Learning that he does not have long to live, a widower needs to figure out how to provide for his developmentally disabled adult son. Taking a job as a census taker, the two leave on a cross-country journey through towns named only by ascending letters of the alphabet. They meet the townspeople, some of whom welcome them into their homes, while others who bear the physical brand of past censuses on their ribs are wary of their presence. As they approach \"Z,\" the man must confront the purpose of the census, and decide how to say good-bye to his son.
MANAGEMENT OF MALIGNANT ASICTES IN TERMINALLY ILL CANCER PATIENTS

The aim of this research is to analyze the effectiveness of abdominal paracentesis and peritoneal drainage in management of malignant ascites of terminally ill cancer patients. The data of 71 patients who received abdominal paracentesis in Palliative Care Department were collected during 2011.10-2012.10. CR(complete remission),PR(partial response),SD(stable disease) and PD (progressive Disease)were used to assess the effective of the paracentesis and drainage. Forty-three (60.56 %) patients got the paracentesis more than twice. The average volume of drainage fluid was 5400 ml. 58 patients received intraperitoneal injection (DDP 60 mg and Thymopentin5 10 mg). 64 patients (90.14 %) got temporary symptoms relief. 60 patients (84.5 %) were considered effective (CR+ PR+SD), while 11 patients (15.49 %) were PD. The median relief time for ascites was 18 days. The biological characteristics of the ascites, primary tumor site have no relationship with the effect of the ascite control. Abdominal paracentesis was still an effective method for relieving symptoms in terminally ill cancer patients with malignant ascites, although the median relief time is short. How to prolong the relief time with few adverse effects for these patients deserved to be studied.

KNOWLEDGE AND ATTITUDES TOWARDS PALLIATIVE SEDATION AT HOME FOR TERMINALLY ILL CANCER PATIENTS: A SURVEY OF PALLIATIVE CARE DOCTORS IN ITALY

Home setting has been reported as the preferred one by most patients and relatives and seems to be the favorite place of death. Just few clearly structured data on clinical practice in palliative sedation (PS) at home are available. In 2015, a structured questionnaire was administered to 125 physicians regarding their attitudes and knowledge about PS, focusing on home care setting. All returned questionnaires were analyzed. 58% were women and 33% were younger than 40. 42% of participants have a formal qualification on palliative care, 45% have less than 5-years work's experience in Home Care. 92% regularly perform PS at home. Our experience was the first one carried out in Italy Palliative sedation at home is feasible and secure but standards for PS at home must be the same as for sedation in an institutional setting. On this way, more studies are requested.
