Clinical and cost-effectiveness of oral sodium bicarbonate therapy for older patients with chronic kidney disease and low-grade acidosis (BiCARB): a pragmatic randomised, double-blind, placebo-controlled trial
Background Chronic kidney disease with metabolic acidosis is common in older people, but the effectiveness of oral sodium bicarbonate therapy in this group is unclear. We tested whether oral sodium bicarbonate provides net health benefit for older people with advanced chronic kidney disease and serum bicarbonate concentrations < 22 mmol/L. Methods Pragmatic multicentre, parallel group, double-blind, placebo-controlled randomised trial. We recruited adults aged ≥ 60 years with estimated glomerular filtration rate of < 30 mL/min/1.73 m 2 , not receiving dialysis, with serum bicarbonate concentration < 22 mmol/L, from 27 nephrology and geriatric medicine departments in the UK. Participants received oral sodium bicarbonate (up to 3 g/day) or matching placebo given for up to 2 years, randomised in a 1:1 ratio. The primary outcome was between-group difference in the Short Physical Performance Battery (SPPB) at 12 months, adjusted for baseline values, analysed by intention to treat. Secondary outcomes included generic and disease-specific quality of life (EQ-5D and KDQoL tools), anthropometry, renal function, walk distance, blood pressure, bone and vascular health markers, and incremental cost per quality-adjusted life year gained. Results We randomised 300 participants between May 2013 and February 2017, mean age 74 years, 86 (29%) female. At 12 months, 116/152 (76%) participants allocated to bicarbonate and 104/148 (70%) allocated to placebo were assessed; primary outcome data were available for 187 participants. We found no significant treatment effect for the SPPB: bicarbonate arm 8.3 (SD 2.5) points, placebo arm 8.8 (SD 2.2) and adjusted treatment effect − 0.4 (95% CI − 0.9 to 0.1, p  = 0.15). We found no significant treatment effect for glomerular filtration rate (0.6 mL/min/1.73 m 2 , 95% CI − 0.8 to 2.0, p  = 0.39). The bicarbonate arm showed higher costs and lower quality of life as measured by the EQ-5D-3L tool over 1 year (£564 [95% CI £88 to £1154]); placebo dominated bicarbonate under all sensitivity analyses. Adverse events were more frequent in those randomised to bicarbonate (457 versus 400). Conclusions Oral sodium bicarbonate did not improve physical function or renal function, increased adverse events and is unlikely to be cost-effective for use by the UK NHS for this patient group. Outcomes after Angiography with Sodium Bicarbonate and Acetylcysteine
Among high-risk patients undergoing angiography, there was no benefit of intravenous sodium bicarbonate over sodium chloride or of oral acetylcysteine over placebo for the prevention of death, dialysis, or contrast-associated acute kidney injury.
The Effects of a Novel Sodium Bicarbonate Ingestion System on Repeated 4 km Cycling Time Trial Performance in Well-Trained Male Cyclists
Background A novel sodium bicarbonate (SB) product has come to market named the “Bicarb System” (M-SB; Maurten AB, Gothenburg, Sweden). It claims to minimise gastrointestinal (GI) discomfort whilst still improving exercise performance. Aim To investigate the effects of M-SB ingestion on repeated 4 km cycling time trials (TT1 and TT2) in well-trained male cyclists. Methods The study recruited ten well-trained cyclists (maximal oxygen uptake ( V ˙ O 2 max ): 67 ± 4 ml kg −1  min −1 BM; peak power output (PPO) at V ˙ O 2 max : 423 ± 21 W) to take part in this randomised, crossover and double-blinded study. Following one visit to determine V ˙ O 2 max , participants completed a second visit to identify individual time to peak blood bicarbonate (HCO 3 − ) (ITTP) in a rested state. Visit three was a familiarisation trial mimicking the experimental procedures. Visits four to seven consisted of completing 2 × 4 km cycling TTs separated by 45 min passive recovery, following one of either: 0.3 g kg −1 BM M-SB, 0.21 g kg −1 BM sodium chloride (placebo; PLA) in vegetarian capsules (size 00), or a control trial (CON). Supplements (M-SB or placebo) were ingested pre-exercise at their respective ITTP. Results Performance in TT1 was faster in the M-SB condition compared with TT1 in CON (− 5.1 s; p  = 0.004) and PLA (− 3.5 s; p  < 0.001). In TT2, performance was also significantly faster in the M-SB condition compared with CON (− 4.4 s; p  = 0.018) or PLA (− 4.1 s; p  = 0.002). Total aggregated GI symptoms were generally low and not significantly different between PLA and the M-SB conditions for a range of symptoms. Conclusions The ingestion of M-SB improves repeated 4 km cycling TT performance and the recovery of acid–base balance between bouts, whilst causing minimal GI discomfort.
Sodium bicarbonate therapy for patients with severe metabolic acidaemia in the intensive care unit (BICAR-ICU): a multicentre, open-label, randomised controlled, phase 3 trial
Acute acidaemia is frequently observed during critical illness. Sodium bicarbonate infusion for the treatment of severe metabolic acidaemia is a possible treatment option but remains controversial, as no studies to date have examined its effect on clinical outcomes. Therefore, we aimed to evaluate whether sodium bicarbonate infusion would improve these outcomes in critically ill patients. We did a multicentre, open-label, randomised controlled, phase 3 trial. Local investigators screened eligible patients from 26 intensive care units (ICUs) in France. We included adult patients (aged ≥18 years) who were admitted within 48 h to the ICU with severe acidaemia (pH ≤7·20, PaCO2 ≤45 mm Hg, and sodium bicarbonate concentration ≤20 mmol/L) and with a total Sequential Organ Failure Assessment score of 4 or more or an arterial lactate concentration of 2 mmol/L or more. We randomly assigned patients (1:1), by stratified randomisation with minimisation via a restricted web platform, to receive either no sodium bicarbonate (control group) or 4·2% of intravenous sodium bicarbonate infusion (bicarbonate group) to maintain the arterial pH above 7·30. Our protocol recommended that the volume of each infusion should be within the range of 125–250 mL in 30 min, with a maximum of 1000 mL within 24 h after inclusion. Randomisation criteria were stratified among three prespecified strata: age, sepsis status, and the Acute Kidney Injury Network (AKIN) score. The primary outcome was a composite of death from any cause by day 28 and the presence of at least one organ failure at day 7. All analyses were done on data from the intention-to-treat population, which included all patients who underwent randomisation. This study is registered with ClinicalTrials.gov, number NCT02476253. Between May 5, 2015, and May 7, 2017, we enrolled 389 patients into the intention-to-treat analysis in the overall population (194 in the control group and 195 in the bicarbonate group). The primary outcome occurred in 138 (71%) of 194 patients in the control group and 128 (66%) of 195 in the bicarbonate group (absolute difference estimate −5·5%, 95% CI −15·2 to 4·2; p=0·24). The Kaplan-Meier method estimate of the probability of survival at day 28 between the control group and bicarbonate group was not significant (46% [95% CI 40–54] vs 55% [49–63]; p=0·09. In the prespecified AKIN stratum of patients with a score of 2 or 3, the Kaplan-Meier method estimate of survival by day 28 between the control group and bicarbonate group was significant (37% [95% CI 28–48] vs 54% [45–65]; p=0·0283). Metabolic alkalosis, hypernatraemia, and hypocalcaemia were observed more frequently in the bicarbonate group than in the control group, with no life-threatening complications reported. In patients with severe metabolic acidaemia, sodium bicarbonate had no effect on the primary composite outcome. However, sodium bicarbonate decreased the primary composite outcome and day 28 mortality in the a-priori defined stratum of patients with acute kidney injury. Effect of oral sodium bicarbonate supplementation on urine TGF-𝜷 in normal serum bicarbonate CKD, a randomized controlled trial
The consequences of chronic subclinical metabolic acidosis, characterized by an increase in single nephron ammonium generation, contribute to the progression of chronic kidney disease (CKD). Therefore, sodium bicarbonate (NaHCO₃) supplementation in CKD with normal serum bicarbonate patients may reduce kidney fibrosis and slow CKD progression. This study aimed to evaluate the impact of high-dose NaHCO₃ supplementation on urinary transforming growth factor-beta (TGF-β), a biomarker of kidney fibrosis, in non-diabetic CKD patients with normal serum bicarbonate levels. We conducted a single-center, randomized, open-label controlled trial in patients with non-diabetic CKD stage 3–4 and normal serum bicarbonate (22–26 mEq/L). Participants were randomized to receive high-dose NaHCO₃ (0.8 mEq/kg/day) or standard care for 12 weeks. The primary outcome was the change in urinary TGF-β-to-creatinine ratio from baseline. Secondary outcomes included changes in urinary albumin-to-creatinine ratio (UACR), urine pH, serum electrolytes, blood pressure, and adverse events. A total of 64 participants were randomized (NaHCO₃ group: n = 32; control group: n = 32). There was no significant difference in the percentage change of urinary TGF-β (NaHCO₃: −1.86% vs. control: 5.75%; p = 0.477). However, the NaHCO₃ group demonstrated a significant increase in urine pH mean difference (0.56; 95% CI: 0.3,0.82 vs. 0,95% CI −0.24,0.24; p = 0.002) compared to the control group. Similarly, no significant differences were observed in UACR, serum electrolytes, blood pressure, or body weight between groups. No serious adverse events were reported. High-dose NaHCO₃ supplementation in non-diabetic CKD patients with normal serum bicarbonate levels did not significantly reduce urinary TGF-β over 12 weeks but effectively increased urine pH without adverse effects. These findings suggest that NaHCO₃ is safe; however, its role in modulating profibrotic biomarkers in CKD requires further investigation. Longer-term studies and alternative alkali therapies should be explored to determine the optimal strategies for preserving kidney function in this population. Feasibility of Sodium Bicarbonate Ingestion in Artistic Swimming Performances
Purpose: We evaluated the feasibility of individualized sodium bicarbonate (NaHCO3) supplementation and its physiological effects on simulated artistic swimming duet performance, including blood buffering responses, perceived exertion, gastrointestinal (GI) tolerance, and performance scores. Methods: Seventeen (n = 17) elite adolescent female artistic swimmers completed an initial trial to determine individual time-to-peak blood bicarbonate concentration (Part 1). Subsequently, a subset (n = 7) completed a randomized, double-blind, crossover intervention (Part 2), performing competition duet routines (4 min) after ingesting either 0.3 g/kg NaHCO3 or a placebo timed to their individual alkalosis peak. Blood gas and lactate samples were taken pre- and post-performance. Performance was scored by blinded FINA adjudicators. GI discomfort was assessed before and after each routine. Results: Peak blood bicarbonate occurred at 52 ± 9 min post-ingestion, with a mean increase of 6.7 ± 1.8 mmol/L (g = 5.03). In Part 2 (n = 7), NaHCO3 significantly elevated pre- and post-performance pH (7.46 ± 0.02 vs. 7.37 ± 0.01; 7.34 ± 0.02 vs. 7.26 ± 0.03), HCO3− (29.5 ± 0.9 vs. 22.4 ± 0.4 mmol/L; 21.5 ± 1.2 vs. 15.7 ± 1.5 mmol/L), and base excess (5.9 ± 0.6 vs. −2.9 ± 0.5 mmol/L; −4.3 ± 0.8 vs. −10.3 ± 1.1 mmol/L) compared to the placebo (all p < 0.05, g = 3.99–14.93). Post-performance lactate was higher (9.3 ± 1.0 vs. 8.4 ± 0.9 mmol/L, g = 0.89), while rating of perceived exertion (RPE) was lower (12.9 ± 0.7 vs. 14.4 ± 0.7, p < 0.05, g = −2.14). Propulsion improved (6.66 ± 0.16 vs. 6.52 ± 0.20, g = 0.85), with no change in execution. Mild gastrointestinal symptoms were transiently elevated with NaHCO3. Conclusions: Individualized NaHCO3 dosing is a feasible and effective ergogenic strategy for artistic swimmers, enhancing systemic alkalosis and perceptual tolerance while preserving technical execution. These findings support the sport-specific integration of NaHCO3 to optimize anaerobic performance elements in high-level artistic swimming.
Randomized control trial of benzydamine HCl versus sodium bicarbonate for prophylaxis of concurrent chemoradiation-induced oral mucositis
PurposeThe purpose of the study is to compare the efficacy of benzydamine HCl with sodium bicarbonate in the prevention of concurrent chemoradiation-induced oral mucositis in head and neck cancer patients.MethodsSixty locally advanced head and neck cancer patients treated with high-dose radiotherapy concurrently with platinum-based chemotherapy were randomly assigned to receive either benzydamine HCl or sodium bicarbonate from the first day of treatment to 2 weeks after the completion of treatment. The total score for mucositis, based on the Oral Mucositis Assessment Scale (OMAS), was used for the assessment, conducted weekly during the treatment period and at the fourth week of the follow-up. Pain score, all prescribed medications, and tube feeding needs were also recorded and compared.ResultsThe median of total OMAS score was statistically significant lower in patients who received benzydamine HCl during concurrent chemo-radiotherapy (CCRT) than in those who received sodium bicarbonate, (p value < 0.001). There was no difference in median pain score, (p value = 0.52). Nineteen percent of patients in sodium bicarbonate arm needed oral antifungal agents whereas none in the benzydamine HCl arm required such medications, (p value = 0.06). Tube feeding needs and the compliance of CCRT were not different between the two study arms.ConclusionsFor patients undergoing high-dose radiotherapy concurrently with platinum-based chemotherapy, using benzydamine HCl mouthwash as a preventive approach was superior to basic oral care using sodium bicarbonate mouthwash in terms of reducing the severity of oral mucositis and encouraging trend for the less need of oral antifungal drugs.
Lactobacillus brevis CD2 for Prevention of Oral Mucositis in Patients With Head and Neck Tumors: A Multicentric Randomized Study
Oropharyngeal mucositis occurs in virtually all patients with head and neck cancer receiving radiochemotherapy. The manipulation of the oral cavity microbiota represents an intriguing and challenging target. A total of 75 patients were enrolled to receive Lactobacillus brevis CD2 lozenges or oral care regimen with sodium bicarbonate mouthwashes. The primary endpoint was the incidence of grade 3 or 4 oropharyngeal mucositis during radiotherapy treatment. There was no statistical difference in the incidence of grade 3-4 oropharyngeal mucositis between the intervention and control groups (40.6% vs. 41.6% respectively, p=0.974). The incidence of pain, dysphagia, body weight loss and quality of life were not different between the experimental and standard arm. Our study was not able to demonstrate the efficacy of L. brevis CD2 lozenges in preventing radiation-induced mucositis in patients with head and neck cancer. Although modulating homeostasis of the salivary microbiota in the oral cavity seems attractive, it clearly needs further study.
Additive effects of beta-alanine and sodium bicarbonate on upper-body intermittent performance
We examined the isolated and combined effects of beta-alanine (BA) and sodium bicarbonate (SB) on high-intensity intermittent upper-body performance in judo and jiu-jitsu competitors. 37 athletes were assigned to one of four groups: (1) placebo (PL)+PL; (2) BA+PL; (3) PL+SB or (4) BA+SB. BA or dextrose (placebo) (6.4 g day−1) was ingested for 4 weeks and 500 mg kg−1 BM of SB or calcium carbonate (placebo) was ingested for 7 days during the 4th week. Before and after 4 weeks of supplementation, the athletes completed four 30-s upper-body Wingate tests, separated by 3 min. Blood lactate was determined at rest, immediately after and 5 min after the 4th exercise bout, with perceived exertion reported immediately after the 4th bout. BA and SB alone increased the total work done in +7 and 8 %, respectively. The co-ingestion resulted in an additive effect (+14 %, p < 0.05 vs. BA and SB alone). BA alone significantly improved mean power in the 2nd and 3rd bouts and tended to improve the 4th bout. SB alone significantly improved mean power in the 4th bout and tended to improve in the 2nd and 3rd bouts. BA+SB enhanced mean power in all four bouts. PL+PL did not elicit any alteration on mean and peak power. Post-exercise blood lactate increased with all treatments except with PL+PL. Only BA+SB resulted in lower ratings of perceived exertion (p = 0.05). Chronic BA and SB supplementation alone equally enhanced high-intensity intermittent upper-body performance in well-trained athletes. Combined BA and SB promoted a clear additive ergogenic effect.
The Efficacy of Sodium Bicarbonate Ringer’s Solution and Lactate Ringer’s Solution in Patients Undergoing Long-Term Abdominal Open Surgery: A Multicenter Prospective Randomized Controlled Study
Sodium bicarbonate Ringer's solution (BRS) is an intravenous fluid that does not rely on hepatic or renal metabolism. It contains bicarbonate ions (HCO ) and lacks lactate ions. This study hypothesizes that BRS is more effective in maintaining acid-base balance during prolonged open abdominal surgeries. The aim is to investigate the effects of sodium bicarbonate Ringer's solution on lactate metabolism, acid-base balance, and clinical outcomes in patients undergoing extended-duration open abdominal surgery, with the objective of refining fluid replacement strategies for this patient population. A multicenter randomized controlled trial was conducted involving 112 patients undergoing open abdominal surgery. Participants were randomly assigned to either the BRS group (n=55) or the lactated Ringer's solution (LRS) group (n=57). The primary outcome was the incidence of postoperative hyperlactacidemia, while secondary outcomes included serum lactate (Lac), pH, base excess (BE), buffered base (BB), bicarbonate (HCO ), blood glucose (Glu), electrolytes, postoperative liver and kidney function, and postoperative complications. Linear regression analysis was performed to identify factors influencing lactate concentration. The incidence of hyperlactic acidemia was lower in Group A compared to Group B (9.1% vs 19.3%, = 0.177), although the difference did not reach statistical significance. No significant differences were observed in lactate (Lac), pH, base excess (BE), bicarbonate (BB), HCO₃⁻, glucose (Glu), or electrolytes between the two groups at any time point ( < 0.001, > 0.05, > 0.05). Linear regression analysis revealed that diabetes, hypotension and blood loss were significant factors influencing blood lactate concentration = 0.349, < 0.001). Sodium bicarbonate Ringer's solution and lactate Ringer's solution are both safe options for fluid replacement during long-term open abdominal surgeries. There is no significant difference observed in perioperative lactate levels, acid-base balance, or clinical postoperative outcomes between the two solutions.
