يتناول الكتاب عددا من المواضيع التي أثارها التقدم الطبي في القرن التاسع عشر ومطلع القرن العشرين فإن كانغيلم ينتقد فكرة الريادة في تاريخ العلوم التي تنتمي إلى فترة ما قبل العلم كما أنه يرى في العقلانية منهجا للبحث وليست مذهبا لذلك يمتلك كل علم معقوليته المتعلقة بخصائص اهتماماته التي ستفرض بدورها مناهجه ولهذا فإن المنهج العلمي يقتضي حصر الاهتمام في ميدان تخصصي محدد هو ما يسمى \"ابستمولوجيا محلية أو جهوية\" تولي اهتماما بالإشكالات المعرفية التي يثيرها هذا الميدان التخصصي.
