The Association between Long Working Hours and Health: A Systematic Review of Epidemiological Evidence
Objectives Many studies have investigated the association between long working hours and health. By focusing on differences in the definition of long working hours and the influence of shift work, we attempt to explain why the results of these studies remain inconclusive. Methods We defined long working hours as working time greater than around 40 hours per week or 8 hours per day. Since previous studies have indicated that shift work is detrimental to health, we minimized the influence of shift work in the studies. We also placed importance on the existence of reference groups since this made the results clearer. Based on these points, we analyzed previous studies to clarify the epidemiological evidence regarding the association between long working hours and health. We established inclusion criteria and carried out a systematic search for articles published in the Mediine and PsycINFO databases between 1995-2012. Results We identified a total of 17 articles and 19 studies (12 prospective cohort and 7 cross-sectional studies). The outcomes were all-cause mortality, circulatory disease, diabetes mellitus, metabolic syndrome, depressive state, anxiety, other psychological disorders, sleep condition, cognitive function, and health-related behavior. Long working hours had significant adverse effects on most health outcomes. Conclusions We concluded that working long hours is associated with depressive state, anxiety, sleep condition, and coronary heart disease. However, further studies that appropriately deal with the definition of long working hours and shift work are needed.
Working hours and health – key research topics in the past and future
OBJECTIVE: This paper discusses the past and present highlights of working hours and health research and identifies key research needs for the future. METHOD: We analyzed over 220 original articles and reviews on working hours and health in the Scandinavian Journal of Work, Environment & Health published during the last 50 years. Key publications from other journals were also included. RESULTS: The majority of identified articles focussed on the effects of shift and night work, with fewer studying long and reduced working hours and work time control. We observed a transition from small-scale experimental and intensive field studies to large-scale epidemiological studies utilizing precise exposure assessment, reflecting the recent emergence of register-based datasets and the development of analytic methods and alternative study designs for randomized controlled designs. The cumulative findings provide convincing evidence that shift work and long working hours, which are often associated with night work and insufficient recovery, increase the risk of poor sleep and fatigue, sickness absence, occupational injuries, and several chronic health conditions such as cardiovascular diseases and cancer. The observed risks are strongly modified by individual and work-related factors. CONCLUSIONS: Although the observed health risks of shift work and long working hours are mostly low or moderate, the widespread prevalence of exposure and the hazardousness of the many associated potential outcomes makes such working time arrangements major occupational health risks. Further research is needed to identify exposure–response associations, especially in relation to the chronic health effects, and to elucidate underlying pathways and effective personalized intervention strategies.
Association of long working hours with accidents and suicide mortality in Korea
Objectives The deleterious health effects of long working hours have been previously investigated, but there is a dearth of studies on mortality resulting from accidents or suicide. This prospective study aims to examine the association between working hours and external-cause mortality (accidents and suicide) in Korea, a country with some of the longest working hours in the world. Methods Employed workers (N=14 484) participating in the Korean National Health and Nutrition Examination Survey (KNHANES) were matched with the Korea National Statistical Office's death registry from 2007-2016 (person-years = 81 927.5 years, mean weighted follow-up duration = 5.7 years). Hazard ratios (HR) for accident (N=25) and suicide (N=27) mortality were estimated according to weekly working hours, with 35-44 hours per week as the reference. Results Individuals working 45-52 hours per week had higher risk of total external cause mortality compared to those working 35-44 hours per week [HR 2.79, 95% confidence interval (CI) 1.22-6.40], adjusting for sex, age, household income, education, occupation, and depressive symptoms. Among the external causes of death, suicide risk was higher (HR 3.89, 95% CI 1.06-14.29) for working 45-52 hours per week compared to working 35-44 hours per week. Working >52 hours per week also showed increased risk for suicide (HR 3.74, 95% CI 1.03-13.64). No statistically significant associations were found for accident mortality. Conclusions Long working hours are associated with higher suicide mortality rates in Korea.
The association between long working hours, shift work, and suicidal ideation: A systematic review and meta-analyses
OBJECTIVES: The association between occupational psychosocial factors and mental health has been studied in several systematic reviews. However, few systematic reviews exist that specifically address suicidal ideation. Therefore, this systematic review and meta-analysis aimed to examine the associations between long working hours, shift work, and suicidal ideation. METHODS: We systematically screened and collected studies published between January 1970 and September 2022 from three electronic databases and Google Scholar. We conducted a meta-analysis using a random-effects model and assessed the risk of bias using a navigation guide. Additionally, the grading of recommendations assessment, development and evaluation (GRADE) approach was used to consider five items, including the risk of bias and publication bias. RESULTS: Twenty-eight articles were included in this meta-analysis. After adjusting for covariates, the overall odds ratio (OR) for long working hours and shift work were 1.44 [95% confidence interval (CI) 1.26–1.63] and 1.34 (95% CI 1.22–1.47), respectively. Compared with those who worked <40 hours per week, those who worked >55 hours per week had a 1.65 (95% CI 1.37–1.98) higher risk of suicidal ideation, and fixed night shift workers had 1.37 (95% CI 1.03–1.83) higher risk than non-shift workers. CONCLUSIONS: Evidence has shown associations between long working hours, shift work, and suicidal ideation. Implementing evidence-based interventions to address these risk factors may help prevent the progression of suicidal ideation. However, further studies are critically needed, especially those using standardized exposure definitions and longitudinal data.
Relationship between long working hours and smoking behaviors: Evidence from population-based cohort studies in Korea
OBJECTIVES: Long working hours and overwork are growing public health concerns in the Western-Pacific region. We explored the relationship between working hours and smoking behaviors of Korean workers. METHODS: This study included 284 782 observations (50 508 workers) from four nationwide cohort studies in Korea. Using generalized estimating equations, we estimated the associations of working hours with current smoking status, smoking initiation, and smoking cessation within each cohort. Cohort-specific estimates were combined through random-effect meta-analysis. Effect sizes were presented as odds ratios (OR) and 95 confidence intervals (CI). RESULTS: The overall smoking prevalence was 26.8% within the cohorts. The adjusted OR (95% CI) of the association between working hours and current smoking were 1.01 (0.94–1.08) for <35 hours/week, 1.04 (1.01–1.09) for 41–48 hours/week, 1.06 (1.01–1.10) for 49–54 hours/week, and 1.07 (1.04–1.10) for ≥55 hours/week compared with 35–40 hours/week. The adjusted OR (95% CI) of the association between working hours and smoking cessation in the follow-up were 0.93 (0.85–1.02) for <35 hours/week, 0.89 (0.83–0.96) for 41–48 hours/week, 0.87 (0.81–0.95) for 48–54 hours/week, and 0.91 (0.85–0.98) for ≥55 hours/week compared with 35–40 hours/week. No clear associations were observed between working hours and smoking initiation. CONCLUSION: Long working hours are associated with high current smoking risk and reduced likelihood of smoking cessation among Korean workers. Policy interventions are required to promote smoking cessation and reduce excess overwork for individuals experiencing long working hours.
Long working hours and depressive symptoms: systematic review and meta-analysis of published studies and unpublished individual participant data
Objectives This systematic review and meta-analysis combined published study-level data and unpublished individual-participant data with the aim of quantifying the relation between long working hours and the onset of depressive symptoms. Methods We searched PubMed and Embase for published prospective cohort studies and included available cohorts with unpublished individual-participant data. We used a random-effects meta-analysis to calculate summary estimates across studies. Results We identified ten published cohort studies and included unpublished individual-participant data from 18 studies. In the majority of cohorts, long working hours was defined as working ≥55 hours per week. In multivariable-adjusted meta-analyses of 189 729 participants from 35 countries [96 275 men, 93 454 women, follow-up ranging from 1-5 years, 21 747 new-onset cases), there was an overall association of 1.14 (95% confidence interval (CI) 1.03-1.25] between long working hours and the onset of depressive symptoms, with significant evidence of heterogeneity (I^2=45.1%, P=0.004). A strong association between working hours and depressive symptoms was found in Asian countries (1.50, 95% CI 1.13-2.01), a weaker association in Europe (1.11, 95% CI 1.00-1.22), and no association in North America (0.97, 95% CI 0.70-1.34) or Australia (0.95, 95% CI 0.70-1.29). Differences by other characteristics were small. Conclusions This observational evidence suggests a moderate association between long working hours and onset of depressive symptoms in Asia and a small association in Europe.
Global, regional and national burden of disease attributable to 19 selected occupational risk factors for 183 countries, 2000–2016
We provide a brief introduction to the objectives, data, methods and results of the World Health Organization (WHO)/International Labor Organization (ILO) Joint Estimates of the Work-related Burden of Disease and Injury (WHO/ILO Joint Estimates), which estimated the burden attributable to 19 selected occupational risk factors. The WHO/ILO Joint Estimates were produced within the global Comparative Risk Assessment framework, which attributes the burden of one specific health outcome (ie, disease/injury) to a specific occupational risk factor. For 39 established occupational risk factor-health outcome pairs, estimates are produced using population attributable fractions (PAF) from recent burden of disease estimates. For two additional pairs, PAF are calculated from new databases of exposure and risk ratios produced in WHO/ILO systematic reviews. Attributable disease burdens were estimated by applying the PAF to total disease burdens. Globally in 2016, it is estimated that 1.88 [95% uncertainty range (UR) 1.84-1.92] million deaths and 89.72 (95% UR 88.61-90.83) million disability-adjusted life years were attributable to the 19 selected occupational risk factors and their health outcomes. A disproportionately large work-related burden of disease is observed in the WHO African Region (for disability-adjusted life years), South-East Asia Region, and Western Pacific Region (for deaths), males and older age groups. The WHO/ILO Joint Estimates can be used for global monitoring of exposure to occupational risk factors and work-related burden of disease and to identify, plan, cost, implement and evaluate policies, programs and actions to prevent exposure to occupational risk factors and their associated burden.
Impact of Shift Work and Long Working Hours on Worker Cognitive Functions: Current Evidence and Future Research Needs
Particular working conditions and/or organization of working time may cause important sleep disturbances that have been proposed to be predictive of cognitive decline. In this regard, circadian rhythm misalignment induced by exposure to night work or long working hours would be responsible for cognitive impairment. Nevertheless, evidence supporting this correlation is limited and several issues still need to be elucidated. In this regard, we conducted a systematic review to evaluate the association between shift/night work and cognitive impairment and address its main determinants. Information provided by the reviewed studies suggested that night work might have serious immediate negative effects especially on cognitive domains related to attention, memory and response inhibition. Furthermore, cognitive performance would progressively worsen over consecutive night shifts or following exposure to very long work shifts. Otherwise, conflicting results emerged regarding the possible etiological role that night work chronic exposure would have on cognitive impairment. Therefore, circadian rhythm desynchronization, lack of sleep and fatigue resulting from night work may negatively impact worker’s cognitive efficiency. However, in light of the considerable methodological variability of the reviewed studies, we proposed to develop a standardized research and evaluation strategy in order to obtain a better and comprehensive understanding of this topic.
Supervising physicians’ perceptions on physician work-hour regulations in Japan: a nationwide cross-sectional study
Purpose Japan introduced physician work-hour regulations in April 2024. Perceptions of such regulations appear to be influenced by history and culture, and thus vary among stakeholders and countries. Here, we aimed to investigate supervising physicians’ perceptions of physician work-hour regulations in Japan. Methods A nationwide cross-sectional study was conducted from March to April, 2024. We distributed an online anonymous self-administered questionnaire, which included closed questions about expected influence of physician work-hour regulations on various factors and an open-ended question regarding expectations or concerns about the regulations. The data were analyzed using descriptive statistics for the closed questions and inductive content analysis for the open-ended question. To explore whether various factors (sex, postgraduate years, specialty, hospital location, hospital type, and hospital size) were associated with the perceptions of the pariticpants on the implementation of physician work-hour regulations (overall, patient care, resident physician well-being, resident physician education, or supervising physician well-being), we also performed multivariable linear regression analysis. Results We included 144 participants in the analysis. Many of the participants raised concerns about the negative impact of the regulations, particularly on the training of medical residents. About one-fifth of the respondents described their thoughts about the gap between the real medical field and the system. Some participants used the Japanese term jikokensan , which literally means self-improvement but is used to refer to study or research conducted by a physician with the aim of acquiring knowledge or enhancing skills apart from their primary duties, in the context of physician work-hour regulations. In multivariable analyses, several organizational factors showed statistically significant associations with supervisors’ perceptions of physician work-hour regulations. Nevertheless, effect sizes were small, there was no clear dose–dependent relationships, and findings were inconsistent across the five outcome domains; thus, no strong or consistent predictors were identified. Conclusions These results highlight the need to more fully explore supervisor perspectives, which could lead to more discourse in policy-making and an improved system of physician work-hour regulations.
Effect of long work hours and shift work on high-sensitivity C-reactive protein levels among Korean workers
Objective We aimed to investigate the association between low-grade inflammation as indicated by high-sensitivity C-reactive protein (hsCRP) level and organizational factors, such as work hours and shift work. Methods We evaluated 7470 young and middle-aged workers who participated in the Korea National Health and Nutrition Examination Surveys from 2015-2018. Work hours were determined from self-reported questionnaires. Shift work was defined as a non-daytime fixed work schedule. An interaction effect between shift work and long work hours on the hsCRP level was estimated using relative excess risk due to interaction (RERI) and attributable proportion (AP) with 95% confidence intervals (CI). Results Increased hsCRP levels were prevalent in 25.2% of the study population. There was a significant association between long work hours and increased hsCRP, especially among middle-aged men [odds ratio (OR) 1.50 (95% CI 1.20-1.87) for moderately increased hsCRP and OR 1.62 (95% CI 1.14-2.30) for highly increased hsCRP]. There was a significant interaction effect between long work hours and shift work on increased hsCRP among middle-aged workers. The RERI were 0.03 (95% CI 0.02-0.04) and 0.56 (95% CI 0.45-0.68) among middle-aged men and women, respectively. The AP were 0.02 (95% CI 0.01-0.03) and 0.36 (95% CI 0.31-0.40) among middle-aged men and women, respectively. Conclusion There was no significant association between shift work and the level of hsCRP. Long work hours were related to low-grade inflammatory processes, but only in middle-aged workers. There was an interaction effect between long work hours and shift work for increased hsCRP, especially in middle-aged women.
