Effect of Educational Interventions on Understanding and Use of Nutrition Labels: A Systematic Review
The potential for nutrition labels to impact on population health is dependent on consumer ability to understand and use this information. Consumer understanding of this information varies across sociodemographic groups and with different label design formats. Labeling legislation requires consumer education on how to use nutrition labels, and recent mandatory changes to the Nutrition Facts Panel (NFP) are underway to improve comprehensibility. This review aimed to evaluate if educational programs can improve understanding and use of nutrition labels. Database searches were performed to identify interventions which delivered education on nutrition labels with outcomes measuring aspects of comprehension or use. A total of 17 studies were selected for review, including nine randomized and eight cohort studies. The majority of studies were conducted in the United States Study participants included school aged children, older adults, and those with diabetes within a range of intervention types involving taught sessions or web-based education. Whilst outcome measures were heterogenous, all studies reported a statistically significant improvement in one or more outcomes of participant understanding or use of nutrition labels. Aspects such as general nutrition knowledge, health literacy, and program delivery format warrant attention in future research. Education which optimizes comprehension and use of nutrition labels may have the potential to improve the impact of this information on dietary health.
Which Front-of-Package Nutrition Label Is Better? The Influence of Front-of-Package Nutrition Label Type on Consumers’ Healthy Food Purchase Behavior
To help consumers understand the healthfulness of food and make healthy food choices, manufacturers are increasingly providing front-of-package nutrition labels. However, not all types of front-of-package nutrition labels can promote consumers’ healthy food purchase behavior. We explored the impact of front-of-package nutrition label type on the consumer purchase behavior of healthy food through three experiments. The results show that evaluative (vs. objective) front-of-package nutrition labels can improve consumer purchase intention and willingness to pay for healthy food. The spokesperson type moderates the influence of front-of-package nutrition labels on consumers’ healthy food purchase behavior. Specifically, when the spokesperson type is a typical consumer, consumers are more willing to buy healthy foods with evaluative nutrition labels than those with objective nutrition labels. When the spokesperson type is a star, consumers are more willing to buy healthy food with objective nutrition labels (vs. evaluative nutrition labels). Finally, this study provides feasible suggestions for marketers to select appropriate front-of-package nutrition labels.
Association between Use of Nutrition Labels and Risk of Chronic Kidney Disease: The Korean National Health and Nutrition Examination Survey (KNHANES) 2008–2019
Nutrition labeling on food packages is increasingly found to promote healthier food choices associated with lower risk of chronic kidney disease (CKD). To examine associations between nutrition labels use and CKD risk, we conducted a nationally representative cross-sectional study of 32,080 adults from the 2008–2019 Korean National Health and Nutrition Examination Survey. Nutrition labels use was collected via self-reported questionnaires. Ascertainment and severity of CKD was determined by estimated glomerular filtration rate or proteinuria. In multivariable-adjusted (MV) logistic regression models, increasing awareness and use of nutrition labels was significantly associated with lower CKD risk (MV-adjusted OR “nutrition labels aware and use” group vs. “nutrition labels unaware” group [95% CIs]: 0.75 [0.59–0.95], Ptrend:0.03). This inverse association varied with CKD’s risk of progression, with 21% and 42% reduced risk observed for CKD subtypes with “moderate” and “high” risk of progression, respectively (all Ptrend ≤ 0.04). Furthermore, the nutrition labels use and CKD risk association significantly differed by age, with 35% reduced risk observed in the older group aged 49 years or older, but not in the younger group (Pinteraction < 0.001). Our results suggest increasing perception and use of nutrition labels may contribute to CKD prevention and its early asymptomatic progression, especially in older adults.
Can nutrition label recognition or usage affect nutrition intake according to age?
The aim of this study was to investigate the effect of the use of nutrition labeling on nutritional intake according to age groups, focusing on Korean elderly. Study participants (N = 5223) were adults at least 20 y of age and had participated in the Korean National Health Examination and Nutrition Survey in 2012. Data for recognition/use of nutrition labels were obtained by self-report. Nutrition intake also was estimated by 24-h dietary recall. Participants were categorized into three age groups: 20 to 39 y, 40 to 59 y, and ≥60 y. Generalized linear model was conducted to test mean differences between nutrition label recognition (NLR) and nutrition label use (NLU) groups for nutrient intake, according to the age groups. Results from this study indicated that younger individuals (age groups of 20–39 and 40–59 y) in the NLU group showed a significant association with nutrient intake compared to those in the NLR group. Additionally, nutrition intake status in the NLU group improved positively. Whereas older participants (≥60 y) in the NLR group showed a significant association with most nutrient intake compared with the NLU group. The study also found that protein intake was reduced in the NLU group compared with the non-NLU group across the age groups, except for older participants (age group 20–39 y: 79.16 versus 86.30 g, P = 0.050; age group 40–59 y: 69.97 versus 75.58 g, P = 0.040; age group of ≥60 y: 64.72 versus 64.89 g, P = 0.967). The present study revealed that nutrition labeling cannot be effective for the elderly, and there were several areas of misunderstanding. Therefore, more systematic education on the topic of nutrition labeling is required to help the elderly make healthier food decisions. •Nutrition labeling is a simple way to help people improve their nutrition intake by making healthier food choices. For older individuals, however, nutrition labeling did not affect nutrition intake.•Regarding to protein and fat intake, the results revealed inappropriate usage of nutrition labeling among all age groups.•Because older individuals are becoming a major target market in the food industry, more studies on nutrition labeling usage should be conducted, keeping this age group in mind. Therefore, more systematic education on the topic of nutrition labels is required in order for consumers, especially for the elderly, to make more informed and healthier food decisions.
Implementation of Nutrition Labels at the 2022 European Athletics Championships: An Observational Study of the Use and Perceptions of Athletes and Athlete Support Personnel
Background/Objectives: Nutrition labels are an effective tool for providing nutrition information. Additionally, nutrient composition is one of the most dictating factors for athletes’ food choices; thus, we aimed to evaluate the use and perceptions regarding the nutrition labels implemented for the meals served at the 2022 European Athletics Championships (EAC). Methods: During mealtime at the team restaurants, participants completed an online self-administered questionnaire. We collected 280 questionnaires, 53.8% of the participants were male, most were athletes (78.9%), and 21.1% were athlete support personnel. Likert-type scales and open-ended questions were included to measure the labels’ importance, layout, influence on food choices, and participants’ understanding of the labels. Mann–Whitney and Kruskal–Wallis tests were used to compare answers. Results: Almost 40% of the participants used the nutrition labels occasionally (38.8%). Most participants were confident (41.9%) or moderately confident (31.3%) in making food choices because they had labels. Nutrition labels were considered important (41.0%) or very important (28.4%) by most participants, and 91.7% would like to have them in future championships. Athlete support personnel versus athletes (p = 0.037) and participants with dietary restrictions versus participants without (p = 0.028) were more confident in their food choices due to nutrition labels. Conclusions: Our results highlight that nutrition labels were helpful for both athletes and athlete support personnel during this EAC and that they should be maintained in future competitions.
Nutrition Label Knowledge and Attitude to Affect Healthy Food Choice among Young Consumers in Malaysia
This research attempts to investigate the direct effect of nutrition label knowledge on attitude, and, nutrition label knowledge and attitude in choosing healthy food among Malaysian consumers as well as the role of nutrition label attitude as a mediator. This research applied the quantitative approach. A sample comprised 381 Malaysian consumers with minimum age of 18 years old. Data from a sample were obtained using a structured questionnaire and analysed using structural equation modelling (SEM). The findings reveal that nutrition label knowledge as a significant predictor of nutrition label attitude and, nutrition label knowledge and attitude as significant predictors of healthy food choice. In addition, the findings reveal the nutrition label attitude as a partial mediator in the relationship between nutrition label knowledge and healthy food choice. Although the findings provide fresh insights on the role of nutrition label knowledge and attitude in affecting healthy food choice, this research requires a replication in future research as to improve the explanatory power of the framework.
Objective understanding of the Nutri-score front-of-pack label by European consumers and its effect on food choices: an online experimental study
Background The effectiveness of Front-of-Pack nutrition Labels (FoPLs) may be influenced by national context. In light of the ongoing efforts to harmonize FoPLs across Europe, this study aimed to compare the effectiveness of five FoPLs (Health Star Rating system, Multiple Traffic Lights, Nutri-Score, Reference Intakes, Warning symbols) on consumer understanding and food choice in 12 European countries. Methods In 2018–2019, for three food categories, approximately 1000 participants per country were asked to select which food they would prefer to purchase between three products with distinct nutritional quality profiles, and then to rank the products by nutritional quality. Participants ( N  = 12,391 in total) completed these tasks first with no FoPL and then, after randomization to one of the five FoPLs, with a FoPL on the food packages. Associations between FoPLs and change in (i) nutritional quality of food choices and (ii) ability to correctly rank the products by nutritional quality were assessed with logistic regression models adjusted for sociodemographic and lifestyle characteristics of participants, conducted overall and by country. Findings Compared with the Reference Intakes, the Nutri-Score (OR = 3.23[2.75–3.81]; p  < 0.0001), followed by the Multiple Traffic Lights (OR = 1.68[1.42–1.98]; p  < 0.0001), was the most effective FoPL in helping consumers identify the foods’ nutritional quality, overall and in each of the 12 countries. Differences between FoPLs regarding food choice modifications were smaller, but the effect of the Nutri-Score seemed slightly higher in eliciting healthier food choices overall compared with the Reference Intakes, followed by the Warning symbols, the Multiple Traffic Lights and the Health Star Rating system. Interpretation In the context of FoPL harmonization in Europe, these findings from an online experiment provide insights into the Nutri-Score’s effectiveness on European consumers.
Consumers’ Perceptions of Five Front-of-Package Nutrition Labels: An Experimental Study Across 12 Countries
Consumers’ perceptions of five front-of-pack nutrition label formats (health star rating (HSR), multiple traffic lights (MTL), Nutri-Score, reference intakes (RI) and warning label) were assessed across 12 countries (Argentina, Australia, Bulgaria, Canada, Denmark, France, Germany, Mexico, Singapore, Spain, the UK and the USA). Perceptions assessed included liking, trust, comprehensibility, salience and desire for the label to be mandatory. A sample of 12,015 respondents completed an online survey in which they rated one of the five (randomly allocated) front-of-pack labels (FoPLs) along the perception dimensions described above. Respondents viewing the MTL provided the most favourable ratings. Perceptions of the other FoPLs were mixed or neutral. No meaningful or consistent patterns were observed in the interactions between country and FoPL type, indicating that culture was not a strong predictor of general perceptions. The overall ranking of the FoPLs differed somewhat from previous research assessing their objective performance in terms of enhancing understanding of product healthiness, in which the Nutri-Score was the clear front-runner. Respondents showed a strong preference for mandatory labelling, regardless of label condition, which is consistent with past research showing that the application of labels across all products leads to healthier choices.
Efficacy of front-of-pack nutrition labels in improving health status
•It remains unseen as to whether front-of-pack nutrition labels affect health status improvement.•Reduction of the intake of a single nutrient does not reduce the risk for non-communicable diseases and mortality.•Variations in single-food intake are rarely associated with an increased risk for mortality from non-communicable diseases.•Healthy dietary patterns are strongly associated with a decrease in mortality from non-communicable diseases. Nutrition labels advise consumers about the nutritional value of packaged foods and their contribution to the overall composition of the diet. They have been proposed as an instrument for the promotion of healthy diets and as a fundamental tool in the prevention of obesity and diet-related non-communicable diseases (NCDs). The aim of this review is to discuss the effectiveness of front-of-pack nutrition labels (FOPNLs) in improving health status, the concern about focusing on single nutrients/foods rather than on eating patterns to prevent obesity and NCDs, and the strength of positive rather than negative messages to promote a healthy and sustainable diet. Although nutrition science investigates individual foods or nutrients, when communicating to the public most of the significant evidence of the favorable health effect largely depends on dietary patterns and not on a single food component or individual nutrient. Therefore, we suggest that a new tool based on positive communication should be developed and implemented to highlight the importance of the diet as a complex matrix.
Modelling the impact of different front-of-package nutrition labels on mortality from non-communicable chronic disease
Background Front-of-Package nutrition labels (FoPLs) are intended to help reduce the incidence of nutrition-related non-communicable diseases through an improvement in diet quality. FoPLs have been shown to improve the nutritional quality of purchases and have been associated with improved diet quality, which is in turn associated with reduced risk of non-communicable diseases. However, the potential impact of FoPLs on reducing mortality from chronic diseases has never been estimated. Methods Data from a laboratory experimental economics test were used to investigate the effects of five different FoPLs (Nutri-Score, Health Star Rating system, Multiple Traffic lights, Reference intakes and SENS ( Système d’Etiquetage Nutritionnel Simplifié )) on the nutritional quality of household purchases. The relative differences in nutrient content and composition of food purchases were then applied to dietary intakes using data from an observational study, thus yielding estimates for ‘reference’ and ‘labelled’ diets. A macro-simulation study using the PRIME model was then conducted to estimate the impact of the modification in dietary intake as a result of FoPL use on mortality from diet-related non-communicable diseases. Results The use of FoPLs led to a substantial reduction in mortality from chronic diseases. Approximately 3.4% of all deaths from diet-related non-communicable diseases was estimated to be avoidable when the Nutri-Score FoPL was used. The remaining FoPLs likewise resulted in mortality reduction, although to a lesser extent: Health Star Rating system (2.8%), Reference Intakes (1.9%), Multiple Traffic Lights (1.6%), and SENS (1.1%). Conclusions FoPLs have the potential to help decrease mortality from diet-related non-communicable diseases, and the Nutri-Score appears to be the most efficient among the five formats tested.
