Vitreolysis for Vitreomacular Traction and Macular Holes
Intravitreal injection of a modified protease that targets components of the vitreomacular interface resolved vitreomacular traction and closed macular holes more often than did placebo, albeit with associated, mainly transient, ocular adverse events.
The human vitreous body is bounded posteriorly by the retina and is variably adherent to it. Collagen fibrils forming the posterior vitreous cortex are firmly attached at the macula, the central part of the retina where visual acuity is best, and are connected to its internal limiting membrane by means of a biochemical glue composed of proteoglycans, including laminin and fibrinectin.

Prophylactic barrage laser in patients with previous vitreous loss during cataract surgery: a randomized clinical trial

Background
One of the significant challenges that can arise during cataract surgery is the occurrence of posterior capsule rupture (PCR) and vitreous loss. It may result in different complications including cystoid macular edema, endophthalmitis, glaucoma, and considerably retinal detachment which consequently contributes to poor functional outcomes.
Methods
This study was a prospective double blinded randomized clinical trial including 42 patients with previous complicated cataract surgery including posterior capsular rupture and consequently vitreous loss that anterior vitrectomy was done for them. The patients were randomly categorized in to two groups by simple randomization. One group (25 patients) received sham laser and the other group (17 patients) received prophylactic 360° barrage laser 4 weeks after cataract surgery. After eighteen months, rate of retinal detachment was compared between two groups.
Results
In terms of gender, age, axial length, maturity, pseudophakia v/s aphakia, type of IOL and type of cataract surgery, there was no statistically significant difference between intervention and control groups. Distribution of RRD was clinically different between 2 groups (intervention and control group). In addition, there was a statistically significant difference between the rate of RRD in control group (non-laser patients) and normal population (0.08 vs. 0.0001). (

Conclusion
Prophylactic barrage laser might be a useful intervention in eyes with posterior capsule rupture and vitreous loss during cataract surgery to prevent rhegmatogenous retinal detachment. However, further studies with larger sample sizes are indicated.

Baseline Predictors of Vitreomacular Adhesion/Traction Resolution Following an Intravitreal Injection of Ocriplasmin

To determine factors predicting response to ocriplasmin (Jetrea; ThromboGenics, Iselin, NJ) response in patients with symptomatic vitreomacular adhesion (VMA).
Combined analysis of two multicenter, prospective, randomized, double-masked trials of intravitreal ocriplasmin 125 µg injection versus placebo. Patients had vitreomacular traction with or without a full-thickness macular hole (FTMH). Multivariate logistic regression was used to determine factors influencing treatment response (complete VMA release [day 28] and non-surgical FTMH closure [month 6]).
Younger age, presence of FTMH (odds ratio [OR] = 2.1; 95% confidence interval [CI], 1.1-3.7), VMA diameter of 1,500 µm or less (OR = 4.9; 95% CI, 2.0-12.4), phakic lens status (OR = 2.8; 95% CI, 1.5-5.2), and absence of epiretinal membrane (OR = 4.1; 95% CI, 2.2-7.9) predicted VMA resolution. FTMHs with apical diameter of 250 µm or less were more likely to close than larger holes (58.3% vs. 24.6%; P = .013). Both FTMH size groups had significantly greater chance of VMA resolution and FTMH closure versus controls.

Cost–effectiveness analysis of ocriplasmin versus watchful waiting for treatment of symptomatic vitreomacular adhesion in the US

Evaluate the cost–effectiveness of ocriplasmin in symptomatic vitreomacular adhesion (VMA) with or without full-thickness macular hole ≤400 μm versus standard of care.
A state-transition model simulated a cohort through disease health states; assignment of utilities to health states reflected the distribution of visual acuity. Efficacy of ocriplasmin was derived from logistic regression models using Ocriplasmin for Treatment for Symptomatic Vitreomacular Adhesion Including Macular Hole trial data. Model inputs were extracted from Phase III trials and published literature. The analysis was conducted from a US Medicare perspective.
Lifetime incremental cost–effectiveness ratio was US$4887 per quality-adjusted life year gained in the total population, US$4255 and US$10,167 in VMA subgroups without and with full-thickness macular hole, respectively.
Ocriplasmin was cost effective compared with standard of care in symptomatic VMA.

Purpose
To determine the incidence of vitreopapillary adhesion (VPA) and to investigate its value as a prognostic factor in the surgical outcome of pseudo- (PMH) and lamellar macular holes (LMH).
Methods
A total of 76 consecutive patients, diagnosed with PMH (41 eyes) or LMH (35 eyes) were included. Eyes with VPA were alternatively assigned to the surgical or control group. Surgery consisted of a 25G vitrectomy and internal limiting membrane peeling with blue dye staining. There were six visits: baseline, the day of surgery, and 1, 3, 6, and 9 post-operative months. Main outcome measures were the incidence of VPA and changes in the outer retinal layers and visual acuity.
Results
VPA was found in 27% (11/41) of patients with PMH and 37% (13/35) with LMH (
P
=0.03). In presence of VPA, the best-corrected visual acuity (BCVA) improved in the surgery group from 32±8 to 47±8 letters, whereas the control group went from 34±7 to 31±8 letters. The difference in letters between the surgery and control groups was statistically significant for both distance (
P
=0.032) and near (
P
=0.04) vision. Intra-retinal cysts were significantly correlated with a poor functional prognosis (
P
=0.01). We found the presence of focal damage to the outer retinal layers in LMH and PMH.
Conclusion
VPA is more frequent in the presence of LMH
vs
PMH. It significantly influences the tangential forces at the vitreoretinal interface, exacerbating anatomical changes, and worsening the functional prognosis. Functional difference between the surgical and control groups was statistically significant for distant and near vision in presence of VPA.

Proliferative diabetic retinopathy (PDR), proliferative vitreoretinopathy (PVR), and neovascular age-related macular degeneration (nAMD) are among the leading causes of blindness. Due to the multifactorial nature of these vitreoretinal diseases, omics approaches are essential for a deeper understanding of the pathophysiologic processes underlying the evolution to a proliferative or neovascular etiology, in which patients suffer from an abrupt loss of vision. For many years, it was thought that the function of the vitreous was merely structural, supporting and protecting the surrounding ocular tissues. Proteomics studies proved that vitreous is more complex and biologically active than initially thought, and its changes reflect the physiological and pathological state of the eye. The vitreous is the scenario of a complex interplay between inflammation, fibrosis, oxidative stress, neurodegeneration, and extracellular matrix remodeling. Vitreous proteome not only reflects the pathological events that occur in the retina, but the changes in the vitreous itself play a central role in the onset and progression of vitreoretinal diseases. Therefore, this review offers an overview of the studies on the vitreous proteome that could help to elucidate some of the pathological mechanisms underlying proliferative and/or neovascular vitreoretinal diseases and to find new potential pharmaceutical targets.

Association between retinal vein occlusion, axial length and vitreous chamber depth measured by optical low coherence reflectometry

Background
Results of ocular biometric measurements in retinal vein occlusion (RVO) eyes are still inconclusive and controversial. The aim of this study was to evaluate the association between ocular axial length (AL), vitreous chamber depth (VCD) and both central (CRVO) and branch retinal vein occlusions (BRVO) using optical low coherence reflectometry (OLCR).
Methods
Both eyes of 37 patients with unilateral CRVO (mean age: 66 ± 14 years, male:female - 21:16) and 46 patients with unilateral BRVO (mean age: 63 ± 12 years, male:female - 24:22) were enrolled in this study. The control group consisted of randomly selected single eyes of 67 age and gender matched volunteers without the presence or history of RVO (mean age: 64 ± 14 years, male:female - 34:33). Optical biometry was performed by OLCR biometer (LenStar LS 900). Average keratometry readings, central corneal thickness (CCT), anterior chamber depth (ACD), lens thickness (LT), AL and VCD of eyes with RVO were compared with those of fellow eyes using paired t-tests and with those of control eyes using independent t-tests.
Results
Mean CCT, ACD and LT, average keratometry readings of affected RVO eyes, unaffected fellow eyes and control eyes was not statistically different in either groups. In eyes with CRVO mean AL and VCD of affected eyes were significantly shorter than those of control eyes (p < 0.001, p < 0.05), mean difference in AL and VCD between the affected and control eyes was 0.56 ± 0.15 mm and 0.45 ± 0.19 mm, respectively. In eyes with BRVO, mean AL of the affected eyes was significantly shorter with a mean difference of 0.57 ± 0.15 mm (p < 0.001) and the VCD was significantly shorter with a mean difference of 0.61 ± 0.15 mm (p < 0.001) comparing with the control eyes.
Conclusion
Shorter AL and VCD might be a potential anatomical predisposing factor for development either of CRVO or BRVO.

AimsTo determine the three-dimensional (3D) structure of the vitreous fluid including the posterior precortical vitreous pockets (PPVP), Cloquet’s canal and cisterns in healthy subjects by AI-based segmentation of the vitreous of swept-source optical coherence tomography (OCT) images. In addition, to analyse the vitreous structures over a wide and deep area using ultrawidefield swept-source OCT (UWF-OCT).MethodsTen eyes of six patients with the mean age was 40.7±8.4 years and the mean refractive error (spherical equivalent) was −3.275±2.2 diopters were examined.ResultsIn the UWF OCT images, the structure of the vitreous was observed in detail over 23 mm wide and 5 mm area. AI-guided analyses showed the complex 3D vitreous structures from any angle. Cisterns were observed to overlie the PPVP from the anterior. The morphology and locations of the cisterns varied among the subjects but tended to be similar in the two eyes of one individual. Cisterns joined the PPVPs superior to the macula to form a large trunk. This joined trunk was clearly seen in 3D images even in eyes whose trunk was not detected in the B scan OCT images. In some eyes, the vitreous had a complex appearance resembling an ant nest without large fluid-filled spaces.ConclusionsA combination of UWF-OCT and 3D imaging is very helpful in visualising the complex structure of the vitreous. These technologies are powerful tools that can be used to clarify the normal evolution of the vitreous, pathological changes of vitreous and implications of vitreous changes in various vitreoretinal diseases.

Aurora borealis and string of pearls in vitreoretinal lymphoma: patterns of vitreous haze

AimsPeculiar retinal signs of vitreoretinal lymphoma (VRL) have been identified. However, limited information on the vitreous features of VRL is available. This study aims to characterise the vitreous involvement in VRL with the help of multimodal imaging.MethodsIn this retrospective, observational study, we reviewed charts and imaging of all patients with biopsy-proven VRL seen from January 2016 to April 2018 at a single referral centre. These included ultrawide-field imaging, ophthalmic ultrasonography and slit-lamp photography. The main outcome measures were patterns of vitreous haze of VRL, as observed by combining clinical and multimodal imaging information.ResultsTwenty-six eyes of 13 patients were included. At presentation, vitreous haze was present in 24 eyes (92%) and was the only sign of VRL in 4 eyes (15%). Three patterns of vitreous haze were identified in VRL. An aurora borealis pattern was present in 12 eyes and showed linear opacities with a myriad of cells aligned along the vitreous fibrils. A string of pearls pattern was present in two eyes at baseline and developed in other four eyes after vitrectomy, showing fine fibrils connecting bunches of inflammatory material. A non-specific pattern was observed in 10 eyes. Ophthalmic ultrasound showed corpuscular material correlating with the grading of vitreous haze.ConclusionVRL shows different patterns of vitreous haze. Multimodal imaging, including ultrawide-field imaging and slit-lamp photography, helps in recognising these patterns, raising suspicion for VRL.

BackgroundCurrent melphalan-based intravitreal regimens for retinoblastoma (RB) vitreous seeds cause retinal toxicity. We assessed the efficacy and toxicity of topotecan monotherapy compared with melphalan in our rabbit model and patient cohort.MethodsRabbit experiments: empiric pharmacokinetics were determined following topotecan injection. For topotecan (15 μg or 30 µg), melphalan (12.5 µg) or saline, toxicity was evaluated by serial electroretinography (ERG) and histopathology, and efficacy against vitreous seed xenografts was measured by tumour cell reduction and apoptosis induction. Patients: retrospective cohort study of 235 patients receiving 990 intravitreal injections of topotecan or melphalan.ResultsIntravitreal topotecan 30 µg (equals 60 µg in humans) achieved the IC90 across the rabbit vitreous. Three weekly topotecan injections (either 15 µg or 30 µg) caused no retinal toxicity in rabbits, whereas melphalan 12.5 µg (equals 25 µg in humans) reduced ERG amplitudes 42%–79%. Intravitreal topotecan 15 µg was equally effective to melphalan to treat WERI-Rb1 cell xenografts in rabbits (96% reduction for topotecan vs saline (p=0.004), 88% reduction for melphalan vs saline (p=0.004), topotecan vs melphalan, p=0.15). In our clinical study, patients received 881 monotherapy injections (48 topotecan, 833 melphalan). Patients receiving 20 µg or 30 µg topotecan demonstrated no significant ERG reductions; melphalan caused ERG reductions of 7.6 μV for every injection of 25 µg (p=0.03) or 30 µg (p<0.001). Most patients treated with intravitreal topotecan also received intravitreal melphalan at some point during their treatment course. Among those eyes treated exclusively with topotecan monotherapy, all eyes were salvaged.ConclusionsTaken together, these experiments suggest that intravitreal topotecan monotherapy for the treatment of RB vitreous seeds is non-toxic and effective.
