MET异常NSCLC诊疗专家共识(2025版)
间质上皮细胞转化因子(mesenchymal-epithelial transition factor, MET)基因位于人类7号染色体上,对细胞的增殖、迁移、侵袭和血管生成等生理过程起着至关重要的调控作用。MET基因是非小细胞肺癌(non-small cell lung cancer, NSCLC)的重要驱动基因之一,其异常形式多样,包括MET 14号外显子(MET exon 14, METex14)跳跃突变、MET基因扩增、MET融合、MET蛋白过表达以及MET活化突变等。随着对MET异常机制的深入理解,针对这些异常的治疗策略日益受到重视,且多项研究已证实携带MET异常的NSCLC患者能从这些治疗中显著获益。中国老年保健协会肺癌专业委员会致力于协助中国医师的临床实践,并为临床工作提供规范化指导,因此组织各专家委员结合临床实践经验并探寻循证医学证据,针对目前MET异常NSCLC诊断和治疗的临床问题给予专业建议,并制定专家共识《MET异常NSCLC诊疗专家共识(2025版)》,以期优化患者的治疗结果。
DLL3-TCE治疗SCLC中国专家共识
小细胞肺癌(small cell lung cancer, SCLC)因其侵袭性强、早期转移率高、预后差,长期以来是肺癌治疗的难点。近年来,免疫检查点抑制剂(immune checkpoint inhibitors, ICIs)的出现显著提升了SCLC患者的总生存,成为SCLC治疗新标准,然而患者整体预后仍有较大提升空间。δ样配体3-T细胞衔接器(delta-like ligand 3-T cell engager, DLL3-TCE)作为一种新兴的靶向T细胞的免疫疗法,可精准地引导T细胞攻击肿瘤细胞,在SCLC治疗中展现出突破性潜力。塔拉妥单抗作为其中代表性药物目前已获批用于临床,多款TCE药物正在临床开发中。尽管DLL3-TCE药物在SCLC临床应用中展现出良好前景,但其作为新兴治疗手段,广大临床医生对该类药物的作用机制、适用人群及临床应用管理尚缺乏深入了解。为规范DLL3-TCE在中国的临床应用,中国临床肿瘤学会小细胞肺癌专家委员会组织共识组专家基于循证医学证据与专家实践经验,制定了《DLL3-TCE治疗SCLC中国专家共识》,旨在为DLL3-TCE类药物的临床应用提供规范化指导。
EGFR 20外显子插入突变非小细胞肺癌规范化诊疗中国专家共识(2024版)
表皮生长因子受体(epidermal growth factor receptor, EGFR)20外显子插入(exon 20 insertion, ex20ins)突变非小细胞肺癌(non-small cell lung cancer, NSCLC)的规范化诊疗在《EGFR 20外显子插入突变非小细胞肺癌规范化诊疗中国专家共识(2023版)》中进行了阐述,提高了临床医生对该罕见靶点的关注。随着疾病领域探索的深入及新型靶向药物的获批,EGFR ex20ins NSCLC的诊疗策略也随之更新。本共识在2023版中国专家共识的基础上进行更新,专家组通过参考国内外文献及临床数据并结合专家自身临床经验,分别从疾病认知、疾病检测、疾病治疗和新型靶向药物研发现状等方面更新共识性建议,以期更好地为临床医师提供用药参考。
Development of Evidence-based Recommendations: Implications for Preparing Expert Consensus Statements
INTRODUCTION In the past decades, the introduction of evidence-based medicine has prompted a paradigm shift in clinical practice. One of the important progress is involving health-care professionals in making informed decisions according to recommendations from evidence-based practice guidelines or other guidance documents (e.g., expert consensus statements) published by national and international authorities (e.g., the National Institute for Health and Care Excellence [NICE]; World Health Organization [WHO]) as well as professional bodies.
EGFR 20外显子插入突变非小细胞肺癌规范化诊疗中国专家共识(2023版)
随着非小细胞肺癌(non-small cell lung cancer, NSCLC)精准诊疗的不断发展,表皮生长因子受体(epidermal growth factor receptor, EGFR)20外显子插入(exon 20 insertion, ex20ins)突变作为罕见突变亚型逐渐被关注,其异质性高,不同插入位点亚型临床获益不同,预后极差,对现有传统治疗方案疗效有限,且常规聚合酶链反应(polymerase chain reaction, PCR)漏检率近50%,因此,在临床诊疗过程中更应该引起高度重视。本共识专家组通过国内外文献及临床数据的参考,并且结合专家自身临床经验,形成EGFR ex20ins突变NSCLC临床规范化诊疗专家共识,分别从疾病认知、疾病检测、疾病治疗和疾病相关新型靶向药物研发现状等方面提出共识性建议,以期为各级临床医师提供用药参考。
