Developmental coordination disorder and dysgraphia: signs and symptoms, diagnosis, and rehabilitation
Developmental coordination disorder (DCD) is a common and well-recognized neurodevelopmental disorder affecting approximately 5 in every 100 individuals worldwide. It has long been included in standard national and international classifications of disorders (especially the ). Children and adults with DCD may come to medical or paramedical attention because of poor motor skills, poor motor coordination, and/or impaired procedural learning affecting activities of daily living. Studies show DCD persistence of 30-70% in adulthood for individuals who were diagnosed with DCD as children, with direct consequences in the academic realm and even beyond. In particular, individuals with DCD are at increased risk of impaired handwriting skills. Medium-term and long-term prognosis depends on the timing of the diagnosis, (possible) comorbid disorders (and their diagnosis), the variability of signs and symptoms (number and intensity), and the nature and frequency of the interventions individuals receive. We therefore chose to investigate the signs and symptoms, diagnosis, and rehabilitation of both DCD and developmental dysgraphia, which continues to receive far too little attention in its own right from researchers and clinicians.
The Adult Developmental Coordination/Dyspraxia Checklist (ADC) in Greek Young Adults: a preliminary reliability study
Developmental Coordination Disorder (DCD) is a neurodevelopmental disorder that affects motor coordination with a negative impact on daily activities. DCD persists from childhood to adulthood in 50–70% of individuals. The present study was a preliminary evaluation of the reliability of the Greek Adult Developmental Coordination Disorders/Dyspraxia Checklist (ADC) version, a self-reported questionnaire to screen adults for probable DCD. One hundred and three (N=103) young adults (aged 19-26), without a previous diagnosis of neurological disorders or disabilities, who completed the Greek version of the ADC checklist twice, with a three-week interval between test-retest measurements. The results showed that Cronbach's alpha values ranged from 0.77 to 0.92 for each of the three subscales and 0.93 for the total ADC scores, indicating a strong internal consistency. Similarly, test-retest reliability results showed intraclass correlation coefficients ranging from 0.63 to 0.87 across each subscale and the total ADC score. Thus, the results revealed acceptable values and moderate to good reliability for the Greek ADC version. In conclusion, given that motor difficulties associated with DCD persist into adulthood and negatively affect motor coordination and daily activities, the Greek ADC version could be a reliable screening tool for identifying probable DCD in young adults. However, further research regarding validity issues in Greek young adults with and without developmental disorders, including DCD, is encouraged.  
Evaluation of the Developmental Coordination Questionnaire (DCDQ) as a Screening Instrument for Co-occurring Motor Problems in Children with Autism Spectrum Disorder
Although motor problems are highly prevalent in children with autism spectrum disorder (ASD), they remain underdiagnosed. Questionnaire-based screening for motor problems could optimize current clinical practice. This study aimed to examine the psychometric properties of the developmental coordination disorder questionnaire (DCDQ) to screen for co-occurring motor problems in individuals with ASD (n = 115; aged 5–15 years). Results indicated an excellent internal consistency; concurrent and discriminant validity with the Movement Assessment Battery for Children, second edition. Sensitivity was excellent, but specificity was lower. The positive and negative predictive values indicate that the DCDQ can be used to detect motor problems in children with ASD and can exclude the presence of developmental coordination disorder.
Evaluation of Motor Coordination in Boys with High-Functioning Pervasive Developmental Disorder Using the Japanese Version of the Developmental Coordination Disorder Questionnaire
Children with high-functioning pervasive developmental disorder (HFPDD) often have motor coordination dysfunction. However, there is no assessment tool for screening developmental coordination disorder (DCD) in Japan, which makes it difficult to evaluate the actual motor impairments of children with HFPDD. We evaluated the motor coordination function of 54 school-age boys with HFPDD using the Japanese version of the Developmental Coordination Disorder Questionnaire (DCDQ-J). We subsequently assessed the relationship between DCDQ-J scores and the results of the Japanese version of the Autism Diagnostic Interview-Revised (ADI-R) of 48 boys. The total and subscale DCDQ-J scores of the boys with HFPDD were significantly lower than the population means in the same grade: 37.0 % were below 2 standard deviations for the total score, 38.9 % for control during movement, 26.0 % for fine motor/handwriting, and 37.0 % for general coordination. Furthermore, the scores of Qualitative Abnormalities in Communication in the ADI-R were negatively correlated with control during movement, fine motor/handwriting, and total scores in the DCDQ-J. This study is the first to show Japanese children with HFPDD frequently exhibit considerably poor motor coordination according to the DCDQ-J. The screening or assessment of motor dysfunction in HFPDD using assessment tools such as the DCDQ could aid the development of interventions for these underestimated problems in Japan.

Psychometric Validation and Reference Norms for the European Spanish Developmental Coordination Disorder Questionnaire: DCDQ-ES
The Developmental Coordination Disorder Questionnaire (DCDQ) is a widely used and well-validated tool that contributes to the diagnosis of Developmental Coordination Disorder (DCD). The aim of this study was to further analyze the psychometric properties of the European Spanish cross-culturally adapted version of the Developmental Coordination Disorder Questionnaire (DCDQ-ES) in a sample of Spanish children aged 6–11 years and to establish reference norms with respect to age groups. Parents of 540 typically developing children completed the DCDQ-ES. A second sample of 30 children with probable DCD (pDCD) was used to test its discriminant validity. Confirmatory factor analysis supported the original three-factor structure and the internal consistency was excellent (Cronbach’s α = 0.907). Significant differences between age groups were found. The pDCD group scored significantly lower than the reference sample in the three subscales and DCDQ-ES total score (p < 0.001; AUC = 0.872). The DCDQ-ES is a reliable and valid tool for screening motor coordination difficulties in Spanish children and for identifying children with probable DCD. The findings of this research suggest that context-specific cut-off scores should be systematically utilized when using cross-cultural adaptations of the DCDQ. Age-specific cut-off scores for Spanish children are provided.
Association Between Developmental Coordination Disorder Traits, Autistic Traits, and Emotional/Behavioral Problems in Japanese Preschool Children
Background/Objectives: Few studies have examined the association between developmental coordination disorder (DCD) traits and emotional/behavioral problems in preschool children, considering the influence of autistic traits. Furthermore, no consistent results have been obtained. The aim of this study was to investigate how DCD traits in preschool children are related to emotional/behavioral problems, controlling for the effects of autistic traits. Methods: A questionnaire survey was administered to 277 parents of children (154 boys, 73.0 ± 3.8 months) who underwent the school physical examination in Y city near Tokyo, Japan, from October to December 2021. The Developmental Coordination Disorder Questionnaire–Japanese version (DCDQ-J) was used to measure DCD traits, the Autism Spectrum Screening Questionnaire (ASSQ) was used to measure autistic traits, and the Strengths and Difficulties Questionnaire (SDQ) was used to measure emotional/behavioral problems. In the hierarchical multiple regression analysis, the SDQ was the dependent variable, with gender entered in Step 1, ASSQ in Step 2, and DCDQ in Step 3. Results: The results showed that autistic traits are associated with preschool children’s emotional/behavioral problems, but even after controlling for autistic traits, higher DCD traits had a relationship with higher conduct problems, hyperactivity/inattention, and peer problems and lower prosocial behavior of preschool children. Conclusions: This result indicates the need for the support of children with motor skill difficulties. Additionally, a focus on mitigating DCD traits not only improves motor skills but also prevents emotional/behavioral problems in preschool children.
Association between General Movements Assessment and Later Motor Delay (excluding Cerebral Palsy) in Low-Birth-Weight Infants
The general movements (GMs) assessment is useful for the prediction of cerebral palsy (CP) and other developmental disorders. Developmental coordination disorder (DCD) is highly prevalent in low-birth-weight (LBW) infants. We investigated the association between aberrant GMs during early infancy and later motor development in LBW infants. The study included infants who fulfilled the following criteria: GMs assessed at 9–20 weeks post-term age; developmental quotient (DQ) assessed at 3 years of age using the Kyoto Scale; intelligence quotient (IQ) assessed at 6 years of age. Participants with normal IQs at 6 years of age without a diagnosis of CP (14 males and 37 females, 23–36 weeks gestation with birth weights of 492–1498 g) were categorized into normal (n = 39) and aberrant (n = 12) groups based on GMs assessment; DQ was compared between the groups. We investigated the items in the DQ assessment and found that the infants in the aberrant group were more frequently unable to perform. Infants in the aberrant group showed a significantly lower DQ in the ‘postural-motor domain’, and were more frequently unable to ‘climb the stairs with alternating legs’ and ‘Jump from a 15–20 cm platform’. This study highlights that GMs aberrancy in early infancy is associated with a delayed gross motor development, even in children with a typical development. The GMs assessment may be useful for the prediction of DCD.
Development and psychometric properties of the DCDDaily: a new test for clinical assessment of capacity in activities of daily living in children with developmental coordination disorder
Objective: To develop the DCDDaily, an instrument for objective and standardized clinical assessment of capacity in activities of daily living (ADL) in children with developmental coordination disorder (DCD), and to investigate its usability, reliability, and validity. Subjects: Five to eight-year-old children with and without DCD. Main measures: The DCDDaily was developed based on thorough review of the literature and extensive expert involvement. To investigate the usability (assessment time and feasibility), reliability (internal consistency and repeatability), and validity (concurrent and discriminant validity) of the DCDDaily, children were assessed with the DCDDaily and the Movement Assessment Battery for Children-2 Test, and their parents filled in the Movement Assessment Battery for Children-2 Checklist and Developmental Coordination Disorder Questionnaire. Results: 459 children were assessed (DCD group, n = 55; normative reference group, n = 404). Assessment was possible within 30 minutes and in any clinical setting. For internal consistency, Cronbach’s α = 0.83. Intraclass correlation = 0.87 for test–retest reliability and 0.89 for inter-rater reliability. Concurrent correlations with Movement Assessment Battery for Children-2 Test and questionnaires were ρ = −0.494, 0.239, and −0.284, p < 0.001. Discriminant validity measures showed significantly worse performance in the DCD group than in the control group (mean (SD) score 33 (5.6) versus 26 (4.3), p < 0.001). The area under curve characteristic = 0.872, sensitivity and specificity were 80%. Conclusions: The DCDDaily is a valid and reliable instrument for clinical assessment of capacity in ADL, that is feasible for use in clinical practice.
Motor Impairments in Children with Autism Spectrum Disorder: A Systematic Review and Meta-analysis
This article comprehensively reviews motor impairments in children with autism spectrum disorder (ASD) to: (1) determine the prevalence of motor problems in children with ASD; (2) understand the nature of motor difficulties in ASD and whether they are consistent with developmental coordination disorder (DCD); and (3) determine if the term DCD was used as a co-occurring diagnosis in children with ASD after publication of the DSM-5 in 2013. The following databases were systematically searched: MEDLINE, EMBASE, CINAHL, and PsycINFO from 2010 to December 2021. Articles were included if they: (1) were peer-reviewed and published in a scientific journal; (2) included children with ASD who were between 5 and 12 years; (3) used motor or function measures to assess motor abilities in children with ASD. Studies that included children with intellectual disabilities were excluded. Two independent reviewers reviewed titles, abstracts, and full-text articles for inclusion. Twenty-seven studies met the inclusion criteria and were assessed for quality by two independent reviewers using the Appraisal tool for Cross-Sectional Studies. The majority of articles (92.5%) indicated that 50–88% of children with ASD had significant motor impairments on standardized motor assessments and/or functional questionnaires. The nature of motor and function problems in ASD were consistent with DCD; however, only three out of 20 papers (15%) that were published from 2014 described the motor problems as DCD. One study reported that 15.1% of children with ASD with motor impairments had a co-occurring diagnosis of DCD, suggesting that DCD is under-recognized in this clinical population.
