A political economy of the tobacco supply chain in an Eastern Mediterranean country: The case of Lebanon
The literature on tobacco has traditionally focused on health effects, public policies for tobacco control, and smoker profiles. However, there is a notable gap in understanding the supply chains and industry practices within the tobacco market. This paper addresses this gap by examining the structure of the tobacco market in Lebanon. Using an exploratory qualitative research approach, this paper maps the tobacco supply chain in Lebanon and investigates the interactions among various stakeholders, including key policymakers, regulators, researchers, and industry experts, as well as their underlying interests. Lebanon is a compelling case study due to its high smoking prevalence, the presence of a state-owned tobacco monopoly (the Regie Libanaise de Tabacs et Tombacs (Regie)), and the ongoing financial crisis that has affected various sectors, including the tobacco industry. The findings reveal three key issues: a complex political economy centered around monopolization and conflicting interests, the absence of a clear national strategy on tobacco leading to ineffective policy formulation, and inefficient tobacco cultivation practices requiring reforms for sustainable agricultural development. To address these issues, it is proposed to foster a more competitive and revenue-efficient tobacco market through the dissolution of the Regie monopoly via horizontal and vertical integration. This includes adopting an excise-specific tax on domestically manufactured tobacco goods and optimizing sales taxes on locally traded items. Secondly, expanding antitrust laws to encompass the tobacco industry and introducing legislative measures for fees and taxes are recommended to create an enabling environment for competition and revenue generation. Thirdly, reforming tobacco cultivation practices requires abolishing the current cultivation licensing framework, offering financial compensation to existing license holders, and supporting farmers in transitioning to alternative crops.
A tobacco-free world: a call to action to phase out the sale of tobacco products by 2040
The time has come for the world to acknowledge the unacceptability of the damage being done by the tobacco industry and work towards a world essentially free from the sale (legal and illegal) of tobacco products. A tobacco-free world by 2040, where less than 5% of the world's adult population use tobacco, is socially desirable, technically feasible, and could become politically practical. Three possible ways forward exist: so-called business-as-usual, with most countries steadily implementing the WHO Framework Convention on Tobacco Control (FCTC) provisions; accelerated implementation of the FCTC by all countries; and a so-called turbo-charged approach that complements FCTC actions with strengthened UN leadership, full engagement of all sectors, and increased investment in tobacco control. Only the turbo-charged approach will achieve a tobacco-free world by 2040 where tobacco is out of sight, out of mind, and out of fashion—yet not prohibited. The first and most urgent priority is the inclusion of an ambitious tobacco target in the post-2015 sustainable development health goal. The second priority is accelerated implementation of the FCTC policies in all countries, with full engagement from all sectors including the private sector—from workplaces to pharmacies—and with increased national and global investment. The third priority is an amendment of the FCTC to include an ambitious global tobacco reduction goal. The fourth priority is a UN high-level meeting on tobacco use to galvanise global action towards the 2040 tobacco-free world goal on the basis of new strategies, new resources, and new players. Decisive and strategic action on this bold vision will prevent hundreds of millions of unnecessary deaths during the remainder of this century and safeguard future generations from the ravages of tobacco use.
Marketing Little Cigars and Cigarillos: Advertising, Price, and Associations With Neighborhood Demographics
Objectives. We have documented little cigar and cigarillo (LCC) availability, advertising, and price in the point-of-sale environment and examined associations with neighborhood demographics. Methods. We used a multimodal real-time surveillance system to survey LCCs in 750 licensed tobacco retail outlets that sold tobacco products in Washington, DC. Using multivariate models, we examined the odds of LCC availability, the number of storefront exterior advertisements, and the price per cigarillo for Black & Mild packs in relation to neighborhood demographics. Results. The odds of LCC availability and price per cigarillo decreased significantly in nearly a dose-response manner with each quartile increase in proportion of African Americans. Prices were also lower in some young adult neighborhoods. Having a higher proportion of African American and young adult residents was associated with more exterior LCC advertising. Conclusions. Higher availability of LCCs in African American communities and lower prices and greater outdoor advertising in minority and young adult neighborhoods may establish environmental triggers to smoke among groups susceptible to initiation, addiction, and long-term negative health consequences.
The Association Between Tobacco Outlet Density and Smoking Among Young People: A Systematic Methodological Review
Abstract Aim Evidence on the association between tobacco outlet density and proximity and smoking behavior among youth is inconsistent, which may be due to methodological problems in some studies. We assessed the association of outlet density or proximity with smoking behavior among young people while taking into account the methodological quality of studies. Methods MEDLINE, EMBASE, and Google Scholar were systematically searched for studies on the relationship between outlet density or proximity and smoking behavior among 12- to 25-year-olds, published between 1997 and 2017. Methodological quality of the included studies was evaluated independently by two reviewers. Results Twenty studies were included in the review. The quality assessment identified five primary sources of potential bias: overadjustment for mediators (problems identified in 14 studies), underadjustment for confounders (six studies), poor statistical model fit (four studies), selection bias (three studies), and misclassification of exposure measurements (eight studies). Four studies were of high methodological quality. In studies with relatively high quality, 10 associations were reported, of which seven were nonsignificant, two positive, and one negative. Similarly, the complete body of evidence demonstrated mostly nonsignificant associations, but a larger proportion of positive associations than negative. Conclusion Although there is some support for a positive direction, current literature does not provide consistent evidence for a positive association between outlet density and smoking among youth. This is not necessarily due to bias in specific studies, but more to fundamental challenges in study design and exposure measurements. These issues need to be addressed in future studies using more rigorous methods. Implications Our findings suggest that, although there is some evidence for a positive association, current scientific literature does not provide consistent support to claim an effect of tobacco outlet density or proximity on youth smoking. This underlines the need for more research with improved methodology. There is a need for quasiexperimental studies, in which the outlet density changes substantially, studies measuring the actual exposure of youth to tobacco outlets, and qualitative research on the mechanisms underlying any association.
Inequalities in tobacco outlet density by race, ethnicity and socioeconomic status, 2012, USA: results from the ASPiRE Study
Background Evidence of racial/ethnic inequalities in tobacco outlet density is limited by: (1) reliance on studies from single counties or states, (2) limited attention to spatial dependence, and (3) an unclear theory-based relationship between neighbourhood composition and tobacco outlet density. Methods In 97 counties from the contiguous USA, we calculated the 2012 density of likely tobacco outlets (N=90 407), defined as tobacco outlets per 1000 population in census tracts (n=17 667). We used 2 spatial regression techniques, (1) a spatial errors approach in GeoDa software and (2) fitting a covariance function to the errors using a distance matrix of all tract centroids. We examined density as a function of race, ethnicity, income and 2 indicators identified from city planning literature to indicate neighbourhood stability (vacant housing, renter-occupied housing). Results The average density was 1.3 tobacco outlets per 1000 persons. Both spatial regression approaches yielded similar results. In unadjusted models, tobacco outlet density was positively associated with the proportion of black residents and negatively associated with the proportion of Asian residents, white residents and median household income. There was no association with the proportion of Hispanic residents. Indicators of neighbourhood stability explained the disproportionate density associated with black residential composition, but inequalities by income persisted in multivariable models. Conclusions Data from a large sample of US counties and results from 2 techniques to address spatial dependence strengthen evidence of inequalities in tobacco outlet density by race and income. Further research is needed to understand the underlying mechanisms in order to strengthen interventions.
Supply and demand effects between tobacco retailer density and smoking prevalence
ObjectivePlaces with more tobacco retailers have higher smoking prevalence levels, but whether this is because retailers locate where people who smoke live or whether tobacco availability prompts tobacco use is unknown. In this study, we compare the role of consumer demand with that of tobacco supply in longitudinal, area-based associations of tobacco retailer density with smoking prevalence.MethodsWe merged annual adult smoking prevalence estimates derived from the USA Behavioural Risk Factor Surveillance System data with annual county estimates of tobacco retailer density calculated from the National Establishment Time Series data for 3080 counties between 2000 and 2010. We analysed relationships between retailer density and smoking in 3080 counties, using random intercept cross-lagged panel models and employing two measures of tobacco retailer density capturing the number of likely tobacco retailers in a county divided by either the population or land area.ResultsBoth density models provided evidence of significant demand and supply effects; in the population-based model, the association of smoking prevalence in 1 year with tobacco retailer density in the next year (standardised coefficient=0.038, p<0.01) was about double the association between tobacco retailer density with subsequent smoking prevalence (0.017, p<0.01). The reverse was true in the land area-based model, where the supply effect (0.042, p<0.01) was more than 10 times stronger than the demand effect (0.003, p<0.01).ConclusionsPolicies that restrict access to retail tobacco have the potential to reduce smoking prevalence, but pairing such policies with interventions to reduce consumer demand remains important.
Neighbourhood inequities in the availability of retailers selling tobacco products: a systematic review
ObjectiveTo examine inequities in tobacco retailer availability by neighbourhood-level socioeconomic, racial/ethnic and same-sex couple composition.Data sourcesWe conducted a 10 November 2022 search of PubMed, PsycINFO, Global Health, LILACS, Embase, ABI/Inform, CINAHL, Business Source Complete, Web of Science and Scopus.Study selectionWe included records from Organisation for Economic Co-operation and Development member countries that tested associations of area-level measures of tobacco retailer availability and neighbourhood-level sociodemographic characteristics. Two coders reviewed the full text of eligible records (n=58), including 41 records and 205 effect sizes for synthesis.Data extractionWe used dual independent screening of titles, abstracts and full texts. One author abstracted and a second author confirmed the study design, location, unit of analysis, sample size, retailer data source, availability measure, statistical approach, sociodemographic characteristic and unadjusted effect sizes.Data synthesisOf the 124 effect sizes related to socioeconomic inequities (60.5% of all effect sizes), 101 (81.5%) indicated evidence of inequities. Of 205 effect sizes, 69 (33.7%) tested associations between retailer availability and neighbourhood composition of racially and ethnically minoritised people, and 57/69 (82.6%) documented inequities. Tobacco availability was greater in neighbourhoods with more Black, Hispanic/Latine and Asian residents (82.8%, 90.3% and 40.0% of effect sizes, respectively). Two effect sizes found greater availability with more same-sex households.ConclusionsThere are stark inequities in tobacco retailer availability. Moving beyond documenting inequities to partnering with communities to design, implement, and evaluate interventions that reduce and eliminate inequities in retail availability is needed to promote an equitable retail environment.PROSPERO registration numberCRD42019124984.
Investigating tobacco presence at retail points of sale around schools in Egypt
Objectives This study aims to identify the locations where tobacco products are sold around schools as well as the different marketing strategies followed at the different points of sale (POS).tify Methods This study was conducted in five Egyptian governorates (Cairo, Giza, Qualiubia, Sharkia and BeniSuif) representing different geographical regions in Egypt. An observational study was conducted around 102 schools from July to September 2024. The study examined location and products marketing tactics in retailers within 100 m of selected schools. Results The study revealed that over half of retailers surrounding the surveyed schools sold at least one type of tobacco and nicotine product. Nearly all schools studied had at least one tobacco POS within a 100-meter radius. All surveyed tobacco POS displayed tobacco products. Different tactics of marketing to children and youth were identified. Conclusions School-age children in Egypt face exposure to tobacco products and advertising. Policymakers must act urgently by enforcing advertising bans and restricting product availability, especially around schools, to protect youth from harm.
Visibility and availability of single cigarettes around schools in four Latin American countries
IntroductionSingle cigarette sales at tobacco-selling points of sale (POS) may promote smoking. We explored visibility and availability of single cigarettes in POS around schools in Argentina, Guatemala, Mexico and Peru, countries where their sale is banned.MethodsBetween April and July 2023, an observational study was conducted at POS around high schools in urban and rural areas in Argentina, Guatemala, Mexico and Peru. A sampling frame of schools was generated from public registries, with schools categorised according to their socioeconomic status and randomly selected within socioeconomic strata. A total of 2081 POS were surveyed based on their proximity to these randomly selected schools. Data were collected on the visibility of single cigarettes (any cigarette sticks or open cigarette packs on display and able to be seen by customers were coded as visible) and their availability (ie, whether singles were sold, whether visible or not).ResultsSingle cigarettes were visible in 37.2% of POS and available in most (84.9%), with neighbourhood stores the primary source. In most (84.1%) POS with visible single cigarettes, they were displayed near candy or toys. Prices of the cheapest single cigarette sticks ranged from US$0.03 in Argentina to US$0.13 in Guatemala.ConclusionThe findings indicate widespread availability of single cigarettes in four Latin American countries where their sale is banned. The low price of single cigarettes, and their proximity to candy/toys, may increase the appeal of smoking to youth. Stricter enforcement of bans on single cigarettes, which may serve as a gateway to experimentation and continued smoking, is needed.
Neighborhood Disparities in the Availability, Advertising, Promotion, and Youth Appeal of Little Cigars and Cigarillos, United States, 2015
Abstract Introduction Between 2012 and 2016, sales of cigars increased by 29% in the United States. In small local studies, greater little cigar and cigarillo (LCC) availability and marketing has been documented in neighborhoods with a greater proportion of Hispanic or Latino and black residents, and near schools. This national study of cigarette retailers assesses whether LCC availability and marketing at the point of sale is associated with neighborhood racial, ethnic, income, and percent youth demographics. Methods In 2015, we collected LCC availability and marketing data through retailer audits of a nationally representative sample of 2128 cigarette retailers. Using 2011–2015 American Community Survey census tract estimates, we modeled associations of neighborhood demographics (in quartiles) with availability of LCC-flavored products, and presence of exterior advertisements, youth marketing, and promotions. Results Nearly 90% of retailers sold LCCs, 83.0% sold flavored LCCs, and 30.9% had youth marketing. Controlling for retailer type and other neighborhood characteristics, neighborhoods with the highest proportion of black residents had significantly higher odds of flavored LCC availability (adjusted odds ratio [AOR] = 2.24, 95% confidence interval [CI] = 1.52, 3.30); exterior advertisements (AOR = 2.84, 95% CI = 1.94, 4.16); price promotions (AOR = 1.62; 95% CI = 1.07–2.45), and youth appeal (AOR = 1.49, 95% CI 1.08–2.08) compared to the lowest. Disparities in flavored LCC availability, exterior advertising, and youth appeal were also present for lower income neighborhoods. Conclusion Neighborhoods with a greater proportion of black or lower income residents have greater flavored LCCs availability and LCC marketing. Without stronger LCC regulation, residents of these neighborhoods may be at a greater risk of LCC use. Implications LCCs are harmful combustible tobacco products that are less regulated than cigarettes (eg, lower taxation, cheaper outlay due to small package sizes, availability of flavors), which makes them an affordable and appealing product to youth. This study documents greater availability, advertising, and marketing of LCCs in neighborhoods with a higher proportion of black or lower-income residents, potentially putting these populations at a greater risk of using or switching to these products in the face of increasing cigarette regulations. These findings underscore the need for local, state, and federal LCC regulatory action.
