Network Analysis Indicates That Avolition Is the Most Central Domain for the Successful Treatment of Negative Symptoms: Evidence From the Roluperidone Randomized Clinical Trial
A recent conceptual development in schizophrenia is to view its manifestations as interactive networks rather than individual symptoms. Negative symptoms, which are associated with poor functional outcome and reduced rates of recovery, represent a critical need in schizophrenia therapeutics. MIN101 (roluperidone), a compound in development, demonstrated efficacy in the treatment of negative symptoms in schizophrenia. However, it is unclear how the drug achieved its effect from a network perspective. The current study evaluated the efficacy of roluperidone from a network perspective. In this randomized clinical trial, participants with schizophrenia and moderate to severe negative symptoms were randomly assigned to roluperidone 32 mg (n = 78), 64 mg (n = 83), or placebo (N = 83). Macroscopic network properties were evaluated to determine whether roluperidone altered the overall density of the interconnections among symptoms. Microscopic properties were evaluated to examine which individual symptoms were most influential (ie, interconnected) on other symptoms in the network and are responsible for successful treatment effects. Participants receiving roluperidone did not differ from those randomized to placebo on macroscopic properties. However, microscopic properties (degree and closeness centrality) indicated that avolition was highly central in patients receiving placebo and that roluperidone reduced this level of centrality. These findings suggest that decoupling the influence of motivational processes from other negative symptom domains is essential for producing global improvements. The search for pathophysiological mechanisms and targeted treatment development should be focused on avolition, with the expectation of improvement in the entire constellation of negative symptoms if avolition is effectively treated.
Break in volition: a virtual reality study in patients with obsessive-compulsive disorder
Research in obsessive-compulsive disorder (OCD) produced inconsistent results in demonstrating an association between patients’ symptom severity and their cognitive impairments. The process involved in volition aspects of behavioral syndromes can be extensively analyzed using specific tests developed in virtual environments, more suitable to manipulate rules and possible breaks of the normal task execution with different, confusing or stopping instructions. The study involved thirty participants (15 OCD patients and 15 controls) during task execution and the relative interferences. At this purpose, the virtual version of Multiple Errands Test was used. Virtual reality setting, with a higher ecological validity respect to a classic neuropsychological battery, allowed us to take into account deficits of volition and the relative dysexecutive functions associated with OCD patients. The proposed paradigm also allows the development of innovative prototypes of coevolving technologies based on new theories and models and deeper understanding of human behavior.
The benefit of making voluntary choices generalizes across multiple effectors
It has been shown that cognitive performance could be improved by expressing volition (e.g., making voluntary choices), which necessarily involves the execution of action through a certain effector. However, it is unclear if the benefit of expressing volition can generalize across different effectors. In the present study, participants made a choice between two pictures either voluntarily or forcibly, and subsequently completed a visual search task with the chosen picture as a task-irrelevant background. The effector for choosing a picture could be the hand (pressing a key), foot (pedaling), mouth (commanding), or eye (gazing), whereas the effector for responding to the search target was always the hand. Results showed that participants responded faster and had a more liberal response criterion in the search task after a voluntary choice (vs. a forced choice). Importantly, the improved performance was observed regardless of which effector was used in making the choice, and regardless of whether the effector for making choices was the same as or different from the effector for responding to the search target. Eye-movement data for oculomotor choice showed that the main contributor to the facilitatory effect of voluntary choice was the post-search time in the visual search task (i.e., the time spent on processes after the target was found, such as response selection and execution). These results suggest that the expression of volition may involve the motor control system in which the effector-general, high-level processing of the goal of the voluntary action plays a key role.
Motivation from control
Human motivation is sensitive to value—to the outcomes of actions. People invest mental and physical resources for obtaining desired results or for stopping and reversing undesired ones. Accordingly, people’s motivation is sensitive to information about their standing in relation to outcome attainment (‘outcome feedback’). In this paper, we argue and present the first evidence for the existence of another motivational sensitivity in humans—a sensitivity to our degree of control on the environment and hence to information about that control (‘control feedback’). We show that when actions have even trivial and constant perceptual effects, participants’ motivation to perform is enhanced. We then show that increased motivation is not because more information about task performance is available and that motivation is increased only in conditions in which control over the effects can be firmly established by the mind. We speculate on the implications for understanding motivation, and potentially, physical and mental health.
Cue integration and the perception of action in intentional binding
‘Intentional binding’ describes the perceived temporal attraction between a voluntary action and its sensory consequence. Binding has been used in health and disease as an indirect measure of awareness of action or agency, that is, the sense that one controls one’s own actions. It has been proposed that binding results from cue integration, in which a voluntary action provides information about the timing of its consequences or vice versa. The perception of the timing of either event is then a weighted average, determined according to the reliability of each of these two cues. Here we tested the contribution of cue integration to the perception of action and its sensory effect in binding, that is, action and tone binding, by manipulating the sensory reliability of the outcome tone. As predicted, when tone reliability was reduced, action binding was diminished and tone binding was increased. However, further analyses showed that cue integration accounted for changes in action binding, but not tone binding. These findings establish a role for cue integration in action binding and support the growing evidence suggesting that action and tone binding are, at least in part, driven by distinct mechanisms.
Volitional limbic neuromodulation exerts a beneficial clinical effect on Fibromyalgia
Volitional neural modulation using neurofeedback has been indicated as a potential treatment for chronic conditions that involve peripheral and central neural dysregulation. Here we utilized neurofeedback in patients suffering from Fibromyalgia - a chronic pain syndrome that involves sleep disturbance and emotion dysregulation. These ancillary symptoms, which have an amplificating effect on pain, are known to be mediated by heightened limbic activity. In order to reliably probe limbic activity in a scalable manner fit for EEG-neurofeedback training, we utilized an Electrical Finger Print (EFP) model of amygdala-BOLD signal (termed Amyg-EFP), that has been successfully validated in our lab in the context of volitional neuromodulation. We anticipated that Amyg-EFP-neurofeedback training aimed at limbic down modulation would improve chronic pain in patients suffering from Fibromyalgia, by reducing sleep disorder improving emotion regulation. We further expected that improved clinical status would correspond with successful training as indicated by improved down modulation of the Amygdala-EFP signal. Thirty-Four Fibromyalgia patients (31F; age 35.6 ± 11.82) participated in a randomized placebo-controlled trial with biweekly Amyg-EFP-neurofeedback sessions or sham neurofeedback (n = 9) for a total duration of five consecutive weeks. Following training, participants in the real-neurofeedback group were divided into good (n = 13) or poor (n = 12) modulators according to their success in the neurofeedback training. Before and after treatment, self-reports on pain, depression, anxiety, fatigue and sleep quality were obtained, as well as objective sleep indices. Long-term clinical follow-up was made available, within up to three years of the neurofeedback training completion. REM latency and objective sleep quality index were robustly improved following the treatment course only in the real-neurofeedback group (time × group p < 0.05) and to a greater extent among good modulators (time × sub-group p < 0.05). In contrast, self-report measures did not reveal a treatment-specific response at the end of the neurofeedback training. However, the follow-up assessment revealed a delayed improvement in chronic pain and subjective sleep experience, evident only in the real-neurofeedback group (time × group p < 0.05). Moderation analysis showed that the enduring clinical effects on pain evident in the follow-up assessment were predicted by the immediate improvements following training in objective sleep and subjective affect measures. Our findings suggest that Amyg-EFP-neurofeedback that specifically targets limbic activity down modulation offers a successful principled approach for volitional EEG based neuromodulation treatment in Fibromyalgia patients. Importantly, it seems that via its immediate sleep improving effect, the neurofeedback training induced a delayed reduction in the target subjective symptom of chronic pain, far and beyond the immediate placebo effect. This indirect approach to chronic pain management reflects the substantial link between somatic and affective dysregulation that can be successfully targeted using neurofeedback. Inducing Disbelief in Free Will Alters Brain Correlates of Preconscious Motor Preparation: The Brain Minds Whether We Believe in Free Will or Not
The feeling of being in control of one's own actions is a strong subjective experience. However, discoveries in psychology and neuroscience challenge the validity of this experience and suggest that free will is just an illusion. This raises a question: What would happen if people started to disbelieve in free will? Previous research has shown that low control beliefs affect performance and motivation. Recently, it has been shown that undermining free-will beliefs influences social behavior. In the study reported here, we investigated whether undermining beliefs in free will affects brain correlates of voluntary motor preparation. Our results showed that the readiness potential was reduced in individuals induced to disbelieve in free will. This effect was evident more than 1 s before participants consciously decided to move, a finding that suggests that the manipulation influenced intentional actions at preconscious stages. Our findings indicate that abstract belief systems might have a much more fundamental effect than previously thought.
Self-Control and Grit: Related but Separable Determinants of Success
Other than talent and opportunity, what makes some people more successful than others? One important determinant of success is self-control—the capacity to regulate attention, emotion, and behavior in the presence of temptation. A second important determinant of success is grit—the tenacious pursuit of a dominant superordinate goal despite setbacks. Self-control and grit are strongly correlated, but not perfectly so. This means that some people with high levels of self-control capably handle temptations but do not consistently pursue a dominant goal. Likewise, some exceptional achievers are prodigiously gritty but succumb to temptations in domains other than their chosen life passion. Understanding how goals are hierarchically organized clarifies how self-control and grit are related but distinct: Self-control entails aligning actions with any valued goal despite momentarily more-alluring alternatives; grit, in contrast, entails having and working assiduously toward a single challenging superordinate goal through thick and thin, on a timescale of years or even decades. Although both self-control and grit entail aligning actions with intentions, they operate in different ways and over different timescales. This hierarchical goal framework suggests novel directions for basic and applied research on success.
Non-invasive brain stimulation enhances fine motor control of the hemiparetic ankle: implications for rehabilitation
We set out to answer two questions with this study: 1. Can stroke patients improve voluntary control of their paretic ankle by practising a visuo-motor ankle-tracking task? 2. Are practice effects enhanced with non-invasive brain stimulation? A carefully selected sample of chronic stroke patients able to perform the experimental task attended three data collection sessions. Facilitatory transcranial direct current stimulation (tDCS) was applied in a random order over the lower limb primary motor cortex of the lesioned hemisphere or the non-lesioned hemisphere or sham stimulation was delivered over the lesioned hemisphere. In each session, tDCS was applied as patients practiced tracking a sinusoidal waveform for 15 min using dorsiflexion-plantarflexion movements of their paretic ankle. The difference in tracking error prior to, and after, the 15 min of practice was calculated. A practice effect was revealed following sham stimulation, and this effect was enhanced with tDCS applied over the lesioned hemisphere. The practice effect observed following sham stimulation was eliminated by tDCS applied over the non-lesioned hemisphere. The study provides the first evidence that non-invasive brain stimulation applied to the lesioned motor cortex of moderate- to well-recovered stroke patients enhances voluntary control of the paretic ankle. The results provide a basis for examining whether this enhanced ankle control can be induced in patients with greater impairments and whether enhanced control of a single or multiple lower limb joints improves hemiparetic gait patterns.
