkable reduction and sometimes total disappearance of FDG accumulation in the involved bony structures, although these structures remain morphologically abnormal. Conversely, an increased focal uptake of FDG in apparent remission patients often precedes clinically overt relapse. PET/CT should be preferred to other imaging techniques to assess the remission status after autologous stem cell transplantation. In patients with primary and remission-induced non-secretory MM, the use of PET/CT may help to early detect single or multiple districts of focal non-secretory relapse. Osteonecrosis of the jaw, its location, and extent in MM patients receiving bis-phosphonates are better defined by both PET/CT and contrast-enhanced MRI compared with dental panoramic views derived from cone beam CT imaging. Little is known as to the possible role of PET/CT in the assessment of disease extension, tumor load, and response to therapy in patients with Waldenstrom's macroglobulinemia (WM). In a study conducted on 35 WM patients, comparative PET/CT before and after therapy was able to detect positive findings in 83 % of the patients, in contrast with the previous results achieved with conventional imaging that reported visceral involvement in much lower percentages. Similarly scanty are the data on the use of PET/CT in localized and systemic amyloidosis, given the small number of patients studied so far. A retrospective study has shown that, at variance from super(123)Iodine-serum amyloid P component ( super(123)I-SAP) scintigraphy, which was found to be positive in about one-third of the patients with localized amyloidosis, an increased FDG uptake was detected at the amyloid site in virtually all of them. On the contrary, none of the patients with systemic amyloidosis showed an increased FDG uptake in sites of known deposition, whereas super(123)I-SAP scintigraphy tested positive in the large majority of them. In another study, however, no such remarkable difference of positive PET/CT scans between localized and systemic amyloidosis was reported. Finally, false-positive and false-negative PET/CT findings can occur in different conditions that should be kept in mind to avoid wrong or omitted diagnoses.

Indicators of Climate Change in Idaho: An Assessment Framework for Coupling Biophysical Change and Social Perception super(a)

Climate change is well documented at the global scale, but local and regional changes are not as well understood. Finer, local- to regional-scale information is needed for creating specific, place-based planning and adaption efforts. Here the development of an indicator-focused climate change assessment in Idaho is described. This interdisciplinary framework couples end users' data needs with observed, biophysical changes at local to regional scales. An online statewide survey of natural resource professionals was conducted to assess the perceived impacts from climate change and determine the biophysical data needed to measure those impacts. Changes to water resources and wildfire risk were the highest areas of concern among resource professionals. Guided by the survey results, 15 biophysical indicator datasets were summarized that included direct climate metrics (e.g., air temperature) and indicators only partially influenced by climate (e.g., wildfire). Quantitative changes in indicators were determined using time series analysis from 1975 to 2010. Indicators displayed trends of varying likelihood over the analysis period, including increasing growing-season length, increasing annual temperature, increasing forest area burned, changing mountain bluebird and lilac phenology, increasing precipitation intensity, earlier center of timing of streamflow, and decreased 1 April snowpack; changes in volumetric streamflow, salmon migration dates, and stream temperature displayed the least likelihood. A final conceptual framework derived from the social and biophysical data provides an interdisciplinary case example useful for consideration by others when choosing indicators at local to regional scales for climate change assessments.

A Cabled Acoustic Telemetry System for Detecting and Tracking Juvenile Salmon: Part 2. Three-Dimensional Tracking and Passage Outcomes

In Part 1 of this paper, we presented the engineering design and instrumentation of the Juvenile Salmon Acoustic Telemetry System (JSATS) cabled system, a nonproprietary sensing technology developed by the U.S. Army Corps of Engineers, Portland District (Oregon, USA) to meet the needs for monitoring the survival of juvenile salmonids through the hydroelectric facilities within the Federal Columbia River Power System. Here in Part 2, we describe how the JSATS cabled system was employed as a reference sensor network for detecting and tracking juvenile salmon. Time-of-arrival data for valid detections on four hydrophones were used to solve for the three-dimensional (3D) position of fish surgically implanted with JSATS acoustic transmitters. Validation tests demonstrated high accuracy of 3D tracking up to 100 m upstream from the John Day Dam spillway. The along-darn component, used for assigning the route of fish passage, had the highest accuracy; the median errors ranged from 0.02 to 0.22 m, and root mean square errors ranged from 0.07 to 0.56 m at distances up to 100 m. For the 2008 case study at John Day Dam, the range for 3D tracking was more than 100 m upstream of the dam face where hydrophones were deployed, and detection and tracking probabilities of fish tagged with JSATS acoustic transmitters were higher than 98%. JSATS cabled systems have been successfully deployed on several major dams to acquire information for salmon protection and for development of more \"fish-friendly\" hydroelectric facilities.

Hydrologic Alterations from Climate Change Inform Assessment of Ecological Risk to Pacific Salmon in Bristol Bay, Alaska: e0143905

We developed an integrated hydrologic model of the upper Nushagak and Kvichak watersheds in the Bristol Bay region of southwestern Alaska, a region under substantial development pressure from large-scale copper mining. We incorporated climate change scenarios into this model to evaluate how hydrologic regimes and stream temperatures might change in a future climate, and to summarize indicators of hydrologic alteration that are relevant to salmon habitat ecology and life history. Model simulations project substantial changes in mean winter flow, peak flow dates, and water temperature by 2100. In particular, we find that annual hydrographs will no longer be dominated by a single spring thaw event, but will instead be characterized by numerous high flow events throughout the winter. Stream temperatures increase in all future scenarios, although these temperature increases are moderated relative to air temperatures by cool baseflow inputs during the summer months. Projected changes to flow and stream temperature could influence salmon through alterations in the suitability of spawning gravels, changes in the duration of incubation, increased growth during juvenile stages, and increased exposure to chronic and acute temperature stress. These climate-modulated changes represent a shifting baseline in salmon habitat quality and quantity in the future, and an important consideration to adequately assess the types and magnitude of risks associated with proposed large-scale mining in the region.

Interactions of Isophorone Derivatives with DNA: Spectroscopic Studies: e0129817

Interactions of three new isophorone derivatives, Isoa Isob and Isoc with salmon testes DNA have been investigated using UV-Vis, fluorescence and circular dichroism spectroscopic methods. All the studied compounds interact with DNA through intercalative binding mode. The stoichiometry of the isophorone/DNA adducts was found to be 1:1. The fluorescence quenching data revealed a binding interaction with the base pairs of DNA. The CD data indicate that all the investigated isophorones induce DNA modifications.

Salmon lice infection on wild salmonids in marine protected areas: an evaluation of the Norwegian jNational Salmon Fjordsr

In Norway, 29 fjords and 52 rivers have been designated for protection in order to prevent the infection of important populations of wild salmonids with salmon lice of farm origin. We evaluated the effect of this protection on the lice infection pressure for wild salmonids based on lice counts performed on wild-caught sea trout and Arctic charr inside onethird of these protected fjords (known as 'National Salmon Fjords'). Results indicate that these areas may provide a certain extent of protection against lice of farm origin, but their configuration will play a key role in their success. When the size and shape of a protected area are such that fish farms are kept at a minimum distance (calculated here as at least 30 km, but this distance is likely site-dependent), wild fish seem unaffected by the direct lice infection pressure imposed by fish farms. In contrast, the effects of small protected fjords were strongly dependent on the production pattern of the aquaculture industry in the surrounding area, and we found a clear correlation between lice levels on wild salmonids and lice production in nearby salmon farms. To establish more precise management practices, both in National Salmon Fjords and other fjord systems along the Norwegian coast, the development and validation of accurate distribution and abundance models for the dispersion of planktonic lice larvae is needed; this could also be the basis for an area management system based on 'maximum sustainable lice loads' or Tice quotas.'

Effects of Ocean Acidification on Temperate Coastal Marine Ecosystems and Fisheries in the Northeast Pacific: e0117533

As the oceans absorb anthropogenic CO2 they become more acidic, a problem termed ocean acidification (OA). Since this increase in CO2 is occurring rapidly, OA may have profound implications for marine ecosystems. In the temperate northeast Pacific, fisheries play key economic and cultural roles and provide significant employment, especially in rural areas. In British Columbia (BC), sport (recreational) fishing generates more income than commercial fishing (including the expanding aquaculture industry). Salmon (fished recreationally and farmed) and Pacific Halibut are responsible for the majority of fishery-related income. This region naturally has relatively acidic (low pH) waters due to ocean circulation, and so may be particularly vulnerable to OA. We have analyzed available data to provide a current description of the marine ecosystem, focusing on vertical distributions of commercially harvested groups in BC in the context of local carbon and pH conditions. We then evaluated the potential impact of OA on this temperate marine system using currently available studies. Our results highlight significant knowledge gaps. Above trophic levels 2-3 (where most local fishery-income is generated), little is known about the direct impact of OA, and more importantly about the combined impact of multi-stressors, like temperature, that are also changing as our climate changes. There is evidence that OA may have indirect negative impacts on finfish through changes at lower trophic levels and in habitats. In particular, OA may lead to increased fish-killing algal blooms that can affect the lucrative salmon aquaculture industry. On the other hand, some species of locally farmed shellfish have been well-studied and exhibit significant negative direct impacts associated with OA, especially at the larval stage. We summarize the direct and indirect impacts of OA on all groups of marine organisms in this region and provide conclusions, ordered by immediacy and certainty.
