COVID-19 and stem cell transplantation; results from an EBMT and GETH multicenter prospective survey
This study reports on 382 COVID-19 patients having undergone allogeneic ( n  = 236) or autologous ( n  = 146) hematopoietic cell transplantation (HCT) reported to the European Society for Blood and Marrow Transplantation (EBMT) or to the Spanish Group of Hematopoietic Stem Cell Transplantation (GETH). The median age was 54.1 years (1.0–80.3) for allogeneic, and 60.6 years (7.7–81.6) for autologous HCT patients. The median time from HCT to COVID-19 was 15.8 months (0.2–292.7) in allogeneic and 24.6 months (−0.9 to 350.3) in autologous recipients. 83.5% developed lower respiratory tract disease and 22.5% were admitted to an ICU. Overall survival at 6 weeks from diagnosis was 77.9% and 72.1% in allogeneic and autologous recipients, respectively. Children had a survival of 93.4%. In multivariate analysis, older age ( p  = 0.02), need for ICU ( p  < 0.0001) and moderate/high immunodeficiency index ( p  = 0.04) increased the risk while better performance status ( p  = 0.001) decreased the risk for mortality. Other factors such as underlying diagnosis, time from HCT, GVHD, or ongoing immunosuppression did not significantly impact overall survival. We conclude that HCT patients are at high risk of developing LRTD, require admission to ICU, and have increased mortality in COVID-19.
Large-scale viral genome analysis identifies novel clinical associations between hepatitis B virus and chronically infected patients
Despite the high global prevalence of chronic hepatitis B (CHB) infection, datasets covering the whole hepatitis B viral genome from large patient cohorts are lacking, greatly limiting our understanding of the viral genetic factors involved in this deadly disease. We performed deep sequencing of viral samples from patients chronically infected with HBV to investigate the association between viral genome variation and patients’ clinical characteristics. We discovered novel viral variants strongly associated with viral load and HBeAg status. Patients with viral variants C1817T and A1838G had viral loads nearly three orders of magnitude lower than patients without those variants. These patients consequently experienced earlier viral suppression while on treatment. Furthermore, we identified novel variants that either independently or in combination with precore mutation G1896A were associated with the transition from HBeAg positive to the negative phase of infection. These observations are consistent with the hypothesis that mutation of the HBeAg open reading frame is an important factor driving CHB patient’s HBeAg status. This analysis provides a detailed picture of HBV genetic variation in the largest patient cohort to date and highlights the diversity of plausible molecular mechanisms through which viral variation affects clinical phenotype.
Impact of COVID-19 Lockdown in Eating Disorders: A Multicentre Collaborative International Study
Background. The COVID-19 lockdown has had a significant impact on mental health. Patients with eating disorders (ED) have been particularly vulnerable. Aims. (1) To explore changes in eating-related symptoms and general psychopathology during lockdown in patients with an ED from various European and Asian countries; and (2) to assess differences related to diagnostic ED subtypes, age, and geography. Methods. The sample comprised 829 participants, diagnosed with an ED according to DSM-5 criteria from specialized ED units in Europe and Asia. Participants were assessed using the COVID-19 Isolation Scale (CIES). Results. Patients with binge eating disorder (BED) experienced the highest impact on weight and ED symptoms in comparison with other ED subtypes during lockdown, whereas individuals with other specified feeding and eating disorders (OFSED) had greater deterioration in general psychological functioning than subjects with other ED subtypes. Finally, Asian and younger individuals appeared to be more resilient. Conclusions. The psychopathological changes in ED patients during the COVID-19 lockdown varied by cultural context and individual variation in age and ED diagnosis. Clinical services may need to target preventive measures and adapt therapeutic approaches for the most vulnerable patients.
Salicylic Acid-Cryotherapy Treatment for Elimination of Potato Virus S from Solanum Tuberosum
Potato virus S (PVS) is one of the most difficult viruses to eliminate by thermotherapy due to its thermal deactivation point. Pre-treatment methods involving salicylic acid (SA) have been successfully carried in conjugation with cryogenic methods to reduce oxidative damage and eliminate viruses. In the present investigation, the effect of SA to protect Solanum tuberosum plants infected with PVS from oxidative damage from subsequent cryotherapy was studied. Vulnerable genotypes to cryogenic protocol were selected, with two SA treatments examined. Potato clones were pretreated with SA (0, 10−5, and 10−6 M), plant development was evaluated and then exposed to cryotherapy. This was followed up by a plant development evaluation and virus testing. After the initial treatment with SA, the plants exhibited an increase in the variables evaluated before cryotherapy. After cryotherapy, between 66.6% and 100% of the treated plants were found to be virus-free compared to control plants which exhibited 0% survival. Thus, the SA-cryotherapy treatment combination described appears to enhance plant survival and eliminate PVS from potato plants.
Comorbidity between lung cancer and COVID-19 pneumonia: role of immunoregulatory gene transcripts in high ACE2-expressing normal lung
Background: SARS-CoV-2 (COVID-19) elicits a T-cell antigen-mediated immune response of variable efficacy. To understand this variability, we explored transcriptomic expression of angiotensin-converting enzyme 2 (ACE2, the SARS-CoV-2 receptor) and of immunoregulatory genes in normal lung tissues from patients with non-small cell lung cancer (NSCLC). Methods: This study used the transcriptomic and the clinical data for NSCLC patients generated during the CHEMORES study [n = 123 primary resected (early-stage) NSCLC] and the WINTHER clinical trial (n = 32 metastatic NSCLC). Results: We identified patient subgroups with high and low ACE2 expression (p = 1.55 × 10−19) in normal lung tissue, presumed to be at higher and lower risk, respectively, of developing severe COVID-19 should they become infected. ACE2 transcript expression in normal lung tissues (but not in tumor tissue) of patients with NSCLC was higher in individuals with more advanced disease. High-ACE2 expressors had significantly higher levels of CD8+ cytotoxic T lymphocytes and natural killer cells but with presumably impaired function by high Thymocyte Selection-Associated High Mobility Group Box Protein TOX (TOX) expression. In addition, immune checkpoint-related molecules – PD-L1, CTLA-4, PD-1, and TIGIT – are more highly expressed in normal (but not tumor) lung tissues; these molecules might dampen immune response to either viruses or cancer. Importantly, however, high inducible T-cell co-stimulator (ICOS), which can amplify immune and cytokine reactivity, significantly correlated with high ACE2 expression in univariable analysis of normal lung (but not lung tumor tissue). Conclusions: We report a normal lung immune-tolerant state that may explain a potential comorbidity risk between two diseases – NSCLC and susceptibility to COVID-19 pneumonia. Further, a NSCLC patient subgroup has normal lung tissue expressing high ACE2 and high ICOS transcripts, the latter potentially promoting a hyperimmune response, and possibly leading to severe COVID-19 pulmonary compromise.
Bluetongue: a review
Bluetongue is a non-contagious disease of domestic and wild ruminants caused by a virus within the Orbivirus genus of the family Reoviridae and transmitted by Culicoides biting midges. It is a reportable disease of considerable socioeconomic concern and of major importance for the international trade of animals and animal products. In the past, bluetongue endemic areas were found between latitudes 40 deg N and 35 deg S; however, bluetongue has recently spread far beyond this traditional range. This is in accordance with the extension of areas in which the biting midge Culicoides imicola, the major vector of the virus in the 'Old World', is active. After 1998 new serotypes of bluetongue virus (BTV) were discovered in Southern European and Mediterranean countries. Since 2006 BTV-serotype 8 has also been reported from the countries in Northern and Western Europe where Culicoides imicola has not been found. In such cases, BTV is transmitted by Palearctic biting midges, such as C. obsoletus or C. dewulfi, and the disease has thus spread much further north than BTV has ever previously been detected. New BTV serotypes have recently been identified also in Israel, Australia and the USA. This review presents comprehensive information on this dangerous disease including its history, spread, routes of transmission and host range, as well as the causative agent and pathogenesis and diagnosis of the disease. It also deals with relevant preventive and control measures to be implemented in areas with bluetongue outbreaks.
A morbillivirus that caused fatal disease in horses and humans
A morbillivirus has been isolated and added to an increasing list of emerging viral diseases. This virus caused an outbreak of fatal respiratory disease in horses and in humans. Genetic analyses show it to be only distantly related to the classic morbilli-viruses rinderpest, measles, and canine distemper. When seen by electron microscopy, viruses had 10- and 18-nanometer surface projections that gave them a \"double-fringed\" appearance. The virus induced syncytia that developed in the endothelium of blood vessels, particularly the lungs
