In the twentieth century, tuberculosis sanatorium architecture was the subject of design experimentation by leading architects, as well as a place for experimenting with modern therapeutic approaches and scientific growth. After the ‘defeat’ of the disease, the former sanatorium buildings were in many cases decommissioned but often retained their hospital function, undergoing numerous incongruous transformations and extensions that made these com-plexes difficult to recognise for their historical and architectural value and landscape significance. Starting with the case of Palermo, with a focus on the first Sicilian public sanatorium built by the physician Vincen-zo Cervello (1905-1909), the paper aims to evaluate the value of these buildings, with a series of proposals for possi-ble repurposing that, between conservation and new additions, reflect the history of the places, between past disso-nances and prospects.

Tuberculosis sanatorium treatment at the advent of the chemotherapy era
Background The discovery of antibiotics in the mid-twentieth century marked a major transition in tuberculosis (TB) treatment and control. There are few studies describing the duration of TB disease and its treatment from the pre-chemotherapy era and little data on how these treatments changed in response to the development of effective antibiotics. The goal of this research is to understand how inpatient treatment for high incidence populations, the First Nations peoples of Saskatchewan, Canada, changed in response to increasing availability of antibiotics effective against TB. We expected that as treatment regimens transitioned from convalescence-only to triple antibiotic therapy, the length of inpatient treatment would shorten. Methods Analyses were performed on records of sanatoria admissions and discharges occurring between 1933 and 1959 in Saskatchewan, Canada. Year of antibiotic discovery was taken as a proxy for treatment regimen: no chemotherapy (pre-1944), mono-therapy (Streptomycin, 1944–1946), dual-therapy (Streptomycin and PAS, 1946–1952), and triple-therapy (Streptomycin, PAS, and INH 1952-). A pooled linear regression of log-transformed length of first admission as predicted by year of admission was modeled to assess the relationship between admission length and year of admission, corrected for clinical and demographic variables. Results First admission length increased 19% in the triple-therapy era as compared to the pre-chemotherapy era, from 316 days (10.4 months) to 377 days (12.4 months). After the discovery of INH (1952), we find statistically significant increases in the proportion of successfully completed therapies (0.55 versus 0.60, p  = 0.035), but also in patients who left hospital against medical advice (0.19 versus 0.29, p  < 0.0001), indicating that as hospitalizations lengthened, more patients chose to discharge without the sanction of their physician. The readmission rate increased from 10 to 50% of all admissions while the province-level TB-specific death rate fell from 63.1 per 10,000 in 1933 to 4.7 per 10,000 in 1958. Conclusion Counterintuitively, we find that the length of first admissions increased with the discovery of TB-treating antibiotics. Increasing admission volume and readmission rate indicate an intensification of inpatient TB treatment during this era. These analyses provide a novel estimate of the effect of changing treatment policy on sanatorium admissions in this population.

TEACHERS' VOICE REHABILITATION IN SANATORIUM CONDITIONS/ SANATORYJNA REHABILITACJA GLOSU U NAUCZYCIELI
Background: Voice diseases occupy the third place among all of the diagnosed occupational disorders in Poland. There still exists the necessity to implement effective and economical methods of the primary and secondary prevention of voice disorders in teachers. The aim of this study was to assess the effectiveness of comprehensive voice rehabilitation in teachers, conducted within a health resort hospital. Material and Methods: The results of comprehensive voice rehabilitation in 100 teachers with job seniority of 10- 30 years (M = 17 years), conducted during 24-day-long staying in a sanatorium, are presented in this research. Phoniatrical and laryngological examinations (maximum phonation time [MPT], perceptual assessment of voice in GRBAS scale, laryngovideostroboscopy, and acoustic analysis of voice), self-assessment of voice (the Voice Handicap Index--VHI), logopedical estimation and audiometry were conducted during the first and the last day of the sanatorium stay. The therapeutic program included educational workshops on vocal hygiene, voice therapy, physiotherapy and psychotherapy. The inpatient therapy effects were also described by the teachers using a questionnaire. Results: Improvements of objective acoustic parameters, a perceptual assessment of voice, and a self-assessment of VHI and MPT were observed. Phonation style changes were confirmed in the laryngovideostroboscopic examination. Phonation closure improved significantly in 30% of the examined teachers. Moreover, 97% of the participants confirmed in the questionnaire the effectiveness and purposefulness of periodic recurrence of such rehabilitation courses. Conclusions: The results of this research as well as the positive feedback from the participants of inpatient rehabilitation confirmed the effectiveness of occupational voice disorder rehabilitation in sanatorium (inpatient) conditions. Treatment and rehabilitation in a stationary form should aim to achieve voice improvement and job continuation. Such a procedure may contribute to reducing the financial outlays related to treatment, health leaves and occupational pensions. W pierwszym i ostatnim dniu turnusu wykonano badania laryngologiczne i foniatryczne [czas fonacji (maximum phonation time--MPT), ocene percepcyjna glosu w skali GRBAS, badanie laryngowideostroboskopowe, analize akustyczna glosu], wypelnienie Kwestionariusza samooceny glosu (Voice Handicap Index--VHI), ocene logopedyczna i audiometrie tonalna. W zakres programu terapeutycznego wchodzily: zajecia edukacyjne dotyczace higieny glosu, terapia glosu, fizjoterapia i psychoterapia. Efekty rehabilitacji sanatoryjnej pacjenci oceniali rowniez w badaniu ankietowym. Wyniki: Stwierdzono poprawe obiektywnych parametrow akustycznych, oceny percepcyjnej glosu, samoceny glosu VHI i MPT. Zmiany w sposobie fonacji potwierdzono w badaniu laryngowideostroboskopowym, w ktorym obserwowane zwarcie fonacyjne poprawilo sie znamiennie u 30% badanych. W ankiecie oceniajacej korzysci z sanatoryjnej rehabilitacji glosu 97% uczestnikow potwierdzilo skutecznosc i celowosc okresowego powtarzania takiej rehabilitacji. Hermann Brehmer’s Tuberculosis Treatment Methods: Potential Applications for Cachexia in Aids and Cancer

Consumer Learning in a Turbulent Market Environment: Modeling Consumer Choice Dynamics After a Product-Harm Crisis
The authors empirically study consumer choice behavior in the wake of a product-harm crisis, which creates consumer uncertainty about product quality. They develop a model that explicitly incorporates the impact of such uncertainty on consumer behavior, assuming that consumers are uncertain about the mean product quality level and learn about product quality through the signals contained in use experience and the product-harm crisis and also that consumers are uncertain about the precision of the signals in conveying product quality and update their perception of this precision over time. They estimate this model using a scanner panel data set that includes consumer purchase history before, during, and after a product-harm crisis that affected Kraft Foods Australia's peanut butter division in June 1996. The proposed model fits the data better than the standard consumer learning model, which assumes consumers are uncertain about product quality level but the precision of information in conveying product quality is known. This study also provides insights into consumers' behavioral choice responses to a product-harm crisis. Finally, the authors conduct counterfactual experiments based on the estimation results and provide insights to managers on crisis management.
