Physiological and perceptual responses during walking at set and preferred pace in normal and overweight adults

BackgroundThis study aimed to investigate the influence of body weight, exercise modality, and pace on physiological and perceptual responses to determine the relation between perceptual and physiological responses.MethodsAerobically untrained, normal weight, and overweight males and females participated. Participants were randomly assigned to one exercise condition for a 1-mile walk.ResultsPrescribed pace resulted in a greater physiological stress than self-selected pace (all p < 0.001) through blood lactate (1.3 ± 0.4 vs 1.7 ± 0.5 mmol), systolic blood pressure (128.9 ± 10.3 vs 139.2 ± 13.2 mmHg), heart rate recovery (2.1 ± 1.7 vs 4.5 ± 2.6 min), oxygen consumption (15.1 ± 2.6 vs 18.1 ± 3.6 ml/kg/min), heart rate (111.7 ± 16.6 vs 134.4 ± 8.3 bpm), and energy expenditure (5.5 ± 1.4 vs 6.8 ± 1.7 kcal/min). Overweight individuals exhibited higher values than normal weight for heart rate (113 ± 8.8 vs 125.5 ± 13.2 bpm, p < 0.001), percentage of max heart rate (60.7 vs 69.0%, p < 0.001), percent of VO2peak (42.2 vs 55.6%, p < 0.001), and total energy expenditure (100.9 ± 27.4 vs 114.1 ± 33.5 kcal, p = 0.022). Greater perceived effort was seen on a treadmill (6.3 ± 0.5 vs 12.5 ± 2.0 RPE, p < 0.001) and at prescribed pace (6.5 ± 1.1 vs 8.9 ± 1.9 RPE, p < 0.001).ConclusionA novel finding of this study was the increased physiological stress and perception of effort on a treadmill and at a prescribed pace, while total energy expenditure showed no significant differences. This could indicate an unfavorable perception and less affective response to the treadmill modality. These results indicate exercise at self-selected pace in preferred environments promotes enjoyable experiences with similar health benefits as those during prescribed higher intensity exercise.

(1) Background: Identifying groups with a misaligned physical capacity (PC) and physical activity (PA) is potentially relevant for health promotion. Although an important health determinant, deeper knowledge of underlying walking behavior patterns in older adults is currently missing. We aim to identify specific PA signatures of misaligned groups and determine PA variables discriminating between groups. (2) Methods: In total, 294 community-dwelling older adults (≥70 years) were divided into four quadrants based on thresholds for PA (≥ or <5000 steps/day) and PC (≤ or >12 s, Timed Up and Go test). Kruskal–Wallis and effect sizes were calculated to compare quadrants’ PA variables and to determine the discriminative power of PA parameters on walking duration, frequency, and intensity. (3) Results: We identified quadrant-specific PA signatures. Compared with “can do–do do”, the “cannot do–do do” group performs shorter continuous and lower-intensity walks; the “can do–do not do” group takes fewer steps and walks with less intensity. The “cannot do–do not do” group presents lower values in all PA variables. “Walking duration greater or equal 3 METs” was the strongest discriminative PA variable. (4) Conclusion: We provide distinct PA signatures for four clinically different groups of older adults. Walking intensity is most useful to distinguish community-dwelling older adults, which is relevant for developing improved customized health promotion interventions.

Background
Safe and stable walking is a complex process involving the interaction of neuromuscular, sensory and cognitive functions. As physical and cognitive functions deteriorate with ageing, training of both functions may have more beneficial effects on walking and falls prevention than either alone. This article describes the study design, recruitment strategies and interventions of the PASSWORD study investigating whether a combination of physical and cognitive training (PTCT) has greater effects on walking speed, dual-task cost in walking speed, fall incidence and executive functions compared to physical training (PT) alone among 70–85-year-old community-dwelling sedentary or at most moderately physically active men and women.
Methods
Community-dwelling sedentary or at most moderately physically active, men and women living in the city of Jyväskylä will be recruited and randomized into physical training (PT) and physical and cognitive training (PTCT). The 12-month interventions include supervised training sessions and home exercises. Both groups attend physical training intervention, which follows the current physical activity guidelines. The PTCT group performes also a web-based computer program targeting executive functions. Outcomes will be assessed at baseline and at 6 and 12 months thereafter. Falls data are collected during the interventions and the subsequent one-year follow-up. The primary outcome is 10-m walking speed. Secondary outcomes include 6-min walking distance, dual-task cost in walking speed, fall incidence and executive function assessed with color Stroop and Trail Making A and B tests. Explanatory outcomes include e.g. body composition and bone characteristics, physical performance, physical activity, life-space mobility, fall-related self-efficacy, emotional well-being and personality characteristics.
Discussion
The study is designed to capture the additive and possible synergistic effects of physical and cognitive training. When completed, the study will provide new knowledge on the effects of physical and cognitive training on the prevention of walking limitations and rate of falls in older people. The expected results will be of value in informing strategies designed to promote safe walking among older people and may have a significant health and socio-economic impact.
