Enhancing Robotic Surgery Training and Reducing Remote Complications with Telesurgery Technology

Introduction: The 2001 Lindbergh operation provided evidence for the feasibility of transatlantic telesurgery.(1-3) However, technological and economic challenges have limited the implementation of this technique.(4-6) This video illustrates details of a telesurgery connection over a 13,000 km distance between Orlando (USA) and Shanghai (China). Surgeons at both locations operated simultaneously on the same animals using telesurgery consoles (MicroPort· MedBot™) for teleproctoring, allowing for a robust evaluation of connectivity and robotic system performance across vast distances. Methods: On July 23rd and 24th, 2024, we conducted a prospective telesurgery study using live animal models (porcine) connecting Orlando to Shanghai. We reproduced a real-life telesurgery scenario where both ends of the connection had control over the robot. Four surgeons were in Orlando and one in Shanghai. We illustrated the communication between surgeons and highlighted the potential of telesurgery to improve outcomes and teaching robotic surgery. Results: Connectivity and robotic technology performed optimally for several hours without troubleshooting or malfunctions. Median delay was 139 milliseconds (137-216) on the first day and 139 milliseconds (137-185) on the second day. The surgeons were able to switch the console control multiple times during the procedures. They could communicate, discuss cases in real-time, and seamlessly transfer control in critical steps of the surgery. Conclusions: This video underscores the practical potential of Telesurgery use in teleproctoring, particularly when an experienced remote surgeon steps in to assist another surgeon during complex or challenging procedures. It highlights Telesurgery's potential for training and improving outcomes in robotic surgery.

Super‐priority lien challenged for its state constitutionality in the United States of America

This note focuses on super‐priority liens (SPLs), which allow homeowner associations to foreclose a property over its mortgage. Twenty‐two states have so far implemented such liens, hoping to balance the interests of mortgage creditors and homeowner associations. The state of Nevada has been facing more challenges from both mortgage creditors and homeowner associations regarding the constitutionality of non‐judicial foreclosure than other states. This paper summaries the cases since 2000 and discusses the three major decisions that the Supreme Court of Nevada made involving SPLs' non‐judicial constitutionality, tender‐extinguishing SPLs, and federal creditors' role in SPLs.

Statutory lien: pre- and post- 2016 amendments to the Malaysian National Land Code version 1; peer review: 2 approved with reservations

Background: A statutory
lien is a non-registrable security dealing under the National Land Code (NLC) in Malaysia. Lien is usually used as a short-term collateral for a repayment of debt by the proprietor. Prior to the 2016 amendments, section 281(1) of the NLC stated that a proprietor may create a lien over his land by depositing the original title with the lender, as security for a loan; and the lender shall enter a lien-holder's caveat under section 330(1) to protect his interests. Case law has expanded the scope of section 281 to include an equitable proprietor under the definition of \"proprietor\" and a third-party loan with the proprietor's consent or authority under the definition of \"loan\". The 2016 amendments removed \"for a loan\" in sections 281(1) and 330(1). This triggers the analysis on the purposes of lien.
Methods: The research methodology adopted is doctrinal legal research. Relevant sections and case law together with the Bill and Parliament's Hansard record are examined to find the intention of the legislature and Parliament.
Results: The amendments allow the lien to be used not merely as security for a loan but also for other purposes (Explanation Note 47 to the Bill). Such deletions have created an ambiguous interpretation; and may be manipulated by fraudsters. In contrast, section 241(1) provides specific purposes such as the repayment of debt, non-debt, or payment of annuity or periodic sum, for the creation of statutory charge.
Conclusions: This research is only confined to the scope of lien within the Peninsular Malaysia and not the States of Sabah and Sarawak which have their respective Sabah Land Ordinance and Sarawak Land Code. A future comparative study could be made amongst the legislations. The research output may be useful for the draftsman to clear up the ambiguity in the relevant provisions.
