Impact of Infant and Maternal Factors on Energy and Macronutrient Composition of Human Milk

The present study investigates the influence of selected infant and maternal factors on the energy and macronutrient composition of mature human milk (HM). The study enrolled 77 mothers at 4–8 weeks postpartum. Each mother provided 1 sample of HM. Each extracted HM sample was formed by mixing four subsamples of HM, each of which were obtained in one predefined 6-h periods of the day. Among maternal factors, the analysis included: anthropometric data before and after pregnancy; weight gain in pregnancy; body composition, assessed using the Maltron BioScan 920-II to analyze bioimpedance; and dietary intake, assessed with three-day dietary records. Among the neonatal factors, birth weight and length, number of daily feedings and type of delivery were included. The composition of HM, including energy content, protein, fat and carbohydrate concentrations, was analyzed using the Miris human milk analyzer. Pearson’s and Spearman’s correlation coefficients and multivariable logistic regression models were used to analyze the association between the selected maternal and infant factors and HM milk composition. It was found that total protein content of HM was correlated with pre-pregnancy BMI (Spearman rho = 0.238; p = 0.037), current lean body mass (Spearman rho = −0.293, p = 0.01) and total water content (Spearman rho = −0.315, p = 0.005). Carbohydrates were the only macronutrients whose composition was significantly affected by the infant factors. It was reported that higher carbohydrate content was associated with male sex (OR = 4.52, p = 0.049). Our results show that maternal and infant factors, especially maternal pre-pregnancy and current nutritional status and infant sex, interact and affect HM composition, suggesting that macronutrient and energy content in HM may be determined in pregnancy and may have unique compositional profile for every mother–infant dyad.

Maternal dietary patterns are associated with human milk composition in Chinese lactating women

Dietary patterns are a useful tool to study the impact of overall maternal diet on human milk (HM) composition beyond single foods or nutrients. The present study aimed to identify dietary patterns among Chinese lactating women and assess their associations with HM macronutrient composition.
Dietary intake data and HM samples were collected from 122 Chinese mothers at three to five study visits during the first 52 d postpartum. Dietary patterns were derived using principal component analysis. Cross-sectional associations of dietary patterns and HM macronutrients were assessed using multivariable linear regression models adjusted for total energy intake. All analyses were done separately for colostrum (postpartum days 0-7) and mature milk (postpartum days 8-52).
Four dietary patterns were identified: high-in-animal-foods, high-in-eggs, high-in-plant-foods, and high-in-fruits. Compared with the lowest tertile (T1), participants in the highest tertile (T3) of the high-in-animal-foods and high-in-plant-foods patterns had lower protein (respectively, T3 − T1 = −1.09 g/100 mL, Ptrend = 0.002; T3 − T1 = −0.54 g/100 mL, Ptrend = 0.001) and higher fat (respectively, T3 − T1 = 0.86 g/100 mL, Ptrend = 0.040; T3 − T1 = 0.40 g/100 mL, Ptrend = 0.004) concentrations in colostrum. In contrast, in mature milk the high-in-animal-foods pattern was positively associated with carbohydrates (T3 − T1 = 0.53 g/100 mL, Ptrend = 0.008) and the high-in-plant-foods pattern was negatively associated with fat (T3 − T1 = −0.64 g/100 mL, Ptrend = 0.002). The high-in-eggs pattern was weakly positively associated with protein concentration in mature milk (T3 − T1 = 0.10 g/100 mL, Ptrend = 0.023).
Maternal dietary patterns with high proportions of animal and plant-based foods were associated with higher fat and lower protein concentrations in colostrum. Different associations were found in mature milk. Dietary-pattern analysis provides an opportunity to characterize total diet and may be more predictive of HM composition than single foods or nutrients.

Background
This study aims to systematically review the effects of maternal vitamin and/or mineral supplementation on the content of breast milk.
Methods
We systematically searched electronic databases including Medline via PubMed, Scopus and ISI Web of Science till May 24, 2018. The following terms were used systematically in all mentioned databases: (“human milk” OR “breast milk” OR “breast milk composition” OR “human breast milk composition” OR “composition breast milk” OR “mother milk” OR “human breast milk” OR “maternal milk”) AND (“vitamin a” OR “retinol” OR “retinal” OR “retinoic acid” OR “beta-carotene” OR “beta carotene” OR “ascorbic acid” OR “l-ascorbic acid” OR “l ascorbic acid” OR “vitamin c” OR “vitamin d” OR “cholecalciferol” OR “ergocalciferol” OR “calciferol” OR “vitamin e” OR “tocopherol” OR “tocotrienol” OR “alpha-tocopherol” OR “alpha tocopherol” OR “α-tocopherol” OR “α tocopherol” OR “vitamin k” OR “vitamin b” OR “vitamin b complex” OR “zinc” OR “iron” OR “copper” Or “selenium” OR “manganese” OR “magnesium”) and we searched Medline via Medical subject Headings (MeSH) terms. We searched Google Scholar for to increase the sensitivity of our search. The search was conducted on human studies, but it was not limited to the title and abstract. Methodological quality and risk of bias of included studies were evaluated by Jadad scale and Cochrane risk of bias tools, respectively.
Results
This review included papers on three minerals (zinc, iron, selenium) and 6 vitamins (vitamin A, B, D, C, E and K) in addition to multi-vitamin supplements. Although studies had different designs, e.g. not using random allocation and/or blinding, our findings suggest that maternal use of some dietary supplements, including vitamin A, D, vitamin B1, B2 and vitamin C might be reflected in human milk. Vitamin supplements had agreater effect on breast milk composition compared to minerals. Higher doses of supplements showed higher effects and they were reflected more in colostrum than in the mature milk.
Conclusion
Maternal dietary vitamin and/or mineral supplementation, particularly fat- soluble vitamins, vitamin B1, B2 and C might be reflected in the breast milk composition. No difference was found between mega dose and single dose administration of minerals.

Gestational Hypertension and Human Breast Milk Composition in Correlation with the Assessment of Fetal Growth—A Pilot Study

Background and aims: 1 in 10 pregnant women is diagnosed with gestational hypertension. Increasing evidence suggests that preeclampsia, gestational diabetes and gestational hypertension may affect human breast milk’s lactogenesis and percentage composition. We aimed to examine whether there is any significant influence of gestational hypertension on the composition of macronutrients in human breast milk and to assess its correlation with fetal growth. Methods: A total of 72 breastfeeding women (34 diagnosed with gestational hypertension and 38 normotensive women during pregnancy) were recruited to the study at the Division of Neonatology, Medical University of Gdańsk, between June and December 2022. Milk samples were collected between the 3rd and 6th day of lactogenesis. Samples were analyzed using the Miris HMA™ Human Milk Analyzer (Upsala, Sweden), which evaluates the milk composition’s energy, fat, carbohydrate and protein quantity. In addition, we assessed the children’s anthropometric measurements (birth weight, body length and head circumference at birth). We used logistic regression to estimate the adjusted odds ratio and 95% confidence interval. Results: The mean (±standard deviation) macronutrient composition per 10 mL of milk in the GH group was 2.5 g (±0.9) of fat, 1.7 g (±0.3) of true protein, 7.7 g (±0.3) of carbohydrates and 63.2 g (±8.1) of energy, in the normotensive women group 1.0 g (±0.9) of fat, 1.7 g (±0.3) of true protein, 7.3 g (±0.4) of carbohydrates and 57.9 g (±8.6) of energy content, respectively. The fat composition was higher in the PIH group by a mean of 0.6 g (p < 0.005). Gestational hypertension had a positive, significant correlation with birth weight (p < 0.013) and the mother’s pre-pregnancy weight (p < 0.005). Conclusions: In conclusion, we found significant differences between milk composition in postpartum women with gestational hypertension compared to healthy, normotensive women. Human milk from women with gestational hypertension was found to contain a higher composition of fat, carbohydrates and energy in comparison to healthy women. Our aim is to further evaluate this correlation, as well as to assess the growth rate of newborns in order to determine the need for individualized formulas for women with pregnancy-induced hypertension, those with poor lactogenesis and those who cannot or choose not to breastfeed.

Assessing strategy to improve dairy production and milk quality under smallholder dairying: Evidence from Ethiopia

Little information exists on effectiveness of the approach in the intervention programme delivered to smallholder dairy farmers. A comparative study was conducted to assess milk production and composition in two districts (Doyogena project area and Angacha as a control) and the associated dairy intervention. Data were collected through household survey and milk testing. The average daily milk yield of crossbred dairy cows was 6.91±1.14 litres, which was significantly (p < 0.05) different across districts. The overall mean fat, solids-not-fat, lactose, salts, protein, and total solids (TS) contents of crossbred cows’ milk were 4.71±0.83, 8.85±0.36, 4.87±0.19, 0.72±0.03, 3.23±0.13 and 13.55±0.19 percent, respectively. The milk composition was significantly (p < 0.05) different across the study areas. Milk fat content is higher than the Ethiopian standard (ES); protein is also comparable. The major constraints for dairy production in the study areas were, in descending order, feed shortage in the dry season, land scarcity, lack of improved breeds, market access, and disease prevalence. To make livestock programs more impactful, strong extension and an integrated approach that encompasses improved feeds, breeding, marketing, and better health management are crucial to alleviate the diverse constraints of smallholder dairying while enhancing productivity and the associated food and nutrition security and livelihoods.

EFFECTS OF FEEDING OLIVE LEAVES ON LACTATING PERFORMANCE OF AWASSI EWES

The current study aimed to study the effects of feeding olive leaves (OL) on milk yield, milk composition, body weight, and blood parameters of lactating Awassi ewes. Thirty-six newly lambed ewes and their single lambs were distributed at random and assigned to one of three diets: 0% OL (CON), 15% OL (OL15), and 30% OL (OL30) to replace the forage source in sheep's diets. The first seven days of the 56-day experiment were dedicated to adaptation, with the remaining 49 days dedicated to the collection of data. Dry matter (DM) intake was greater (p = 0.014) in the OL15 and OL30 groups compared with the CON group. Crude protein and ether extract intake was also the highest in the OL30 group (p=0.0172) followed by the OL15 group and was lowest for the CON group. The consumption of acid detergent fiber and neutral detergent fiber was higher (p < 0.0001) in CON compared to other groups. A decrease (p = 0.0510) in milk production cost was seen in the group-fed diets containing OL. There were no differences in initial body weight in ewes and lambs, while ewes fed on the CON lost more body weight compared with the OL30, while OL15 was intermediate. The average daily gain in lambs was greater in OL15 and OL30 compared to the CON group. In summary, substituting OL for wheat straw did not impact the lactation performance of ewes whilst it decreased the cost of producing milk and may benefit the livestock farmers.

While human milk composition is characterised by marked dynamicity, we are far from having a clear picture of what factors drive this variation. Hormones in human milk are known to vary according to specific maternal phenotypes, but limited evidence shows the infant also has a role in determining milk composition. The present study aimed to investigate the interplay between maternal and infant characteristics in relation to human milk hormonal profile. In total, 501 human milk samples from mothers recruited in the Finnish STEPS cohort study (Steps to the healthy development) were analysed. Pre-pregnancy and pregnancy maternal data, socioeconomic status and infant characteristics at birth were collated. Leptin, adiponectin, insulin-like growth factor-1 and cyclic Glycine-Proline in milk were measured. Multivariate analysis of variance (MANOVA) and linear regression were utilised for statistical analysis. Sex-specific interactions with maternal factors were observed, as the infant sex mediated associations between gestational diabetes and milk adiponectin (p = 0.031), birth-mode and total protein (p = 0.003), maternal education and insulin-like growth factor-1: cyclic Glycine-Proline ratio (p = 0.035). Our results suggest that changes in human milk composition are associated with interactions between maternal and infant characteristics and pathophysiological factors. Future work should expand on these findings and further explore the link between hormonal profiles in human milk and infant outcomes.

Third-Trimester Glucose Homeostasis in Healthy Women Is Differentially Associated with Human Milk Oligosaccharide Composition at 2 Months Postpartum by Secretor Phenotype

Human milk oligosaccharides (HMOs) are bioactive molecules in human milk that play a critical role in infant health. Obesity and associated metabolic aberrations can negatively impact lactation and alter milk composition. Here, the relationship between maternal glucose homeostasis and HMO composition from 136 healthy women was examined. Maternal glucose homeostasis (fasting plasma glucose and insulin, homeostatic model assessment for insulin resistance, and insulin sensitivity index) was evaluated at 30 weeks of gestation in healthy women (body mass index = 18.5–35 kg/m2). Human milk samples were collected at two months postpartum. HMO concentrations were measured via high performance liquid chromatography. Women were categorized into “secretor” and “non-secretor” groups based on 2′-Fucosyllactose concentrations (<100 nmol/mL, non-secretor). Pearson’s correlation analysis and linear models were used to assess the relationships between maternal glucose homeostasis and HMO concentrations. In non-secretors, third trimester fasting plasma glucose and insulin were negatively associated with total HMO-bound sialic acid and concentrations of the sialylated HMOs 3′-sialyllactose and disialylacto-N-tetraose. In secretors, difucosyllactose and lacto-N-fucopentaose-II concentrations increased and sialyllacto-N-tetraose c and sialyllacto-N-tetraose b decreased as insulin sensitivity increased. This study is the first to demonstrate a relationship between obesity-associated maternal factors and HMO composition in both secretor and non-secretor populations.

The effects of β-lactoglobulin (β-LG), β-casein (β-CN), and κ-CN variants and β-κ-CN haplotypes on the relative concentrations of the major milk proteins α-lactalbumin (α-LA), β-LG, αS1-CN, αS2-CN, β-CN, and κ-CN and milk production traits were estimated in the milk of 1,912 Dutch Holstein-Friesian cows. We show that in the Dutch Holstein-Friesian population, the allele frequencies have changed in the past 16 years. In addition, genetic variants and casein haplotypes have a major impact on the protein composition of milk and explain a considerable part of the genetic variation in milk protein composition. The β-LG genotype was associated with the relative concentrations of β-LG (A » B) and of α-LA, αS1-CN, αS2-CN, β-CN, and κ-CN (B>A) but not with any milk production trait. The β-CN genotype was associated with the relative concentrations of β-CN and αS2-CN (A2>A1) and of αS1-CN and κ-CN (A1>A2) and with protein yield (A2>A1). The κ-CN genotype was associated with the relative concentrations of κ-CN (B>E>A), αS2-CN (B>A), α-LA, and αS1-CN (A>B) and with protein percentage (B>A). Comparing the effects of casein haplotypes with the effects of single casein variants can provide better insight into what really underlies the effect of a variant on protein composition. We conclude that selection for both the β-LG genotype B and the β-κ-CN haplotype A2B will result in cows that produce milk that is more suitable for cheese production.
