Multivitamin consumption and childhood asthma: a cross-sectional study of the NHANES database

Background
Dietary intakes of vitamins are associated with asthma. However, previous studies mainly explored the association between a single vitamin intake and asthma, which did not take the multivitamins into consideration. Herein, this study aims to explore the overall effect of dietary multivitamins consumption on childhood asthma.
Methods
Data of children and adolescents (aged 2-17 years old) were extracted from the National Health and Nutrition Examination Survey (NHANES) database in 2015-2018 in this cross-sectional study. Weighted univariate logistic regression analysis was used to screen covariates. The association between multivitamins (including vitamin A, C, D, E, B1, B2, B6, B12, K, niacin, folic acid, and choline) and childhood asthma was explored using univariate and multivariate logistic regression analyses. The evaluation indexes were odds ratio (OR) and 95% confidence interval (CI). We further introduced the Bayesian kernel machine regression (BKMR) to assess the joint effect of the twelve vitamins on childhood asthma, the impact of an individual vitamin as part of a vitamin mixture, and the potential interactions among different vitamins.
Results
Among 4,715 eligible children and adolescents, 487 (10.3%) had asthma. After adjusting for covariates including race, family history of asthma, pregnant smoking, BMI Z-score, energy intake, breast feeding, and low birth weight, we found that for each 1-unit increase in vitamin K consumption, the odds of childhood asthma decreased 0.99 (

=0.028). The overall effect analysis reported a trend of negative relationship between the multivitamins and childhood asthma, especially at the 75th percentile and over. According to the BKMR models, when other vitamins are fixed at the median level, the odds of childhood asthma increased along with the elevated vitamin D (VD) and vitamin B2 (VB2), whereas along with the depressed vitamin C (VC). In addition, no potential interaction has been found between every two vitamins of multivitamins on childhood asthma.
Conclusion
Among children and adolescents who have high-risk of asthma, it may be beneficial to increase dietary consumption of multivitamins. Our findings recommended that children and adolescents should increase the intake of VC-rich foods, whereas control the dietary consumption of VD and VB2 in daily life.

Background
Congenital heart disease (CHD) is a common birth defect. Our previous studies suggest that indoor air pollution, especially total volatile organic compounds (TVOCs), may increase fetal CHD risk, whereas vitamin and folic acid (FA) supplements in early pregnancy might offer protection against CHD. However, limited research has explored whether FA or multivitamin supplementation can mitigate the effects of TVOCs exposure on CHD.
Methods
We conducted a case-control study to investigate the association between maternal nutrient supplementation, household indoor air pollutant exposure during pregnancy, and CHD in offspring. Pregnant women with 22–30 gestational weeks were recruited from two hospitals in East China between January 2016 and March 2022. A comprehensive approach was used, incorporating questionnaires to collect nutrient supplement information, blood sample analysis to detect serum folate, vitamin B12, and homocysteine (HCY) concentrations, and field investigations to assess indoor benzene, toluene, xylene, formaldehyde, and TVOCs exposures. Logistic regression analysis was performed to identify CHD risk factors, and stratified analysis was used to evaluate the combined effects of nutrient supplementation and TVOCs on CHD.
Results
The study included 53 cases and 77 controls. Logistic regression analysis identified high maternal serum HCY (> 6.125 µmol/L) and high household TVOCs exposure (> 0.0165 mg/m³) as risk factors for CHD in offspring, with adjusted odds ratios of 2.98 (95% CI: 1.31–6.36) and 9.23 (95% CI: 3.78–22.53), respectively. Regular multivitamin supplementation mitigated the risk of high TVOCs exposure on fetal CHD, while the adverse effect of high serum HCY-related CHD risk was attenuated in the group with regular FA supplementation.
Conclusion
Exposure to high indoor TVOCs concentrations increases the risk of fetal CHD. Regular multivitamin supplementation may reduce the adverse effects of high TVOCs exposure on fetal CHD.

The association between maternal prenatal folic acid and multivitamin supplementation and autism spectrum disorders in offspring: An umbrella review

Previous reviews have examined the association between maternal prenatal use of folic acid and multivitamin supplements and autism spectrum disorder (ASD) in children, but findings remain inconclusive. This umbrella review aims to synthesise the existing evidence on the association between prenatal folic acid and multivitamin supplementation and the risk of ASD in offspring.
This umbrella review followed the PRISMA guidelines to synthesise and report evidence from existing systematic reviews and meta-analyses (SRMs). Articles were searched in PubMed, Scopus, Web of Science, and Google Scholar. The quality of included studies was assessed using the Assessment of Multiple Systematic Reviews (AMSTAR). A weighted inverse variance random-effects model was applied to estimate pooled effects. The association was quantified using relative risks (RRs) with 95% confidence intervals (CIs). Subgroup analysis and sensitivity analysis were also conducted. Heterogeneity and publication bias were assessed.
Eight SRMs comprising 101 primary studies and over three million mother-offspring pairs were included. Prenatal folic acid and/or multivitamin supplementation was associated with a 30% reduced risk of ASD in offspring (RR = 0.70, 95% CI: 0.62, 0.78; GRADE: highly suggestive). Subgroup analysis by supplement type showed that maternal prenatal multivitamin supplementation reduced the risk of ASD by 34% (RR = 0.66, 95% CI: 0.55-0.80; GRADE: highly suggestive), while folic acid supplementation was associated with a 30% reduction in ASD risk (RR = 0.70, 95% CI: 0.60-0.83; GRADE: highly suggestive).
Maternal prenatal folic acid and multivitamin supplementation are associated with a reduced risk of ASD in offspring. These findings have important public health implications, suggesting that prenatal supplementation could help mitigate the risk of ASD in children.

Stunting in children is caused by severely inadequate nutrients; therefore, balanced nutritious foods such as multivitamins is the most promising alternative interventions. Concentrated deep seawater is rich in minerals, thus provide potent multivitamin sources. The hygroscopic nature of minerals makes them prone to moisture. Consequently, the fabrication of a non-hygroscopic dosage form and the study on its potential for stunting conditions are highly needed. The concentrate was encapsulated into granules by using different fillers with Florite as desiccant. Physicochemical evaluations were conducted using Differential Scanning Calorimetry (DSC), X-ray Diffraction (XRD), and Scanning Electron Microscopy (SEM). An in vivo anti-stunting study was performed using zebrafish as an animal model. The results indicated that encapsulation using Avicel PH 102 as filler, either with or without addition of Florite 10%, provided good stability after 5 months of storage. Antistunting activity test showed that, after induction by rotenone, treatments with deep seawater either in form of concentrate water or granule improved the stunted condition of the zebra fish embryos shown as the improvement in the hatching rate, brain length, and body length. Deep seawater concentrate was successfully formulated into granules which provided moisture protection of its highly hygroscopic nature as well as antistunting activity.

Effects of multivitamin and mineral supplementation on adiposity, energy expenditure and lipid profiles in obese Chinese women

Background: Obese individuals are more likely to have either lower blood concentrations or lower bioavailability of minerals and/or vitamins. However, there are limited data on the effects of nutritional supplementation on body weight (BW) control, energy homeostasis and lipid metabolism in obese subjects. Objective: The purpose of this study is to evaluate the effects of supplementation with multivitamin and multimineral on adiposity, energy expenditure and lipid profiles in obese Chinese women. Design: A total of 96 obese Chinese women (body mass index (BMI) 28 kg m−2) aged 18–55 years participated in a 26-week randomized, double-blind, placebo-controlled intervention study. Subjects were randomized into three groups, receiving either one tablet of multivitamin and mineral supplement (MMS), or calcium 162 mg (Calcium) or identical placebo daily during the study period. BW, BMI, waist circumference (WC), fat mass (FM), fat-free mass, resting energy expenditure (REE), respiratory quotient (RQ), blood pressure, fasting plasma glucose and serum insulin, total cholesterol (TC), low- and high-density lipoprotein-cholesterol (LDL-C and HDL-C) and triglycerides (TGs) were measured at baseline and 26 weeks. Results: A total of 87 subjects completed the study. After 26 weeks, compared with the placebo group, the MMS group had significantly lower BW, BMI, FM, TC and LDL-C, significantly higher REE and HDL-C, as well as a borderline significant trend of lower RQ (P=0.053) and WC (P=0.071). The calcium group also had significantly higher HDL-C and lower LDL-C levels compared with the placebo group. Conclusion: The results suggest that, in obese individuals, multivitamin and mineral supplementation could reduce BW and fatness and improve serum lipid profiles, possibly through increased energy expenditure and fat oxidation. Supplementation of calcium alone (162 mg per day) only improved lipid profiles.

Relationships between multivitamins, blood biochemistry markers, and BMC and BMD based on RF: A cross-sectional and population-based study of NHANES, 2017–2018

Previous studies have separately suggested a possible association between the vitamin exposure, blood biochemical indicators, and bone density. Our study aimed to investigate the relationship between vitamin exposure serum concentrations, blood biochemical indicator serum concentrations, and BMC and BMD using the NHANES 2017-2018 nutrient survey data. This population-based cross-sectional study aimed to explore these associations.
In this study, we measured vitamin serum concentrations, serum ion serum concentrations, and serum biochemical indicators in adults participating in the NHANES. Skeletal status was assessed by evaluating BMC and BMD in the whole body. Given the inclusion of multiple variables and diverse data types, we used the RF to fit a multivariable model to estimate the associations between vitamin serum concentrations, blood biochemical indicator serum concentrations, and skeletal status.
Under the dimension reduction and comparison selection of RF model, we identified ALP, CPK, and creatinine serum concentrations as the most important factors associated with BMC and BMD in multiple skeletal sites, and the gender, age, height, weight, and body mass index which were found to be related to BMC and BMD in different skeletal sites. Vitamin D and blood calcium serum concentrations were not the important factors associated with BMC and BMD and the three blood biochemical indexes were more important than the vitamin level for BMC and BMD.
The effect of vitamin serum concentrations and blood calcium serum concentrations on human bone density was not significant. ALP, CPK and creatinine serum concentrations body development indicators were identified as the most important factors related to bone status. The RF model can be used to comprehensively evaluate the effects of vitamin content and blood biochemistry serum concentrations in adults on BMC and BMD.

Maternal use of folic acid and multivitamin supplements and infant risk of birth defects in Norway, 1999–2013

The association between folic acid supplementation and birth defects other than neural tube defects (NTD) remains unclear. We used a log-binomial regression model to investigate if periconceptional folic acid and/or multivitamin use was associated with birth defects in Norway with prospectively collected data from the Medical Birth Registry of Norway (MBRN) during 1999–2013. We used the European Surveillance of Congenital Anomalies (EUROCAT) classification system to define eleven organ-specific major birth defect groups (nervous system, eye, ear–face–neck, cardiovascular system, respiratory system, oral clefts, digestive system, abdominal wall, urinary system, genital organs and limb), with additional subgroups. Fetuses or infants whose mothers used folic acid and/or multivitamin supplements before and during pregnancy were classified as exposed. During the years 1999–2013, 888 294 (99·0 %) live-born infants, 6633 (0·7 %) stillborn infants and 2135 (0·2 %) fetuses from terminated pregnancies due to fetal anomalies were registered in the MBRN. Among the live- and stillborn infants of women who used vitamin supplements compared with infants of non-users, the adjusted relative risk (aRR) was 0·94 (95 % CI 0·91, 0·98) for total birth defects (n 18 382). Supplement use was associated with reduced risk of abdominal wall defects (aRR 0·58; 95 % CI 0·42, 0·80, n 377), genital organ defects (aRR 0·81; 95 % CI 0·72, 0·91, n 2299) and limb defects (aRR 0·81; 95 % CI 0·74, 0·90, n 3409). Protective associations were also suggested for NTD, respiratory system defects and digestive system defects although CI included the null value of 1. During the full study period, statistically significant associations between supplement use and defects in the eye, ear–face–neck, heart or oral clefts were not observed.

The Effects of Electrolytic Multivitamins and Neomycin on Antioxidant Capacity and Intestinal Damage in Transported Lambs

Transport stress can cause damage to animals. In this experiment, 60 four-month-old lambs were randomly divided into three groups: CG (basal diet), EG (basal diet + 375 mg/d/lamb electrolytic multivitamin), and NG (basal diet + 200 mg/d/lamb neomycin). The results were as follows: during road transport, in all groups, the levels of SOD, T-AOC, and GSP-Px, and mRNA expressions of CAT, SOD, Nrf2, HO-1, and Bcl-2 in the jejunum and colon decreased (p < 0.01). However, mRNA expressions of Keap1, IL-1β, IL-2, IL-12, Bax, and Caspase3 in the jejunum and colon and the level of MDA increased (p < 0.01). The concentrations of IgA, IgG, and sIgA in the jejunum and colon also decreased (p < 0.01). In the EG and NG, the levels of SOD (p < 0.05) and T-AOC (p < 0.01) increased, and the level of MDA decreased (p < 0.01). However, in the jejunum, the levels of SOD and T-AOC, the concentrations of IgA and IgG, and mRNA expression of Bcl-2 increased (p < 0.05). mRNA expressions of IL-1, IL-2, and Caspase 3 (p < 0.05), and mRNA expression of IL-12 (p < 0.01) decreased. In the colon, SOD activity and the concentration of sIgA increased (p < 0.01). The level of MDA and mRNA expressions of IL-2 and Caspase 3 also decreased (p < 0.05). In the jejunum and colon, mRNA expression of SOD (p < 0.05) and mRNA expression of Nrf2 increased (p < 0.01). mRNA expression of Keap1 (p < 0.05) and Bax (p < 0.01) decreased. In summary, road transport can cause a decrease in antioxidant activity and immunity of lambs and an increase in oxidative damage. Electrolytic multivitamins and neomycin can improve immune function and potentially reduce oxidative damage to the jejunum and colon.
