Shift Work and Cardiovascular disease-pathways from Circadian Stress to Morbidity
In order to establish a causal relation between shift work and cardiovascular disease (CVD), we need to verify the pathways from the former to the latter. This paper aims to review the current knowledge of the mechanisms between shift work and CVD. Shift work can increase the risk of CVD by several interrelated psychosocial, behavioral, and physiological mechanisms. The psychosocial mechanisms relate to difficulties in controlling working hours, decreased work-life balance, and poor recovery following work. The most probable behavioral changes are weight gain and smoking. The plausible physiological and biological mechanisms are related to the activation of the autonomie nervous system, inflammation, changed lipid and glucose metabolism, and related changes in the risk for atherosclerosis, metabolic syndrome, and type II diabetes. The data provide evidence for possible disease mechanisms between shift work and CVD, but compelling evidence on any specific mechanism is missing.
Shift Work, Sleep, and sleepiness-differences between Shift Schedules and Systems
In this narrative review, we examined what level of research evidence is available that shift workers' sleep-wake disturbances can be minimized through ergonomie shift scheduling. We classified the pertinent studies conducted on real shift workers in field conditions by the type of shift system and study design (ie, whether the shift systems were modified or not— \"treatment\" versus \"no treatment\"). The results of the observational studies in which no changes to the shift system were made (ie, no treatment) showed that, irrespective of the shift system, night and early-morning shifts and quick returns are associated with short sleep and increases in sleepiness. The same is true for very long shifts (> 16 hours) and extremely long weekly working hours (> 55 hours). For all categories of shift systems, there were a lack of controlled intervention studies, limiting the possibility to provide solution-focused recommendations for shift scheduling. Most of the controlled intervention studies had been conducted on workers under regular 3-shift systems. These studies suggested that a change from slowly backward-rotating shifts to rapidly forward-rotating shifts is advantageous for alertness and, to some degree, sleep. We also found that a change from an 8-to 12-hour shift system does not necessarily result in impairments in the sleep-wake pattern. The level of research evidence was affected by many of the studies' frequent methodological limitations in measuring sleep and sleepiness. In all, to have reliable and solution-focused recommendations for shift scheduling, methodologically sound controlled intervention studies are required in different categories of shift systems.
Association between time-related work factors and dietary behaviors: results from the Japan Environment and Children’s Study (JECS)
Background Few studies have examined the association of workhours and shift work (referred to here as “time-related work factors”) with dietary behaviors. We aimed to investigate this association, as well as the dietary behaviors among individuals with occupations characterized by time-related work factors. Methods A cross-sectional study was performed using data from the Japan Environment and Children’s Study. The study included 39,315 working men. Dietary behaviors (i.e., skipping breakfast, eating out, eating instant food, overeating, and eating fast) were assessed with a self-reported information from the Food Frequency Questionnaire. Logistic regression analysis was conducted to examine the associations of time-related work factors with dietary behaviors and dietary behavior tendencies among those in occupations characterized by long workhours and/or shift work. Results Long workhours were associated with high frequencies of skipping breakfast, eating out, eating instant food, overeating, and eating fast. The frequency of having shift work was associated with high frequencies of skipping breakfast, eating out, and eating instant food. Several occupations involving long workhours and/or shift work showed specific dietary behaviors; in some occupations, the level of significance changed after adjusting for time-related work factors in addition to other potential confounding factors. Conclusions Time-related work factors may help explain workers’ dietary behaviors. Long workhours and shift work may lead to poor dietary behaviors. Other factors influenced by occupation itself, such as food environment, may also influence workers’ dietary behaviors. Workhours and/or shift work, and these other work factors, should be given attention in workplace health promotion.
Objectively measured physical activity of hospital shift workers
Objectives Shift work may alter workers' leisure-time and occupational physical activity (PA) levels, which might be one of the potential underlying mechanisms of the negative health effects of shift work. Therefore, we compared objectively measured PA levels between hospital shift and non-shift workers. Methods Data were used from Klokwerk+, a cohort study examining the health effects of shift work among healthcare workers employed in hospitals. In total, 401 shift workers and 78 non-shift workers were included, all of whom wore Actigraph GT3X accelerometers for up to seven days. Time spent sedentary, standing, walking, running, stairclimbing, and cycling during leisure time and at work was estimated using Acti4 software. Linear regression was used to compare proportions of time spent in these activities between hospital shift and non-shift workers. Results Average accelerometer wear-time was 105.9 [standard deviation (SD) 14.0] waking hours over an average of 6.9 (SD 0.6) days. No differences between hospital shift and non-shift workers were found in leisure-time PA (P>0.05). At work, shift workers were less sedentary [B=-10.6% (95% CI -14.3- -6.8)] and spent larger proportions of time standing [B=9.5% (95% CI 6.4-12.6)] and walking [B=1.2% (95% CI 0.1- 2.2)] than non-shift workers. However, these differences in occupational PA became smaller when the number of night shifts during accelerometer wear-time increased. Conclusions Leisure-time PA levels of hospital shift workers were similar to those of non-shift workers, but shift workers were less sedentary and more physically active (ie, standing/walking) at work. Future research to the role of occupational activities in the health effects of shift work is recommended.
Health-related Effects of Worksite Interventions Involving Physical Exercise and Reduced Workhours
Objectives This study examined the health-related effects of two worksite interventions, physical exercise and reduced workhours, on women employed in dentistry. Methods Six workplaces were randomized to one of the following three conditions: (i) 2.5 hours of weekly, mandatory physical exercise of middle-to-high intensity to be performed during workhours (N=62), (ii) a reduction of full-time weekly workhours from 40 to 37.5 hours (N=50), and (iii) reference. In all, 177 women participated. Biomarkers and self-ratings in questionnaires were obtained before the intervention (T₁), and six (T₂) and 12 months (T₃) after the intervention. Results The results showed increased levels of physical activity and exercise in all of the groups, the level of physical exercise being significantly greater in the physical exercise group. Repeated-measures analyses of variance using data from T₁ and T₃ for biological measures and all three time points for self-ratings produced significant interaction effects for glucose, waist-to-hip ratio, and work ability and clear trends for general symptoms and upper-extremity disorders. Posthoc analyses showed that the results of the health-related measures differed between the interventions, decreased glucose and upper-extremity disorders in the exercise group, and increased high-density lipoprotein and waist-to-hip ratio among those working reduced hours. Conclusions These results show that the two interventions had small and varied effects on biomarkers and self-reports of different aspects of health among women. It is suggested that interventions involving a modest reduction in workhours seem to be more effective if these hours are used for physical exercise.
Working hours and depressive symptoms over 7 years: evidence from a Korean panel study
PurposeThis study aims to examine how working hours influence depressive symptoms and the association between working hours and depressive symptoms differently across genders.MethodThe sample consists of salaried workers aged 25–64 years who participated in two consecutive waves of the seven-wave Korean Welfare Panel Study (2007–2013) (n = 6813 individuals, 27,986 observations) which is a survey of a nationally representative sample of the South Korean population. I apply logit regression and fixed-effects logit regression to examine the causal relation between (intra-)individual changes of working hours and depressive symptoms over a 7-year period.ResultsResults from logit model and fixed-effects logit model show that less than 30 h of work per week and more than 60 h of work per week are associated with significantly higher levels of depressive symptoms. Sex-stratified analyses reveal that women who worked over 60 h per week were at increased risk of showing depressive symptoms compared with women who worked 30–40 h per week. No significant increase in depressive symptoms was seen in men who worked more than 60 h per week. However, men working less than 30 h per week are more likely to report higher levels of depressive symptoms.ConclusionsThese results suggest that work arrangement affects the mental health of men and women differently.
Tarzan, Jane, the baby and the boss
PurposeThis study aims to examine the two (and perhaps the most) important outcome variables of the interface between work and family, namely, overall job performance and parental functioning, in the context of performance appraisal.Design/methodology/approachEach of 844 respondents (managers or self-employed who supervise workers, half of them men) evaluated a briefly portrayed employed married parent on his/her job performance and parental functioning. Male and female respondents were randomly and equally allocated to one of 16 research conditions. They evaluated an employed married parent portrayed as a mother or a father, who increased or decreased his/her weekly workhours following the mother's return from maternity leave, invested relatively high or low effort in his/her work and exhibited relatively high or low work achievements.FindingsParents who invest a relatively high effort in their work were evaluated as having a higher level of job performance than those who invest a relatively low effort. Parents who exhibit relatively high work achievements were evaluated as having higher levels of job performance and parental functioning than those who exhibit relatively low work achievements. Parents who increased their weekly workhours following the mother's return from maternity leave were evaluated as having a lower level of parental functioning than those who decreased their weekly workhours.Originality/valueThis is a rare study implementing a factorial design with five independent variables (parent's time investment in work following the mother's return from maternity leave, his/her relative work effort, his/her relative work achievements, parent's gender and the evaluator’s gender) never manipulated simultaneously before.
Different Levels of work-related Stress and the Effects on Sleep, Fatigue and Cortisol
Objectives The aim of the study was to relate different levels of work stress to measures of sleep and the diurnal pattern of salivary cortisol and subjective sleepiness. Methods Thirty-four white-collar workers participated under two different conditions. One workweek with a relatively high stress level (H) and one with a lower stress level (L) as measured through self-rated stress during workdays. The workers wore activity monitors, filled out a sleep diary, gave saliva samples (for cortisol), and rated their sleepiness and stress during one workday and one free day. Results During the week with stress the number of workhours increased and total sleep time decreased. Sleepiness showed a significant interaction between weeks and time of day, with particularly high levels towards the evenings of the stress week. Cortisol also showed a significant interaction, with a more flattened pattern, probably due to increased evening levels during the stress week. Stress (restlessness) at bedtime was significantly increased during the stress week. Conclusions The results demonstrate that a workweek with a high workload and much stress increases sleepiness and workhours, impairs sleep, and affects the pattern of diurnal cortisol secretion.
Long Workhours, Work Scheduling and work-related Injuries among Construction Workers in the United States
Objectives The objectives of this study were (i) to examine work scheduling in construction and (ii) to establish whether there is any connection between workhours and safety outcomes among construction workers. Methods The National Longitudinal Survey of Youth, 1979 cohort (NLSY79), was used for the data analysis. Odds ratios were used to measure the risk of work-related injury in different worker groups. Results The findings showed that (i) construction workers started work earlier, worked longer days and fewer weeks a year, and were more likely to hold multiple jobs and change jobs than their nonconstruction counterparts and (ii) long workhours and irregular work schedules were significantly associated with a higher work-related injury rate after control for possible confounders. Conclusion The results provide evidence that overtime and irregular work scheduling have an adverse effect on worker safety.
Working Hours and Depressive Symptomatology among Full-Time Employees: Results from the Fourth Korean National Health and Nutrition Examination Survey (2007-2009)
Objective This study aimed to examine the distribution of working hours and the association between working hours and depressive symptomatology using representative data from a national, population-based survey. Method Data came from the fourth Korean National Health and Nutrition Examination Survey (2007—2009), which employed a systematic, stratified cluster-sampling method. We used logistic regression procedures to estimate the importance of weekly working hours as a predictor of depressive symptomatology. Results The prevalence of depressive symptomatology was 10.2%. The work week, which averaged 48.3 hours for the sample as a whole, was longer for men (49.8 hours) than women (45.3 hours), and 12.1% of respondents were engaged in shift work. In logistic regression analyses, compared to those working <52 hours per week, the odds ratios (OR) of working hours as a predictor of depressive symptomatology were 1.19 [95% confidence interval (95% CI) 0.77—1.85] for those working 52—59 hours per week and 1.62 (95% CI 1.20—2.18) for those working ≥60 hours per week, after adjustment for demographic characteristics, health behaviors, socioeconomic status, employment status, and work schedules. It showed a positive dose—response relationship between working hours and depressive symptomatology (P=0.0059). Conclusions Working hours in Korea are long. There is an association between working hours and depressive symptomatology, and there seems be a trend in working hours and depressive symptomatology.
